The current status of hormone treatment for prostate cancer patients in Korean real-world practice:a multi-institutional observational study

We aimed to evaluate the current nationwide trend,efficacy,safety,and quality of life(QoL)profiles of hormone treatment in real-world practice settings for prostate cancer(PCa)patients in Korea.A total of 292 men with any biopsy-proven PCa(TanyNanyMany)from 12 institutions in Korea were included in this multi-institutional,observational study of prospectively collected data.All luteinizing hormone-releasing hormone(LHRH)agonists were allowed to be investigational drugs.Efficacy was defined as(1)the rate of castration(serum testosterone≤50 ng dl^-1)at 4-week visit and(2)breakthrough(serum testosterone>50 ng dl^-1 after castration).Safety assessments included routine examinations for potential adverse events,laboratory tests,blood pressure,body weight,and bone min era I density(BMD,at baseli ne and at the last follow-up visit).QoL was assessed using the Expa nded Prostate Cancer Index Composite-26(EPIC-26).The most comm on initial therapeutic regimen was LHRH agonist with an ti-androge n(78.0%),and the most commonly used LHRH agonist for combination and mono therapy was leuprolide(64.0%for combi natio n and 58.0%for monotherapy).The castration and breakthrough rates were 78.4%and 6.6%,respectively.The laboratory results related to dyslipidemia worsened after 4 weeks of hormone treatment.In addition,the mean BMD T-score was significantly lower at the last follow-up(mean:-1.950)compared to baseline(mean:-0.195).The mean total EPIC-26 score decreased from 84.8(standard deviation[s.d.]:12.2)to 78.3(s.d.:8.1),with significant deterioration only in the urinary domain(mean:23.5 at baseli ne and 21.9 at the 4-week visit).These findi ngs dem on strate the n ationwide trend of current practice setti ngs in hormone treatment for PCa in Korea.

Ultrasound-guided intra-articular corticosteroid injection in a patient with manubriosternal joint involvement of ankylosing spondylitis:A case report

BACKGROUND Manubriosternal joint(MSJ)disease is a rare cause of anterior chest pain but can be a major sign of systemic arthritic involvement.In patients with ankylosing spondylitis(AS),a type of systemic arthritis,chest pain can be due to MSJ involvement and can be improved by ultrasound-guided corticosteroid injection into the joint.CASE SUMMARY A 64-year-old man visited our pain clinic complaining of anterior chest pain.There were no abnormal findings on lateral sternum X-ray,but arthritic changes in the MSJ were observed on single-photon emission computed tomography-computed tomography.We performed additional laboratory tests,and he was finally diagnosed with AS.For pain relief,we performed ultrasound-guided intra-articular(IA)corticosteroid injections into the MSJ.After the injections,his pain nearly resolved.CONCLUSION For patients complaining of anterior chest pain,AS should be considered,and single-photon emission computed tomography-computed tomography can be helpful in diagnosis.In addition,ultrasound-guided IA corticosteroid injections may be effective for pain relief.

Undetected traumatic cardiac herniation like playing hide-and-seek-delayed incidental findings during surgical stabilization of flail chest:A case report

BACKGROUND Post-traumatic blunt pericardial injury is a rare condition with only a few reported cases which were generally diagnosed during initial examinations upon admission.However,pericardial injuries not bad enough to dislocate the heart may only cause intermittent electrocardiogram(ECG)changes or be asymptomatic.CASE SUMMARY In this case,we report a blunt pericardial injury undetected on preoperative transthoracic echocardiography and chest computed tomography.We misjudged intermittent ECG changes and blood pressure fluctuations as minor symptoms resulting from cardiac contusion and did not provide intensive treatment.The pericardial injury was found incidentally during surgical stabilization of rib fractures and was successfully repaired.CONCLUSION Post-traumatic blunt pericardial ruptures should be considered in patients with blunt chest trauma showing abnormal vital signs and ECG changes.

