BACKGROUND In severe cases of coronary artery disease,percutaneous coronary intervention provide promising *** stent used could be a drug-eluting stent(DES)or a titanium-nitride-oxide coated stent(TiNOS).AIM To compare the 5-year effectiveness and safety of the two stent *** The following systematic review and meta-analysis was conducted in accordance with the preferred reporting items for systematic reviews and meta-analysis guidelines,and PubMed/MEDLINE,Scopus,and Cochrane Central were searched from inception till August *** outcomes were major adverse cardiac events(MACE),cardiac death,myocardial infarction(MI),cardiac death or MI,and ischemia-driven total lesion revascularization(ID-TLR).RESulTS Four randomized controlled trials(RCT),which analyzed a sum total of 3045 patients with acute coronary syndrome(ACS)after a median follow-up time of 5 years were *** statistically insignificant,an increase in the ID-TLR was observed in patients receiving TiNOSs vs *** addition,MI,cardiac death and MI,and definite stent thrombosis(DST)were significantly decreased in the TiNOS *** analysis revealed no significant results with meta-regression presenting non-ST elevated MI(NSTEMI)as a statistically significant covariate in the outcome of *** TiNOS was found to be superior to DES in terms of MI,cardiac death or MI,and DST outcomes,however,the effect of the two stent types on ID-TLR and MACE was not significant.A greater number of studies are required to establish an accurate comparison of patient outcomes in TiNOS and DES.

Diagnosis and Management of Childhood Tuberculosis: A Cross-Sectional Study of Practices in Gilgit Baltistan, Pakistan

Background: Tuberculosis (TB) is one of the major killer diseases among infectious diseases. The success of TB control depends on the capability of the health care system to detect and accurately manage TB cases. Tuberculosis in children remained a low public health priority with limited epidemiologic studies. Struggles for TB control in children need to be enhanced as children are providing the reservoir for future cases to develop. Objectives: The study evaluated diagnostic and treatment practices related to childhood pulmonary tuberculosis in Gilgit Baltistan (GB), Pakistan. Methods: A descriptive, cross-sectional study based on retrospective record review of childhood Pulmonary Tuberculosis patients registered in the year 2020 with self-administered questionnaire. Results: Data of 557 childhood cases were collected. Most childhood cases were in age group 1 - 4 years (54%) with male predominance. More than 90% were diagnosed and treated at public sector facilities. 99% of the cases were clinically diagnosed with passive case finding. Cough was considered as a symptom of childhood Tuberculosis (TB) by 94% of physicians. Other important features included failure to thrive (13%), contact with a family history of TB (15%), Malnutrition(24%) and respiratory signs (21%). 99% physicians advised chest X-ray, Complete blood count (CBC) (95%) and Erythrocytes sedimentation rate (ESR) (72%) for diagnosis and fewer physicians (2%) used sputum smear microscopy and induced sputum (0.1%). Combining data on dosage, frequency and duration for drugs, 99% of the cases were found receiving correct regimen. The treatment outcomes of the cases were cured 4 (0.8%), treatment completed 551 (99.3%) and died 2 (0.4%) with no lost to follow up. Conclusions: The study highlights inappropriate diagnostic and treatment practices for managing childhood pulmonary TB among physicians in public and private sectors of Gilgit Baltistan. Weed growth, herbicide efficacy, and rice productivity in dry seeded paddy field under different wheat stubble management methods

