Systematic review: Renin-angiotensin system inhibitors in chemoprevention of hepatocellular carcinoma

BACKGROUND Neoangiogenesis is one of the key pathogenetic mechanisms in hepatocellular carcinoma (HCC). Modulation of the renin-angiotensin system (RAS) by angiotensin-converting enzyme inhibitors (ACE-Is) and angiotensin receptor blockers (ARBs) seems to be a possible adjuvant therapy for HCC, due to the antiangiogenic and anti-fibrogenic activity of these drugs. AIM To elucidate the role of ARBs and ACE-Is in HCC. METHODS We performed an electronic search of the literature using the most accessed online databases (PubMed, Cochrane library, Scopus and Web of Science), entering the query terms "angiotensin-converting enzyme inhibitors" OR "ACE inhibitors" OR "ACE-I" AND "hepatocarcinoma*" OR "hepatocellular carcinoma;moreover "angiotensin II type 1 receptor blockers" OR "ARBs" AND "hepatocarcinoma*" OR "hepatocellular carcinoma". Eligibility criteria were:(1) prospective or retrospective clinical studies;(2) epidemiological studies;and (3) experimental studies conducted in vivo or in vitro. Abstracts, conference papers, and reviews were excluded a priori. We limited our literature search to articles published in English, in peer-reviewed *** Thirty-one studies were selected. Three interventional studies showed that ACEIs had a significant protective effect on HCC recurrence only when used in combination with vitamin K or branched chain aminoacids, without a significant increase in overall survival. Of six retrospective observational studies, mainly focused on overall survival, only one demonstrated a prolonged survival in the ACE-Is group, whereas the two that also evaluated tumor recurrence showed conflicting results. All experimental studies displayed beneficial effects of RAS inhibitors on hepatocarcinogenesis. Numerous experimental studies, conducted either on animals and cell cultures, demonstrated the anti-angiogenetic and antifibrotic effect of ACE-Is and ARBs, thanks to the suppression of some cytokines such as vascular endothelial growth factor

金线莲液(喷雾用)在COVID-19大流行期间表现出良好的临床治疗效果,但其物质基础及作用机制尚不明确。本研究拟通过网络药理学及分子对接的方法预测金线莲液防治新冠的分子机制,结合体外药效学实验对当前具有明确新冠防治效果的靶点与金线莲液重点成分的相互作用进行研究。首先运用UPLC-MS结合数据库比对分析金线莲液中活性成分,经SwissADME数据库体内药代动力学过程筛选得到类药性较好的潜在活性成分17个,SwissTargetPrediction、GeneCards检索获得化合物疾病共有靶点93个。随后运用Cytoscape 3.8.2软件构建“成分−靶点”网络图,以Metascape平台进行基因功能注释和富集通路分析,发现金线莲液主要通过异鼠李素、山柰酚、木犀草素、槲皮素、芹菜素等活性成分与共有靶点(NOS3、MPO、MMP3等)结合,调节对外部刺激的积极反应、炎症反应、细胞因子产生等生物过程,发挥抗COVID-19的作用。血管紧张素转换酶2(angiotensin-converting enzyme 2,ACE2)活性测定实验表明金线莲液(喷雾用)原液及除去多糖后的上清液(主要为黄酮类成分)对ACE2酶活性有一定的抑制作用。最后,采用2019-nCOV-S蛋白假病毒感染HEK-293T-ACE2细胞,发现金线莲液可能主要通过阻断新冠病毒S蛋白与ACE2的结合发挥抗病毒作用。

幼苗是森林生物多样性保育的重点关注对象,以往研究对云南普洱季风常绿阔叶林幼苗的关注较少。为探究该地幼苗的群落构建机制,该研究利用30hm2季风常绿阔叶林动态监测样地野外幼苗调查数据,分析幼苗的物种组成,划分不同优势种样方,根据逐步群落构建模型分析群落构建机制,进一步分析幼苗的群落功能性状。结果表明:季风常绿阔叶林幼苗以短刺锥(Castanopsis echidnocarpa)和枹丝锥(***)为优势种,根据优势种是否出现将样方划分为4种类型:短刺锥样方、枹丝锥样方、混合优势种样方及非优势种样方。幼苗的群落构建过程均包含随机扩散构建(贡献率43.1%-61.3%)、生境过滤(贡献率27.4%-33.9%)及限制相似性(贡献率5.7%-27.2%)机制。短刺锥样方和枹丝锥样方以确定过程为主,贡献率分别为56.9%和54.6%,而混合优势种样方及非优势种样方则以随机过程为主,贡献率分别为60.4%和61.3%。在非优势种样方中Rao二次熵(Rao’Q)最高,而在混合优势种样方中最低。短刺锥样方具有最低的比叶面积和较高的叶厚度、比茎长度、根质量分数及潜在株高,而枹丝锥样方及非优势种样方有更大的比叶面积。不同幼苗样方中,比茎长度和潜在株高变异系数较大,而其他功能性状变异水平较低。多元回归分析显示,不同幼苗样方中,比叶面积和潜在株高均和Rao’Q显著正相关,除了非优势种样方外,其他样方叶厚度和生活型均与Rao’Q显著正相关。因此,季风常绿阔叶林幼苗的群落构建同时包含随机过程和确定过程,两种过程的贡献率随群落类型的不同而不同。

