Efficacy and feasibility of gemcitabine and carboplatin as first-line chemotherapy in elderly patients with advanced non-small cell lung cancer

Background Although platinum-based chemotherapy is a standard first-line treatment in advanced non-small cell lung cancer （NSCLC）, further research for the safety and efficacy of combination chemotherapy in elderly patients has been required. The purpose of this study was to evaluate the efficacy and safety of gemcitabine and carboplatin as first-line treatment in elderly patients with advanced NSCLC and to evaluate the prognostic factors. Methods Eligibility included：（1） age of 70 years or more, （2） histologically confirmed NSCLC, （3） chemotherapy-na＇l＇ve, （4） advanced disease with stage IIIB or IV, （5） Eastern Cooperative Oncology Group （ECOG） performance status （PS） 0-2, （6） adequate organ function. Patients received intravenous carboplatin （area under curve （AUC）=5） on day 1 and gemcitabine （1000 mg/m2） on days 1 and 8, every 3 weeks. Results The medical records of forty patients were reviewed retrospectively. Median age was 73.9 years （range, 70- 84.6）, and there were 27 men （67.5%）. Thirty-seven patients （92.5%） had ECOG PS 0-1. Adenocarcinoma was found in 57.5%. Median cycles were administrated with 4.5 per person （range： 1-6）. Best responses were partial response in 22 （55.0%） patients and stable disease （SD）in 13 （32.5%）. The median progression free survival （PFS） and overall survival （OS） were 5.9 months （95% CI： 4.5-7.3 months） and 9.6 months （95% CI： 8.2-11.0 months）, respectively. Grade 4 hematologic toxicities for neutropenia （7.5%）, thrombocytopenia （7.5%） and anemia （5.0%） were observed. Histology was significant prognostic factor for PFS （P=0.024）. Conclusion Gemcitabine and carboplatin combination chemotherapy is an effective and manageable treatment option in elderly advanced NSCLC patients with good performance status.

Incomplete High Radial Nerve Palsy Due to Schwannoma of Radial Nerve in the Upper Arm—A Case Report

A schwannoma is a benign soft tissue tumor composed of Schwann cells. Clinically, the tumor is slow-growing and usually asymptomatic, rarely developing neurological symptoms. We present a case of a 44-year-old female who was diagnosed with an incomplete high radial nerve palsy, showing a wrist drop and extension of fingers due to schwannoma on the radial nerve of the upper arm. The patient showed excellent result after excision of the schwannoma and neurorrhaphy of the radial nerve.

Endoscopic fibrin glue injection for closure of pancreatocutaneous fistula following transgastric endoscopic necrosectomy

Transgastric endoscopic necrosectomy has been recently introduced as the effective and alternative management of infected pancreatic necrosis and pancreatic abscess. However,up to 40% of patients who undergo endoscopic necrosectomy may need an additional percutaneous approach for subsequent peripancreatic fluid collection or non-resolution of pancreatic necrosis. This percutaneous approach may lead to persistent pancreatocutaneous fistula,which remains a serious problem and usually requires prolonged hospitalization,or even open-abdominal surgery. We describe the first case of pancreatocutaneous fistula and concomitant abdominal wall defect following transgastric endoscopic necrosectomy and percutaneous drainage,which were endoscopically closed with fibrin glue injection via the necrotic cavity.

Generally the gold investment material consists of cristobalite,quartz and *** physical property of gold investment materials depends on its thermal expansion coefficients,compressive strength,and particles size *** the thermal expansion coefficient of cristobalite and quartz are 2.6×10^-6/℃and 2.32×10^-6/℃respectively,the composition ratio of each components influence the thermal and physical properties of gold investment *** the clinical applications,it is necessary to improve the properties of gold investment materials such as homogeneous size distribution and thermal expansion *** the present study,effect of inorganic fillers such as cristobalite and quartz on gold alloy investment was investigated to improve the properties of *** compressive strength and thermal expansion coefficients of the specimens were *** results showed that cristobalite and quartz were homogeneously distributed by milling. The optimum compressive strength was obtained at the ratio of 42：22 cristobalite and quartz,respectively.

BACKGROUND Due to a shortage of donor kidneys, many centers have utilized graft kidneys from brain-dead donors with expanded criteria. Kidney transplantation(KT)from donors on extracorporeal membrane oxygenation(ECMO) has been identified as a successful way of expanding donor pools. However, there are currently no guidelines or recommendations that guarantee successful KT from donors undergoing ECMO treatment. Therefore, acceptance of appropriate allografts from those donors is solely based on clinician *** SUMMARY We report a case of successful KT from a brain-dead donor supported by ECMO for the longest duration to date. A 69-year-old male received a KT from a 63-yearold brain-dead donor who had been on therapeutic ECMO treatment for the previous three weeks. The recipient experienced slow recovery of graft function after surgery but was discharged home on post-operative day 17 free from hemodialysis. Allograft function gradually improved thereafter and was comparatively acceptable up to the 12 mo follow-up, with serum creatinine level of 1.67 mg/d *** This case suggests that donation even after long-term ECMO treatment could provide successful KT to suitable candidates.

BACKGROUND In most cases,it is not difficult to differentiate common bile duct(CBD)stone from Ascaris infection because they are different disease entities and have different imaging *** two diseases usually demonstrate unique characteristic findings on computed tomography or magnetic resonance ***,we report a rare case from our experience in which a CBD stone mimicked and was misdiagnosed as *** SUMMARY A 72-year-old male presented with elevated serum liver *** tomography showed a hyper-attenuated,elongated lesion in the CBD lumen and associated biliary *** resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography revealed a linear filling defect in the bile ***,elongated echogenic material with a central hypoechogenic area was seen on endoscopic *** the imaging findings caused us to suspect infection with the nematode Ascaris,the lesion was revealed to be a dark-brown-colored CBD stone through endoscopic *** We report a rare case of a CBD stone that mimicked *** also review the literature for side-by-side comparisons of the imaging features of CBD stones and ascariasis.
