Complications and management of forgotten long-term biliary stents

AIMTo evaluate complications and management outcomes of retained long-term plastic biliary *** plastic biliary stent placement was performed in 802 patients at Yeungnam University Hospital between January 2000 and December 2014. Follow-up loss with a subsequently forgotten stent for more than 12 mo occurred in 38 patients. We retrospectively examined the cause of biliary stent insertion, status of stents, complications associated with biliary stents and management outcomes of long-term plastic biliary stents. Continuous variables were analyzed using the t test. Observed frequencies in subsets of the study population were compared using Fisher’s exact test and χ2 tests. Statistical significance was defined as P sertion were bile duct stones (63.2%, 24/38) and benign bile duct stricture (52.6%, 20/38). Mean duration of retained plastic stent was 22.6 ± 12.2 mo, and in 10 cases (26.3%), stents were retained for more than 24 mo. Common bile duct (CBD) stones or sludge were found in most cases (92.1%, 35/38). The most common complication was acute cholangitis (94.7%, 36/38). Stent removal by endoscopic approach was successfully performed in 92.1% (35/38) of the cases. In 3 cases, an additional plastic stent was inserted alongside the previous stent due to failure of the stent removal. Endoscopic removal of bile duct stones was successful in 73.7% (28/38) of the cases. When patients were divided into two groups by duration of stent placement (12 to 24 mo vs over 24 mo), there were no differences in the development of cholangitis, presence of biliary stones, and success rate of endoscopic removal of stones and biliary *** most common complication of retained long-term plastic biliary stents was acute cholangitis associated with CBD stones. Oral chemotherapy for second-line treatment in patients with gemcitabine-refractory advanced pancreatic cancer

Effect of barley straw biochar application on greenhouse gas emissions from upland soil for Chinese cabbage cultivation in short-term laboratory experiments

Chinese cabbage was cultivated in upland soil with the addition of biochar in order to investigate the potential for reduction of greenhouse gas emissions. Barley straw biochar(BSB) was introduced in a Wagner pot(1/5000a) in amounts of 0(BSB0, control), 100(BSB100), 300(BSB300), and 500(BSB500) kg 10a^(-1). After the addition of BSB into the upland soil, carbon dioxide(CO_2) emission increased while methane(CH_4) and nitrous oxide(N_2O) emissions decreased. The highest CO2 flux was measured for the BSB500 sample,(84.6 g m^(-2)) followed by BSB300, BSB100, and BSB0 in decreasing order. Relative to those of control, the total CH_4 flux and N_2O flux for the BSB500 treatment were lower by 31.6% and 26.1%, respectively. The global warming potential(GWP) of the treatment without biochar was 281.4 g CO_2 m-2 and those for treatments with biochar were in the range from 194.1 to 224.9 g CO_2 m^(-2). Therefore, introducing BSB into upland soil to cultivate Chinese cabbages can reduce the global warming potential.

Long-term outcomes of focal laser photocoagulation for the treatment of polypoidal choroidal vasculopathy

AIM:To evaluate the long-term effect and safety of focal laser photocoagulation treatment in eyes with polypoidal choroidal vasculopathy(PCV).METHODS:Medical records of 13 eyes of 13 patients with PCV were followed-up for more than 2 y after focal laser photocoagulation *** patients were diagnosed with PCV using indocyanine green angiography,and eyes with other comorbid ocular diseases were *** measurement outcomes of the study were the post-treatment regression and recurrence of polyps,complications,and changes in visual *** t-test was performed to compare visual outcome before and after the ***:The mean age of the 13 patients was 70.2±5.5 y,and the follow-up period was 72.3±31.0(range,25-118)*** eyes had juxtafoveal polyps and 10 eyes had extrafoveal *** the 13 eyes,9 eyes(69.2%)had regression of polyps 1.7±1.2(range,0.9-4)mo after focal laser *** eyes(55.6%)showed recurrence of polyps during the follow-up periods,and the recurrence period was 12.8±18.9(range,1.9-48)*** subretinal hemorrhage occurred in two eyes(15.4%)27 and 72 d after laser treatment,*** were no statistically significant differences in visual acuities at baseline;1,2,3 y post-treatment(all P>0.05);and last follow-up(0.63±0.5,0.73±0.70,0.67±0.57,0.75±0.7,and 0.95±0.8 log MAR,respectively).CONCLUSION:Focal laser photocoagulation is beneficial for early regression of polyps in eyes with PCV and does not result in significant submacular hemorrhage during the long-term ***,it can be primarily considered in eyes with PCV with extrafoveal or juxtafoveal polyps to regress risky polyps as well as to maintain visual acuity without serious hemorrhagic complications.

Measuring nasolacrimal duct volume using computed tomography images in nasolacrimal duct obstruction patients in Korean

AIM: To investigate nasolacrimal duct(NLD) volume in Korean patients and to examine the correlation between NLD volume and obstruction. METHODS: Of patients who underwent orbital computed tomography from March 2013 to January 2016, patients diagnosed with NLD obstruction were classified into the patient group and patients without obstruction were classified into the control group. The NLD volume was measured using the Image J program, which showed the NLD in axial,coronal, and sagittal images on computed tomography. RESULTS: The average value of men's NLD volume,265.33±90.57 mm3, was significantly larger than women's,211.87±68.61 mm3(P=0.009). In the patient group, the NLD volume of the obstructed eyes, 242.49±82.93 mm3,and the non-obstructed eyes, 225.20±73.20 mm3, were significantly higher than the control group, 217.61±82.04 mm3(Psed a chronic change in the bone around the NLD and affect the measurement of NLD volume.

Pathophysiological mechanisms underlying gastrointestinal symptoms in patients with COVID-19

Gastrointestinal(GI)symptoms,such as diarrhea,abdominal pain,vomiting,and anorexia,are frequently observed in patients with coronavirus disease 2019(COVID-19).However,the pathophysiological mechanisms connecting these GI symptoms to severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infections remain *** studies indicate that the entry of SARS-CoV-2 into intestinal cells leads to downregulation of angiotensin converting enzyme 2(ACE2)receptors resulting in impaired barrier *** intestinal ACE2 functions as a chaperone for the amino acid transporter B0AT1,the B0AT1/ACE2 complex within the intestinal epithelium acts as a regulator of gut microbiota composition and *** to the B0AT1/ACE2 complex lead to microbial dysbiosis through increased local and systemic immune *** studies have also suggested that altered serotonin metabolism may be the underlying cause of GI disorders involving *** findings of elevated plasma serotonin levels and high fecal calprotectin in COVID-19 patients with diarrhea indicate that the viral infection evokes a systemic inflammatory response that specifically involves the ***,the elevated proinflammatory cytokines correlate with elevated serotonin and fecal calprotectin levels further supporting the evidence of GI inflammation,a hallmark of functional GI ***,the finding that rectal swabs of COVID-19 patients remain positive for SARS-CoV-2 even after the nasopharynx clears the virus,suggests that viral replication and shedding from the GI tract may be more robust than that of the respiratory tract,further indicating fecal-oral transmission as another important route of viral *** review summarized the evidence for pathophysiological mechanisms(impaired barrier function,gut inflammation,altered serotonin metabolism and gut microbiota dysbiosis)underlying the GI symptoms in patients with COVID-19.
