Catheter ablation of atrial fibrillation in the elderly

Background Atrial fibrillation （AF） catheter ablation has emerged as a promising treatment strategy for AF, but has not been widely adopted in the elderly population. The present study aimed to determine the safety and efficacy of AF catheter ablation in the elderly popula-tion. Methods and Results The study population consisted of 316 patients with paroxysmal AF who underwent left atrial ablation. Ninety-five patients were≥65 years （48 males, mean age 68.9 ± 3.0 years old） and 221 patients were〈65 years old （130 males, mean age 52.5 ± 10.4 years old）. After a mean follow-up period of 34.0 ± 15.1 months, 55 （57.9%） patients in the elderly group were free from ar-rhythmia recurrence compared with 149 （67.4%） patients in the younger group （p=0.169）. procedural complications were uncommon in both study groups. In logistic regression analysis, left atrial diameter （p=0.003）, hypertension （p=0.001）, dyslipidemia （p=0.039）, and coronary artery disease （p=0.018） were independent predictors of AF recurrence in the elderly population. Conclusions Catheter ablation of AF is safe and effective in older patients. Invasive strategies should be considered as an alternative choice in symptomatic elderly patients with AF.

Elevated Ferritin Levels and the Relationship with Fasting Insulin Levels in Elderly patients with Metabolic Syndrome

Introduction: Elevated serum ferritin levels are associated with insulin resistance, type 2 diabetes and metabolic syndrome (MetS) as well as systemic inflammation and cardiovascular disease. The associations between ferritin and hemoglobin levels with individual components of MetS are unclear. The aims of the study were 1) to compare the ferritin levels, and 2) to investigate the relationships between ferritin, high-sensitivity CRp (hs-CRp), fasting glucose, fasting insulin and homeostasis model assessment (HOMA-IR) levels in elderly patients. Subjects and Methods: Study population included 121 (mean age 64.3 ± 14.1 yrs) (80 female, 41 male) elderly patients. The study population was evaluated for MetS by Adult Treatment panel III (ATpIII). Demographic and biochemical data such as fasting insulin, hs-CRp, fasting glucose and ferritin levels were evaluated. Biochemical data were evaluated retrospectively. Insulin resistance (IR) was estimated using the HOMA. Results: Metabolic syndrome was diagnosed in 39 elderly patients (32.2%). In elderly patients with MetS, mean levels of ferritin, hs-CRp, fasting glucose, fasting insulin and HOMA were found to be 72.9 ± 33.1 ng/ml, 0.90 ± 0.01, 99.1 ± 20.1 mg/dl, 13.4 ± 1.1 μU/l, 3.0 ± 0.1, respectively. However, mean levels of ferritin, hs-CRp, fasting glucose, fasting insulin and HOMA were found to be 54.1 ± 33.1 ng/ml, 0.67 ± 0.1, 91.9 ± 17.0 mg/dl, 8.4 ± 2.7 μU/l, 2.71 ± 0.9, in the other elderly patients, (p?= 0.0012), (p?= 0.70), (p?= 0.70), (p?= 0.003), (p?= 0.80) respectively. Mean levels of ferritin were positively correlated with diastolic (r = 0.850,?p =?0.03), systolic blood pressures (r = 0.700,?p =?0.02), and fasting insulin (r = 0.444,?p =?0.003) in elderly with MetS. Conclusions: Mean levels of ferritin were increased in elderly patients with metabolic syndrome. Listening to Turkish classical music decreases patients’anxiety,pain,dissatisfaction and the dose of sedative and analgesic drugs during colonoscopy:A prospective randomized controlled trial

AIM： To determine whether listening to music decreases the requirement for dosages of sedative drugs, patients＇ anxiety, pain and dissatisfaction feelings during colonoscopy and makes the procedure more comfortable and acceptable. METHODS： patients undergoing elective colonoscopy between October 2005 and February 2006 were randomized into either listening to music （Group 1, n = 30） or not listening to music （Group 2, n = 30）. Anxiolytic and analgesic drugs （intravenous midazolam and meperidine） were given according to the patients＇ demand. Administered medications were monitored. We determined their levels of anxiety using the State-Trait Anxiety Inventory Test form. patients＇ satisfaction, pain, and willingness to undergo a repeated procedure were self-assessed using a visual analog scale. RESULTS： The mean dose of sedative and analgesic drugs used in group 1 （midazolam： 2.1 ± 1.4, meperidine： 18.1 ± 11.7） was smaller than group 2 （midazolam： 2.4 ± 1.0, meperidine： 20.6 ± 11.5）, but without a significant difference （p 〉 0.05）. The mean anxiety level in group 1 was lower than group 2 （36.7 ± 2.2 vs 251.0 ± 1.9, p 〈 0.001）. The mean satisfaction score was higher in group 1 compared to group 2 （87.8 ± 3.1 vs 58.1 ± 3.4, p 〈 0.001）. The mean pain score in group i was lower than group 2 （74.1 ± 4.7 vs 39.0 ± 3.9, p 〈 0.001）. CONCLUSION： Listening to music during colonoscopy helps reduce the dose of sedative medications, as well as patients＇ anxiety, pain, dissatisfaction during the procedure. Therefore, we believe that listening to music can play an adjunctive role to sedation in colonoscopy. It is a simple, inexpensive way to improve patients＇ comfort during the procedure.

