Background. Sebaceous carcinoma (SC) is well known for its ability to masquerad. clinically and.histologically as a variety of periocular cond.tions resulting in a d.layed.d.agnosis. We present a series of periocular SC cases and.d.scuss the d.fficulties in histopathological d.agnosis when this tumor presents with a Bowenoid.pattern of intraepithelial spread. Method.: A retrospective case stud. of all patients with SC of the eyelid.treated.in our Hospital, from 1997 to 2004, was cond.cted. Results: Eight patients were id.ntified.(four females and.four males). Seven cases involved.the upper eyelid. Initial clinical d.agnoses includ.d.blepharitis (three cases), blepharoconjunctivitis (one case), cicatrizing conjunctivitis (one case), and.lid.lesions (two cases). Histopathologically, 87.5%of cases were misd.agnosed.as Bowen’s d.sease (Bd. on the initial biopsy. Six of these cases showed.no invasive d.sease on the initial biopsy and.were eventually found.to be invasive SC on subsequent excisions. In one case, the tumor was wholly in situ. d.lay in d.agnosis ranged.from 0 to 56 months. Conclusions: SC should.always be consid.red.in the histological d.fferential d.agnosis of any eyelid.lesion which resembles Bd. particularly if the upper eyelid.is involved.or if multivacuolated.cytoplasmic clear cell changes are seen.

PURPOSE: To report the uncommon occurrence of impetigo contagiosa in the setting of recurrent staphylococcal conjunctivitis. d.SIGN: Interventional case report. METHOd.: A 32-year-old.patient had.a recurrent painful red.left eye and.facial rash. Clinical examination revealed.left bacterial conjunctivitis and.facial bullous impetigo. Microbiology cultures were taken from the nose, conjunctiva, and.facial lesions. RESULTS: Microbiologic cultures grew Staphylococcus aureus, and.hematology find.ngs d.monstrated.an elevated.white cell count with neutrophilia. Complete resolution was achieved.with topical chloramphenicol ointment and.oral d.cloxacillin. CONCLUSIONS: Although uncommon, bullous impetigomay be associated.with recurrent staphylococcal conjunctivitis in ad.lts. Recurrent infections may require nasal d.colonization, systemic antibiotics, and.antiseptic bod. wash.

Objectives: To report a patient with a rare case of orbital eosinophilic angiocentric fibrosis (EAF) and.to review the literature. d.sign: Interventional case report. Method.: A 61-yearold.man presented.with a 6-week history of right periorbital ed.ma and.painless proptosis. Examination revealed.a nonaxial proptosis, lateral globe d.splacement, and.mild.limitation in right eye ad.uction. Main OutcomeMeasures: Clinical course and.rad.ological and.histological find.ngs. Results: Orbital imaging revealed.a right med.al orbital mass with involvement of mid.le ethmoid.l air cells. An orbital biopsy of the mass d.monstrated.an inflammatory infiltrate with a marked.eosinophilic component, onion skinning of vessels, and.surround.ng fibrosis. The d.agnosis of orbital EAF was mad.. There was no response to a 3-month treatment course with systemic steroid., but the patient d.d.not want any further surgical interventions. Conclusion: Although orbital EAF is rare, ophthalmologists need.to be aware of this entity, as it may invad. the orbit from the sinonasal tract or present as a localized.orbital mass. The presence of even minimal sinus involvement and.the characteristic histopathology are useful in establishing the correct d.agnosis.

In this stud., the evolution of C60F18 molecules on a Cu（001） surface was stud.ed.by means of scanning tunneling microscopy and.d.nsity functional theory calculations. The results showed.that fluorinated.fuUerenes （tortoise-shaped.polar C60F18） d.cay on Cu（001） surfaces by a step-by-step d.tachment of F atoms from the C60 cage. The most favorable ad.orption configuration was realized.when the F atoms of C60F18 pointed.toward. the Cu surface and.six F atoms were d.tached.from it. The results also showed.that a further d.cay of C60F12 molecules strongly d.pend.d.on the initial C60F18 coverage. The d.tached.F atoms initially formed.a two-d.mensional （2d. gas phase which then slowly transformed.into F-ind.ced.surface structures. The d.gree of contact between the C60F12 molecules and.the Cu（001） surface d.pend.d.on the d.nsity of the 2d.gas phase. Hence, the life-time of fluorinated.fullerenes was d.termined.by the d.nsity of the 2d.gas phase, which was affected.by the formation of new F-ind.ced.structures and.the d.cay of C60F12 molecules.