Question-Answering Pair Matching Based on Question Classification and Ensemble Sentence Embedding

Question-answering(QA)models find answers to a given question.The necessity of automatically finding answers is increasing because it is very important and challenging from the large-scale QA data sets.In this paper,we deal with the QA pair matching approach in QA models,which finds the most relevant question and its recommended answer for a given question.Existing studies for the approach performed on the entire dataset or datasets within a category that the question writer manually specifies.In contrast,we aim to automatically find the category to which the question belongs by employing the text classification model and to find the answer corresponding to the question within the category.Due to the text classification model,we can effectively reduce the search space for finding the answers to a given question.Therefore,the proposed model improves the accuracy of the QA matching model and significantly reduces the model inference time.Furthermore,to improve the performance of finding similar sentences in each category,we present an ensemble embedding model for sentences,improving the performance compared to the individual embedding models.Using real-world QA data sets,we evaluate the performance of the proposed QA matching model.As a result,the accuracy of our final ensemble embedding model based on the text classification model is 81.18%,which outperforms the existing models by 9.81%∼14.16%point.Moreover,in terms of the model inference speed,our model is faster than the existing models by 2.61∼5.07 times due to the effective reduction of search spaces by the text classification model.

We demonstrate a new type of multifocal and extended depth of focus(EDOF)intraocular lenses(IOLs)embeddingμm-thin geometric phase(GP)lens layers.As an emerging approach for lens phase design,the GP modulated IOLs outperform conventional diffractive IOLs in multifocality while completely avoiding the clinically undesirable demand for additional surface patterns to standard monofocal IOL designs.The number of foci and light splitting ratio of the GP IOLs are adjusted by changing the number of stacked GP layers and the thickness of each layer.Bifocal and trifocal GP IOLs are fabricated by radial alignment of anisotropic orientation in UV-curable liquid crystal polymers.After characterizing the defocus image and modulation transfer function of the GP IOLs,it is expected that GP IOLs will alleviate the most common problems associated with multifocal and EDOF IOLs,blurred vision and photic phenomena caused by light scattering and posterior capsule opacification.

The fourth industrial revolution indispensably brings explosive data processing and storage;thus,a new computing paradigm based on artificial intelligence-enabling device structure is urgently required.Memristors have received considerable attention in this regard because of their ability to process and store data at the same location.However,fundamental problems with abrupt switching characteristics limit their practical application.To address this problem,we utilized the concept of metaplasticity inspired by biosystems and observed gradual switching in the peptide-based memristor at high proton conductivity.An unexpectedly high slope value>1.7 in the logI–V curve at low voltage(≤400 mV)was considered the main origin,and it might arise from the modulatory response of proton ions on the threshold of Ag ion migration in the peptide film.With the obtained gradual switching property at high proton conductivity,the device showed significantly increased accuracy of image recognition(~82.5%).We believe that such a demonstration not only contributes to the practical application of neuromorphic devices but also expands the bioinspired functional synthetic platform.

Humidity-induced synaptic plasticity of ZnO artificial synapses using peptide insulator for neuromorphic computing

Neuromorphic devices inspired by the human brain have attracted significant attention because of their excellent ability for cognitive and parallel computing.This study presents ZnO-based artificial synapses with peptide insulators for the electrical emulation of biological synapses.We demonstrated the dynamic responses of the device under various environmental conditions.The proton-conducting property of the tyrosine-rich peptide enables time-dependent responses under ambient conditions such that various aspects of synaptic behaviors are emulated by the devices.The transition from short-term memory to longterm memory is achieved via electrochemical doping of ZnO by protons.Furthermore,we demonstrate an image classification simulation using a multi-layer perceptron model to evaluate the potential of the device for use in neuromorphic computing.The neural network based on our device achieved a recognition accuracy of 87.47% for the MNIST handwritten digit images.This work proposes a novel device platform inspired by biosystems for brain-mimetic hardware systems.

