The global incidence rates of in-hospital cardiac arrest(IHCA)range from1.2 to 9.0 per1,000 hospitalized patients,as per the National Cardiac Arrest Database.[1] While IHCAs tend to exhibit superior 30-day survival rates relative to out-of-hospital cardiac arrests (OHCA) due to situational advantages,such as immediate access to medical personnel and treatments.

Herein,the purpose of this study is to evaluate the effects of kaempferol on bioavailability and pharmacokinetics of nifedipine and its metabolite dehydronifedipine in *** experimental design is based on with or without kaempferol in the oral and intravenous administration of nifedipine in ***,the pharmacokinetic parameters including nifedipine and dehydronifedipine were evaluated in *** in vitro studies of kaempferol were investigated on P-glycoprotein(P-gp)and cytochrome P450(CYP)3A4 *** reduced a 50%inhibitory concentration(IC50)of 8.6μmol·L-1 on CYP3A4 enzyme ***,kaempferol clearly improved the cell internalization of rhodamine-123 in MCF-7/ADR cells overexpressing *** on increased concentrations of kaempferol,the areas under the plasma concentration–time curve(AUC0-∞)and the peak concentration(Cmax)of nifedipine were increased after oral and intravenous ***,the absolute bioavailability(AB)and relative bioavailability(RB)of nifedipine in the presence of kaempferol was significantly higher than those of the control group after oral and intravenous *** of bioavailability of nifedipine by kaempferol may be mainly because of the inhibition of the P-gp-mediated efflux transporter in the small intestine and CYP3A4-mediated metabolism in the small intestine or liver,or both.

To evaluate the effect of methotrexate on collagen-induced arthritis,micro-computed tomography(micro-CT)and histopathological analyses were used in male Wistar *** were divided randomly into three *** 1 was treated with 0.9% saline,and groups 2 and 3 were boosted with type II *** day 21 to 42,groups 1 and 2 were orally treated with 0.9% saline and group 3 was orally treated with 1.5 mg/kg *** rats were sacrificed at day 42 after the first collagen ***-CT analyses showed bony parameters,such as bone volume and trabecular number,were decreased in group 2 compared to group 1,and these parameters were recovered in group *** examination and pathological parameter scoring showed that the knee joints of rats in group 2 had severe joint destruction,showing cartilage and bone erosion,enlarged cavities with inflammatory cell infiltration and activation of synovial *** contrast,these changes were reduced in group *** together,methotrexate treatment showed therapeutic potential in male rat collageninduced arthritis model,and micro-CT analysis and histopathological tools could be integrated to assess the quantification/qualification of arthritic lesions.

Electrospinning is a simple and versatile method to produce nanofiber ***,owing to bending instability that occurs during the electrospinning process,electrospinning has frequently produced a non-uniform-thickness nanofiber filter,which deteriorates its air ***,an adaptive electrospinning system based on reinforcement learning(E-RL)was developed to produce uniform-thickness nanofiber *** E-RL accomplished a real-time thickness measurement of an electrospun nanofiber filter by measuring the transmitted light through the nanofiber filter using a camera placed at the bottom of the collector and converting it into thickness using the Beer–Lambert *** on the measured thickness,the E-RL detected the non-uniformity of the nanofiber filter thickness and manipulated the movable collector to alleviate the non-uniformity of the thickness by a pre-trained reinforcement learning(RL)*** the training of the RL algo-rithm,the nanofiber production simulation software based on the empirical model of the deposition of the nanofiber filter was developed,and the training process of the RL algorithm was repeated until the optimal policy was *** the training process with the simulation software,the trained model was transferred to the adaptive electrospinning *** the movement of the collector under the optimal strategy of RL algorithm,the non-uniformity of such nanofiber filters was significantly reduced by approximately five times in standard deviation and error for both simulation and *** finding has great potential in improving the reliability of electrospinning process and nanofiber filters used in research and industrial fields such as environment,energy,and biomedicine.

