Real time endoscopic ultrasound elastography and strain ratio in the diagnosis of solid pancreatic lesions

AIM To evaluate the accuracy of the elastography score combined to the strain ratio in the diagnosis of solid pancreatic lesions(SPL). METHODS A total of 172 patients with SPL identified by endoscopic ultrasound were enrolled in the study to evaluate the efficacy of elastography and strain ratio in differentiating malignant from benign lesions. The semi quantitative score of elastography was represented by the strain ratio method. Two areas were selected, area(A) representing the region of interest and area(B) representing the normal area. Area(B) was then divided by area(A). Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy were calculated by comparing diagnoses made by elastography, strain ratio and final *** SPL were shown to be benign in 49 patients and malignant in 123 patients. Elastography alone had a sensitivity of 99%, a specificity of 63%, and an accuracy of 88%, a PPV of 87% and an NPV of 96%. The best cut-off level of strain ratio to obtain the maximal area under the curve was 7.8 with a sensitivity of 92%, specificity of 77%, PPV of 91%, NPV of 80% and an accuracy of 88%. Another estimated cut off strain ratio level of 3.8 had a higher sensitivity of 99% and NPV of 96%, but with less specificity, PPV and accuracy 53%, 84% and 86%, respectively. Adding both elastography to strain ratio resulted in a sensitivity of 98%, specificity of 77%, PPV of 91%, NPV of 95% and accuracy of 92% for the diagnosis of SPL. CONCLUSION Combining elastography to strain ratio increases the accuracy of the differentiation of benign from malignant SPL.

Crop discrimination through satellite imagery is still problematic. Accuracy of crop classification for high spatial resolution satellite imagery in the intensively cultivated lands of the Egyptian Nile delta is still low. Therefore, the main objective of this research is to determine the optimal hyperspectral wavebands in the spectral range of (400 - 2500 nm) to discriminate between two winter crops (Wheat and Clover) and two summer crops (Maize and Rice). This is considered as a first step to improve crop classification through satellite imagery in the intensively cultivated areas in Egypt. Hyperspectral ground measurements of ASD field Spec3 spectroradiometer was used to monitor the spectral reflectance profile during the period of the maximum growth stage of the four crops. 1-nm-wide was aggregated to 10-nm-wide bandwidths. After accounting for atmospheric windows and/or areas of significant noise, a total of 2150 narrow bands in 400 - 2500 nm were used in the analysis. Spectral reflectance was divided into six spectral zones: blue, green, red, near-infrared, shortwave infrared-I and shortwave infrared-II. One Way ANOVA and Tukey’s HSD post hoc analysis was performed to choose the optimal spectral zone that could be used to differentiate the different crops. Then, linear regression discrimination (LDA) was used to identify the specific optimal wavebands in the spectral zones in which each crop could be spectrally identified. The results of Tukey’s HSD showed that blue, NIR, SWIR-1 and SWIR-2 spectral zones are more sufficient in the discrimination between wheat and clover than green and red spectral zones. At the same time, all spectral zones were quite sufficient to discriminate between rice and maize. The results of (LDA) showed that the wavelength zone (727:1299 nm) was the optimal to identify clover crop while three zones (350:712, 1451:1562, 1951:2349 nm) could be used to identify wheat crop. Self-compacting concrete （SCC） is defined so that no vibration is necessary for the compaction. The main criteria of producing SCC have to satisfy the following characteristics [1, 2, 3]：（1） Ability to flow into and completely fill complex forms under its own weight; （2） Ability to pass through and bond to congested reinforcements; （3） High resistance to aggregate segregation. Self-compacting concrete presents a significant sign in improving the product quality and efficiency of the building industry. It also enhances the working conditions and the quality and appearance of concrete. Japan has been used self-compacting concrete in bridge, building and tunnel construction since the early 1990s. In the last decade, SCC has been produced a high potential for greater acceptance and wider applications in highway bridge construction in the Europe and U.S.. However, till now, there is no application of SCC in the construction industry in Egypt. Therefore, the main objective of this research is to produce SCC by using the locally available materials in our region such as basalt, gravel, sand, limestone powder and silica fume. Experimental programme was designed to characterize the properties of fresh and hardened SCC. It comprises different concrete mixes thbricated with different types and percentages of constituent materials. Three full-scale reinforced concrete beams were fabricated from the SCC mixes and tested under flexure. For the purpose of comparison, an extra RC beam was made of conventional normal concrete to serve as a reference beam. This study, in general, demonstrated that the applications of SCC in construction industry oiler products with enhanced characteristics as well as could be economical.

