AIM:To assess the hepatitis B virus(HBV)-DNA and the prevalence of occult HBV infection in end-stage renal failure(ESRF)patients from Central Greece. METHODS:Sera from 366 ESRF patients attending five out of six dialysis units from Central Greece were investigated for HBV-DNA by real-time polymerase chain *** serum samples with repeatedly detectable HBV-DNA were considered *** antibodies to hepatitis C virus(anti-HCV)were tested by a third generation enzyme linked immunosorbent assay(ELISA),while IgG antibodies to hepatitis E virus (anti-HEV)were tested by two commercially available ***:HBV-DNA was detected in 15/366 patient (4.1%)and HBsAg in 20/366(5.5%).The prevalenc of occult HBV infection was 0.9%(3/346 HBsAg negative patients).Occult HBV was not associate with a specific marker of HBV infection or anti-HCV o anti-HEV *** was no significant differenc in HBV-DNA titres,demographic and biochemica features,between patients with occult HBV infectio and those with HBsAg-positive chronic HBV infection. CONCLUSION:In central Greece,4%of ESRF patient had detectable HBV-DNA,though in this setting,th prevalence of occult HBV seems to be very low(0.9%).

AIM: Various side effects have been reported in patients infected with hepatitis C virus (HCV) who were treated with interferon-alpha (IFN-α), including the appearance or exacerbation of underlying autoimmune diseases and the development of a variety of organ and non-organ specific autoantibodies (NOSA). However, very few studies in adults have been strictly designed to address: whether the prevalence and the titre of organ and NOSA in serial samples of HCV-treated patients were affected by IFN-α, and the impact of these autoantibodies on the treatment outcome of HCV patients. METHODS: We investigated whether parietal cell autoantibodies (pCA) and/or NOSA were related with treatment-outcome in 57 HCV-treated patients (19 sustained-responders, 16 relapsers, 22 non-responders). Serum samples from patients were studied blindly at three time-points (entry, end of treatment and end of followup). For the detection of autoantibodies we used indirect immunofluorescence, commercial and in-house ELISAs. RESULTS: Sustained biochemical response was associated with ANA-negativity at the entry or end of follow up. Sustained virological response was associated with the absence of pCA at the entry. Combined virological and biochemical sustained response (CVBSR) was associated with the absence of antinuclear antibodies (ANA) at the end of follow up and pCA-negativity at the entry. Sustained virological and CVBSR were associated with a reduction of ANA and SMA titers during therapy. CONCLUSION: Although pCA and/or NOSA seropositivity should not affect the decision to treat HCV patients, the presence of some of them such as ANA, pCA and SMA before treatment or their increase during therapy with IFN- a may predict a worse response, indicating the need for a closer monitoring during treatment of HCV patients positive for these autoantibodies.

Reversion of severe hepatopulmonary syndrome in a non cirrhotic patient after corticosteroid treatment for granulomatous hepatitis:A case report and review of the literature

Hepatopulmonary syndrome （HpS） is defined as a clinical triad including liver disease, abnormal pulmonary gas exchange and evidence of intrapulmonary vascular dilatations. We report a 61-year-old male presented with fatigue, long-lasting fever, loss of weight, signs of portal hypertension, hepatosplenomegaly, cholestasis and progressive dyspnoea over the last year. Clinical, laboratory and histological findings confirmed the diagnosis of granulomatous hepatitis. HpS due to hepatic granulomainduced portal hypertension was proved to be the cause of severe hypoxemia of the patient as confirmed by contrast-enhanced echocardiography. Reversion of HpS after corticosteroid therapy was confirmed by a new contrastenhanced echocardiography along with the normalization of cholestatic enzymes and improvement of the patient＇ s conditions. This is the first case of complete reversion of HpS in a non-cirrhotic patient with hepatic granuloma, indicating that intrapulmonary shunt in liver diseases is a functional phenomenon and HpS can be developed even in miscellaneous liver involvement as in this case.

Africa’s participation in Digital Earth is *** is a tendency to ascribe this state to prevailing governance and cultural challenges in ***,foreign actors such as donors have an apparent role in shaping geospatial policies and *** far,the complex linkages between external aid and improved social and environmental monitoring and decision-making have been handled as a kind of‘black box’.To better understand the situation,we open the box and focus on the interaction between donors and *** use a heuristic from political science,as well as empirical evidence,to describe the policyinfluencing tools that donors employ based on four basic resources donors possess:organization,authority,treasure,and *** show an evolution of tool usage as donors shift from‘old aid’to‘new aid’*** new tools include:technical assistance for geospatial curriculum development,inscription of standards and data access requirements in contracts and grants,cross-agency project design,best-practice analysis,portfolio management,and the use of language to promote participation and *** these tools reflect donor intent to partner in the realization of Digital Earth,the tools stem from a persisting asymmetric power dynamic between donors and policy-makers.

