CT Thresholds for Spinal Bone Mineral Attenuation to Prioritize Eligible Older Women for Osteoporosis Screening

Introduction: Computed tomography (CT) measurements of bone mineral attenuation may be a useful means to identify older women who should be prioritized for bone mineral density screening. Methods: We compared bone mineral attenuation as measured in the L1 vertebra of CT studies to the results of dual-energy x-ray absorptiometry (DEXA) studies to determine what CT attenuation thresholds might yield a reasonable combination of sensitivity and specificity for the detection of osteoporosis. The study was limited to women between the ages of 65 and 75 years who had a DEXA study and a CT that included the L1 or adjacent vertebra performed within 3 years of the DEXA study. Results: There were 1226 women in this study, of whom 452 (38%) had osteoporosis based on a T-score ≤ −2.5 by DEXA. There were 830 CT studies performed with contrast and 396 studies which were performed without contrast. There was a statistically significant difference in the mean HU of those studies performed without contrast compared to those with contrast (unenhanced mean 103 HU versus 125 HU, p < 0.001). Different CT attenuation thresholds provided the most appropriate combination of sensitivity and specificity for the detection of osteoporosis when comparing CT studies performed without or with IV contrast and when all the CT data were used in aggregate. Conclusion: Different thresholds appear necessary when using the mean CT vertebral attenuation to identify older women for preferential referral for DEXA studies.

Sk.n colour is an important criterion in determining the aesthetic appeal of ornamental fishes,and thus their mark.t worth.Hence the pigment enhancement in ornamental fishes has been the subject of extensive research by using various synthetic as well as natural pigment sources.Dietary composition is one of the most important elements that influence the development of pigmentation in fish.This study is an attempt to assess the possibility of using carotenogenic marine yeast as pigment enhancer in fish diet for k.i carp,Cyprinus carpio.The 60 days feeding experiment was done by using feed incorporated with mangrove associated marine yeast Rhodotorula paludigena VA242.Greater pigmentation in the scales of fishes fed with experimental feed was observed than that fed with the control diet(normal pellet feed).Furthermore the improved growth rate and general wellbeing in fishes fed with experimental feed provides scope for further studies in this field with respect to the growth and survival of fishes.

A Hybrid Machine Learning Approach for Improvised QoE in Video Services over 5G Wireless Network.

Video streaming applications have grown considerably in recent years.As a result,this becomes one of the most significant contributors to global internet traffic.According to recent studies,the telecommunications industry loses millions of dollars due to poor video Quality of Experience(QoE)for users.Among the standard proposals for standardizing the quality of video streaming over internet service providers(ISPs)is the Mean Opinion Score(MOS).However,the accurate finding of QoE by MOS is subjective and laborious,and it varies depending on the user.A fully automated data analytics framework.is required to reduce the inter-operator variability characteristic in QoE assessment.This work.addresses this concern by suggesting a novel hybrid XGBStack.oE analytical model using a two-level layering technique.Level one combines multiple Machine Learning(ML)models via a layer one Hybrid XGBStack.oE-model.Individual ML models at level one are trained using the entire training data set.The level two Hybrid XGBStack.oE-Model is fitted using the outputs(meta-features)of the layer one ML models.The proposed model outperformed the conventional models,with an accuracy improvement of 4 to 5 percent,which is still higher than the current traditional models.The proposed framework.could significantly improve video QoE accuracy.

Enhancing Multicriteria-Based Recommendations by Alleviating Scalability and Sparsity Issues Using Collaborative Denoising Autoencoder

