The effects of total ankle replacement on ankle joint mechanics during walking

Background:End-stage ankle arthritis impairs joint function and patients' *** ankle replacement is a surgical procedure to treat severe ankle *** Talaris Anatomic Ankle^(TM)(STAA) was designed to mimic the normal ankle anatomy and flexion/extension of the ankle *** purpose of this study was to examine the effect of an STAA ankle replacement on ankle joint function and mechanics during ***:Five patients with end-stage unilateral ankle arthritis were *** performed level walking in a laboratory setting on 2occasions,prior to and 3 months after the STAA ankle *** Orthopedic Foot and Ankle Society(AOFAS) hindfoot score was obtained.A 12-camera motion capture system was used to perform walking *** temporo-spatial parameters and ankle joint mechanics were *** Student's t tests and non-parametric Wilcoxon matched tests were performed to examine the differences in biomechanical variables between the pre-and post-surgery walking ***:Compared to the pre-surgical condition,at 3 months of post-STAA surgery,patients experienced greater improvement in AOFAS hindfoot score(p = 0.0001);the STAA ankle demonstrated a 31% increase in ankle joint excursion(p = 0.045),a 22% increase in ankle plantarflexor moment(p = 0.075),a 60% increase in ankle power absorption(p = 0.023),and a 68% increase in ankle power production(p = 0.039).Patients also demonstrated a 26% increase in walking speed(p = 0.005),a 20% increase in stride length(p = 0.013),a 15% decrease in double support time(p = 0.043),and a 5% decrease in total stance time(p = 0.055).Conclusion:Three months after surgeries,the STAA patients experienced improvements in ankle function and gait *** STAA ankle demonstrated improved ankle mechanics during daily activities such as walking.

Estimates of voluntary activation in individuals with anterior cruciate ligament reconstruction:Effects of type of stimulator,number of stimuli,and quantification technique

Background:Accurate quantification of voluntary activation is important for understanding the extent of quadriceps dysfunction in individuals with anterior cruciate ligament reconstruction(ACLr).Voluntary activation has been quantified using both percent activation derived from the interpolated twitch technique and central activation ratio(CAr)derived from the burst superimposition technique,as well as by using different types of electrical stimulators and pulse train ***,it is unclear how these parameters affect voluntary activation estimates in individuals with *** study was performed to fill this important knowledge gap in the anterior cruciate ligament ***:Quadriceps strength and voluntary activation were examined in 18 ACLr participants(12 quadriceps/patellartendon graft,6 hamstring tendon graft;time since ACLr:1.06±0.82 years,mean±SD)at 90°of knee flexion using 2 stimulators(Digitimer and Grass)and pulse train conditions(3-pulse and 10-pulse).Voluntary activation was quantified by calculating both CAr and percent ***:results indicated that voluntary activation was significantly overestimated by CAr when compared with percent activation(pry activation estimates were not affected by pulse train conditions when using percent activation;however,3-pulse stimuli resulted in greater overestimation than 10-pulse stimuli when using CAr(p=0.003).Voluntary activation did not differ between stimulators(p>0.05);however,the Digitimer evoked greater torque at rest than the Grass(presults indicate that percent activation derived from the interpolated twitch technique provides superior estimates of voluntary activation than CAr derived from burst superimposition and is less affected by pulse train conditions or stimulators in individuals with ACLr.

Cannabinoids are lipophilic substances derived from Cannabis sativa that can exert a variety of effects in the human *** have been studied in cellular and animal models as well as in human clinical trials for their therapeutic benefits in several human *** of these include central nervous system(CNS)diseases and dysfunctions such as forms of epilepsy,multiple sclerosis,Parkinson’s disease,pain and neuropsychiatric *** addition,the endogenously produced cannabinoid lipids,endocannabinoids,are critical for normal CNS function,and if controlled or modified,may represent an additional therapeutic avenue for CNS *** review discusses in vitro cellular,ex vivo tissue and in vivo animal model studies on cannabinoids and their utility as therapeutics in multiple CNS *** addition,the review provides an overview on the use of cannabinoids in human clinical trials for a variety of CNS *** and endocannabinoids hold promise for use as disease modifiers and therapeutic agents for the prevention or treatment of neurodegenerative diseases and neurological disorders.

Morphometric studies of 3D micro CT-scanned images can provide insights into the evolution of the brain and sensory structures but such data are still scarce for the most diverse mammalian order of *** reviewed and new data,we tested for convergence to extreme aridity and high elevation in the sensory and brain morphology of rodents,from morphometric data from micro-CT X-ray scans of 174 crania of 16 species of three distantly re-lated African murid(soft-furred mice,Praomyini,laminate-toothed rats,Otomyini,and gerbils,Gerbillinae)clades and one North American cricetid(deer mice and white-footed mice,Peromyscus)*** studies demon-strated convergent evolution acting on the oval window area of the cochlea(enlarged in extremely arid-adapted species of Otomyini and Gerbillinae)and on endocranial volume(reduced in high elevation taxa of Otomyini and Peromyscus).However,contrary to our predictions,we did notfind evidence of convergence in brain struc-ture to aridity,or in the olfactory/respiratory system(turbinate bones)to high *** structure differed,particularly in the petrosal lobules of the cerebellum and the olfactory bulbs,between Otomyini and Gerbillinae,with extreme arid-adapted species in each clade being highly divergent(not convergent)from other species in the same *** observed greater“packing”of the maxillary turbinate bones,which have important respiratory functions,in Peromyscus mice from high and low elevations compared to the high-elevation African Praomyini,but more complex patterns within Peromyscus,probably related to trade-offs in respiratory physiology and heat exchange in the nasal epithelium associated with high-elevation adaptation.

