AIM: To compare adherence, response, and remission with light treatment in African-American and Caucasian patients with Seasonal Affective ***: Seventy-eight study participants, agerange 18-64(51 African-Americans and 27 Caucasians)recruited from the Greater Baltimore Metropolitan area, with diagnoses of recurrent mood disorder with seasonal pattern, and confirmed by a Structured Clinical Interview for the Diagnostic and Statistical Manual of Mental Disorders-Ⅳ, were enrolled in an open label study of daily bright light treatment. The trial lasted6 wk with flexible dosing of light starting with 10000 lux bright light for 60 min daily in the morning. At the end of six weeks there were 65 completers. Three patients had Bipolar Ⅱ disorder and the remainder had Major depressive disorder. Outcome measures were remission(score ≤ 8) and response(50% reduction)in symptoms on the Structured Interview Guide for the Hamilton Rating Scale for Depression(SIGH-SAD)as well as symptomatic improvement on SIGH-SAD and Beck Depression Inventory-Ⅱ. Adherence was measured using participant daily log. Participant groups were compared using t-tests, chi square, linear and logistic regressions. RESULTS: The study did not find any significant group difference between African-Americans and their Caucasian counterparts in adherence with light treatment as well as in symptomatic *** symptomatic improvement and rate of treatment response were not different between the two groups,African-Americans, after adjustment for age, gender and adherence, achieved a significantly lower remission rate(African-Americans 46.3%; Caucasians 75%; P =0.02).CONCLUSION: This is the first study of light treatment in African-Americans, continuing our previous work reporting a similar frequency but a lower awareness of SAD and its treatment in African-Americans. 背景与目的我们开展了一项帕博利珠单抗用于伴未治疗脑转移的非小细胞肺癌(non-small cell lung cancer,NSCLC)或黑色素瘤患者的疗效和安全性的II期试验,旨在评估程序性死亡受体1(programmed cell death 1,PD-1)抑制剂在中枢神经系统(central nervous system,CNS)中的疗效。中期结果已发表,现报道对NSCLC队列的更新分析结果。方法这是一项开放性、单中心、II期试验。纳入标准:年龄≥18岁,诊断为晚期NSCLC并伴有≥1个5 mm-20 mm脑转移病灶,既往从未治疗或之前放疗后进展,无神经系统症状,不需要激素治疗且美国东部肿瘤协作组(Eastern Cooperative Oncology Group,ECOG)<2分。患者每2周接受一次帕博利珠单抗(10 mg/kg)治疗。队列1为程序性死亡配体1(programmed cell death ligand 1,PD-L1)≥1%的患者,队列2为PD-L1<1%或未评估的患者。主要终点是脑转移患者缓解比例。所有经治患者均纳入疗效与安全性终点的分析。该研究已结束入组,并于***登记注册,注册号为NCT02085070。结果2014年3月31日-2018年5月21日,共42例患者接受治疗。中位随访时间为8.3个月(IQR:4.5个月-26.2个月)。队列1的37例患者中11例有脑转移缓解[29.7%(95%CI:15.9%-47.0%)]。队列2未观察到缓解。治疗相关的3级-4级不良事件(adverse events,AEs)包括2例肺炎、1例全身症状、1例结肠炎、1例肾上腺皮质功能不全、1例高血糖症和1例低钾血症。6例(14%)患者发生了治疗相关的严重不良事件,包括肺炎、急性肾损伤、低钾血症和肾上腺皮质功能不全。没有观察到治疗相关死亡病例。结论帕博利珠单抗治疗PD-L1≥1%的NSCLC伴脑转移患者有效,且对所有纳入的未经治疗的脑转移患者安全。需要进一步探索免疫治疗用于NSCLC合并CNS转移。