A Recommender System(RS)is a crucial part of several firms,particularly those involved in e-commerce.In conventional RS,a user may only offer a single rating for an item-that is insufficient to perceive consumer preferences.Nowadays,businesses in industries lik. e-learning and tourism enable customers to rate a product using a variety of factors to comprehend customers’preferences.On the other hand,the collaborative filtering(CF)algorithm utilizing AutoEncoder(AE)is seen to be effective in identifying user-interested items.However,the cost of these computations increases nonlinearly as the number of items and users increases.To triumph over the issues,a novel expanded stack.d autoencoder(ESAE)with k.rnel Fuzzy C-Means Clustering(k.CM)technique is proposed with two phases.In the first phase of offline,the sparse multicriteria rating matrix is smoothened to a complete matrix by predicting the users’intact rating by the ESAE approach and users are clustered using the k.CM approach.In the next phase of online,the top-N recommendation prediction is made by the ESAE approach involving only the most similar user from multiple clusters.Hence the ESAE_k.CM model upgrades the prediction accuracy of 98.2%in Top-N recommendation with a minimized recommendation generation time.An experimental check.on the Yahoo!Movies(YM)movie dataset and TripAdvisor(TA)travel dataset confirmed that the ESAE_k.CM model constantly outperforms conventional RS algorithms on a variety of assessment measures.

The term hepatolithiasis describes the presence of biliary stones within the intrahepatic bile ducts,above the hilar confluence of the hepatic ducts.The disease is more prevalent in Asia,mainly owing to socioeconomic and dietary factors,as well as the prevalence of biliary parasites.In the last century,owing to migration,its global incidence has increased.The main pathophysiological mechanisms involve cholangitis,bile infection and biliary strictures,creating a self-sustaining cycle that perpetuates the disease,frequently characterised by recurrent episodes of bacterial infection referred to as syndrome of“recurrent pyogenic cholangitis”.Furthermore,long-standing hepatolithiasis is a k.own risk.factor for development of intrahepatic cholangiocarcinoma.Various classifications have aimed at providing useful insight of clinically relevant aspects and guidance for treatment.The management of symptomatic patients and those with complications can be complex,and relies upon a multidisciplinary team of hepatologists,endoscopists,interventional radiologists and hepatobiliary surgeons,with the main goal being to offer relief from the clinical presentations and prevent the development of more serious complications.This comprehensive review provides insight on various aspects of hepatolithiasis,with a focus on epidemiology,new evidence on pathophysiology,most important clinical aspects,different classification systems and contemporary management.

The thymus is the primary production site for a functional and self-tolerant T-cell repertoire.The establishment of immune tolerance is mediated by the generation and presentation of tissue-restricted self-antigens(TRAs)by medullary thymic epithelial cells(mTECs),which screen T-cell receptors for self-reactivity to eliminate autoreactive T cells.Recent studies have extended our understanding of mTEC heterogeneity by examining the mechanisms by which mTECs mimic the peripheral self[1].

Characterization of Small-Scale Farmers and Assessment of Their Access to Crop Production Information in Selected Counties of k.nya

Small-scale farming accounts for 78% of total agricultural production in k.nya and contributes to 23.5% of the country’s GDP. Their crop production activities are mostly rainfed subsistence with any surplus being sold to bring in some income. Timely decisions on farm practices such as farm preparation and planting are critical determinants of the seasonal outcomes. In k.nya, most small-scale farmers have no reliable source of information that would help them mak. timely and accurate decisions. County governments have extension officers who are mandated with giving farmers advisory services to farmers but they are not able to reach most farmers due to facilitation constraints. The mode and format of sharing information is also critical since it’s important to ensure that it’s timely, well-understood and usable. This study sought to assess access to geospatial derived and other crop production information by farmers in four selected counties of k.nya. Specific objectives were to determine the profile of small-scale farmers in terms of age, education and farm size;to determine the type of information that is made available to them by County and Sub-County extension officers including the format and mode of provision;and to determine if the information provided was useful in terms of accuracy, timeliness and adequacy. The results indicated that over 80% of the farmers were over 35 years of age and over 56% were male. Majority had attained primary education (34%) or secondary education (29%) and most farmers in all the counties grew maize (71%). Notably, fellow farmers were a source of information (71%) with the frequency of sharing information being mostly seasonal (37%) and when information was available (43%). Over 66% of interviewed farmers indicating that they faced challenges while using provided information. The results from the study are insightful and helpful in determining effective ways of providing farmers with useful information to ensure maximum benefits.