The Prognostic Value of BMI, Serum Glucose, Endometrial Echo Pattern and Uterine Artery Doppler Velocimetry as a Predictor for Endometrial Pathology in Women with Postmenopausal Bleeding (Prospective Observational Study)

Background: Post-menopausal bleeding is a warning sign that accounts for about 5% of all outpatient gynaecologic visits and is a common indication for referral to rapid access clinics because of the fear of underlying malignancy. Endometrial malignancies differ from other malignancies in that early symptomization is common, allowing early cure. Patients and Methods: During the study period, 100 women with post-menopausal bleeding having inclusion criteria were evaluated in Al Hussein University Hospital. For each patient full history, general, abdominal and pelvic examination was performed. Routine pre-operative investigations were done. Patients were divided into four groups: Group 1 included 29 patients with endometrial polyp. Group 2 included 34 patients with endometrial hyperplasia. Group 3 included 21 patients with atrophic endometrium. Group 4 included 16 patients with endometrial carcinoma. Results: As regards the predictive value of BMI, in the study there was a high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. When discussing the predictive value of blood glucose level, in the study there was a high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. It is worth to mention that the predictive value of endometrial thickness, in the study, was with high statistical significance in comparison between the endometrial carcinoma group and all other benign groups providing the highest specificity and sensitivity. At the last the predictive value of uterine artery velocimetry, in the study, was with high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. Conclusion: BMI, blood glucose level, endometrial thickness and uterine artery velocimetry indices, improve the prediction of endometrial carcinoma in women with post-menopausal bleeding.

In this paper, we study the dynamics of the atomic inversion, scaled atomic Wehrl entropy and marginal atomic Wehrl density for a single two-level atom interacting with SU(1,1) quantum system. We obtain the expectation values of the atomic variables using specific initial conditions. We examine the effects of different parameters on the scaled atomic Wehrl entropy and marginal atomic Wehrl density. We observe an interesting monotonic relation between the different physical quantities for different values of the initial atomic position and detuning parameter.

Introduction: Ovarian cancer is the fourth most prevalent cancer among women and the seventh most common cancer overall. Every year, an estimated 200,000 cases and 125,000 deaths related to ovarian cancer are reported around the world. It is most common in high-resource countries, with an incidence rate of 9.3 per 100,000 women. Ovarian cancer is detected at an advanced stage in about 70% of instances, and only 30% of women with such cancers live for more than 5 years. Although only around 20% of ovarian cancers are limited to the ovaries at diagnosis, patients with localized disease have a 5-year survival rate of more than 90%. Peritonectomy is a crucial part of the surgical treatment for ovarian cancer. Visual inspection and palpation are not reliable methods for determining the extent of tumour involvement. The majority of gynecologic oncologists are suspicious about the benefits of a full peritonectomy, concerned about the benefits and hazards. They believe that the tumor’s fundamental biology, not surgical aggressiveness, dictates the tumor’s resectability. The aim of this work was primarily to compare the differences between total peritoneal gutter removal and random peritoneal biopsy in cases of early ovarian cancer. A secondary aim of this work is to show ability of total peritoneal gutter removal in relation to that of random peritoneal biopsy to detect positivity of metastasis in the histopathological specimens. Objective:To study the antioxidant and anti-inflammatory activity of Butia capitata(***)leaf extracts along with phytochemical analysis of the proposed bioactive ***:Different successive extracts of *** *** were prepared with selective organic solvents and screened for their anti-inflammatory activities in tested animals and invitro antioxidant *** extensive phytochemical investigation of the bioactive extracts through paper chromatography,thin layer chromatography,column chromatography,gasliquid chromatography(GLC),high pressure liquid chromatography and spectral ***,ultraviolet,hydrogen and carbon nuclear magnetic resonance,electron ionization-mass spectrometry,heteronuclear multiple bond correlation and heteronuclear multiple quantum correlation were carried out Results:Results showed that different extracts possess promising antioxidant effect and significant anti-inflammatory activity with variable *** results of the phytochemical investigation of the bioactive extracts revealed the presence of volatile substances,lipoidal matter,α-tocopherol,free sugars,polysaccharides and flavonoidal ***:*** leaf extracts were shown to possess variable antioxidant effect,the most promising was methanol extract Both polar and non polar extracts were proved to have anti-inflammatory activity,the non polar extract was superior in this respect The bioactivity of the extracts was ascribed to the presence of flavonoids,sterols andα-tocopherol.
