Financial time series forecasting could be beneficial for individual as well as institutional investors. But, the high noise and complexity residing in the financial data make this job extremely challenging. Over the years, many researchers have used support vector regression (SVR) quite successfully to conquer this challenge. In this paper, an SVR based forecasting model is proposed which first uses the principal component analysis (PCA) to extract the low-dimensional and efficient feature information, and then uses the independent component analysis (ICA) to preprocess the extracted features to nullify the influence of noise in the features. Experiments were carried out based on 16 years’ historical data of three prominent stocks from three different sectors listed in Dhaka Stock Exchange (DSE), Bangladesh. The predictions were made for 1 to 4 days in advance targeting the short term prediction. For comparison, the integration of PCA with SVR (PCA-SVR), ICA with SVR (ICA-SVR) and single SVR approaches were applied to evaluate the prediction accuracy of the proposed approach. Experimental results show that the proposed model (PCA-ICA-SVR) outperforms the PCA-SVR, ICA-SVR and single SVR methods.

Background: Endometriosis is an important cause of infertility and chronic pelvic pain. Our study aims to understand the cause and effect relationship of endometriosis with these two clinical presentations. Objectives: 1) To study the correlation between stage of endometriosis with pain and infertility;2) To evaluate role of transvaginal ultrasound for diagnosis of endometriosis using laparoscopic diagnosis as gold standard. Methodology: Total of 89 women presenting with infertility and or pain and with laparoscopic findings of endometriosis were included in the study. The results were analysed by logistic regression. Results: The mean age of women was 29.67 years. Seventy three percent of patients had moderate to severe disease while 31.46% of patients with endometriosis were infertile. Symptom of pain could be categorised as dysmenorrhea (71.47%), dyspareunia (41.5%) and dyschezia (15.7%). There was no association of endometriosis stage with the pain type, duration of symptoms. Transvaginal ultrasound seems a reliable test for diagnosis of ovarian endometriosis and advanced stage disease. 87.5% of patients with ovarian endometrioma had advanced disease (higher than Stage II rAFS). Conclusion: No correlation was found between the stage of endometriosis and severity of symptoms. Presence of endometrioma on USG was associated with advanced stage of endometriosis.

Objective: To assess the perinatal outcome in patients with cord presentation and cord prolapse over a period of 2 years and to assess the obstetric risk factors associated with these cord complications. Design: Retrospective case study. Setting: A tertiary health care centre of India. Sample: All cases of cord presentation and cord prolapse diagnosed in the department over a period of 2 years (2009-2010). Methods: The information regarding maternal presentation at the time of admission, mode of delivery, and neonatal outcome was retrieved from case sheets of patients available in Medical Record Department. Maternal factors in terms of age, parity, gestational age, mode of presentation, mode of delivery were analysed. Foetal outcome was assessed by Diagnosis to Delivery Interval, APGAR score at 0 minutes and 5 minutes, foetal weight and need for NICU admission. Results: There were 53 cases of cord complications, out of which 7 (13.20%) had cord presentation and 46 (86.80%) had cord prolapse. Among these 46 patients, 11 patients developed cord prolapse in labour room (3 after artificial rupture of membranes and 8 after spontaneous rupture of membranes), rest 34 patients presented in emergency with cord prolapse. There were 79.24% multiparous patients, 16 patients had malpresentations and breech was commonest malpresentation. Forty one patients had positive cord pulsations at the time of admission, all these patients underwent emergency caesarean section and all had live babies. Twelve patients reported to us with absent cord pulsation, 3 among these underwent section for obstructed labour with hand and cord prolapse, rest 9 delivered vaginally. The average DDI was 26.00 minutes, DDI in patients developed cord prolapse in hospital was 18.90 minutes ± 5.48 minutes, whereas in patients admitted with cord prolapse as emergency, the DDI was 29.34 minutes ± 6.37 minutes (p < 0.05). Echocardiographic Evaluation of Cardiac Dyssynchrony in Patients with Congestive Heart Failure