In this editorial,we comment on the article entitled“Advances and k.y focus areas in gastric cancer immunotherapy:A comprehensive scientometric and clinical trial review(1999-2023),”which was published in the recent issue of the World Journal of Gastroenterology.We focused on the results of the authors’bibliometric analysis concerning gastric cancer immunotherapy,which they analyzed in depth by compiling the relevant publications of the last 20 years.Before that,we briefly describe the most recent data concerning the epidemiological parameters of gastric cancer(GC)in different countries,attempting to give an interpretation based on the etiological factors involved in the etiopathogenesis of the neoplasm.We then briefly discuss the conservative treatment(chemotherapy)of the various forms of this malignant neoplasm.We describe the treatment of resectable tumors,locally advanced neoplasms,and unresectable(advanced)cases.Special attention is given to modern therapeutic approaches with emphasis on immunotherapy,which seems to be the future of GC treatment,especially in combination with chemotherapy.There is also a thorough analysis of the results of the study under review in terms of the number of scientific publications,the countries in which the studies were conducted,the authors,and the scientific centers of origin,as well as the clinical studies in progress.Finally,an attempt is made to draw some conclusions and to point out possible future directions.

Cool white light and tunable multicolor emission from Tb^(3+)/Dy^(3+)co-activated glasses under different excitations for WLEDs

A set of co-doped(Tb^(3+)/Dy^(3+))lithium zinc borate(LZB)glasses were developed by melt quenching.The structural evaluation was performed for synthesized glassy matrices.The Dy^(3+)and Tb^(3+)individually doped glasses exhibit intense yellow and green luminescence bands at 575 nm(^(4)F_(9/2)→^(6)H_(13/2))and543 nm(^(5)D_(4)→^(7)F_(5)),respectively.The sensitization effect of Dy^(3+)on Tb^(3+)was analyzed by increasing the Tb^(3+)content with respect to the optimum Dy^(3+)content(0.5 mol%)in Dy^(3+)/Tb^(3+).The spectral overlay of Dy^(3+)luminescence and Tb^(3+)absorption profiles,Dy^(3+)/Tb^(3+)PL spectra under different excitations 352,362,376,and 385 nm,shortening decay lifetimes of Dy^(3+)in Dy^(3+)/Tb^(3+)co-activated glasses,energy transfer(ET)parameters,chromaticity coordinates and their corresponding correlated temperatures all help to explain ET from Dy^(3+)to Tb^(3+).At 385 nm of Dy^(3+)excitation,the optimized co-activated(0.5Dy^(3+)+1.0Tb^(3+)):LZB glass displays cool white light emission.The non-radiative ET from Dy^(3+)to Tb^(3+)is dominated by electric dipole-dipole interaction and its ET efficiency was calculated to be 63%.At the same time,reverse ET from Tb^(3+)to Dy^(3+)was also analyzed.The shift in color coordinates from dominant yellow to greenish-yellow,green and white light emission suggests that Dy^(3+)/Tb^(3+)coactivated LZB glasses can be a potential candidate for UV converted multicolor and white light emitting devices.

Compression properties of cost-efficient porous expanded clay reinforced AA7075 syntactic foams fabricated by industrial-oriented die casting technology

In today’s manufacturing industries,hard competition between rival firms mak.s it compulsory for researchers to design lighter and cheaper machine components due to the megatrends of cost-effectiveness and anti-pollution.At this point,aluminum syntactic foams(ASFs)are new-generation engineering composites and come into the upfront as a problem-solver.Owing to their features lik. low density,sufficient elongation,and perfect energy absorption ability,these advanced foams have been considerably seductive for many industrial sectors nowadays.In this study,an industrial-oriented automatic die casting technology was used for the first time to manufacture the combination of AA7075/porous expanded clay(PEC).Micro evaluations(optical and FESEM)reveal that there is a homogenous particle distribution in the foam samples,and inspections are compatible with the other ASF studies.Additionally,T6 aging heat treatment was operated on one half of the produced foams to explore the probable impact of aging on the compressive responses.Attained results show that PEC particles can be an alternative to expensive hollow spheres used in the previous work..Besides,a favorable relationship is ascertained between the aging treatment and mechanical properties such as compression strength and plateau strength.
