Thyroglossal Duct Anomalies in Pediatric and Adult Population: A 10-Year Experience in a Tertiary Care Hospital

Introduction: Thyroglossal duct cyst is a well-recognized congenital midline neck swelling observed in early childhood and rarely in adults. It may reveal itself as a painless cyst, abscess, or as fistula. Several studies have been conducted across the world, but the literature is scarce on its presentation and complications in the Middle East. This indexing study aims to report a 10-year experience with thyroglossal duct cyst (TGDC) presentation, excision, and recurrence at a teaching hospital in Saudi Arabia (KSA). Methods: A retrospective chart review was conducted at the Armed Forces Hospital Southern Region, KSA from December 2008 to December 2018. Data were retrieved from the electronic medical record system of the hospital and validated with the histopathology records. A total of 48 patients diagnosed as TGDC, sinus or fistula were identified. Stata ver. 16.1 was used to analyze the data and results formulated using regression model and Pearson’s chi-square test. Results: The majority of our patients were female (60.4%), and the most common presentation was a midline neck swelling which moved with tongue protrusion. Surgical excision via Complete Sistrunk procedure was the operation of choice in 60.5% followed by excision of the cyst with tract and sinus tract. Previous infection and surgical history contributed to complications in our patients, but no association of age or gender was observed. Patients presenting with infected and discharging cysts were found to be much more likely to develop a recurrence (9 patients). Cysts deep to the hyoid showed more recurrence [n = 7 (78%)] compared to superficial cysts [n = 2 (22%)] and posterior and multi-tract lesions also showed significant recurrence. Conclusion: Despite being a common cause of neck swelling, thyroglossal duct cyst is often missed on physical exams. To reduce complications, timely diagnosis and treatment are necessary.

Several mathematical models have been developed to investigate the dynamics of tuberculosis(TB)and hepatitis B virus(HBV).Numerous current models for TB,HBV,and their co-dynamics fall short in capturing the important and practical aspect of *** is crucial to take into account a stochastic co-infection HBV–TB epidemic model since different random elements have a substantial impact on the overall dynamics of these *** provide a novel stochastic co-model for TB and HBV in this study,and we establish criteria on the uniqueness and existence of a nonnegative global *** also looked at the persistence of the infections as long its dynamics are governable by the proposed *** verify the theoretical conclusions,numerical simulations are presented keeping in view the associated analytical *** infections are found to finally die out and go extinct with certainty when L´evy intensities surpass the specified thresholds and the related stochastic thresholds fall below *** findings also demonstrate the impact of noise on the decline in the co-circulation of HBV and TB in a given *** results provide insights into effective intervention strategies,ultimately aiming to improve the management and control of TB and HBV co-infections.

Cancer is a frightful disease and represents one of the biggest health-care issues for the human race and demands a proactive strategy for cure. Plants are reservoirs for novel chemical entities and provide a promising line for research on cancer. Hitherto, being effective, chemotherapy is accompanied by certain unbearable side effects. Nevertheless,plants and plant derived products is a revolutionizing field as these are Simple, safer, ecofriendly, low-cost, fast, and less toxic as compared with conventional treatment *** are selective in their functions and acts specifically on tumor cells without affecting normal cells. Carcinogenesis is complex phenomena that involves many signaling cascades. Phytochemicals are considered suitable candidates for anticancer drug development due to their pleiotropic actions on target events with multiple manners. The research is in progress for developing potential candidates(those can block or slow down the growth of cancer cells without any side effects) from these phytochemicals. Many phytochemicals and their derived analogs have been identified as potential candidates for anticancer therapy. Effort has been made through this comprehensive review to highlight the recent developments and milestones achieved in cancer therapies using phytomolecules with their mechanism of action on nuclear and cellular factors. Furthermore, drugs for cancer treatment and their limitations have also been discussed.

Breast cancer is a frightful disease and serious concern in women around the world causing significant health care burden in both developed and developing countries. Extensive research work has shown that breast cancer provides strong resistance to chemical agents, U V radiation,and hormonal treatments. It is generally accepted that cell genetics is not the only main reason for breast cancer and genetic risk factors, for example, mutations in RRCAI and BRCA2 genes constitute 5%-10% of all breast cancer rates. Other related factors include age, gender,race, ethnicity, weight, reproductive factors, exo-and endogenous hormonal exposures, oral contraceptives use, ultraviolet radiation, diet, and night work(circadian disruption). Many studies have revealed that dietary isoflavones regulate breast cancer occurrence, recurrence and prognosis. Dietary isoflavones have long been part of Asian population diet and there is a significant increase as compared to dietary isoflavones intake among other populations. Dietary isoflavones are natural phytoestrogens having both estrogenic and anti-estrogenic potentials on breast cancer cells in culture, animal models and in experimental trials. This literature survey provides a comprehensive overview on the tumor preventive and tumor promoting potentials of dietary isoflavones on breast cancer. In addition, this paper provides a literature review of dietary isoflavones and their effects on up-regulation and down-regulation of different signaling pathways, genes and proteins. Finally, future perspectives of dietary isoflavones and breast cancer researchers are also critically discussed, which will provide a deeper insight regarding the inner molecular mechanisms of action.

