Evaluation of the Surface of Nitinol after MR Polishing Process

Nitinol is used in various industries, especially for biomaterials. Generally, the surface integrity of sub-micron level and non-surface flaw is required in nitinol for biomaterial. However, surface flaws, such as scratches, pits, and residual stress, can occur on the polished surface, and it is difficult to remove these surface flaws. The corrosion is accelerated continuously and causes bad influence in the body. Therefore, in this study, conventional polishing and MR polishing were used to minimize the corrosion and residual stress when the nitinol is used as biomaterial. X-ray diffraction and Tafel extrapolation were used to investigate the residual stress and the corrosion, and the strain hardening was investigated by nanoindentation. Surface integrities were also evaluated by SEM image and surface roughness results. As a result, the increased residual stress in conventional polishing was decreased by MR polishing. The result of corrosion test was improved from ?301 mV to ?280 mV. The residual stress was improved from ?38.18 MPa to ?31.17 MPa. Finally, it is certified that MR polishing can be used to minimize surface flaws and to improve the corrosion properties.

Helicobacter pylori in middle ear of children with

Background Otitis media with effusion （OME） is a common pediatric disease, but its pathogenesis remains uncertain. The relationship between OME and Helicobacter pylori （HP） is currently being studied, and a relationship has not yet been confirmed. The purpose of this study was to show that a relationship does exist between HP and OME. Methods The study consisted of 60 patients who were diagnosed with OME and had ventilation tube insertions with or without an adenoidectomy. This study included an additional 30 patients who had only received an adenoidectomy without being diagnosed with OME. The effusion samples were analyzed with polymerase chain reaction （PCR） and the campylobacter-like organism （CLO） test. The adenoid tissue samples were analyzed with the CLO test. Results Eighteen patients among the 60 patients （30%） tested positive for HP. In the cases with adenoids, 15.6% of the OME patients and 13.3% of the adenoidectomy only patients were positive for HP. There were no differences between the prevalence of HP in the adenoids of OME patients and the patients without OME. Conclusion HP can be considered one of the causes of OME.

Timing of upper gastrointestinal endoscopy does not influence short-term outcomes in patients with acute variceal bleeding

AIM To examine the association between the timing of endoscopy and the short-term outcomes of acute variceal bleeding in cirrhotic *** This retrospective study included 274 consecutive patients admitted with acute esophageal variceal bleeding of two tertiary hospitals in Korea. We adjusted confounding factors using the Cox proportional hazards model and the inverse probability weighting(IPW) method. The primary outcome was the mortality of patients within 6 *** A total of 173 patients received urgent endoscopy(i.e., ≤ 12 h after admission), and 101 patients received nonurgent endoscopy(> 12 h after admission). The 6-wk mortality rate was 22.5% in the urgent endoscopy group and 29.7% in the non-urgent endoscopy group, and there was no significant difference between the two groups before(P = 0.266) and after IPW(P = 0.639). The length of hospital stay was statistically different between the urgent group and non-urgent group(P = 0.033); however, there was no significant difference in the inhospital mortality rate between the two groups(8.1% vs 7.9%, P = 0.960). In multivariate analyses, timing of endoscopy was not associated with 6-wk mortality(hazard ratio, 1.297; 95% confidence interval, 0.806-2.089; P = 0.284).CONCLUSION In cirrhotic patients with acute variceal bleeding, the timing of endoscopy may be independent of short-term mortality.

AIM: To evaluated the value of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) scan in diagnosis of hepatocellular carcinoma (HCC) and extrahepatic metastases. METHODS: A total of 138 patients with HCC who had both conventional imaging modalities and 18F-FDG PET/CT scan done between November 2006 and March 2011 were enrolled. Diagnostic value of each imaging modality for detection of extrahepatic metastases was evaluated. Clinical factors and tumor characteristics including PET imaging were analyzed as indicative factors for metastases by univariate and multivariate methods. RESULTS: The accuracy of chest CT was significantly superior compared with the accuracy of PET imaging for detecting lung metastases. The detection rate of metastatic pulmonary nodule ≥ 1 cm was 12/13 (92.3%), when < 1 cm was 2/10 (20%) in PET imaging. The accuracy of PET imaging was significantly superior compared with the accuracy of bone scan for detecting bone metastases. In multivariate analy- sis, increased tumor size (≥ 5 cm) (P = 0.042) and increased average standardized uptake value (SUV) uptake (P = 0.028) were predictive factors for extrahepatic metastases. Isometabolic HCC in PET imaging was inversely correlated in multivariate analysis (P = 0.035). According to the receiver operating characteristic curve, the optimal cutoff of average SUV to predict extrahepatic metastases was 3.4. CONCLUSION: 18F-FDG PET/CT scan is invaluable for detection of lung metastases larger than 1 cm and bone metastases. Primary HCC having larger than 5 cm and increased average SUV uptake more than 3.4 should be considered for extrahepatic metastases.