背景与目的对于严重面容损毁的成年患者而言，其术后长期心理学影响的评估仍有限。本研究旨在确定此类患者的长期心理学机能，并评估获得性面容损毁患者与无面容损毁者间的不同。同样的，先天性面容损毁者的心理机能程度与其对面容的满意度相关，此类患者会因其他人对其面容的负面评价、自尊心和面部畸形的严重程度而感到不同程度的忧虑。方法本研究共对59例严重先天性面容损毁者、59例创伤获得性面容损毁者和120例无面容损毁者进行分析。让所有被调查者进行标准的心理学、身体检查和人口统计学问卷调查，包括面容负面评价忧虑量表（Fear of Negative Appear—ance Evaluation Scale）、自信心量表（Rosenberg Self-Esteem Scale）、住院焦虑和抑郁量表（Hospital Anxietyand Depression Scale）、阿成贝切自评表（Achenbach Adult Self-Repor）、SF-36健康调查表（SF-36Health Survey）和视觉类比量表（Visual Analog Scale）。结果先天性面容损毁的成年患者有相对正常的心理机能，但相对于无面容损毁的成年患者，其表现出存在藏在心底的问题。与获得性面容损毁者相比，先天性者仅在身体检查量表部分的生活质量方面表现出较少的问题。除了身体检查部分的量表的有关生活质量的方面以外，患者对面容的满意度、对面容负面评价的忧虑和自尊心都是很好的可预测其心理机能不同方面的因素。结论（通过手术）改善患者外观评价满意度、（通过心理支持）增强其自尊心或降低其对面容负面评价的忧虑，将有助于其提高长期心理机能。今后的研究将会把重点放在对个体患者的分析和对不良适应相关危险因素的分析。

肌萎缩侧索硬化(ALS)是一种神经退行性疾病,其特征是中枢神经系统中运动神经元的进行性丧失。星形胶质细胞在ALS的疾病进程中起重要作用。星形胶质细胞通过缝隙连接蛋白家族,如连接蛋白(Connexin,Cx)互相连接。Cx43是中枢神经系统中保持稳态的主要星形胶质细胞连接蛋白。在病理状态下,连接蛋白的表达和功能发生改变。本文发现,在ALS中Cx43异常增高是星形胶质细胞介导的毒性作用机制之一。笔者观察到,在ALS SOD1^(G93A)小鼠模型的疾病进程中,Cx43表达进行性增高。尤其是,在ALS患者的运动皮质和脊髓也能检测到Cx43增高。从SOD1^(G93A)小鼠中分离的星形胶质细胞,与人诱导多能干细胞衍生的星形胶质细胞一样,都检测到Cx43蛋白增高,且为独立于神经元共培养的内源性现象。与对照的星形胶质细胞过表达的野生型SOD1(SOD1^(WT))相比,SOD1^(G93A)星形胶质细胞中增高的Cx43表达可产生重要的功能性影响。本文发现,SOD1^(G93A)星形胶质细胞表现出缝隙连接偶联增强,半通道介导活动升高,细胞内钙离子水平上升。最后,本文检测Cx43蛋白表达升高对MN生存的影响,发现应用pan-Cx43阻滞剂和Cx43半通道阻滞剂均对与SOD1^(G93A)星形胶质细胞共培养的MNs有神经保护作用。这些新发现展示出未被认知的Cx43在ALS相关的运动神经元丢失中所起作用。

Carcinoma of Unknown Primary Site: Outcomes in Patients with a Colorectal Molecular Profile Treated with Site Specific Chemotherapy

