The standard of care in the management of d-transposition of the great arteries(TGA)includes the arterial switch operation(AsO).1,2 Although some institutions have begun to publish the results of performing the AsO within the first few hours of life,most centers continue to"electively"delay AsO for several days to allow for a reduction in pulmonary vascular resistance and the stabilization of renal and liver function.1,2 The early initiation of prostaglandin E1(PGE)infusion to maintain ductal patency is a nearly universal component of therapy in order to maintain adequate arterial oxygen saturation and systemic oxygen delivery during the pre-operative period via intercirculatory mixing at the atrial septal and patent ductus arteriosus levels1,3.

Objective:To evaluate the combination therapy of pyronaridine tetraphosphate and diminazene aceturate against Babesia in vitro and in ***:Bioinformatic analysis was performed using atom pair *** in vitro combination test was performed against Babesia bovis and Theileria ***,the in vivo chemotherapeutic efficacy of pyronaridine tetraphosphate in combination with diminazene aceturate was investigated against the growth of Babesia microti in mice using a fluorescence inhibitory ***:Pyronaridine tetraphosphate and diminazene aceturate exhibited nearly similar molecular *** in vitro combination of pyronaridine tetraphosphate and diminazene aceturate was synergistic on Babesia bovis and additive on Theileria *** addition,5 mg/kg pyronaridine tetraphosphate combined with 10 mg/kg diminazene aceturate inhibited Babesia microti growth significantly compared with those observed after treatment with 25 mg/kg diminazene aceturate alone from day 6 post treatment to day 12 post *** combination therapy also normalized the hematological parameters of infected ***:An oral dose of pyronaridine tetraphosphate combined with a subcutaneous dose of diminazene aceturate inhibits Babesia in vitro and in mice,suggesting it might be a new paradigm for the treatment of babesiosis.

Three different bismuth-lead systems namely, Wood＇s alloy （Bi50Pb25sn12.5Cd12.5）, Newton＇s alloy （Bi50Pb31.2sn18.8） and Rose＇s alloy （B50Pb28sn22）, with one used as fusible alloys were quenched from melt by melt spinning technique. Thermal analysis, structure and mechanical properties of all alloys have been studied and analyzed. From X-ray diffraction analysis, an intermetallic compound phase, designated Pb7Bi3 is detected. The formation of an intermetallic compound phase causes a pronounced increase in the electrical resistivity. The Wood＇s alloy containing-cadmium exhibits mechanical properties superior to both the Newton＇s and Rose＇s alloys. The presence of cadmium in Wood＇s alloy decreases its melting point. Wood＇s alloy has better properties, which make it useful in various applications such as in protection shields for radiotherapy, locking of mechanical devices and welding at low temperature.

This paper proposed a new scheme for smooth handoff over F-HMIPv6 networks based on Location Based services(LBs). It uses the available information about mobile node(MN) such as user mobility patterns and MN's velocity to reduce handoff latency. In the proposed scheme, the movement pattern of users plays an important role in the performance analysis. The proposed scheme develops LBs that records a number of mobility patterns based on user daily behavior, and exploits these information to reduce the handoff latency. The proposed scheme is analyzed and compared with F-HMIPv6 protocol. The results show that it improves the performance in terms of handoff latency, packet delivery cost, and location update cost. specifically, the proposed scheme achieves the tradeoff between realizing a smooth handoff and minimizing the cost that is the demand for all customers of mobile services.

Potentiometric electrodes were developed for the rapid determination of proguanil hydrochloride in pure samples, pharmaceutical preparations and spiked serum and urine samples using PVC membrane,screen printed（sPE）, coated wired（CWE）, carbon paste（CPE） and modified carbon paste（MCPE）electrodes based on the ion-exchanger of proguanil with phosphotungestic acid（Pr-PT） as a chemical modifier. The prepared electrodes showed Nernestian slopes of 59.7, 58.1, 58.5, 58.5 and 57.0 for the PVC,sPE, CWE, CPE and MCPE for the proguanil ions in a wide concentration range of 1.0 ＊ 10^-5–1.0 ＊ 10^-2mol L^-1 at 25°C with detection limits of 7.94 ＊ 10^-6, 1.0 ＊ 10^-5, 1.0 ＊ 10^-6, 7.07 ＊ 10^-6 and 2.5 ＊ 10^-6 mol L^-1, respectively. The prepared electrodes exhibited high proguanil selectivity in relation to several inorganic ions and sugars and they could be successfully utilized for its determination in pure solutions, pharmaceutical preparations and serum and urine samples using the direct potentiometry and standard addition methods with very good recovery values.