The present study investigated the application of echocardiography to evaluation of cardiac dyssynchrony in patients with congestive heart failure（CHF）. A total of 348 consecutive CHF patients who were admitted for cardiac resynchronization（CRT） and presented with low ejection fraction（EF） and wide QRS duration were enrolled in this study, along with 388 healthy individuals. Dyssynchrony was assessed based on filling time ratio（FT/RR）, left ventricular pre-ejection delay（PED）, interventricular mechanical delay（IVMD）, longitudinal opposing wall delay（LOWD） and radial septal to posterior wall delay（RSPWD）. Response to CRT was defined as a ≥15% increase in EF. The results showed that FT/RR was decreased while PED, IVMD, LOWD and RSPWD were increased in the CHF group compared with the control group（P〈0.01）. In the CHF group, FT/RR was negatively correlated with the QRS duration, LV end-diastolic diameter（LVESd）, LV end-diastolic volume（LVEDV） and LV end-systolic volume（LVESV）（P〈0.01）, but positively with the LVEF（P〈0.01）. Additionally, PED, IVMD, LOWD and RSPWD were positively correlated with the QRS duration, LVESd, LVEDV and LVESV（P〈0.01）, but negatively with the LVEF（P〈0.01）. The CHF group was divided into three subgroups according to the varying degrees of LVEF. FT/RR decreased successively from the LVEF-1 group to the LVEF-2 group to the LVEF-3 group, while the PED, IVMD, LOWD and RSPWD successively increased in the same order（P〈0.01）. The CHF group was divided into three subgroups according to the varying degrees of QRS duration, and FT/RR decreased successively in a sequence from the QRS-1 group to the QRS-2 group to the QRS-3 group, while the PED, IVMD, LOWD and RSPWD successively increased in the same order（P〈0.01）. Speckle tracking radial dyssynchrony ≥130 ms was predictive of an EF response in patients in QRS-1 group（78% sensitivity, 83% specificity）, those in QRS-2 group（83% sensitivity, 77% specificity） and in QRS-3 group（89% sensitivity, 79% specificity）.

AIM To report our one-year experience with computer assisted propofol sedation(CAPS) for colonoscopy as the first United States Medical Center to adopt CAPS technology for routine clinical *** Between September 2014 and August 2015, 2677 patients underwent elective outpatient colonoscopy with CAPS at our center. All colonoscopies were performed by 1 of 17 gastroenterologists certified in the use of the CAPS system, with the assistance of a specially trained nurse. Procedural success rates, polyp detection rates, procedure times and recovery times were recorded and compared against corresponding historical measuresfrom 2286 colonoscopies done with midazolam and fentanyl from September 2013 to August 2014. Adverse events in the CAPS group were *** The mean age of the CAPS cohort was 59.9 years(48.7% male); 31.3% were ASA?Ⅰ, 67.3% ASA Ⅱ and 1.4% ASA Ⅲ. 45.1% of the colonoscopies were for screening, 31.5% for surveillance, and 23.4% for symptoms. The mean propofol dose administered was 250.7 mg(range 16-1470 mg), with a mean fentanyl dose of 34.1 mcg(0-100 mcg). The colonoscopy completion and polyp detection rates were similar to that of historical measures. Recovery times were markedly shorter(31 min vs 45.6 min, P < 0.001). In CAPS patients, there were 20(0.7%) cases of mild desaturation(< 90%) treated with a chin lift and reduction or temporary discontinuation of the propofol infusion, 21(0.8%) cases of asymptomatic hypotension(< 90 systolic blood pressure) treated with a reduction in the propofol rate, 4(0.1%) cases of marked agitation or discomfort due to undersedation, and 2 cases of pronounced transient desaturation requiring brief(< 1 min) mask ventilation. There were no sedation-related serious adverse events such as emergent intubation, unanticipated hospitalization or permanent injury. CONCLUSION CAPS appears to be a safe, effective and efficient means of providing moderate sedation for colonoscopy in relatively healthy patients. Recovery
