Factors Associated with Need for Drainage of Pleural Effusion after Diaphragm Surgery

Background: Diaphragm surgery is common with advanced ovarian malignancies. The purpose of this study is to determine associated factors with the need for drainage of pleural effusion after diaphragm surgery. Methods and Materials: A retrospective chart review was undertaken in all women undergoing debulking surgery for stage IIIc/IV ovarian cancer from 2007-2009. Results: One hundred and eight patients were found to be eligible for the study, but 73 were the primary focus of this paper due to having undergone debulking surgery, including diaphragmatic surgery, from 2007-2009. All 73 had ablation with the argon beam coagulator, 7 had a full thickness resection, and 7 had extensive peritoneal peel. Five patients had preoperative effusions. Only 7 patients required chest drainage postoperatively. Pre-operative ascites correlated closely with postoperative effusion (p-value = 0.031) but not with drainage (p-value = 0.068). The mean age of patients requiring drainage was significantly older (73 years) than that of patients who did not require drainage (60 years) (p-value = 0.002). Conclusion: Older patients undergoing diaphragm surgery are more likely to require chest tube or thoracentesis due to concurrent symptoms. Pre-operative ascites correlates closely with the development of postoperative effusion after diaphragm surgery.

A comparison of duloxetine hydrochloride with fluoxetine hydrochloride in major depressive disorders: a pilot study

To compare remission rate, relapse rate and tolerability of duloxetine, a dual reuptake inhibitor of 5-hydroxy serotonin (5-HT) and norepinephrine (NE), versus fluoxetine, a reuptake inhibitor of 5-HT during follow up period of 16 weeks in major depressive disorder (MDD) a open label comparative trial was conducted. Trial was comprising of 60 patients, diagnosed with MDD, were allocated to fluoxetine group (n*-30, 20-60mg od) or duloxetine group (n*-30, 40-60mg od) for 16 weeks. The end points were remission and relapse assessed by Hamilton Rating scale for Depression-24 items (HAMD-24). In results the mean fall in HAMD-24 scores between groups was comparable till 4 weeks. Thereafter, at 8 weeks the mean fall in HAMD-24 score was significantly greater in duloxetine group (p value < 0.05). At 16 weeks the mean fall was highly significant (p value < 0.01) in duloxetine group. Thirty percent patients in duloxetine group achieved remission in comparison to none in fluoxetine group. None of the patient, in any group, reported relapse. Adverse effects were mild to moderate in *** conclusion duloxetine has a better pharmacological profile over fluoxetine in terms of efficacy and safety.

The bioactive triterpenoid 3-oxo-6-β-hydroxy-β-amyrin(1)has been isolated from multiple plant *** this study,chloroform fraction of Pistacia integerrima extract was processed for the isolation of the *** compound identity was confirmed by advanced spectroscopy technique.X-ray crystallography was applied for molecular structure *** addition,compound 1 was screen for its activity on reversal of MDR(multidrug resistance)mediated by P-gp(P-glycoprotein).This was accomplished by using rhodamine123 exclusion on multidrug-resistant human ABCB1 gene transfected mouse T-lymphoma cell *** revealed that MDR reversing effect was comparable to verapamil as positive control in *** of TPA-induced tumor promotion with 3-oxo-6β-hydroxy-β-amyrin led to reduction in the applied anti-tumor promotion *** chemo-preventive effect of 3-oxo-6β-hydroxy-β-amyrin was comparable to curcumin as positive control based on the reduction of immediate early tumor antigen *** docking by applying Autodock Vina 1 and i-GEMDOCK v 2.1 tools indicated that compound 1 gives good docking results,as determined by their fitness score and ***,results showed that compound 1 isolated from Pistacia integerrima precisely attached to a region where co-crystallized ligand for receptor previously *** findings may explain the use of Pistacia integerrima plant extracts as an anticancer agent in folk medicine.

Right vocal cord paralysis in our present case was diagnosed on clinical and radiological examination which is precipitated by an anomalous right aortic arch with diverticulum. This is a very uncommon vascular etiology of hoarseness and is extremely rare. Because of this rarity, the practicing otolaryngologist may miss this finding while evaluating a case of idiopathic right vocal cord paralysis. Thus, the authors feel that idiopathic or unexplained right vocal cord paralysis should be routinely investigated with a CT or MRI of neck and chest with or without contrast to avoid such shortcomings. There is only one such case of right vocal cord paralysis by right aortic which has been reported earlier in literature.

Subdural hematomas are often life-threatening when acute but chronic subdural hematomas, however, have better prognosis if properly managed. Chronic subdural hematomas are common in the elderly due to shrinkage of brain tissue, but in young patient mostly associated with head injury. It is seen also in young having various coagulopathies associated with blood disorders or drug-induced, but it is very rare. Propylthiouracil (PTU) is an oral medication that is used in treatment of hyperthyroidism approved by FDA in July 1947. This medication may rarely cause very serious blood disorders (such as a low number of red cells, white cells, and platelets), especially during the first few months of treatment. We are reporting a rare case of PTU-induced thrombocytopenia leading to chronic subdural haematoma, which presented with established papilledema and signs of raised ICP in a hyperthyroid female and she responded well to surgical management.

A method for allocating Walsh codes by group in a CDMA(Code Division Multiple Access)cellular system is *** proposed system provides a method for grouping,allocating,removing and detecting of the minimum traffic group to minimize the time for allocating a call or transmitted data to an idle Walsh code, thereby,improving the performance of the system and reducing the time required to set up the *** new concept of CGIWC has been presented to solve the calls or data allocating and removal from the Walsh Code. Preferably,these steps are performed by a BCS(Base station Call control Processor)at a CDMA base station. Moreover,a comparison with the previous work has been shown for the support of our related *** the end,the future direction in which the related work can be employed,are highlighted.

Ductal paucity and Warkany syndrome in a patient with congenital extrahepatic portocaval shunt

An eleven-year-old clinically dysmorphic and devel-opmentally retarded male child presenting with com-plaints of 5 episodes of recurrent cholestatic jaundice since 3 years of age was evaluated. Imaging revealed features consistent with congenital extrahepatic porto-caval shunt(Abernethy type 1b), multiple regenerative liver nodules and intrahepatic biliary radical dilatation. The presence of ductal paucity and trisomy 8 were con-firmed on liver biopsy and karyotyping. The explanation for unusual and previously unreported features in the present case has been proposed.
