Pre-operative total parenteral nutrition improves post-operative outcomes in a subset of crohn’s disease patients undergoing major abdominal surgery

Protocol for the development and validation of a clinical measurement tool for fear of disease progression and recurrence in cardiac patients

Introduction: One in two cardiac patients fear having another heart event or their heart condition getting worse. Research in other chronic illnesses demonstrates that screening for fear of progression and recurrence is vital for adequately addressing such concerns in clinical care. The current project aims to develop and validate a measure for fear of progression and recurrence in cardiac ***: The Fear of cardiac Recurrence and Progression Scale (FcRP) will be developed through a multistep process. An initial item pool will be generated through a review of the literature and existing measures and consultation with and feedback from key informants. The item pool will be tested in a sample of over 250 adults who have ever had an acute coronary event, undergone cardiac surgery, or a chronic cardiac condition. Exploratory factor analysis will be used to identify the underlying factors, and Rasch analysis will be used to reduce the number of items. A short form version of the FcRP will be developed for use as a brief screening tool, informed by clinical relevance and Rasch psychometric ***: While many cardiac patients experience fears related to the progression or recurrence of their illness, there remains the need for a validated tool with which these concerns can be identified and measured. It is expected that the design and validation of the FcRP will aid identification of cardiac patients suffering from clinically significant levels of fear of progression and recurrence and facilitate the design of tailored psychological interventions to target these fears.

This study explores the role of Bedouins in preserving Palestinian land through their own means of community resilience through the concept of“Art of being”and its entailed“Arts of presence and persistence”.This is examined through the correlation between Bedouin lifestyle and the spatial distribution of structures within“Al-Kurshan”Bedouin clan in Area c in the West *** on a mixed qualitative and spatial methodologies,information was gathered through semi-structured interviews with Al-Kurshan Bedouins and employees of organizations supporting Bedouins,on-site observation,and analyses of maps and photos,in addition to drawings and *** spatial distribution of Bedouin built structures is analyzed in relation to landscape and climate,social aspects within the families,and available service and sectorial *** analyses give insight on how Bedouins struggle against adopting other *** study shows that the Bedouins are slowly forced to negotiate their semi-nomadic lifestyle,but resist giving up their right to exist against the threat of forced *** maintain their“Art of being”which partly represents their own way of safeguarding Palestinian lands from *** study contributes to the wider discussion of spatial dynamics of Bedouins and their community resilience within the Palestinian geopolitical context.

BAcKGROUND Due to development of an immune-dysregulated phenotype,advanced liver disease in all forms predisposes patients to sepsis acquisition,including by opportunistic pathogens such as *** data exists on fungal infection within a medical intensive liver unit(MILU),particularly in relation to acute on chronic liver *** To investigate the impact of fungal infections among critically ill patients with advanced liver disease,and compare outcomes to those of patients with bacterial *** From our prospective registry of MILU patients from 2018-2022,we included 27 patients with culture-positive fungal infections and 183 with bacterial *** compared outcomes between patients admitted to the MILU with fungal infections to bacterial *** was extracted through chart *** All fungal infections were due to candida species,and were most frequently blood *** among patients with fungal infections was significantly worse relative to the bacterial cohort(93%vs 52%,Pcohort developed grade 2 or 3 acute on chronic liver failure(AcLF)(90%vs 64%,P=0.02).Patients in the fungal cohort had increased use of vasopressors(96%vs 70%,P=0.04),mechanical ventilation(96%vs 65%,Pcute kidney injury(78%vs 52%,P=0.014).On MILU admission,the fungal cohort had significantly higher Acute Physiology and chronic Health Evaluation(108 vs 91,P=0.003),Acute Physiology Score(86 vs 65,P=0.003),and Model for End-Stage Liver Disease-Sodium scores(86 vs 65,P=0.041).There was no significant difference in the rate of central line use preceding culture(52%vs 40%,P=0.2).Patients with fungal infection had higher rate of transplant hold placement,and lower rates of transplant;however,differences did not achieve statistical *** Mortality was worse among patients with fungal infections,likely attributable to severe AcLF *** studies examinin

Macroalgae are multicellular,aquatic autotrophs that play vital roles in global climate maintenance and have diverse applications in biotechnology and eco-engineering,which are directly linked to their multicellularity ***,their genomic diversity and the evolutionary mechanisms underlying multicellularity in these organisms remain *** this study,we sequenced 110 macroalgal genomes from diverse climates and phyla,and identified key genomic features that distinguish them from their microalgal *** for cell adhesion,extracellular matrix formation,cell polarity,transport,and cell differentiation distinguish macroalgae from microalgae across all three major phyla,constituting conserved and unique gene sets supporting multicellular *** genes show phylum-and climate-specific expansions that may facilitate niche ***,our study reveals genetic determinants of convergent and divergent evolutionary trajectories that have shaped morphological diversity in macroalgae and provides genome-wide frameworks to understand photosynthetic multicellular evolution in aquatic environments.

