Development of a Measurement Scale to Evaluate Self-Care Stability for Patients Diagnosed with Type 2 Diabetes at an Older Age

Objectives: For the medical care of older adults diagnosed with type 2 diabetes later in life, it is necessary to provide medical staff support considering their life stage and to ensure that they live a stable life without fatigue. However, there is no measure for evaluating whether older diabetic patients have a stable life at the time of diagnosis. This study aimed to develop a scale to evaluate self-care stability for diabetic patients and to verify its reliability and validity. Methods: A cross-sectional study was conducted at four hospitals in Japan. A draft scale comprising 39 items was developed to identify patients’ stability of diabetes self-care in life. A total of 69 patients diagnosed with diabetes for the first time at age 60 or older responded effectively to questionnaires. Results: As a result of item analysis and factor analysis, seven factors comprising 20 items were extracted. The cumulative contribution ratio before rotation was 70.577%. Cronbach’s α was 0.700 overall. The total score on this scale and the sum of the revised Philadelphia Geriatric Center Morale Scale scores were significantly positively correlated (r = 0.379). On comparing the total scale score based on glycated hemoglobin level, the scale score of the stable group was found to be higher. Therefore, this scale was found to be reliable and valid. Conclusions: This scale may have utility in the measurement of self-care stability in patients diagnosed with type 2 diabetes later in life.

Diabetic neuropathy is one of the most serious complications of diabetes, and its increase shows no sign of stopping. Furthermore, current clinical treatments do not yet approach the best effectiveness. Thus, the development of better strategies for treating diabetic neuropathy is an urgent matter. In this review, we first discuss the advantages and disadvantages of some major mouse models of diabetic neuropathy and then address the targets for mechanism-based treatment that have been studied. We also introduce our studies on each part. Using stem cells as a source of neurotrophic factors to target extrinsic factors of diabetic neuropathy, we found that they present a promising treatment.

Sm-based perovskite-type oxide (SmMeO3: Me = Cr, Mn, Fe, Co) thin-films could be synthesized by a wet-chemical method using an acetylacetone—Poly(Vinyl Pyrrolidone) (PVP) polymeric precursor method at 750℃. The perovskite-type oxide thin-films were tried to apply an acetylene gas sensor based on AC impedance spectroscopy. Among the oxides tested, SmFeO3 thin-film sensor showed good sensor responses in which the AC impedance at 20 kHz was depending on acetylene gas concentration between 2 ppm and 80 ppm at 400℃.

We examined the effect of androgens on bladder blood flow （BBF）, bladder function and histological changes in castrated male rats. Male Wistar rats were classified into unoperated group （control group）, groups castrated at the age of 8weeks （group 8wPC） and groups castrated at the age of 4weeks （group 4wPC）. Each rat was used at the age of 20weeks. BBF was measured using fluorescent microspheres. Bladder cystometry was performed without anesthesia or restraint; the bladder was first irrigated with saline and then with 0.25% acetic acid （AA） solution. Maximum voiding pressure and voiding interval were measured. The bladder and lilac artery were histologically examined for differences in smooth muscle and quantity of collagen fiber to analyze the effect of castration on the smooth muscle content. No differences were noted in BBF following castration. The voiding intervals for all groups were shortened （P 〈 0.001） following AA irrigation. No significant difference was noted in the maximum voiding pressure. Histological changes were observed in bladder and lilac artery. Smooth muscle/collagen ratio at the bladder was lower in groups 8wPC and 4wPC compared to the control group （P 〈 0.01）, while that at the lilac artery was decreased in group 4wPC compared to the control group （P〈 0.001）. In conclusion, our findings indicate that castration does not alter BBF, but leads to histological changes in the bladder as well as its associated blood vessels.

Low-grade myofibroblastic sarcoma （LGMS） is a rare, malignant tumor with myofibroblastic differentiation. Despite it being classified as a distinct entity by the World Health Organization, a few cases were reported in the oral and maxillofacial region. Here, a LGMS developed on the palate of a 73-year-old man who presented with a 1-cm tumor on the posterior border of the palate. Based on the histological and immunohistochemical features, a diagnosis of LGMS was established. The tumor was resected, and no recurrence was observed over 2 years. Although the tongue is the most preferred site for LGMS, it may occur in any region of the oral cavity.

The dependence of transformer performance on the material properties was investigated using two laboratory-processed 0.23 mm thick grain-oriented electrical steels domain-refined with elec-trolytically etched grooves having different magnetic properties. The iron loss at 1.7 T, 50 Hz and the flux density at 800 A/m of material A were 0.73 W/kg and 1.89 T, respectively; and those of material B, 0.83 W/kg and 1.88 T. Model stacked and wound transformer core experiments using the tested materials exhibited performance well reflecting the material characteristics. In a three-phase stacked core with step-lap joints excited to 1.7 T, 50 Hz, the core loss, the exciting current and the noise level were 0.86 W/kg, 0.74 A and 52 dB, respectively, with material A; and 0.97 W/kg, 1.0 A and 54 dB with material B. The building factors for the core losses of the two materials were almost the same in both core configurations. The effect of higher harmonics on transformer performance was also investigated.
