Long-term mental health outcomes after unintentional burns sustained during childhood:a retrospective cohort study

Background:Burns are a devastating injury that can cause physical and psychological issues. Limited data exist on long-term mental health (mH) after unintentional burns sustained during childhood. This study assessed long-term mH admissions after paediatric burns. methods:This retrospective cohort study included all children (matched uninjured comparison cohort (n=46,548). Linked hospital, mH and death data were examined. multivariable negative binomial regression modelling was used to generate incidence rate ratios (IRR) and 95%confidence intervals (CI). Results:The burn cohort had a significantly higher adjusted rate of post-burn mH admissions compared to the uninjured cohort (IRR, 95%CI:2.55, 2.07–3.15). Post-burn mH admission rates were twice as high for those younger than 5 years at index burn (IRR, 95%CI 2.06, 1.54–2.74), three times higher for those 5–9 years and 15–18 years (IRR, 95%CI:3.21, 1.92–5.37 and 3.37, 2.13–5.33, respectively) and almost five times higher for those aged 10–14 (IRR, 95%CI:4.90, 3.10–7.76), when compared with respective ages of uninjured children. The burn cohort had higher admission rates for mood and anxiety disorders (IRR, 95%CI:2.79, 2.20–3.53), psychotic disorders (IRR, 95%CI:2.82, 1.97–4.03) and mental and behavioural conditions relating to drug and alcohol abuse (IRR, 95%CI:4.25, 3.39–5.32). Conclusions:Ongoing mH support is indicated for paediatric burn patients for a prolonged period after discharge to potentially prevent psychiatric morbidity and associated academic, social and psychological issues.

Understanding the factors that facilitate the emergence of cooperation among organisms is central to the study of social evolution. Spotted hyenas Crocuta crocuta frequently cooperate to mob lions Panthera leo, approaching the lions as a tightknit group while vocalizing loudly in an attempt to overwhelm them and drive them away. Whereas cooperative mobbing behavior has been well documented in birds and some mammals, to our knowledge it has never been described during interactions between 2 apex predators. Using a 27-year dataset, we characterize lion-hyena encoun- ters, assess rates of mobbing behavior observed during these interactions, and inquire whether mobbing results in successful acquisition of food. Lions and hyenas interacted most often at fresh kills, especially as prey size and the number of hyenas present increased. Possession of food at the beginning of an interaction positively affected retention of that food by each predator species. The presence of male lions increased the probability of an interspecific interaction but decreased the likelihood of hyenas obtaining or retaining possession of the food. Hyena mobbing rates were highest at fresh kills, but lower when adult male lions were present. The occurrence of mobbing was predicted by an increase in the number of hyenas present. Whether or not mobbing resulted in acquisition of food from lions was predicted by an increase in the number of mobs formed by the hyenas present, suggesting that cooperation among hyenas enhances their fitness.

The Tongariro Power Development Scheme（TPDS） is used to regulate flow in the headwaters of the largest catchment on the North Island of New Zealand（the Waikato）.Two small dams,the Rangipo Dam and the Poutu Intake Dam,were constructed in 1973 and *** flow regime of the river is managed to divert freshes into the power scheme,but allows flows larger than 100 m;s;to be released,to rework and transport sediment through the catchment. The reproductive potential of overwintering boll weevil, Anthonomus grandis grandis （Boheman）, females collected from pheromone traps in September, November and January, fed for 1, 3, and 5 weeks on plant pollens, and then provided cotton squares, was determined in the laboratory at 27 ~ I^C, 65% RH, and a photoperiod 13 ： 11 （L ： D） h. Duration of pollen feeding by overwintering boll weevils did not significantly influence egg and feeding punctures, or puncture ratios （egg to total punctures） for any of the three months of parent weevil collections when provided cotton squares on a daily basis. However, punctures and puncture ratios are significantly different when comparing mean data between months of boll weevil collections. When boll weevils were provided with cotton squares daily, the pre-ovipositional periods of female parents captured in September, November and January were 5, 9 and 14 days, respectively. The rate of eggs by females was significantly lower during November and January than September. Female parents collected in September produced a significantly higher percentage of eggs yielding adult progeny than those collected in November and January. Life table parameters indicated that net reproductive rate （Ro） of boll weevil females collected in September was 1.2- fold higher than those collected in November and 10.7-fold higher than those collected in January. Except for testes size, no differences in male reproductive parameters were observed during the cotton-free period compared with males captured during mid-cotton （June）. The number of oocytes in the ovarioles and the number of ooeytes containing yolk were significantly lower during September, November and January compared with June. The reproductive potential ofoverwintering boll weevil females collected in different months is an important consideration in determining the success of any control strategy.

Zinc(Zn) deficiency in paddy soils is often a problem for rice *** can decrease metal availability in some noncalcareous soils through different mechanisms associated with soil redox *** experiments were performed in order to better understand the processes that governed the dynamics of Zn in non-calcareous paddy soils at varying redox potentials(Eh).Airdried non-calcareous soil samples collected from four different paddy field sites in the Philippines were submerged and incubated in a reaction cell with continuous stirring and nitrogen purging for 4 weeks,and then purged with compressed air for another week to reoxidize the *** Eh of the four soils started at 120 to 300 mV,decreased to —220 to —300 mV after 100 to 250 h of reduction,and was maintained at this low plateau for about 2 weeks before increasing again upon *** solubility showed contrasting patterns in the four soils,with two of the soils showing a decrease in soluble Zn as the Eh became low,probably due to zinc sulfide(ZnS) *** contrast,the other two soils showed that Zn solubility was maintained during the reduced phase which could be due to the competition with iron(Fe) for precipitation with *** in the relative amounts of S,Fe,and manganese(mn) oxides in the four soils apparently influenced the pattern of Zn solubility after flooding.

Endoscopic ultrasound staging for early esophageal cancer: Are we denying patients neoadjuvant chemo-radiation?

AIm To evaluate the accuracy of endoscopic ultrasound(EUS) in early esophageal cancer(EC) performed in a highvolume tertiary cancer center. mETHODS A retrospective review of patients undergoing esophagectomy was performed and patients with c T1 N0 and c T2 N0 esophageal cancer by EUS were evaluated. Patient demographics, tumor characteristics, and treatment were reviewed. EUS staging was compared to surgical pathology to determine accuracy of EUS. Descriptive statistics was used to describe the cohort. Student's t test and Fisher's exact test or χ~2 test was used to compare variables. Logistic regression analysis was used to determine if clinical variables such as tumor location and tumor histology were associated with EUS *** Between 2000 and 2015, 139 patients with clinical stage Ⅰ or Ⅱ?A esophageal cancer undergoing esophagectomy were identified. There were 25(18%) female and 114(82%) male patients. The tumor location included the middle third of the esophagus in 11(8%) and lower third and gastroesophageal junction in 128(92%) patients. Ninety-three percent of patients had adenocarcinoma. Preoperative EUS matched the final surgical pathology in 73/139 patients for a concordance rate of 53%. Twenty-nine patients(21%) were under-staged by EUS; of those, 19(14%) had unrecognized nodal disease. Positron emission tomography(PET) was used in addition to EUS for clinical staging in 62/139 patients. Occult nodal disease was only found in 4 of 62 patients(6%) in whom both EUS and PET were negative for nodal involvement. CONCLUSION EUS is less accurate in early EC and endoscopic mucosal resection might be useful in certain settings. The addition of PET to EUS improves staging accuracy.