Comparison of outcomes between laparoscopicvs. open liver resection for intermediate stage hepatocellular carcinoma

Aim: The Barcelona Clinic Liver Score (BCLC) currently limits hepatic resection only for small, solitary tumors measuring 2-3 cm with no signs of portal hypertension (PHT) or macrovascular invasion. The aim of this study was to show the benefit of surgical resection, and to compare the peri-operative and long-term outcomes between laparoscopic liver resection (LLR) and open liver resection (OLR) for hepatocellular carcinoma (HCC) classified as intermediate stage (B) under BCLC. Methods: From 2004 to 2013, 49 patients staged as intermediate (BCLC B) and who underwent hepatic resection was included. These patients were divided into LLR or OLR. Demographics, tumor characteristics, recurrence rates and overall survival (OS) were compared between the 2 groups. Results: Forty-nine patients were included and grouped into LLR (n = 28) and OLR (n = 21). The average tumor number was 2 ± 1 for both groups, while the mean tumor size was 4.4 ± 1.7 cm and 5.3 ± 2.6 cm for the LLR and OLR group, respectively. When compared with OLR, LLR had lower post-operative complications (14.3% vs. 33.3%, P = 0.114), and a statistically significant shorter hospital stay than the OLR group (9 vs. 21 days, P = 0.023). The LLR group also achieved a statistically significant difference in complete R0 resection as compared with the OLR group (P =0.016). The OS and disease-free survival (DFS) at 1, 3 and 5 years were comparable between LLR and OLR (OS: 89.1% vs. 76.2%;70.4% vs. 55.9%;58.6% vs. 43.5%, P = 0.583;DFS: 59.3% vs. 51.0%;20.2% vs. 44.6%;16.2% vs. 37.2%, P = 0.947, respectively). Conclusion: LLR showed comparable outcomes compared to OLR in the treatment of HCC staged BCLC B. Therefore, LLR as well as OLR can be considered in selective patients in the BCLC B group.

采用韩国当地原料及废瓷粉制备梅子青裂纹釉。为了改善釉面针孔及解决当地废瓷大量堆放等问题,考察废瓷粉含量对梅子青裂纹釉的影响。研究结果表明:随着废瓷粉含量的增加,釉面的裂纹逐渐由稠密向稀疏直裂纹转化,釉面梅子青色调变淡。当废瓷粉含量为3%时,釉面呈现交叉网状的细碎裂纹,梅子青色调并具有玉质感。这主要是由于釉中析出石英晶相,并且釉中有许多的小气泡存在,对光造成了漫反射和散射,因此釉面呈现玉质感。该研究结果不仅拓宽了废瓷粉在青瓷釉中的应用,而且利于保护环境,节约生产成本。

An Experimental Study on the Behavior of Shear Keys According to the Curing Time of UHPC

Precast segmental construction has been recently developed to reduce the construction cost and shorten the construction term as compared to the cast-in-place method in a will to establish the design and erection system of structures using Ultra High Performance Concrete (UHPC). However, this method requires the presence of segmental joints to transfer the loads between neighboring segments, which stresses the importance of securing structural safety and serviceability. Therefore, need is for research on the behavior of the segmental joint for the structures erected by the precast segmental construction method. To that goal, this paper presents an experimental study on the behavior of shear keys with respect to the curing time of UHPC in the segmental joint. Analysis is done on the load-displacement relation according to the curing time of the shear keys and on the failure modes of the shear keys according to the cracking pattern at failure.

Chronic gastrointestinal symptoms and quality of life in the Korean population

AIM: To evaluate the prevalence of chronic gastroin-testinal symptoms and their impact on health-related quality of life (HRQOL) in the Korean population. METHODS: A cross-sectional survey, using a reliable and valid Rome Ⅱ based questionnaire, was per-formed on randomly selected residents, between 18 and 69 years in age. All respondents were interviewed at their homes or offices by a team of interviewers. The impact of chronic gastrointestinal symptoms on HRQOL was assessed using the Korean version of the 36-item Short-Form general health survey (SF-36). RESULTS: Of the 1807 eligible subjects, 1417 (78.4%: male 762; female 655) were surveyed. Out of the respondents, 18.6% exhibited at least one chronic gastrointestinal symptom. The prevalence of gastro- esophageal reflux disease (GERD), defined as heart- burn and/or acid regurgitation experienced at least weekly, was 3.5% (95% CI, 2.6-4.5). The prevalence of uninvestigated dyspepsia, irritable bowel syndrome (IBS) and chronic constipation based on Rome Ⅱ cri- teria were 11.7% (95% CI, 10.1-13.5), 2.2% (95%CI, 1.5-3.1), and 2.6% (95% CI, 1.8-3.5) respectively. Compared with subjects without chronic gastrointesti- nal symptoms (n = 1153), those with GERD (n = 50), uninvestigated dyspepsia (n = 166) and IBS (n = 31) had signifi cantly worse scores on most domains of the SF-36 scales. CONCLUSION: The prevalence of GERD, uninvesti-gated dyspepsia and IBS were 3.5%, 11.7% and 2.2% respectively, in the Korean population. The health- related quality of life was signifi cantly impaired in sub-jects with GERD, uninvestigated dyspepsia and IBS in this community.
