The gut microbiome is recognized as a key modulator of sepsis ***,the contribution of the gut mycobiome to sepsis development is still not fully ***,we demonstrated that the level of Candida albicans was markedly decreased in patients with bacterial sepsis,and the supernatant of Candida albicans culture significantly decreased the bacterial load and improved sepsis symptoms in both cecum ligation and puncture(CLP)-challenged mice and Escherichia coli-challenged *** metabolomics and the genetic engineering of fungi revealed that Candida albicans-derived phenylpyruvate(PPA)enhanced the bactericidal activity of macrophages and reduced organ damage during ***,PPA directly binds to sirtuin 2(SIRT2)and increases reactive oxygen species(ROS)production for eventual bacterial ***,PPA enhanced the bacterial clearance capacity of macrophages in sepsis patients and was inversely correlated with the severity of sepsis in *** findings highlight the crucial contribution of commensal fungi to bacterial disease modulation and expand our understanding of the host-mycobiome interaction during sepsis development.

AIM To explore the applicability of the Asia-Pacific Association for the Study of the Liver(APASL) and European Association for the Study of the Liver(EASL) guidelines for acute-on-chronic liver failure(ACLF) in profiling patients and determining the *** Patients admitted to a tertiary hospital in Singapore with acute decompensation of liver disease from January 2004to July 2014 are screened for ACLF according to the APASL and EASL criteria. The patients' data(including basic demographics, information about existing chronic liver disease, information about the acute decompensation, relevant laboratory values during admission, treatment, and outcome) are retrospectively analyzed to determine the background, precipitating factors and *** A total of 458 liver patients is analyzed, and 78 patients with ACLF are identified. Sixty-three patients(80.8%) meet the APASL criteria, 64 patients(82.1%) meet the EASL criteria, and 49 patients(62.8%) fulfilled both criteria. The most common causes of acute liver injury are bacterial infections(59.0%), hepatitis B flare(29.5%), and variceal bleeding(24.4%). The common aetiologies of the underlying chronic disease included hepatitis B(43.6%), alcoholic(20.5%) and cryptogenic(11.5%) liver disease. The overall mortality rate is 61.5%. Increased age, the number of organ failures(as per CLIF-SOFA score), peak creatinine, INR, and amylase levels are associated with increased mortality or the need for liver transplantation. 14.3% of patients undergo liver transplantation with a 100% 1-year survival rate. CONCLUSION Both APASL and EASL criteria have identified ACLF patients with high three-month mortality, but those who fulfill APASL criteria alone have a better survival.

This work was aimed at identifying genes that show altered expression profiles in response to changes in ploidy and/or reproductive mode (from sexual to apomictic) in the African grass Eragrostis curvula. A differential display analysis was performed on leaf and flower transcriptomes from a series of genetically related euploid plants, including tetraploid apomictic, diploid sexual, and tetraploid sexual plants. More than 100 primer combinations were used to generate 11,864 total markers, yielding 1293 differential bands. Of these bands, 11.84% to 6.74% were related to ploidy and 0.71% to 2.17% to the reproductive mode, depending on the tissue. A small percentage of bands showed similar expressions between the tetraploid apomictic and the diploid sexual plants. Expression-based similarity dendrograms were constructed. Our data suggested that ploidy is more decisive than tissue type in defining the transcriptome structure. Out of 102 fragments sequenced, 50 showed strong homology to known genes. The differentially expressed genes were mapped in silico onto maize chromosomes. Several candidates mapped within the linkage group syntenic to the Tripsacum dactyloides diplospory-governing region. The evidence indicates that expression of genes located around the diplospory-associated region may be strongly influenced by ploidy and may be silenced in the apomictic genotype. These findings are discussed in the context of diplospory molecular control and its connection with ploidy.

[目的]探索自拟方甘草养阴汤(GNY)对咪喹莫特(IMQ)诱导的银屑病样皮肤损伤小鼠模型的疗效。[方法]将小鼠分为健康对照组、模型组(IMQ诱导银屑病小鼠)和治疗组(给予银屑病小鼠GNY,浓度为2.5、5 g/kg);共4组(动物数=8/组)。采用银屑病病灶面积和严重程度指数(PASI)、苏木-精伊红染色和Baker评分用于评估疾病的严重程度。采用免疫组织化学或免疫荧光染色法对真皮的Gr-1,CD11c,RORγt和TGF-β阳性的炎性细胞进行计数。采用流式细胞仪分析脾脏细胞中的CD4+IL-17+Th17细胞和CD4+CD25+Foxp3+Treg细胞比例。使用qPCR半定量IL-6、IL-10、IL-17A、IL-22、IL-23、TNF-α、TGF-β和AMPK-1的相对mRNA表达。[结果]与模型组相比,GNY治疗的小鼠在两种浓度下均能够改善了病灶的形态学特征,包括PASI评分和上皮角质细胞增生。较高浓度的GNY可显着降低银屑病小鼠模型真皮中的Gr-1,CD11c,RORγt和TGF-β阳性细胞比例;下调脾脏Th17细胞比例,增加Treg细胞的表达。此外,IMQ模型组小鼠的真皮内IL-6、IL-10、IL-17A、IL-22、IL-23、TNF-α、TGF-β和AMPKɑ1的相对mRNA表达升高,在以5 g/kg浓度的GNY治疗的模型小鼠中表达明显降低。[结论]研究表明GNY可改善IMQ诱导的小鼠银屑病样皮肤损伤表现、减轻炎症反应;为临床治疗银屑病提供依据。

