We propose an efficient algorithm for solving strictly convex quadratic programs with box constraints (i.e., lower and upper bounds on each variable). our algorithm is based on the active set and the Newton method. We repeatedly compute relevant inverse matrices efficiently and, starting from an initial feasible poillt, we find an optimal solution of the problem in finitely many steps. Our experimental results show that the new algorithm is practically efficient.

Fistula between digestive tract and airway is one of the complications after esophagectomy with lymph node dissection. A case of esophagotracheal fistula secondary to esophagitis 9 years after esophagectomy and gastric pull-up for treatment of esophageal carcinoma is described. It was successfully treated with transposition of a pedicled pectoralis major muscle flap.

Successful treatment for esophageal carcinoma with lung metastasis by induction chemotherapy followed by salvage esophagectomy: Report of a case

We here report a case of a 51-year-old man with lung metastasis from esophageal carcinoma that was initially treated by combination chemotherapy consisting of fluorouracil and nedaplatin. Because metastatic disease disappeared, salvage esophagectomy was performed. Although chest wall recurrence developed at the thoracotomy wound, prolonged survival of 48 months was achieved by local tumor resection and additional chemotherapy. This combination chemotherapy is regarded as a promising and considerable treatment for metastatic esophageal carcinoma.

For a healthy and productive life, good steep is essential, which has prompted studies on how comfortable sleep can be achieved. Understanding the relationship between thermal environment and physiological responses such as skin and core temperatures, and psychological responses such as thermal and sleep sensations is necessary to identify the most suitable thermal environment for steep. As an energy-saving and practical method of creating the most appropriate thermal environment for sleep, local heating or cooling is sometimes used, which takes into consideration the differences in local thermal responses. We performed this study to identify the most effective thermal environment for inducing comfortable sleep by identifying the physiological responses during sleep on the basis of sleep experiments conducted under local body heating or cooling conditions. We also used a human thermal model, which can be applied for predicting physiological responses. In the experiments, the feet of the subject were the primary area to be heated or cooled, which was achieved by installing a flexible duct with an outlet placed close to the subject＇s feet and inlet connected to an air conditioner. Differences in the fluctuation of body temperature and sleep stage depended on the airflow direction from the duct to the feet. When air was blown downward towards the feet, body temperature decreased and the subject was able to sleep well. Measured skin and core temperatures were calculated using an improved 27-node human thermal model that was originally developed for use in subjects who are not in steep. Although skin

This report describes a rare case of metastatic hepatocellular carcinoma (HCC) presenting a huge mass in the left external auditory canal (EAC). The patient was a 55-year-old man with hepatitis B virus-related HCC. He presented to our department with a three-month history of increasing left otalgia,and hearing loss with recent fresh aural bleeding. Histopathologic examination indicated that the tumor was secondary to HCC. Although external irradiation was not effective,the tumor was treated with surgical debulking and high dose rate 192 Ir remote afterloading system (RALS) for postoperative intracavitary irradiation. A review of the literature revealed only five other cases of HCC metastasis to the temporal bone,all of which mainly metastasized in the internal acoustic meatus. The present case is the first report of HCC metastasis to the EAC.

Factors Predicting Late Rectal Disorders after Radiation Therapy for Prostate Cancer

Background： Although various studies have been conducted on the effects of radiation therapy for prostate cancer, rectal toxicity after radiation therapy for prostate cancer, which is an important late adverse event associated with radiation therapy, has not been sufficiently examined. This study aimed to assess the associations of late rectal disorder （LRD） with dosimetric, anatomic, and clinical factors in patients with prostate cancer who underwent three-dimensional conformal radiation therapy （3D-CRT）. Methods： We retrospectively analyzed 104 patients undergoing 3D-CRT between January 2009 and October 2011. Thirty patients were administered anticoagulation/antiplatelet （AC/AP） agents. The standard dose was 74 Gy. Uni- and multi-va,iate analyses we,e performed to identify factors predictive of LRD after 3D-CRT. Results： The median follow-up period was 66 （range： 14 87） months. LRD occurred in 10.6% （11/104） of patients. The median time fi＇om RT to LRD was 15 months （range： 7MI months）. Sixty-four percent of those with LRD （7/11 patients） had been given AC/AP agents. Fitly-five （6/11 ） patients had severe internal iliac artery calcification. By univariate analysis, significant predictors of LRD were internal iliac artery calcification, administration of AC/AP agents, and age. Being very elderly was the significant predictor identified by multivariate analysis （P = 0.0276）. For patients receiving AC/AP agents and those with severe internal iliac artery calcification, the LRD incidences were 23.3% （7/30 patients） and 23.1% （6/26 patients）, respectively, and being 75 years of age or older was a significant predictor in these subsets. Conclusions： Our results suggest advanced age, administration of AC/AP agents, and severe internal iliac artery calcification to be risk factors tbr LRD in patients undergoing standard RT.
