Outcomes assessment in surgery traditionally has included data regarding peri-operative mortality and morbidity, as well as long-term survival and recurrence in surgical oncology. However, quality of life （QOL） is another important patient-related outcome measure. QOL data can be used to tailor treatment and improve clinical outcomes by detecting physical or psychological problems in patients that otherwise might be overlooked, but which have profound implications for the effective delivery of care. We review several well-validated QOL instruments developed specifically for use in patients with gastrointestinal malignancies, including esophageal, pancreatic and colorectal cancers, and conclude that QOL assessment routinely should be included in clinical trials of novel treatments.

Neurodegenerative disorders are often associated with cellular dysfunction caused by underlying protein-misfolding signalling. Numerous neuropathologies are diagnosed at late stage symptomatic changes which occur in response to these molecular malfunctions and treatment is often too late or restricted only to the slowing of further cell death. Important new strategies to identify early biomarkers with predictive value to intervene with disease progression at stages where cell dysfunction has not progressed irreversibly is of paramount importance. Thus, the identification of these markers presents an essential opportunity to identify and target disease pathways. This review highlights some important metabolic alterations detected in neurodegeneration caused by misfolded prion protein and discusses common toxicity pathways identified across different neurodegenerative diseases. Thus, having established some commonalities between various degenerative conditions, detectable metabolic changes may be of extreme value as an early diagnostic biomarker in disease.

Environmental management of military activities is of growing global concern by defence forces. As one of the largest landholders in Australia, the Australian Defence Force （ADF） is increasingly concerned with sustainable environmental management. This paper focuses on how the ADF is maintaining effective environmental management, especially in environmentally sensitive marine protected areas. It uses Shoalwater Bay Training Area （SWBTA） as a research example to examine environmental management strategies conducted by the ADF. SWBTA is one of the most significant Defence training areas in Australia, with a large number of single, joint and combined military exercises conducted in the area. With its maritime component contained in the Great Barrier Reef Marine Park （GBRMP）, the Great Barrier Reef World Heritage Area （GBRWHA）, and abutting Queensland＇s State Marine Parks, it has high protection values. It is therefore vital for the ADF to adopt environmentally responsible management while they are conducting military activities. As to various tools employed to manage environmental performance, the ISO 14001 Environmental Management System （EMS） is widely used by the ADE This paper exanlines military activities and marine environmental management within SWBTA, using the Talisman Saber （TS） exercise series as an example. These are extensive joint exercises conducted by the ADF and the United States defence forces. The paper outlines relevant legislative framework and environmental policies, analyses how the EMS operates in environmental management of military activities, and how military activities comply with these regulations. It discusses the implementation of the ADF EMS, including risk reduction measures, environmental awareness training, consultation and communication with stakeholders. The emergence of mutual knowledge is a major cognitive mechanism for the robustness of complex socio-technical systems. It has been extensively studied from an ethnomethodological point of view and empirically reproduced by multi-agent simulations. Whilst such simulations have been used to design real work settings the underlying theoretical grounding for the process is vague. The aim of this paper is to investigate whether the emergence of mutual knowledge(MK) in a group of colocated individuals can be explained as a percolation phenomenon. The followed methodology consists in coupling agent-based simulation with dynamic networks analysis to study information propagation phenomena: After using an agent-based simulation the authors generated and then analyzed its traces as networks where agents met and exchanged knowledge. Deep analysis of the resulting networks clearly shows that the emergence of MK is comparable to a percolation process. The authors specifically focus on how changes at the microscopic level in the proposed agent based simulator affect percolation and robustness. These results therefore provide theoretical basis for the analysis of social organizations.

Management of preoperative biliary drainage (PBD) in patients undergoing pancreaticoduodenectomy (PD) is still debated. Obstructive jaundice is associated with a proinflammatory state, impaired immune function and disturbances in coagulation. Patients with jaundice are at high risk to develop renal dysfunction and bacteriobilia. In this context, PBD was traditionally performed before surgery to improve postoperative outcomes.

·AIM:To report a successful osteo-odonto keratoprosthesis(OOKP) procedure in a case of end stage of corneal blindness due to Stevens-Johnson syndrome (SJS).·METHODS:An interventional case report.·RESULTS:We describe a 35-year-old Indian woman,a known case of SJS with bilateral dry eyes and corneal blindness (failed corneal graft with vascularised total corneal opacity in the right eye and non-healing corneal ulcer in the left eye).Vision was hand movement only in both *** corneal ulcer healed with medical treatment resulting in vascularised total corneal opacity with no improvement in *** was performed in the right eye and the vision was improved from hand movement to 6/*** same vision was maintained in the right eye at the last follow-up 5 years after surgery.·CONCLUSION:OOKP provides good visual rehabilitation with long-term anatomically stable prosthesis in patients with end-stage of ocular surface disorders and corneal blindness secondary to SJS.·

To address the management of Clostridium difficile (C. difficile) infection (CDI) in the setting of suspected inflammatory bowel disease (IBD)-flare. METHODSA systematic search of the Ovid MEDLINE and EMBASE databases by independent reviewers identified 70 articles including a total of 932141 IBD patients or IBD-related hospitalizations. RESULTSIn those with IBD, CDI is associated with increased morbidity, including subsequent escalation in IBD medical therapy, urgent colectomy and increased hospitalization, as well as excess mortality. Vancomycin-containing regimens are effective first-line therapies for CDI in IBD inpatients. No prospective data exists with regards to the safety or efficacy of initiating or maintaining corticosteroid, immunomodulator, or biologic therapy to treat IBD in the setting of CDI. Corticosteroid use is a risk factor for the development of CDI, while immunomodulators and biologics are not. CONCLUSIONStrong recommendations regarding when to initiate IBD specific therapy in those with CDI are precluded by a lack of evidence. However, based on expert opinion and observational data, initiation or resumption of immunosuppressive therapy after 48-72 h of targeted antibiotic treatment for CDI may be considered.

We presented a quantity judgment task that involved comparing two sequentially presented sets of items to preschoolers and chimpanzees using nearly identical procedures that excluded verbal instructions to children. Trial difficulty in this task reflected the ratio difference between sets of discrete items where larger ratios （e.g., 0.80 as from comparing 4 to 5） were more difficult than smaller ones （e.g., 0.50 as from comparing 4 to 8）. Children also completed verbal-based tasks probing the relationskip between counting proficiency and performance on the quantity judgment task of sequentially presented identical sized items. Both species＇ performance was best when ratios between comparison sets were small regardless of set size in all types of tasks. Generally, chimpanzees and older children performed better than younger children except at larger ratios. Children＇s counting proficiency was not related to success in choosing the larger of two quantities of identical-sized items. These results indicate that chimpanzees and children share an approximate number sense that is reflected through analog magnitude estimation when comparing quantities [Current Zoology 57 （4）： 419-428, 2011].
