BACKGROUND Mitral valvuloplasty using artificial chordae tendineae represents an effective surgical approach for treating mitral regurgitation.Achieving precise measurements of artificial chordae tendineae length(CL)is an important factor in the procedure;however,no objective index currently exists to facilitate this measurement.Therefore,preoperative assessment of CL is critical for surgical planning and support.Four-dimensional x-ray micro-computed tomography(4D-CT)may be useful for accurate CL measurement considering that it allows for dynamic three-dimensional(3D)evaluation compared to that with transthoracic echocardiography,a conventional inspection method.AIM To investigate the behavior and length of mitral chordae tendineae during systole using 4D-CT.METHODS Eleven adults aged>70 years without mitral valve disease were evaluated.A 64-slice CT scanner was used to capture 20 phases in the cardiac cycle in electrocardiographic synchronization.The length of the primary chordae tendineae was measured from early systole to early diastole using the 3D image.The primary chordae tendineae originating from the anterior papillary muscle and attached to the A1-2 region and those from the posterior papillary muscle and attached to the A2-3 region were designated as cA and cP,respectively.The behavior and maximum lengths[cA(ma),cP(max)]were compared,and the correlation with body surface area(BSA)was evaluated.RESULTS In all cases,the mitral anterior leaflet chordae tendineae could be measured.In most cases,the cA and cP chordae tendineae could be measured visually.The mean cA(max)and cP(max)were 20.2 mm±1.95 mm and 23.5 mm±4.06 mm,respectively.cP(max)was significantly longer.The correlation coefficients(r)with BSA were 0.60 and 0.78 for cA(max)and cP(max),respectively.Both cA and cP exhibited constant variation in CL during systole,with a maximum 1.16-fold increase in cA and a 1.23-fold increase in cP from early to mid-systole.For cP,CL reached a plateau at 15%and remained elongated until end-systole,whereas for cA,aft

BACKGROUND Although intracorporeal anastomosis(IA)for colon cancer requires longer operative time than extracorporeal anastomosis(EA),its short-term postoperative results,such as early recovery of bowel movement,have been reported to be equal or better.As IA requires opening the intestinal tract in the abdominal cavity under pneumoperitoneum,there are concerns about intraperitoneal bacterial infection and recurrence of peritoneal dissemination due to the spread of bacteria and tumor cells.However,intraperitoneal bacterial contamination and medium-term oncological outcomes have not been clarified.abdominal cavity in IA.METHODS Of 127 patients who underwent laparoscopic colon resection for colon cancer from April 2015 to December 2020,75 underwent EA(EA group),and 52 underwent IA(IA group).After propensity score matching,the primary endpoint was 3-year disease-free survival rates,and secondary endpoints were 3-year overall survival rates,type of recurrence,surgical site infection(SSI)incidence,number of days on antibiotics,and postoperative biological responses.RESULTS Three-year disease-free survival rates did not significantly differ between the IA and EA groups(87.2%and 82.7%,respectively,P=0.4473).The 3-year overall survival rates also did not significantly differ between the IA and EA groups(94.7%and 94.7%,respectively;P=0.9891).There was no difference in the type of recurrence between the two groups.In addition,there were no significant differences in SSI incidence or the number of days on antibiotics;however,postoperative biological responses,such as the white blood cell count(10200 vs 8650/mm^(3),P=0.0068),C-reactive protein(6.8 vs 4.5 mg/dL,P=0.0011),and body temperature(37.7 vs 37.5℃,P=0.0079),were significantly higher in the IA group.CONCLUSION IA is an anastomotic technique that should be widely performed because its risk of intraperitoneal bacterial contamination and medium-term oncological outcomes are comparable to those of EA.

Immune checkpoint inhibitor therapy has dramatically improved patient prognosis,and thereby transformed the treatment in various cancer types including esophageal squamous cell carcinoma(ESCC)in the past decade.Monoclonal antibodies that selectively inhibit programmed cell death-1(PD-1)activity has now become standard of care in the treatment of ESCC in metastatic settings,and has a high expectation to provide clinical benefit during perioperative period.Further,anti-cytotoxic T-lymphocyte–associated protein 4(CTLA-4)monoclonal antibody has also been approved in the treatment of recurrent/metastatic ESCC in combination with anti-PD-1 antibody.Well understanding of the existing evidence of immune-based treatments for ESCC,as well as recent clinical trials on various combinations with chemotherapy for different clinical settings including neoadjuvant,adjuvant,and metastatic diseases,may provide future prospects of ESCC treatment for better patient outcomes.

Background The development of a sustainable business model with social acceptance,makes necessary to develop new strategies to guarantee the growth,health,and well-being of farmed animals.Debaryomyces hansenii is a yeast species that can be used as a probiotic in aquaculture due to its capacity to i)promote cell proliferation and differen-tiation,ii)have immunostimulatory effects,iii)modulate gut microbiota,and/or iv)enhance the digestive function.To provide inside into the effects of D.hansenii on juveniles of gilthead seabream(Sparus aurata)condition,we inte-grated the evaluation of the main key performance indicators coupled with the integrative analysis of the intestine condition,through histological and microbiota state,and its transcriptomic profiling.Results After 70 days of a nutritional trial in which a diet with low levels of fishmeal(7%)was supplemented with 1.1%of D.hansenii(17.2×10^(5) CFU),an increase of ca.12%in somatic growth was observed together with an improve-ment in feed conversion in fish fed a yeast-supplemented diet.In terms of intestinal condition,this probiotic modu-lated gut microbiota without affecting the intestine cell organization,whereas an increase in the staining intensity of mucins rich in carboxylated and weakly sulphated glycoconjugates coupled with changes in the affinity for certain lectins were noted in goblet cells.Changes in microbiota were characterized by the reduction in abundance of several groups of Proteobacteria,especially those characterized as opportunistic groups.The microarrays-based transcrip-tomic analysis found 232 differential expressed genes in the anterior-mid intestine of S.aurata,that were mostly related to metabolic,antioxidant,immune,and symbiotic processes.Conclusions Dietary administration of D.hansenii enhanced somatic growth and improved feed efficiency param-eters,results that were coupled to an improvement of intestinal condition as histochemical and transcriptomic tools indicated.This probiotic yeast stimulated host-microbiota interactions with