The National Cancer Institute had projected breast cancer(BC) as one of the topmost prevalent malignancies around the *** many cases,BC becomes resistant to chemotherapy,radiation and hormonal *** BC therapies are associated with adverse side effects,drug resistance and *** research work has shown that these dietary phytochemicals(DPs) may exert therapeutic effects by regulating the miRNA expression.A large number of DPs have been researched as miRNA regulatory agents against BC and some other DPs have not yet been tested against *** have discussed the effects of curcumin,diallyl disulphide,3,3′ diindolylmethane,ellagic acid,genistein,indole-3-carbinol,quercetin,resveratrol,and sulforaphane on regulation of expression of BC miRNAs in a wide range of in vitro and in vivo *** have also shown some of the possible DPs(Oleanolic acid,capsaicin,benzyl isothiocyanate,epigallocatechin gallate,phenethyl isothiocyanate and ursolic acid) that have shown miRNA regulatory activities and have not yet been tested against BC ***,current limitations,challenges,future perspectives of DPs and BC research are also critically discussed.

目的研究植物碱蓬内生真菌曲霉TQ67次级代谢产物。方法采用ODS反相柱色谱、高效液相色谱、半制备HPLC等方法对发酵液及菌丝体部分分离纯化,运用核磁、质谱等现代波谱分析方法对化合物进行结构鉴定。结果得到8个化合物:aspermutarubrol(1)、sydowiols D(2)、sydowiols E(3)、细交链孢酮酸(4)、(E)-5-(羟甲基)-2-(6'-甲基-2'-庚烯-2'-基)酚(5)、sydonol(6)、(7S)-(+)-7-O-methylsydonol(7)和hydroxysydonic acid(8)。化合物2/3和化合物1其DPPH的IC_(50)分别为(9.79±0.33)和(32.30±1.02)μg·mL^(-1),FRAP值分别为(230.55±2.50)和(278.99±8.32)μg·mL^(-1)。结论从碱蓬内生真菌曲霉TQ67中分离得到的化合物2/3和化合物1有较强的抗氧化活性。

目前已明确番茄抗晚疫病R基因表现明显的株龄相关抗性(Age-related Resistance,ARR),但抗晚疫病QTL的抗性规律尚不明确。以携带抗晚疫病基因Ph-3的栽培种番茄(Solanum lycopersicum)‘CLN2037B’和野生种醋栗番茄(***)‘L3708’以及易感病材料栽培种番茄(***)‘LA2818’为对照,对含有抗晚疫病QTL的多毛番茄(***)‘LA2099’、‘LA1033’和‘LA1777’是否也存在株龄相关抗性进行了分析。结果表明,携带抗晚疫病QTL的3个材料的6叶期和9叶期植株病情级数均比3叶期明显降低,表明QTL抗性与株龄相关。利用乙烯、水杨酸和茉莉酸素合成或缺失的突变体和病毒诱导基因沉默(Virus Induced Gene Silencing,VIGS)技术,初步明确了乙烯和水杨酸参与6叶期Ph-3基因介导的对晚疫病的抗性,而茉莉酸不参与。

Resistance(R) genes against plant pathogens often have age-related resistance(ARR) effects. However, the mechanism involved in this phenomenon remains unknown. In this paper, Solanum lycopersicum ‘CLN2037B' and S. pimpinellifolium ‘L3708' harboring the Ph-3 gene, as well as S. habrochaites ‘LA2099', ‘LA1777' and ‘LA1033' harboring quantitative trait loci(QTLs), were tested to investigate age-related resistance against late blight(LB; caused by Phytophthora infestans) in the three-leaf stage of the plants. The results demonstrated that the QTL-related LB resistance showed the same age-related resistance as the Ph-3-mediated resistance at the six- and nine-leaf stages compared with the three-leaf stage. This indicated that there is a common defense mechanism in tomatoes against P. infestans via ARR. In addition, we combined ethylene(ET), salicylic acid(SA) and jasmonic acid(JA) mutants with virus-induced gene silencing(VIGS) to study the Ph-3-dependent resistance signaling pathway. The results showed that ethylene and salicylic acid, but not jasmonic acid, are involved in the LB resistance mediated by the Ph-3 gene.

采用田间自然接种番茄黄花曲叶病毒(TYLCV)的鉴定方法,对来自番茄野生种类番茄茄*** LA2951的渐渗系(Introgression Line,IL)群体进行了筛选,发现类番茄茄LA2951对TYLCV的抗性受多个位点控制。通过分析不同IL的抗性,共鉴定出7个QTL,分别位于染色体1、3、4、5、6、7和12上,其中位于染色体1上的QTL有待于进一步确定。
