Background: Evidence regarding the effectiveness of prenatal nutritional supplements has mainly considered anthropometric pregnancy outcomes. The effect on markers of health and disease， such as offspring telomere length (TL) and mitochondrial DnA content (mtDnAc) is unknown. Objectives: We assessed the efficacy of maternal multiple micronutrient (MMn)-fortified balanced-energy protein (BEP) and iron-folic acid (IFA) supplementation on newborn TL as a secondary outcome and mtDnAc as a non-declared outcome. Design: We conducted a randomized controlled trial in rural Burkina Faso， among pregnant females (15-40 years old) enrolled at n. Mothers received either MMn-fortified BEP and IFA (intervention) or IFA only (control) throughout pregnancy. Whole arterial blood samples were collected from the umbilical cord of 104 control and 90 intervention group infants， respectively. Average relative TL and mtDnAc were measured using quantitative polymerase chain reaction. Linear regression models were fitted to assess TL and mtDnAc differences across trial arms. Results: We found that a combined daily MMn-fortified BEP supplement and IFA tablet did not affect newborn TL [β = -0.010 (95% CI: -0.057， 0.036); P = 0.662] or mtDnAc [β = 0.065 (95% CI: -0.203， 0.073); P = 0.354]， as compared to an IFA tablet alone. These findings were confirmed (P > 0.05) by adjusting the regression models for potential prognostic factors of study outcomes at enrollment. Exploratory analyses indicated higher， but non-significantly different mtDnAc among children born either small-for-gestational age， low birthweight， or preterm. Conclusions: newborns from mothers who received daily nutritional supplements across gestation did not have different relative TL or mtDnAc.

Software systems have been employed in many fields as a means to reduce human efforts;consequently,stakeholders are interested in more updates of their *** smells arise as one of the obstacles in the software *** are characteristics of software source code that indicate a deeper problem in *** smells appear not only in the design but also in software *** smells introduce bugs,affect software maintainability,and lead to higher maintenance *** code smells can be formulated as an optimization problem of finding the best detection *** researchers have recommended different techniques to improve the accuracy of code smell detection,these methods are still unstable and need to be *** research has sought only to discover a few at a time(three or five types)and did not set rules for detecting their *** research improves code smell detection by applying a search-based technique;we use the Whale Optimization Algorithm as a classifier to find ideal detection *** this algorithm,the Fisher criterion is utilized as a fitness function to maximize the between-class distance over the withinclass *** proposed framework adopts if-then detection rules during the software development life *** rules identify the types for both medium and large *** are conducted on five open-source software projects to discover nine smell types that mostly appear in *** proposed detection framework has an average of 94.24%precision and 93.4%*** accurate values are better than other search-based algorithms of the same *** proposed framework improves code smell detection,which increases software quality while minimizing maintenance effort,time,and ***,the resulting classification rules are analyzed to find the software metrics that differentiate the nine code smells.

DnA methylation is a chromatin modification that is often associated with the exciti ng and sometimes unpredictable patter ns of inheritanee that can unfold with epigenetic phenomena. The stability and heritability of DnA methylati on patter ns perhaps allow us to utilize DnA methylation profiles to distil the suite of potentially functional elements in large crop genomes. Here, we discuss the potential and possible ways to use the absence of DnA methylation to identify potential regulatory regions within intergenic sequences and the presence of DnA methylation to identify pseudogenes.

Distribution of licensed acupuncturists and educational institutions in the United States in early 2015