A Case Report of Kissing Molars Class II

Kissing molars (KMs) is a condition of occlusal molar surfaces in a single follicular space with roots extending in opposite directions. Here, we have reported a case of KMs in a 58-year-old woman. The patient complained of pain in the right mandibular molar region and was diagnosed with KMs consisting of the right mandibular second and third molars. Because of the patient’s pain having subsided at the time of the visit and her unwillingness to undergo tooth extraction, the patient was followed up. KMs is classified into three classes (I-III) and is either true-KMs or pseudo-KMs and presents with or without cystic variants of dental follicles. The presents as true-KMs class II without a cystic variant. With reference to the literature and based on our analysis, the mean age of patients affected by this specific case of KMs is 31.7 years and unilateral KMs is relatively more common (85.7%). Histopathological findings of dentigerous cysts are more often indicated (42.9%). The treatment policy for KMs should therefore be based on the classification of KMs. Importantly, the focus should be on preserving the first and second molars as much as possible. The treatment approach, such as the employment of surgical removal or orthodontics, should be determined by considering the associated factors such as the crowns, tissues, and age of the patient.

Effects of tropical forest conversion into oil palm plantations on nitrous oxide emissions:A meta-analysis

Oil palm plantations have dramatically expanded in tropical Asia over the past decades.Although their establishment has been projected to increase nitrous oxide(N_(2)O)emissions,earlier reports have shown inconsistent results.This study analyzed these previously published data to compare N_(2)O emissions in oil palm plantations to reference forests.A linear mixed-eff ects model was used to examine the signifi cance of the eff ect of establishing oil palm plantations on N_(2)O emissions,rather than to calculate mean eff ect sizes because of limitations in the data structure.The results indicated that N_(2)O emissions were signifi cantly greater from oil palm plantations than from reference forests,as expected.This is the fi rst study to report the eff ect of oil palm plantations on N_(2)O emissions by synthesizing previously published data.To quantify the size of this eff ect,additional studies with frequent and long-term monitoring data are needed.

This work uses quantum chemistry calculations and machine learning to explore design rules for singlet fission in a chemical space of four million indigoid derivatives.We identify~400,000 derivatives of 2,2′-diethenyl cibalackrot,which theoretically fulfil the energy conditions for exoergic singlet fission above the silicon band gap energy.Probing this database with a random forest classifier,we observe that small substituents with positive mesomeric effects and weak negative inductive effects reinforce the desired energetic conditions when placed at specific positions.Finally,a subset of molecules that reflects the random forest classifier’s rules are investigated for their quantum chemical properties to translate the desirable structural motifs into wavefunction-based design rules.Here,direct correlations between the energetic condition for singlet fission,the biradical character and the charge and triplet spin density in prominent molecular regions are identified,providing insights that may serve as a guide for singlet fission core structure development.

Do predators prefer toxic animals?A case of chemical discrimination by an Asian snake that sequesters firefly toxins

Several Asian natricine snakes of the genus Rhabdophis feed on toads and sequester steroidal cardiac toxins known as bufadienolides(BDs)from them.A recent study revealed that species of the Rhabdophis nuchalis Group ingest lampyrine fireflies to sequester BDs.Although several species of fireflies are distributed in the habitat of the R.nuchalis Group,only lampyrine fireflies,which have BDs,are included in the diet of these snakes.Thus,we hypothesized that the R.nuchalis Group chemically distinguishes fireflies that have BDs from those that do not have BDs.We also predicted that the R.nuchalis Group detects BDs as the chemical cue of toxin source.To test these predictions,we conducted 3 behavioral experiments using Rhabdophis chiwen,which belongs to the R.nuchalis Group.In the first experiment,R.chiwen showed a moderate tongue flicking response to cinobufagin,a compound of BDs.On the other hand,the snake showed a higher response to the chemical stimuli of lampyrine fireflies(BD fireflies)than those of lucioline fireflies(non-BD fireflies).In the second experiment,in which we provided live BD and non-BD fireflies,the snake voluntarily consumed only the former.In the third,a Y-maze experiment,the snake tended to select the chemical trail of BD fireflies more frequently than that of non-BD fireflies.These results demonstrated that R.chiwen discriminates BD fireflies from non-BD fireflies,but the prediction that BDs are involved in this discrimination was not fully supported.To identify the proximate mechanisms of the recognition of novel toxic prey in the R.nuchalis Group,further investigation is necessary.

The modification of the electronic band structure is of critical importance for thermoelectric materials whose heat to electricity power generation is related to carrier effective mass and alignment of semiconductor band edges.One approach to optimize the electronic band structure is by modification of orbital interactions through doping or alloying.While the current ab-initio quantum chemical calculations can help us to investigate orbital components of electronic bands,they reveal little information on the relative tunability of electronic states and transport properties with respect to orbital interactions.In this work,we present a method,based on a symmetry-adapted tight-binding model and sensitivity analysis,that can pinpoint the role of orbital interactions in determining electronic band structure and transport properties.As an application,a systematic theoretical analysis is used to show how the power factor of PbTe can/cannot be improved by playing with interatomic orbital interactions.The orbital interaction sensitivity analysis introduced in this work can easily be applied to other compounds or properties.