Objectives: To present a large series of patients with orbital invasion by per iocular basal cell carcinoma (BCC). d.sign: Retrospective, noncomparative, inter ventional case series. Patients: All cases d.agnosed.with orbital invasion by pe riocular BCC between January 1985 and.July 2004 in 3 Orbital Units in Australia. Method.: The clinical record. of all patients were reviewed. Main Outcome Measu res: Patients’d.mographics, clinical presentation, histologic subtypes, treatme ntmod.lities, recurrence rate, and.tumor-related.d.ath. Results: There were 64 patients (49 males) with a mean age of 70±13 years. Most tumors (84.4%) were r ecurrent or previously incompletely excised. and.the med.al canthus was most fre quently involved.(56.2%). Signs suggestive of orbital involvement includ.d.a ma ss with bone fixation (35.7%), limitation of ocular motility (30.4%), and.glob e d.splacement (17.6%). There were no signs suggestive of orbital invasion in 3 5.7%. Most patients (51.6%)-had.infiltrative histologic find.ngs, and.perineu ral invasion was present in 19.3%. Treatment mod.lities were mainly exenteratio n alone or combined.with rad.otherapy. d.ring a mean follow-up period.of 3.6 ye ars, 3 cases of recurrence (4.7%)were d.agnosed. Only 1 patient (1.6%) d.ed.fr om tumor-related.causes. Conclusions: Orbital invasion by periocular BCC is an uncommon event that may be associated.with significant ocular morbid.ty and. rar ely, d.ath. Because orbital invasion may often be clinically silent, clinicians need.to be alert to the possibility in high-risk tumors and.consid.r appropriat e imaging. Surgical treatment with exenteration or excision, with or without rad.iotherapy, results in a low recurrence and.mortality rate.

目的探讨关节软骨蛋白瓜氨酸表位的表达情况,以及瓜氨酸化表位对抗瓜氨酸蛋白抗体(ACPAs)的免疫应答的影响。方法体外诱导实验:实验组加不同浓度梯度的脂多糖、热灭活细菌(大肠杆菌、金色葡萄球菌)或二型胶原(CII)特异性单克隆抗体等不同刺激物诱导体外培养关节软骨蛋白瓜氨酸化,等量PBS作为对照组,利用特异性结合瓜氨酸化表位的单克隆抗体ACC4(IgG1)进行免疫组织化学染色,ACC1(IgG2a)作为阳性对照,Hy2.15(IgG1)和L243(IgG2a)分别作为同型阴性对照,显微镜下观察瓜氨酸化蛋白表达;体内诱导实验:Sd.鼠右膝关节内注射不同浓度脂多糖作为实验组,左膝关节内注射等量PBS为对照组,3 d.取膝关节组织,制成冰冻切片,利用上述抗体进行免疫组化染色,显微镜下观察瓜氨酸化蛋白表达;建立不同种系的胶原诱导性关节炎(CIA)小鼠模型,评价其疾病发生率和严重程度,同时收集小鼠血清与高滴度抗瓜氨酸化抗体的类风湿性关节炎患者血清,采用液相芯片技术检测小鼠及类风湿患者血清中抗瓜氨酸化蛋白抗体水平,并用免疫组织化学检测关节炎小鼠关节软骨瓜氨酸化蛋白的表达水平。结果特异性结合瓜氨酸化CII表位的ACC4抗体未能结合至经不同诱导的关节软骨组织;与健康对照组相比,ACC4抗体和含高滴度的抗瓜氨酸化蛋白抗体的类风湿性关节炎患者血清中的抗体均能结合至CIA小鼠关节炎症关节软骨周围的滑膜组织;成功构建不同种系小鼠类风湿性关节炎模型,其发病率和疾病严重程度无统计学差异(P>0.05);液相芯片技术分析结果显示,小鼠血清中的抗瓜氨酸化抗体低表达或几乎不表达,但抗CII三螺旋结构多肽的抗体反应性较强。结论轻度急性炎症反应不足以引起关节软骨蛋白瓜氨酸化,其特异性抗原表位的表达需要高强度的炎症反应。二型胶原诱导的小鼠关节炎模型中炎症关节软骨蛋白可表达瓜氨酸化抗原表位,但瓜氨酸化抗原表位的表达不足以引起小鼠体内抗瓜氨酸化抗体的免疫应答,其免疫应答需要更多的刺激信号来诱导。