Assessment of disease activity by fecal immunochemical test in ulcerative colitis

AIM To evaluate the efficacy of quantitative fecal immunochemical test(FIT) as biomarker of disease activity in ulcerative colitis(UC).METHODS Between February 2013 and November 2014, a total of 82 FIT results, obtained in conjunction with colonoscopies, were retrospectivelyevaluated for 63 patients with UC. The efficacy of FIT for evaluation of disease activity was compared to colonoscopic findings. Quantitative fecal blood with automated equipment examined from collected feces. Endoscopic disease severity were assessed using the Mayo endoscopic subscore(MES) classification. The extent of disease were classified by proctitis(E1), left sided colitis(E2), and extensive colitis(E3). Clinical activity were subgrouped by remission or *** All of 21 patients with MES 0 had negative FIT( 134.89 ng/m L. The sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV) and accuracy of negative FIT about mucosal healing were 73.33%, 81.82%, 91.49%, 51.43% and 73.17%, respectively. The sensitivity, specificity, PPV, NPV and accuracy of predictive value of positive FIT(cutoff value > 100 ng/mL) about active disease status were 45.45%, 93.33%, 71.43%, 82.35%and 26.83%, respectively. Among patients with clinical remission, FIT was negative in 31(81.6%) of 38 cases, with a mean fecal hemoglobin concentration of 6.12 ng/mL(range, negative to 80.9 ng/mL) for this group of patients. Among patients with clinical active disease, FIT was negative in 16(36.4%) out of 44 cases, with a mean fecal hemoglobin concentration > 167.4 ng/mL for this group of patients. FIT was positively correlated with endoscopic activity(r = 0.626, P < 0.01) and clinical activity(r = 0.496, P < 0.01). To evaluate the safety and efficacy of limited endoscopic sphincterotomy (ES) before placement of self-expandable metal stent (SEMS).METHODSThis was a retrospective analysis of 244 consecutive patients with unresectable malignant biliary obstruction, who underwent placement of SEMSs following limited ES from December 2008 to February 2015. The diagnosis of malignant biliary obstruction and assessment of patient eligibility for the study was established by a combination of clinical findings, laboratory investigations, imaging and pathological results. All patients were monitored in the hospital for at least 24 h following endoscopic retrograde cholangio pancreatography (ERCP). The incidence of immediate or early post-ERCP complications such as post-ERCP pancreatitis (PEP) and bleeding related to limited ES were considered as primary outcomes. Also, characteristics and complications according to the cancer type were *** the 244 patients included, the underlying diagnosis was cholangiocarcinoma in 118 patients, pancreatic cancer in 79, and non-pancreatic or non-biliary malignancies in the remaining 47 patients. Early post-ERCP complications occurred in 9 patients (3.7%), with PEP in 7 patients (2.9%; mild, 6; moderate, 1) and mild bleeding in 2 patients (0.8%). There was no significant association between the incidence of post-ERCP complications and the type of malignancy (cholangiocarcinoma vs pancreatic cancer vs others, P = 0.696) or the type of SEMS used (uncovered vs covered, P = 1.000). Patients who had more than one SEMS placed at the first instance were at a significantly higher risk of post-ERCP complications (one SEMS vs two SEMS, P = 0.031). No other factors were predictive of post-ERCP *** ES is feasible and safe, and effectively facilitates the placement of SEMS, without any significant risk of PEP or severe bleeding.

Described is an initial attempt to simulate the suspended sediment dynamics relating to tidal and wave forcing during summertime in June 1980 and August 1981 for the Yellow and East China Seas continental shelf. The cohesive/non-cohesive sediment resuspension and movement generated by the interaction between current and wave are modeled by use of ECOMSED and WAM Cycle 4. Model results are compared with observations in US-China Marine Sedimentation Dynamics Program performed for 1980-81 at off the Changjiang estuary. The main features of simulations show that suspended sediment concentrations during the summer decreased markedly offshore as observed during the simulation periods. As for some discrepancies for the mouth of the estuary with high river discharges, i.e., the Changjiang River, the model did not properly reproduce the over-mixing situation in the summer; thus distinct vertical concentration variation in this local region is not agreeable with observation. However, general dispersal patterns of suspended sediment movement seem to be agreeably reproduced for the nearshore shallow region. Some of the procedures of simulation and results are presented and discussed.
