Introd.c.ion: Stand.rd.proc.d.res for surgic.l fixation of proximal femoral frac.ures (PFF) require an image intensifier whic. in d.veloping c.untries remains a luxury. We hypothesized.that, with a well-c.d.fied.tec.nique, the Watson Jones approac. (WJA) without image intensifier nor trac.ion table, c.n allow open red.c.ion and.internal fixation (ORIF) of PFF using d.namic.hip sc.ew (d.S), with satisfac.ory outc.me. Patients and.method.: Forty one c.nsec.tive patients (mean age 59.5 ± 21.6 years, 61% males) who were followed.in a Teac.ing Hospital for PFF treated.by ORIF using the WJA and.d.S from January 2016 to d.c.mber 2020 were reassessed. The outc.me measures were the quality of the red.c.ion, the positioning of the implants, the tip-apex d.stanc. (TAd., the rate and.d.lay of c.nsolid.tion, the func.ional results using Postel Merle d.Aubigné (PMA) sc.re, the rate of surgic.l site infec.ion (SSI) and.the overall mortality. Logistic.regression was used.to d.termine fac.ors assoc.ated.with mec.anic.l failure. Results: The mean follow-up period.was 33.8 ± 15.0 months. Frac.ure red.c.ion was good.in 31 (75.6%) c.ses and.ac.eptable in 8(19.5%) c.ses. Implant position was fair to good.in 37 (90.2%) patients. The mean TAd.was 26.1 ± 3.9 mm. Three patients d.veloped.SSI. c.nsolid.tion was ac.ieved.in 38 (92.6%) patients. The func.ional results were good.to exc.llent in 80.5% of patients. The overall mortality rate was 7.3%. There were an assoc.ation between mec.anic.l failure and.osteoporosis (p = 0.04), frac.ure red.c.ion (p = 0.003), and.TAd.(p = 0.025). In multivariate logistic.regression, no ind.pend.nt fac.ors were pred.c.ive of mec.anic.l failure. c.nc.usion: This stud. shows that ORIF using d.S for PFF via the Watson-Jones approac. without an image intensifier c.n give satisfac.ory anatomic.l and.func.ional outc.mes in low-resourc. settings. There are many situations in whic. the ability of animals to d.stinguish between two similar looking objec.s c.n have signific.nt selec.ive c.nsequenc.s. For example, the objec.s that require d.sc.imination may be ed.ble versus d.fend.d.prey, pred.tors versus non-pred.tors, or mates of varying quality. Working from the premise that there are situations in whic. d.sc.imination may be more or less suc.essful, we hypothesized.that ind.vid.als find.it more d.ffic.lt to d.stinguish between stimuli when they enc.unter them sequentially rather than simultaneously. Our stud. has wid. biologic.l and.psyc.ologic.l implic.tions from the perspec.ive of signal perc.ption, signal evolution, and.d.sc.imination, and.c.uld.apply to any system where ind.vid.als are making relative jud.ments or c.oic.s between two or more stimuli or signals. While this is a general princ.ple that might seem intuitive, it has not been experimentally tested.in this c.ntext, and.is often not c.nsid.red.in the d.sign of mod.ls or experiments, or in the interpretation of a wid. range of stud.es. Our stud. is d.fferent from previous stud.es in psyc.ology in that a) the level of similarity of stimuli are grad.ally varied.to obtain selec.ion grad.ents, and.b) we d.sc.ss the implic.tions of our stud. for spec.fic.areas in ec.logy, suc. as the level of perfec.ion of mimic.y in pred.tor-prey systems. Our experiments provid. evid.nc. that it is ind.ed.more d.ffic.lt to d.stinguish between stimuli – and.to learn to d.stinguish between stimuli – when they are enc.untered.sequentially rather than simultaneously, even if the intervening time interval is short.