Objectives: To assess the margins required for excision of lentigo maligna (LM) and lentigo maligna melanoma (LMM) by the technique of mapped serial excision (MSE), and to assess the efficacy of MSE. Design: An interventional, prospective, noncontrolled case series. Setting: Tertiary referral, dermatologic surgery unit. Patients: Consecutive patients with head and neck LM or LMM who underwent MSE between March 1, 1993, and October 31, 2002. Intervention: The MSE of LM or LMM. Main Outcome Measures: The number of 5-mm levels for excision of LM and LMM and recurrence. Results: One hundred sixty-one LMs or LMMs in 155 patients were treated. Thirty percent (37 of 125) of LMs required more than 5-mm margins. For LMMs less than 1 mm in Breslow thickness, 12%(4/32) required more than 10-mm margins. For primary tumors, 20%of LMs (18 of 91) required more than 5-mm margins, while 10%of LMMs less than 1 mm in Breslow thickness (2 of 21) requiredmore than a 10-mm margin. For recurrent tumors, 56%of LMs (19/34) required more than a 5-mm margin. Mean follow-up of 38 months (range, 5-100 months) showed 4 recurrences (2%) after MSE. The extrapolated recurrence at 5 years was 5.0%Conclusions: The current recommendations of 5-mm margins for LM and 10-mm margins for LMM less than 1 mm in Breslow thickness are often insufficient. Our results demonstrate the importance of margin-controlled excision, particularly in recurrent lesions. The use of MSE offers a high cure rate, in conjunction with tissue conservation.

Oxidative stress has been involved in neurodegenerative diseases. The growth hormone (GH) counteracts the levels of reactive oxygen species. Previously, we showed that the prolonged exposure to ozone causes oxidative stress in the hippocampus and memory deficits. In this work, we analyzed the effects of the growth hormone on the memory deficit generated by ozone exposure, growth hormone effects on the Insulin-like growth factor I (IGF-I), and the serinethreonine protein kinase (Akt) activation in the dentate gyrus. Our results show that GH prevents memory deficits in early stages of the neurodegenerative process.

To evaluate the rate of recurrence of horizontal laxity or lid malposition and suture- related complications associated with the use of an absorbable suture in the lateral tarsal strip procedure. Prospective, interventional, consecutive case series. A prospective study. setting: Institutional. patient population: One hundred patients with horizontal lid laxity associated with ectropion or entropion. Patients with paralytic ectropion or ocular prostheses were excluded. intervention: One hundred and five lateral tarsal strip procedures were performed using a 6- 0 polyglactin suture. These were combined with full- thickness skin grafts (34), internal (55) or external (23) retractor plications, and inverting sutures (4) as required to correct the lid malposition. main outcome measures: Recurrence of horizontal laxity or lid malposition and suture related complications were the main outcome measures. Suture related wound infection was diagnosed clinically by the presence of pus associated with wound breakdown. One patient failed to reach 3- month follow- up and was excluded. There was one recurrence of horizontal laxity after a mean follow- up period of 9.1 months. Four patients developed lateral canthal wound infections, compared with none at the other operative sites. The 95% confidence intervals for infections in the tarsal strip procedure were 1.1% to 9.6% , which were significantly greater than zero (P < .0001), the infection rate for the other procedures. A 6- 0 absorbable suture may be used for the lateral tarsal strip and does not lead to a recurrence of horizontal laxity. It does not appear to reduce the wound infection rate associated with this procedure.

Chronic exposure to low doses of ozone similar to a day of high pollution in Mexico City causes a state of oxidative stress. This produces a progressive neurodegeneration in hippocampus of rats exposed to the gas. The aim of this study was to analyze the effect of chronic exposure on the changes in the blood-brain barrier in rats exposed to low doses of ozone. Method: each group received one of the following treatments, control group received air without ozone, and groups 2, 3, 4, 5, and 6 received ozone doses of 0.25 ppm for 4 h daily during 7, 15, 30, 60 and 90 days respectively. Each group was processed to inmunohistochemical technique against of the following antibody: blood-brain barrier, guanylyl cyclase, Iba-1, GFAP, NFκ-B, TNF-α. The results show that there is a correlation between the time exposure of ozone and the progressive damage, on the blood-brain barrier rupture, finally causing edema of endothelial cell, increase in guanylyl cyclase type 1, thickening of the processes and astrocytes foot, and an increase in the expression of factors NFκ-B and TNF-α at 30 and 60 days of exposure to this gas. All the above indicates that the chronic state of oxidative stress causes a neurodegeneration process, accompanied by disruption of the blood-brain barrier likely to occur in the Alzheimer’s disease.
