Medical decision making for older adults： an international perspective comparing the United States and India

There has been a significant decline in cardiovascular morbidity and mortality amidst pervasive advances in care, including percutane- ous revascularization, mechanical circulatory support, and transcatheter valvular therapies. While advancing therapies may add significant longevity, they also bring about new end-of-life decision-making challenges for patients and their families who also must weigh the advan- tages of reduced mortality to the possibility of longer lives consisting of high morbidity, frailty, pain, and poor quality of living. Advance care entails options of withholding or withdrawing therapies, and has become a familiar part of cardiovascular care for older patients in Western countries. However, as advanced cardiovascular practices extend to developing countries, the interrelated concept of advance care is rarely straight forward as it is affected by local cultural traditions and mores, and can lead to very different inferences and use. This paper discusses the concepts of advance care planning, surrogate decision-making, orders for resuscitation and futility in patients with cardiac dis- ease with comparisons of West to East, focusing particularly on the United States versus India.

Toothaches have received widespread attention and commentary dating back to ancient times,appearing in Egyptian texts from over 3000 years ago and the writings of Hippocrates,Aristotle,and *** studies of the innervation of dental pulp and dentin date back to at least the 1800’s,yet our understanding of the basis for tooth pain is still *** treatments for toothache continue to be drastic and irreversible,including removal of the dental pulp or extraction of the *** the trigeminal sensory innervation of the tooth pulp is commonly the origin of toothache pain,it is also indispensable for normal function and physiology,and should be retained as an integral part of the repaired *** or regeneration of the dental pulp and its innervation could represent the next step in restorative dentistry,but a better understanding of the roles of the tooth’s sensory nerve supply and the mechanisms underlying its development and repair is needed.

Acoustic radiation-free surface phononic crystal resonator for in-liquid low-noise gravimetric detection

Acoustic wave resonators are promising candidates for gravimetric ***,they generally suffer from strong acoustic radiation in liquid,which limits their quality factor and increases their frequency *** article presents an acoustic radiation-free gravimetric biosensor based on a locally resonant surface phononic crystal(SPC)consisting of periodic high aspect ratio electrodes to address the above *** acoustic wave generated in the SPC is slower than the sound wave in water,hence it prevents acoustic propagation in the fluid and results in energy confinement near the electrode *** energy confinement results in a significant quality factor improvement and reduces frequency *** proposed SPC resonator is numerically studied by finite element analysis and experimentally implemented by an electroplating-based fabrication *** results show that the SPC resonator exhibits an in-liquid quality factor 15 times higher than a conventional Rayleigh wave resonator at a similar operating *** proposed radiation suppression method using SPC can also be applied in other types of acoustic wave ***,this method can serve as a general technique for boosting the in-liquid quality factor and sensing performance of many acoustic biosensors.

In October 2022,the European Association for the Study of the Liver(EASL)published in Journal of Hepatology the Clinical Practice Guidelines(CPG)in regard to the diagnosis and management of cystic liver diseases(1).American College of Gastroenterology published similar guidelines in 2014 regarding focal liver lesions,but this is the first comprehensive guideline focusing on cystic liver lesions with robust background support and knowledge(2).

Genomewide selection for fruit quality traits in apple: breeding insights gained from prediction and postdiction

Many fruit quality traits in apple(Malus domestica Borkh.)are controlled by multiple small-effect quantitative trait loci(QTLs).Genomewide selection(genomic selection)might be an effective breeding approach for highly quantitative traits in woody perennial crops with long generation times like *** goal of this study was to determine if genomewide prediction is an effective breeding approach for fruit quality traits in an apple scion breeding *** apple scion breeding germplasm(nindividuals=955),high-quality single nucleotide polymorphism(SNP)data(nSNPs=977),and breeding program fruit quality trait data at harvest were *** parents‘Honeycrisp’and‘Minneiska’were highly *** to high predictive abilities were observed for most fruit quality traits at *** example,when 25%random subsets of the germplasm set were used as training sets,mean predictive abilities ranged from 0.35 to 0.54 across ***,training and test sets,family size for within family prediction,and number of SNPs per chromosome affected model predictive *** of large-effect QTLs as fixed effects resulted in higher predictive abilities for some traits(*** red overcolor).Postdiction(***)analyses demonstrated the impact of culling threshold on selection *** results of this study demonstrate that genomewide selection is a useful breeding approach for certain fruit quality traits in apple.

An understanding of the roles and representations of women in classical Chinese philosophy is here derived from central texts such as the Analects,the Lienii Zhuan,and the I *** argue that the roles of women during the classical period of Chinese philosophy tended to be as part of the "inner," working domestically as a housewife and *** will be shown from three passages from the *** were represented as submissive and passive,as with the qualities ascribed to yin energy,and therefore as rightfully subordinate to ***,despite representations of women in philosophy being thus at this time,there were exceptions,specific women who could take a male "outer" political *** story of Jing Jiang from the Lienii Zhuan suggests that although women being involved in "outer" affairs were looked down on,there were still women who would be and who would occasionally get praised for doing *** shows that it was realized,explicitly or otherwise,that women were capable of taking those roles,but also that they were not allowed to take such roles at that time.

Frailty is a multidimensional dynamic condition characterized by decreased physiological reserves. In this issue,Wu et al.(2019)describe the largest whole-genome resequencing dataset published to date for rapeseed (Brassica napus),an allopolyploid crop species that originated just a few thousand years ago under anthropogenic influence and rapidly evolved into one of the world's most important oilseed crops. In almost 1000 *** spanning species-wid.e germplasm for oilseed rape,a comprehensive analysis of sequence diversity related to flowering-related traits uncovered selective sweeps associated with eco-geographic adaptation and human selection,attributable particularly to divergence among homoeologs of key flowering-time regulation and ethylene synthesis/signaling genes.