A new polyglycerol phthalate extractant was prepared and characterized using Fourier transform infrared spectroscopy, proton nuclear magnetic resonance(~1H NMR) spectroscopy, and thermogravimetric analysis. A preliminary study of the extraction of long-lived(^(90) sr and ^(152+154)Eu) and short-lived(^(90) Y) radionuclides by polyglycerol phthalate was performed. The influence of different diluents, concentration of the acid, type of acid, and various interfering ions was explored. The stripping of ^(152+154)Eu radionuclide was studied by using different acid solutions, and it was found that sulfuric acid was the ideal acid for this purpose. Furthermore,^(152+154)Eu was extracted from the fission products(^(90) sr) and the extraction% of^(90) sr was found to improve in the presence of interfering ions.

Natural-orifice specimen-extraction surgery(NOsEs)for colorectal disease is not a new surgical procedure,although its lack of penetration into common colorectal surgical practice may make it a relatively new *** et al.[1]were among the first to report the extraction of a colectomy specimen through the vagina in 1991 and,shortly thereafter,Franklin et al.[2]published the first report of partial colectomy with naturalorifice specimen extraction(NOsE)via the *** then,there have been several publications on the extraction of both malignant and benign colonic diseases from the caecum to the distal rectum through natural-orifice extraction *** procedures such as laparoscopic cholecystectomy,which was introduced in 1985 and rapidly diffused into the surgical community over a 2-to 3-year period,the dispersion of NOsEs has not been equivocal[3].As there are several reasons why some new surgical innovations may be taken up more quickly than others,the adoption curves of new procedures can take many ***,the tipping point that describes the onset of the peak rate of diffusion of the new technology usually occurs after the first 10%–20%of users have adopted it[3].From a global perspective,the adoptions curve for NOsEs has not obtained a peak rate of diffusion and appears to have arrested in the developmental and explorative phase of innovation[3].

Background:In Egypt,around 1 million children are on the *** children's health is an issue that is not well *** objective of the current study was to determine the health problems of a targeted group of street children seeking medical care and to generate a hypothesis about the positive/negative role of street children care ***:A cross-sectional study was done on 2169 street children who sought medical care at El-Basma clinic at the Center for social and Preventive Medicine in Cairo University Children Hospital from January 2011 to July ***:respiratory illnesses and skin problems were the most prevalent conditions,affecting 485 (22.4%) and 359 (16.6%) of the patients,*** issues and trauma were significantly higher among the adolescent *** and gastrointestinal problems were significantly higher among children aged less than 2 *** and cardiac problems were significantly higher among those referred from organizations whereas trauma was significantly higher among those coming directly from the streets.A significant proportion of children at the extremes of the studied age spectrum were coming directly from the *** transmitted infections were rare ***:respiratory and skin diseases were the major morbidity problems among street children,especially adolescent *** afforded opportunities for access to different health care services.

BACKGROUND Hepatitis C virus(HCV)infection and its consequent complications are undeniably a public health burden worldwide,particularly in *** evidence suggests that many lncRNAs have relevant roles in viral infections and antiviral *** To investigate the expression profiles of circulating lncRNAGAs5,lncRNAHEIH,lncRNABIsPR and mRNABsT2 in naïve,treated and relapsed HCV Egyptian patients,to elucidate relation to HCV infection and their efficacy as innovative biomarkers for the diagnosis and prognosis of HCV *** One hundred and thirty HCV-infected Egyptian patients and 20 healthy controls were included in this *** lncRNAs and mRNABsT2 were measured using quantitative real-time polymerase chain reaction(qRT-PCR).resULTs Our results indicated that serum lncRNAGAs5 and LncRNABIsPR were upregulated,whereas mRNA BsT2 and LncRNA HEIH were downregulated in naïve *** contrast,HCV patients treated with sofosbuvir and simeprevir;with sofosbuvir and daclatasvir;or with sofosbuvir,daclatasvir and ribavirin exhibited lower levels of lncRNAGAs5 and lncRNABIsPR with higher mRNABsT2 compared to naïve ***,patients relapsed from sofosbuvir and simeprevir showed higher levels of these lncRNAs with lower mRNABsT2 compared to treated ***5 and lncRNABIsPR were positively correlated with viral load and ALT at Preas mRNABsT2 was negatively correlated with viral load at Psignificant positive correlation between lncRNA HEIH and AFP was observed at Prential expression of these RNAs suggests their involvement in HCV pathogenesis or antiviral response and highlights their promising roles in diagnosis and prognosis of HCV.

Cyclization of 4,5-Diamino Pyrazole Derivatives and Their Antibacterial Activities

A convenient synthesis of intermediate 4,5-diamino-3-aryl-l-phenylpyrazoles 4a--4c was reported. The different cyclization reactions were carried out with chalcone, 2-mercaptoacetic acid and p-anisialdehyde, ethyl chloroformate, glyoxal and thiourea to afford different N and s containing heterocycles. The reaction conditions were compared by conventional heating and microwave irradiation. The structures of the cyclization products were de- termined by analytical and spectroscopic data. All the synthesized compounds were screened for antibacterial activities in vitro.