Efficient management of secondary haemophagocytic lymphohistiocytosis with intravenous steroids and γ-immunoglobulin infusions

BACKGROUND Secondary haemophagocytic lymphohistiocytosis(sHLH)is a rare lifethreatening condition mainly associated with underlying infections,malignancies,and autoimmune or immune-mediated *** To analyse all sHLH cases that were diagnosed and managed under real-world circumstances in our department focusing on the treatment schedule and the *** prospectively collected data from all adult patients fulfilling the criteria of sHLH who diagnosed and managed from January 1,2010 to June 1,2018,in our department of the tertiary care university hospital of Larissa,Greece,were analysed retrospectively(n=80;52%male;median age:55 years).The electronic records and/or written charts of the patients were reviewed for the demographic characteristics,clinical manifestations,underlying causes of sHLH,laboratory parameters,treatment schedule and 30-d-mortality *** of patients had received after consent intravenousγ-immunoglobulin(IVIG)for 5 d(total dose 2 g/kg)in combination with intravenous steroid pulses followed by gradual tapering of *** Seventy-five patients(94%)reported fever>38.5°C,47(59%)had liver or spleen enlargement and 76(95%)had ferritin>500 ng/mL including 20(25%)having considerably high levels(>10000 ng/mL).Anaemia and thrombocytopenia occurred in 72%and leucopoenia in 47%of *** infections were diagnosed in 59 patients(74%)as follows:leishmaniasis alone in 15/80(18.9%),leishmaniasis concurrently with Coxiella Burnetti or non-Hodgkin lymphoma in 2/80(2.5%),bacterial infections in 14/80(17.5%)including one case with concurrent non-Hodgkin lymphoma,viral infections in 13/80(16.3%),fungal infections in 2/80(2.5%),infections by mycobacteria in 1/80(1.3%)and unidentified pathogens in 12/80(15%).Seventy-two patients(90%)had received combination treatment with IVIG and intravenous ***,sHLH resolved in 76%of patients,15%died within the first month but 82.5%of patients were still alive 6 mo after ***

Multifunctional FeCo-Graphitic Carbon Nanocrystals for Combined Imaging, Drug Delivery and Tumor-Specific photothermal Therapy in Mice

Ultrasmall FeCo-graphitic carbon shell nanocrystals （FeCo/GC） are promising multifunctional materials capable of highly efficient drug delivery in vitro and magnetic resonance imaging in vivo. In this work, we demonstrate the use of FeCo/GC for highly effective cancer therapy through combined drug delivery, tumor-selective near-infrared photothermal therapy, and cancer imaging of a 4T1 syngeneic breast cancer model. The graphitic carbon shell of the ~4 nm FeCo/GC readily loads doxorubicin （DOX） via π-π stacking and absorbs near-infrared light giving photothermal heating. When used for cancer treatment, intravenously administrated FeCo/GC-DOX led to complete tumor regression in 45% of mice when combined with 20 min of near-infrared laser irradiation selectively heating the tumor to 43-45 ℃. In addition, the use of FeCo/GC-DOX results in reduced systemic toxicity compared with free DOX and appears to be safe in mice monitored for over 1 yr. FeCo/GC-DOX is shown to be a highly integrated nanoparticle system for synergistic cancer therapy leading to tumor regression of a highly aggressive tumor model.

Geoengineering Research Moves from Laboratory to Field

In March 2023,a converted bomber took off from Eielson Air Force Base in Alaska,USA,carrying a payload of delicate scientific instruments(Fig.1).Operated by the US National Oceanic and Atmospheric Administration(NOAA),the flight aimed to collect a plethora of information about the Earth’s *** exploration sounds routine,but the mission’s goal is unusual for a government entity:to inform policy decisions surrounding large-scale,deliberate interventions to alter the stratosphere for climate change mitigation[1].The approach of deflecting sunlight to cool the planet,called solar radiation management or solar geoengineering,was long considered a fringe *** with recent funding and attention from government agencies like NOAA,the approach is moving mainstream.

First pig-to-Live Human Xenotransplant produces Mixed Results

On 7 January 2022,a 57-year-old man with terminal heart disease became the first living human to receive a genetically modified pig organ—a heart—transplanted into his chest during a 8 h surgery at the University of Maryland Medical Center in Baltimore,MD,USA(Fig.1)[1].The pig heart,provided by the biotechnology company Revivicor(Blacksburg,VA,USA),had ten gene modifications intended to boost the chances of a successful pig-to-human transplant.

Comparative analysis of the biodiversity of fungal endophytes in insect-induced galls and surrounding foliar tissue

Insect-induced galls are abnormal plant growths that can provide food and shelter to their inhabitants,resulting in stressed plant tissue that may alter the conditions for the colonization or proliferation of endophytic *** investigated the effect gall formation has on fungal endophyte communities and *** three closely-related gallforming aphid species that specialize on poplars,we characterized fungal endophyte diversity in galls and surrounding petiole and leaf lamina tissue.A total of 516 fungal endophyte samples were isolated from 272 tissue samples(32 leaves,31 petioles,and 209 galls),resulting in 23 distinct *** sharing a common host plant and often forming spatially contiguous galls,the endophyte profiles within the galls of each aphid species were distinct,not only from the galls of the other species,but also from surrounding plant *** results suggest that insect galls can affect the composition of fungal endophyte species in plant tissues,by altering either the colonization or proliferation of their endophytic ***,fungal endophytes may be important in the ecology and evolution of insect galls.