Supersonically sprayed self-aligne d rGO nanosheets and ZnO/ZnMn_(2)O_(4)nanowires for high-energy and high-power-density supercapacitors

Core-shell-type bimetallic oxide and carbon composites comprising zinc oxide(ZnO)nanospheres and zinc manganese oxide(ZnMn_(2)O_(4))nanowires were produced by a hydrothermal method,and supersoni-cally sprayed together with reduced graphene oxide(rGO)nanosheets onto Ni foil to fabricate flexible su-percapacitors.The supersonic impact facilitated the exfoliation of the rGO nanosheets,thereby increasing the surface area and adhesion of the composite particles to the substrate.The rGO nanosheets were vertically aligned during the supersonic impact and formed localized zones,enabling optimal accommodation of the ZnO/ZnMn_(2)O_(4)particles.This localization,with the addition of rGO,reduced the agglomeration of ZnO/ZnMn_(2)O_(4)particles.The molar concentration of MnSO_(4)used in the synthesis of ZnO/ZnMn_(2)O_(4)was varied from 0.05 to 0.15 mol/L to determine the optimal MnSO_(4)concentration that would result in the highest energy storage capacitance.The unique nanostructure of ZnO/ZnMn_(2)O_(4)and the self-alignment of rGO sheets facilitated a favorable environment for high energy storage capability with a specific capaci-tance of 276.3 mF·cm^(−2)at a current density of 0.5 mA·cm^(−2)and an energy density of 98.2μWh·cm^(−2)at a power density of 1600μW·cm^(−2).The width of the potential window was increased to 1.2 V,imply-ing a significant increase in the energy storage capability of the supercapacitor.Capacitance retention of 88%was achieved after 10,000 charge/discharge cycles for the supercapacitor fabricated using an optimal MnSO_(4)concentration(0.10 mol/L)during the composite synthesis.

BACKGROUND Functional vocal cord disorders can be a differential diagnosis for postoperative upper airway obstruction requiring urgent intervention.However,this may be unfamiliar to anesthesiologists who would favor inappropriate airway intervention and increased morbidity.CASE SUMMARY A 61-year-old woman underwent cervical laminectomy,followed by laparoscopic cholecystectomy 10 mo later.Despite adequate reversal of neuromuscular blockade,the patient experienced repetitive respiratory difficulty with inspiratory stridor after extubation.After the second operation,the patient was diagnosed with paradoxical vocal fold motion(PVFM)by an otolaryngologist based on the clinical features and fiberoptic bronchoscopy results,and the patient was successfully treated.CONCLUSION PVFM should be considered a differential diagnosis if a patient presents with stridor after general anesthesia.

Inhibition of CD82 improves colitis by increasing NLRP3 deubiquitination by BRCC3

CD82 is a transmembrane protein that is involved in cancer suppression and activates immune cells;however, information on the NLRP3 inflammasome is limited. Herein, we show that although CD82 suppressed the activation of the NLRP3 inflammasome in vivo and in vitro, CD82 deficiency decreased the severity of colitis in mice. Furthermore, two binding partners of CD82, NLRP3 and BRCC3, were identified. CD82 binding to these partners increased the degradation of NLRP3 by blocking BRCC3-dependent K63-specific deubiquitination. Previous studies have shown that CD82-specific bacteria in the colon microbiota called Bacteroides vulgatus (B. vulgatus) regulated the expression of CD82 and promoted the activation of the NLRP3 inflammasome. Accordingly, we observed that B. vulgatus administration increased mouse survival by mediating CD82 expression and activating NLRP3 in mice with colitis. Overall, this study showed that CD82 suppression reduced the pathogenesis of colitis by elevating the activation of the NLRP3 inflammasome through BRCC3-dependent K63 deubiquitination. Based on our findings, we propose that B. vulgatus is a novel therapeutic candidate for colitis.