Purpose: Accurate identification of carcinoma of unknown primary site (CUP) patients with colorectal sites of origin may improve outcomes by directing colorectal chemotherapy rather than empiric chemotherapy. Clinical features, standard pathologic evaluation, treatment response, and survival of CUP patients whose tumors had colorectal genetic signatures were examined. Patients and Methods: We prospectively tested paraffin-embedded biopsies from 213 CUP patients using mRNA-based RT-PCR assays, and identified patients with colorectal genetic signatures. Results: Assays were successful in 185 specimens (87%);32 (17%) had colorectal genetic signatures. All 32 patients had carcinoma;colonoscopy was normal in 30 patients studied;29 patients (90%) had typical metastatic sites;17 (53%) had consistent IHC profiles (CK20+, CDX2+). Twenty-nine patients (90%) received first- and/or second-line colorectal chemotherapy regimens (response rates of 69% and 54%, respectively). Median, 2- and 4-year survivals for all 32 patients were 21 months, 42%, and 35%, respectively. Conclusion: The majority of CUP patients with colorectal molecular profile diagnoses responded to site-specific chemotherapy;median survival was similar to patients with known advanced colorectal carcinoma and superior to expected survival of CUP patients receiving empiric chemotherapy. Molecular profile assays can identify CUP patients with colorectal carcinoma;colorectal chemotherapy appears to improve outcomes.

本文旨在为IV期非小细胞肺癌患者的治疗提供更新版推荐。本文资料检索源自2002年以来公布的相关随机试验文献。此指南范围限于化疗与生物治疗。更新委员会对这些文献进行了总结并提供了推荐更新。162篇文献符合标准被纳入参考。本推荐基于可改善总生存期的治疗方法。仅改善无进展生存期的治疗方法推动了对毒性及生存质量的监测。对于体力状态评分为0分或1分患者的一线治疗,可推荐以铂类为基础的细胞毒性药物的两药联用。对铂类治疗有禁忌的患者,可采用非铂类细胞毒性两药联合。对于体力状态评分为2分的患者,单一细胞毒性药物即可。对于疾病进展或经过4个周期的治疗仍对治疗无反应的患者,应停止一线细胞毒性化疗。即使在6个周期后患者对治疗仍有反应,亦应停止两药细胞毒性化疗。对于伴有明确的表皮生长因子受体(epidermal growth factor receptor,EGFR)突变的患者,可推荐一线采用吉非替尼治疗;对于EGFR突变为阴性或不明确的患者,细胞毒性化疗更佳。除具有特定临床特征的患者外,可推荐贝伐单抗与卡铂-紫杉醇联用。对于通过免疫组化证实EGFR阳性的肿瘤患者,可推荐西妥昔单抗与顺铂-长春瑞滨联用。多西紫杉醇、厄洛替尼、吉非替尼或培美曲塞被推荐作为二线治疗。对于未曾接受过厄洛替尼或吉非替尼治疗的患者,可推荐厄洛替尼作为三线治疗。现有数据不足以推荐常规三线采用细胞毒性药物。已有的证据也不足以推荐常规应用分子标记物选择化疗。

Thirteen patients with Schistosoma mansoni myelopathy are reported. Neurologic syndromes included acute areflexic flaccid paraplegia (three), thoracic myelopathy with hyperreflexia and Babinski sign (six), and a cauda equina syndrome (four). Inflammatory granulomas and a schistosome worm in a leptomeningeal vein of the spinal cord were observed in the one patient coming to necropsy.

Suicide is the second leading cause of death in youth globally; however, there is uncertainty about how best to intervene. Suicide rates are typically higher in males than females, while the converse is true for suicide attempts. We review this "gender paradox" in youth, and in particular, the age-dependency of these sex/gender differences and the developmental mechanisms that may explain them. Epidemiologic, genetic,neurodevelopmental and psychopathological research have identified suicidal behaviour risks arising from genetic vulnerabilities and sex/gender differences in early adverse environments, neurodevelopment, mental disorder and their complex interconnections. Further, evolving sex-/gender-defined social expectations and norms have been thought to influence suicide risk. In particular, how youth perceive and cope with threats and losses(including conforming to others' or one's own expectations of sex/gender identity) and adapt to pain(through substance use and helpseeking behaviours). Taken together, considering brain plasticity over the lifespan, these proposed antecedents to youth suicide highlight the importance of interventions that alter early environment(s)(e.g., childhood maltreatment) and/or one's ability to adapt to them. Further, such interventions may have more enduring protective effects, for the individual and for future generations, if implemented in youth.
