We report a rare case of a small-bowel lymphangioma causing massive gastrointestinal (GI) bleeding that we successfully diagnosed and treated using double-balloon enteroscopy (DBE). An 81-year-old woman suffering from repeated GI bleeding of unknown origin underwent a capsule endoscopy at a previous hospital. She was suspected of having bleeding from the jejunum, and was referred to our department for diagnosis and treatment. An oral DBE revealed a 20 mm × 10 mm, regularly surfaced, white to yellowish, elongated, pedunculated jejunal polyp with small erosions at 10 cm distal to the ligament of Treiz. Since no other source of bleeding was identified by endoscopy in the deep jejunum, anendoscopic polypectomy (EP) was performed for this lesion. A subsequent histopathological examination of the resected polyp showed clusters of lymphatic vessels with marked cystic dilatation in the submucosa and the deep layer of the lamina propria mucosae. These characteristics are consistent with the typical features of small-bowel lymphangioma with erosions. Although clip-ping hemostasis was performed during EP, re-bleeding occurred. Finally, a complete hemostasis was achieved by performing an additional argon plasma coagulation.

Solvent Composition Effect on PDMS Composite Dielectric Elastomer Actuator

Polydimethylsiloxane (PDMS) is inactive to electric fields, but when combined with ferroelectric cyanoethyl sucrose (CR-U), it becomes an electrically active actuator material. The PDMS/CR-U composites were prepared by casting method using tetrahydrofuran (THF) and acetone (ACT) as solvents. The effect of a mixed solvent composition was investigated. The viscosity of the PDMS/CR-U/solvent solution decreased as the THF composition increased. The composite film obtained by evaporation of the solvent and cross-linkage of PDMS showed a phase-separated structure. Spherical CR-U dispersed in the PDMS matrix with a skin layer on one surface side. Electrical resistance, dielectric constant, space-charge distribution, and electrically induced bending deformation behavior were investigated for these composite films. The composite films prepared from a THF-rich solvent exhibited lower surface resistivity than those prepared from a THF-poor solvent. Applying an electric field to the composite film resulted in an asymmetric space-charge distribution with charge accumulation in the skin layer. THF content decreased the viscosity of the solution, meaning the decrease of the apparent size of PDMS chain aggregates. It allows the dispersion of CR-U in the PDMS matrix and also results in the decline of resistivity, the increase of permittivity, and the increase of charge injection. The results explained the structure formation of the composite film and the electric field response as an actuator. A casting solvent of THF content of 87.5 wt% or more is essential for the function of the PDMS/CR-U composite.

Trends in Dietary Intakes and Serum Cholesterol Levels over 50 Years in Tanushimaru in Japanese Men

We investigated the relationship between time trends in dietary intakes and serum cholesterol levels in a Japanese cohort of the Seven Countries Study, in Tanushimaru, a typical farming town on Kyushu Island. Total subjects were 628 in 1958, 539 in 1977, 602 in 1982, 752 in 1989, 402 in 1999, and 329 in 2009, and all of the subjects were men aged 40 - 64 years. Eating patterns were evaluated by 24-hour dietary recall from 1958 through 1989, and by a food frequency questionnaire in 1999 and 2009. We also measured serum cholesterol levels in each health examination. The total energy intake decreased from 2,837 kcal in 1958 to 2,289 kcal in 2009. The carbohydrate intake in percentage of total energy intake decreased markedly, from 84% in 1958 to 58% in 2009, in contrast to large increases during this period in protein intake (from 11% to 16%) and fat intake (from 5% to 22%). In proportion to the dramatic change in protein and fat intake, serum cholesterol levels showed large increases (from 152.5 mg/dl to 207.7 mg/dl). Despite of such big dietary changes toward a westernized diet, incidence of coronary heart disease has not increased in our cohort for a couple of decades. However, careful surveillance is needed in the future because of the remarkably increasing intake of fats.

Usefulness of Immuno-Magnetic Beads Conjugated with Anti-EpCAM Antibody for Detecting Endometrial Cancer Cells

A simple and non-invasive method for detecting endometrial cancer in women with abnormal uterine bleeding is required. For this purpose, we prepared immuno-magnetic beads conjugated with anti-human EpCAM rat monoclonal antibody (mAb) for isolating exfoliated endometrial cells including endometrial cancer cells in vaginal discharge. The affinities of the anti-human EpCAM rat mAbs were analyzed by flow cytometry and immunocytochemistry and then magnetic beads were conjugated with the mAbs. The rate of retrieval of endometrial cells using the immuno-magnetic beads was calculated. Endometrial cells were isolated using the immuno-magnetic beads from the vaginal discharges of 22 patients with endometrial cancer and 16 non-malignant controls. The isolated cells were stained using endometrial cancer specific-mAbs and analyzed by flow cytometry and imaging cytometry. The immuno-magnetic beads conjugated with high-affinity mAb (clone 1456) appeared to have very low auto-fluorescence. Sufficient enrichment of Ep-CAMpositive cells using immuno-magnetic beads was observed in both simulation and clinical samples. The overall sensitivities of flow cytometry and imaging cytometry to detect endometrial cancer cells were 72.7% and 45.5%, respectively. Meanwhile, the overall specificities of flow cytometry and imaging cytometry for healthy controls were 75.0% and 81.3%, respectively. Our immuno-magnetic beads have very low auto-fluorescence, so they could be useful for fluorescent analysis, such as fluorescent immunochemical staining. In the future, these novel immuno-magnetic beads could be used for cytological study.