Climate refugia along Lake Superior’s shores:disjunct arctic-alpine plants rely on cool shoreline temperatures but are restricted to highly exposed habitat under climate warming

Climate refugia can serve as a remnant habitat or stepping stones for species dispersal under climate *** largest freshwater lake by surface area,Lake Superior,USA and Canada,serves as a model system for understanding cooling-mediated local refugia,as its cool water temperatures and wave action have maintained shoreline habitats suitable for southern disjunct populations of arctic-alpine plants since ***,we seek to explain spatial patterns and environmental drivers of arctic-alpine plant refugia along Lake Superior’s shores,and assess future risk to refugia under moderate(+3.5℃)and warmest(+5.7℃)climate warming ***,we examined how the interactive effects of summer surface water temperatures and wind affected onshore temperatures,resulting in areas of cooler ***,we developed an ecological niche model for the presence of disjunct arctic-alpine refugia(pooling 1253 occurrences from 58 species)along the lake’s ***,we fit species distribution models for 20 of the most common arctic-alpine disjunct species and predicted presence to identify refugia ***,we used the two climate warming scenarios to predict changes in the presence of refugia and disjunct *** type,elevation above water,inland distance,July land surface temperature from MODIS/Terra satellite and near-shore depth of water were the best predictors of disjunct ***,we predicted 2236 km of the shoreline(51%)as disjunct refugia habitat for at least one species under current conditions,but this was reduced to 20% and 7% with moderate(894 km)and warmest(313 km)climate change projections.

Purpose:This paper presents an update of the 2011 Wheelchair Compendium of Physical Activities designed for wheelchair users and is referred to as the 2024 Wheelchair *** Wheelchair Compendium aims to curate existing knowledge of the energy expenditure for wheelchair physical activities(PAs).Methods:A systematic review of the published energy expenditure of PA for wheelchair users was completed between 2011 and May *** added these data to the 2011 Wheelchair Compendium data that was compiled previously in a systematic review through ***:A total of 47 studies were included,and 124 different wheelchair PA reported energy expenditure values ranging from 0.8 metabolic equivalents for wheelchair users(filing papers,light effort)to 11.8 metabolic equivalents for wheelchair users(Nordic sit skiing).Conclusion:In introducing the updated 2024 Wheelchair Compendium,we hope to bridge the resource gap and challenge the prevailing narratives that inadvertently exclude wheelchair users from physical fitness and health PAs.

Development of a Proof-of-Concept Multi-Method Computer Simulation to Support rural Healthcare Disaster Preparedness Planning

Due to a lack of resources, rural communities often face challenges when planning catastrophic events. This project involved applying systems thinking and model-based systems engineering to develop a proof-of-concept, multi-method computer simulation and then determining whether the simulation could be used to assess the efficacy of disaster planning approaches on health outcomes in rural communities, as a function of primary healthcare. The project focus was a rural or non-urban healthcare system experiencing a natural hazard. Both system dynamics and discrete event models were incorporated to represent subsystem operations, crucial disaster responses, as well as three key response systems: public health, emergency management, and healthcare. The subsystem models included several components: policies/procedures, communications,resources, exercises/drills/training, healthcare space and staff, and the flow of affected people into and through the *** combined simulation can serve as a first step to a more comprehensive approach to helping rural communities achieve more efficient and effective healthcare planning for disaster responses.

The *** Institute of Standards and Technology(NIST)published the Community resilience Planning Guide in *** NIST Guide advocates for a participatory process for developing a performance measurement framework for the jurisdiction’s resilience against a scenario *** framework centers around tables of expected and desired recovery times for selected community assets,such as electricity,water,and natural gas *** NIST Guide does not provide a method for estimating the expected recovery ***,building high-fidelity computer models for such estimations requires substantial resources that even larger ju-risdictions cannot *** most promising approach to recovery time estimation is to systematically use data elicited from people to tap into the wisdom of the(knowledgeable)*** paper describes a novel research-through-design project to enable the computer-supported elicitation of recovery time series *** work is the first in the literature to examine people’s ability to estimate recovery curves and how design in-fluences such *** main contribution to resilience planning is three-fold:development of a new elicitation tool called restimate,understanding its potential user base,and providing insights into how it can facilitate resilience *** is the first tool to enable evidence-based expert elicitation in any community with limited resources for resilience *** resilience planning,those who facilitate high-stakes planning activities under large uncertainties(e.g.,mission-critical system design and planning)will benefit from a similar research-through-design process.

Piezoelectric resonant de-icing systems are attracting great *** paper aims to assess the implementation of these systems at the aircraft *** article begins with the model to compute the power requirement of a piezoelectric resonant de-icing system sized from the prototype detailed in Part 1/2 of this *** the mass,drag,and fuel consumption of this system and the subcomponents needed for its implementation are *** features of a piezoelectric resonant de-icing system are finally computed for aircraft similar to Airbus A320 aircraft and aircraft of different categories(Boeing 787,ATr 72 and TBM 900)and compared with the existing thermal and mechanical ice protection systems.A sensitivity analysis of the main key sizing parameters of the piezoelectric de-icing system is also performed to identify the main axes of improvement for this *** study shows the potential of such ice protection *** particular,for the realistic input parameters chosen in this work,the electro-mechanical solution can provide a 54% reduction in terms of mass and a 92% reduction in terms of power consumption for an A320 aircraft architecture,leading to a 74% decrease in the associated fuel consumption compared to the actual air bleed system.