In recent decades, acupuncture has been used more widely and extensively in the United States （U.S.）. However, there have been no national surveys or analyses reported in academic journals on the number of practicing or licensed acupuncturists, This study was conducted to identify the approximate number of licensed acupuncturists active in 2015. The Board of Acupuncture or Board of Medicine in each state or U.S. territory was contacted to collect data, Online license information searching was also performed in order to get accurate numbers of licensed acupuncturists for those states in which a board was unable to be contacted. The study found that the number of licensed acupuncturists in 2015 in the U,S, was 34,481. Of this, more than 50% were licensed in three states alone： California （32.39%）, new York （11,89%） and Florida （7.06%）. The number of licensed acupuncturists increased 23.30% and 52.09%, compared to the year 2009 （n = 27,965） and 2004 （n = 22,671）, respectively; increasing about 1,266 per year. There were 62 and 10 accredited acupuncture institutions providing master and doctoral degrees, respectively. The West Coast comprised 51.39% of degree granting programs, while the East Coast comprised 29.17%; together the coastal states housed more than 80% of all programs, with the remainder sprinkled across the southern （9.72%）, northern （8.33%）, and the middle/central states （1.39%）. BACKGROUnD Secondary haemophagocytic lymphohistiocytosis(sHLH)is a rare lifethreatening condition mainly associated with underlying infections,malignancies,and autoimmune or immune-mediated *** To analyse all sHLH cases that were diagnosed and managed under real-world circumstances in our department focusing on the treatment schedule and the *** Prospectively collected data from all adult patients fulfilling the criteria of sHLH who diagnosed and managed from January 1,2010 to June 1,2018,in our department of the tertiary care university hospital of Larissa,Greece,were analysed retrospectively(n=80;52%male;median age:55 years).The electronic records and/or written charts of the patients were reviewed for the demographic characteristics,clinical manifestations,underlying causes of sHLH,laboratory parameters,treatment schedule and 30-d-mortality *** of patients had received after consent intravenousγ-immunoglobulin(IVIG)for 5 d(total dose 2 g/kg)in combination with intravenous steroid pulses followed by gradual tapering of *** Seventy-five patients(94%)reported fever>38.5°C,47(59%)had liver or spleen enlargement and 76(95%)had ferritin>500 ng/mL including 20(25%)having considerably high levels(>10000 ng/mL).Anaemia and thrombocytopenia occurred in 72%and leucopoenia in 47%of *** infections were diagnosed in 59 patients(74%)as follows:leishmaniasis alone in 15/80(18.9%),leishmaniasis concurrently with Coxiella Burnetti or non-Hodgkin lymphoma in 2/80(2.5%),bacterial infections in 14/80(17.5%)including one case with concurrent non-Hodgkin lymphoma,viral infections in 13/80(16.3%),fungal infections in 2/80(2.5%),infections by mycobacteria in 1/80(1.3%)and unidentified pathogens in 12/80(15%).Seventy-two patients(90%)had received combination treatment with IVIG and intravenous ***,sHLH resolved in 76%of patients,15%died within the first month but 82.5%of patients were still alive 6 mo after ***

Profile and determinants of unsuccessful tuberculosis outcome in rural nigeria:Implications for tuberculosis control

AIM: To determine the treatment outcomes and predictors for unsuccessful tuberculosis(TB) outcomes in rural ***: Adult rural TB patients treated during 2011 and 2012 in two healthcare facilities(one urban public and one rural private) were identified from the TB treatment registers and retrospectively reviewed. Tuberculosis treatment outcomes were assessed according to World Health Organisation guidelines. Determinants of unsuccessful treatment outcomes were identified using a multivariable logistic regression ***: Between January 2011 to December 2012, 1180 rural TB patients started treatment, of whom 494(41.9%) were female. The treatment success rate was 893(75.7%), while the rates of death, loss-tofollow-up, and treatment failure were 129(10.9%), 100(8.5%), and 18(1.5%) respectively. In the final multivariable logistic regression model, the odds of unsuccessful treatment outcome were higher among patients who received care at the urban public facility(a OR = 2.9, 95%CI: 1.9-4.4), smear-negative(1.3,1.0-1.8) and extrapulmonary(2.7, 1.3-5.6) TB patients, human immunodeficiency virus(HIV) co-infected(2.1, 1.5-3.0), and patient who received the longer(8-mo) anti-TB regimen(1.3, 1.1-1.8).COnCLUSIOn: Treatment success among rural TB patient in nigeria is low. High risk groups should be targeted for closer monitoring, socio-economic support, and expansion of TB/HIV activities.