Insulin receptor substrate gene polymorphisms are associated with metabolic syndrome but not with its components

Aim: Metabolic syndrome (MetS) is a major risk factor for both diabetes mellitus and cardiovascular disease (CVD). The aims of the study were 1) to investigate the insulin receptor substrate-1 (IRS-1) and insulin receptor substrate-2 (IRS-2) gene polymorphisms in patients with MetS and 2) to examine the relationships between gene polymorphisms and components of MetS. patients & Methods: The study population included 100 patients with MetS and 30 patients without MetS as control group. Metabolic syndrome (MS) was defined as in ATp III. Entire coding exons of IRS-1 and IRS-2 genes were amplified by polymerase chain reaction (pCR). Insulin resistance (IR) was estimated using the homeostasis model assessment (HOMA). Results: In patients with MetS, 34 (34%), had G972R (rs1801278) gene polymorphism and 66 (66%) had no nucleotide substitutions at the IRS-1 gene (p circumference, blood pressure, triglyceride, HDL-Cholesterol, LDL-Cholesterol and HOMA-IR levels. Conclusion: Insulin receptor substrate-1 and 2 gene polymorphisms were associated with metabolic syndrome but not its components.

Therapeutic effect of caffeic acid phenethyl ester on cerulein-induced acute pancreatitis

AIM: To evaluate the therapeutic role of caffeic acid phenethyl ester (CApE) in a rat model of ceruleaninduced acute pancreatitis (Ap).METHODS: Seventy male Wistar albino rats were divided into seven groups. Acute edematous pancreatitis was induced by subcutaneous cerulein injection (20 μg/kg) four times at 1-h intervals. CApE (30 mg/kg) was given by subcutaneous injection at the beginning (CApE 1 group) and 12 h after the last cerulein injection (CApE 2 group). Serum amylase, lipase, white blood cell count, and tumor necrosis factor (TNF)-α levels were measured, and pancreatic histopathology was assessed. RESULTS: In the Ap group, amylase and lipase levels were found to be elevated and the histopathological evaluation showed massive edema and inflammation of the pancreas, with less fatty necrosis when compared with sham and control groups. Amylase and lipase levels and edema formation decreased signif icantly in the CApE therapy groups (p pecially in the CApE 2 group, edema was improved nearly completely (p = 0001). Inflammation and fatty necrosis were partially recovered by CApE treatment. The pathologicalresults and amylase level in the placebo groups were similar to those in the Ap group. White blood cell count and TNF-α concentration was nearly the same in the CApE and placebo ***: CApE may be useful agent in treatment of Ap but more experimental and clinical studies are needed to support our observation of benef icial effects of CApE before clinical usage of this agent.

postdilation is frequently used during coronary interventions to prevent stent *** there are contradictory fi ndings regarding the benefi ts of postdilation for both intraprocedural and long-term *** evaluated the impact of postdilation among patients who presented with acute coronary syndromes(ACS)and underwent percutaneous coronary interventions(pCI).A total of 258 consecutive patients who presented with ACS and underwent pCI were included in the *** patients were followed up for 25±1.7 months for the occurrence of major adverse cardiovascular events(MACE).During follow-up,65 patients(25.2%)had *** patients without MACE,intracoronary nitrate infusion was less frequently used(p=0.005),myocardial blush grade was higher(pp=0.005).No signifi cant differences were noted between groups regarding the predilation,recurrent dilation,postdilation,and other angiographic *** multivariate analysis,female sex(p=0.047),myocardial blush grade(p=0.038),previous coronary artery disease(p=0.030),and peak troponin level(p=0.002)were found to be predictors of *** patients who were treated with pCI for ACS,performing postdilation did not predict fi nal Thrombolysis in Myocardial Infarction(TIMI)fl ow grade,corrected TIMI frame count,myocardial blush grade,or MACE.

The predictive Role of New Markers in Early Diagnosis of Acute Kidney Injury in patients with Acute pancreatitis

Objective: Acute Kidney Injury (AKI) is a serious early complication that significantly increases the mortality rate in patients with acute pancreatitis (Ap). AKI can improve with an early diagnosis and appropriate treatment protocols. We aimed to evaluate the role of IL-6, IL-18, NGAL, IL-1β, TNF-α, Cys-C and KIM-1 in serum and IL-18 in urine in the early detection of AKI in patients with Ap. Materials and Methods: Fifty-six patients, who were diagnosed with Ap and underwent standard treatment, between July 2011 and December 2013, were included in the study. patients were sampled for markers at admission and discharge, or when the patients died. patients were grouped by presence or absence of AKI. Results: The mean age of the cases was 56.41 ± 20.23 (17 - 91), of which 21 (37.5%) of the cases were male and 35 (62.5%) were female. AKI was detected in 13 (23.2%) patients. The Cys-C level was significantly higher in AKI patients (at hospitalization p pitalization were significantly higher than discharge levels (p patients with AKI was higher than the patients without AKI during admission (p positive correlation between serum KIM-1 and creatinine, but a negative correation between GFR and spot urine amylase/creatinine ratio during AKI patients’ admission (respectively;p p p Conclusion: In this study, Cys-C and KIM-1 proved to be statistically significant to predict AKI in patients with Ap. However further studies are required to support these conclusions.