Prevention and treatment of hepatic encephalopathy: Focusing on gut microbiota

The gut flora plays an important role in the pathogenesis of the complications of cirrhosis. Hepatic encephalopathy (HE) represents a broad continuum of neuropsychological dysfunction in patients with acute or chronic liver disease and/or porto-systemic shunting of blood flow and it manifests with progressive deterioration of the superior neurological functions. The pathophysiology of this disease is complex, as it involves overproduction and reduced metabolism of various neurotoxins, particularly ammonia. Management of HE is diversified and requires several steps: elimination of precipitating factors, removal of toxins, proper nutritional support, modulation of resident fecal flora and downregulation of systemic and gut-derived inflammation. This review will provide an overview of gut barrier function and the influence of gut-derived factors on HE, focusing on the role of gut microbiota in the pathogenesis of HE and the recent literature findings on its therapeutic manipulation.

Vertebroplasty and delayed subdural cauda equina hematoma: Review of literature and case report

Vertebroplasy is considered an alternative and effective treatment of painful oncologic spine disease. Major complications are very rare, but with high morbidity and occur in less than 1% of patients who undergo vertebroplasty. Spinal subdural hematoma(SDH) is an extremely rare complication, usual developing within 12 h to 24 h after the procedure. We report the case of a tardive SDH in an oncologic patient who underwent VP for Myxoid Liposarcoma metastasis. Trying to explain the pathogenesis, we support the hypothesis that both venous congestion of the vertebral venous plexus of the vertebral body and venous congestion due to a traumatic injury can provoke SDH. To our best knowledge, only 4 cases of spinal subdural hematoma following a transpedicular vertebroplasty have been previously described in International literature and only one of them occurred two weeks after that surgical procedures. Percutaneous verteboplasty is a wellknown treatment of pain oncologic spine disease, used to provide pain relief and improvement of quality life and is considered a simple surgical procedure, involving a low risk of complications, but related to high morbidity, such as SDH. Therefore it has to be performed by experienced and skilled surgeons, that should also recognize possible risk factors, making SDH more risky.

Intracameral lidocaine as supplement to classic topical anesthesia for relieving ocular pain in cataract surgery

AIM: To evaluate safety, efficacy, and patient adherence of intracameral lidocaine as supplement of classic topical anesthetic drops in cataract ***: A prospective and controlled trial including a large cohort of 1650 individuals suffering with bilateral cataract not complicated, in program by phacoemulsification surgery, were randomly assigned to 2 different groups for the type of anesthesia received, 0.4% oxybuprocaine hydrochloride(INN) drops, and INN drops associated to intracameral 1% lidocaine hydrochloride monohydrate. At the end of surgery, tables were assigned to each patient indicating the degree of pain(0-3) felt during the ***: Thirty-two percent of patients in group 1 declared to have not felt any pain against the 77% of patients in group 2. Fifty-nine percent of patients in group 1 complained about only a slight discomfort against 20% of group 2 patients. Only a small percentage of patients in group 1(5%) admitted severe pain, while no patient in group 2 admitted severe pain. Four patients of group 2 reported an episode of transient amaurosis, lasting several hours after ***: Intracameral administration of lidocaine is a simple and secure method able to increase the analgesia during the cataract surgery, eliminating the discomfort and increasing also the cooperation of the patients during the steps of manipulation.

近50年来青藏高原的土壤温度、水分条件发生显著改变。前期许多研究关注了温度或水分变化对青藏高原微生物群落的影响,对于放牧背景下增温增水对微生物群落多样性和组成的耦合影响研究相对较少。本研究在青藏高原纳木错典型高寒草甸生态系统展开,采用增温增雪耦合放牧的原位控制实验,研究了土壤细菌群落组成、多样性和功能类群对增温、增雪和放牧及其交互作用的响应。结果表明,增温增雪和放牧及其交互作用对土壤细菌整体的多样性和组成没有显著的影响,增雪放牧显著影响了细菌群落的丰富度;增温、增雪和放牧改变了植物种群和微生物部分种群的相对丰度;增温增雪和放牧都降低了多种细菌功能类群的数量。在气候暖湿化的趋势下,青藏高原细菌多样性、组成和相对丰度不会在短时间内发生巨大的改变。