A discoid meniscus is a morphological abnormality wherein the meniscus loses its normal‘c’*** most patients are asymptomatic,patients might still present with symptoms such as locking,pain,swelling,or giving *** resonance imaging is usually needed for confirmation of *** on a constellation of factors,including clinical and radiological,different approaches are chosen for the management of discoid *** purpose of this review is to outline the treatment of discoid meniscus,starting from conservative approach,to the different surgical options for this *** PubMed and Google Scholar databases were used for this *** discussing the treatment of discoid meniscus from 2018 to 2023 were *** there were 369 studies retrieved,and after removal of studies using the exclusion criteria,26 studies were included in this *** such as stability,presence of tear,and morphology can help with surgical *** approaches have been used to treat discoid meniscus,where the choice is tailored for each patient ***,factors that may positively impact patient outcomes include male sex,body mass indexconventional approach is partial meniscectomy with or without repair;however,recently,there has been an increased emphasis on discoid-preserving techniques such as meniscoplasty,meniscopexy,and meniscal allograft transplantation.

We present radial velocity(RV)curve templates of RR Lyrae first-overtone(RRc)stars constructed with the Mg I b triplet and Hαlines using time-domain Medium-Resolution Survey spectra of seven RRc stars from Large Sky Area Multi-Object Fiber Spectroscopic Telescope(LAMOST)Data Release ***,we derive the relation between the stellar RV curve amplitudes and g-band light curve amplitudes from Zwicky Transient Facility(ZTF)public *** those RRc stars without ZTF g-band light curves,we provide the conversions from the light curve amplitudes in ZTF r-and i-bands,Gaia G-band,and V-band from the All-Sky Automated Survey for Supernovae to those in ZTF *** validate our RV curve templates using the RRc star SV Scl and find the uncertainties of systemic RV are less than 2.11 km s~(-1)and 6.08 km s~(-1)based on the Mg I b triplet and Hαlines,*** calculate the systemic RVs of 30 RRc stars using the RV curve templates constructed with the Mg I b triplet and Hαlines and find the systemic RVs are comparable with each *** RV curve template will be particularly useful for obtaining the systemic RV of RRc using the LAMOST spectroscopy.

The global increase in the prevalence of type 2 diabetes mellitus(T2DM)and its complications presents significant challenges to public ***,periodontal disease(PD)was recognized as a factor that is likely to influence the progression of T2DM and its complications due to its potential to exacerbate systemic inflammation and oxidative *** this editorial,we comment on the article published by Thazhe Poyil et al in the very recent issue of the World Journal of Diabetes in 2024,which investigated the correlation between PD and diabetic retinopathy(DR)in T2DM patients,with emphasis on the association between periodontal swollen surface area,glycated hemoglobin(HbA1c),interleukin-6(IL-6),and lipoprotein(a).The findings by Thazhe Poyil et al are significant as they demonstrate a strong link between PD and DR in T2DM *** correlation highlights the importance of addressing periodontal health in diabetes management to potentially reduce the risk and severity of DR,a complication of *** integration of periodontal evaluation and treatment into diabetes care protocols may lead to improved glycemic control and better overall outcomes for T2DM patients.A few studies have established an interconnection between PD and diabetic complication,specifically DR,in T2DM patients,which we aim to highlight in this *** was placed on the different mechanisms that suggest a bidirectional relationship between PD and T2DM,where the presence of periodontal inflammation negatively influenced glycemic control and contributed to the development and progression of DR through shared inflammatory and vascular *** article highlights the importance of collaboration amongst diabetes specialists,ophthalmologists,periodontists,and public health professionals to advance the prevention,early detection,and treatment of PD and *** will improve the health and quality of life of T2DM ***,the editorial highlights the need for further re