To ascertain the influence of wheat stubble management options and chemical weed control methods on weed growth and productivity of dry direct-seeded fine rice, a two years' field study was undertaken at the Agronomic Research Farm, University of Agriculture, Faisalabad, Pakistan in 2013 and 2014. Different wheat stubble management methods, viz., incorporation, burning and retention were executed during seed-bed preparation. While, herbicide treatments comprised of a weed check, weed free, pendimethalin followed by tank mixture of fenoxaprop p-ethyl+ethoxysulfuron ethyl, and bispyribac sodium followed by tank mixture of fenoxaprop p-ethyl+ethoxysulfuron ethyl. Results revealed that weed control efficacy of both herbicide treatments ranged from 84 to 94%. Herbicide treatments significantly reduced weed density(88–90%) and dry weight(86–88%), while improved the rice growth attributes compared with weed check. Application of bispyribac sodium followed by tank mixture of fenoxaprop p-ethyl+ethoxysulfuron ethyl in stubble retention recorded 226 and 273% increase in kernel yield over weedy check in 2013 and 2014, respectively. In stubble incorporation, pendimethalin followed by tank mixture of fenoxaprop p-ethyl+ethoxysulfuron ethyl was more effective in increasing(256–293%) rice yields over weedy check. among different treatment combinations, the maximum net benefits(1 397.49^(–1) 472.22 USD ha^(–1)), net returns(636–700 USD ha^(–1)), benefit cost ratio(1.77^(–1).83) and marginal rate of return(2 187–2 330%) were recorded with the application of bispyribac sodium followed by fenoxaprop p-ethyl+ethoxysulfuron ethyl in stubble retention. In crux, application of bispyribac sodium followed by tank mixture of fenoxaprop p-ethyl+ethoxysulfuron ethyl in stubble retention is efficient approach to control weeds, and get maximum rice productivity and net economic returns under dry seeded system.

Enhancing Dielectric and Mechanical Behaviors of Hybrid Polymer Nanocomposites Based on Polystyrene,Polyaniline and Carbon Nanotubes Coated with Polyaniline

The dielectric and mechanical properties of hybrid polymer nanocomposites of polystyrene/polyaniline/carbon nanotubes coated with polyaniline （pCNTs） have been investigated using impedance analyzer and extensometer. The blends of PS/PANI formed the heterogeneous phase separated morphology in which pCNTs are dispersed uniformly. The incorporation of a small amount of pCNTs into the blend of PS/PANI has remarkably increased the dielectric properties. Similarly, the AC conductivity of PS/PANI is also increased five orders of magnitude from 1.6 × 10^-10 to 2.0×10^-5 ***-1 in the hybrid nanocomposites. Such behavior of hybrid nanocomposites is owing to the interfacial polarization occurring due to the presence of multicomponent domains with varying conductivity character of the phases from insulative PS to poor conductor PANI to highly conductive CNTs. Meanwhile, the tensile modulus and tensile strength are also enhanced significantly up to 55% and 160%, respectively, without much loss of ductility for three phase hybrid nanocomposites as compared to the neat PS. Thereby, the hybrid nanocomposites of PS/PANI/pCNTs become stiffer, stronger and tougher as compared to the neat systems.

Pharmacological basis for the medicinal use of Michelia champaca in gut, airways and cardiovascular disorders

Objective: To discover the mechanism behind ameliorative effects of Michelia champaca(M. champaca) in gastrointestinal, respiratory and cardiovascular disorders. Methods: Antispasmodic potential was evaluated by trying the M. champaca extract(aqueous:ethanolic) on rabbit aorta, trachea and jejunum in vitro. Isotonic and isometric transducers coupled with Power Lab data acquisition system was used to record the responses of isolated tissues. Results: M. champaca extract relaxed the spontaneous and high K^+(80 mmol/L)-induced contractions of isolated jejunum preparation of rabbit showing a Ca^(2+) channel blocking mechanism. Moreover, extract shifted calcium concentration response curves towards right like standard calcium channel blocker verapamil. In rabbit tracheal preparation, M. champaca relaxed both carbachol(1 μmol/L) and high K^+-induced contractions, likewise verapamil. In rabbit aorta preparation, M. champaca relaxed phenylephrine(1 μmol/L) and high K^+-induced contractions similar to verapamil. Conclusion: M. champaca possesses spasmolytic, airways relaxant and vasodilator actions mediated perhaps due to blocking of Ca^(2+) channels, hence validating its therapeutic usage in diarrhea, asthma and hypertension.

背景:正常的颈椎曲度是前凸,开始于齿突而结束于C7,成人C2到C7椎体后线的平均角度为34°±9°,正常颈椎前凸曲度的消失主要出现在各种疾病进程中和颈椎损伤时,其次常见于胸腰病变及下肢疾病及畸形时,颈椎病或颈椎畸形手术的最佳效果和患者满意度水平在一定程度上取决于颈椎前凸曲度的矫正和恢复。因此,颈椎病患者的手术治疗,除改善神经系统异常和缓解临床症状的作用外,手术治疗的主要目标是建立一个理想的颈椎生理曲度,从而起到保持适当的矢状面平衡、恰当的头部和颈部姿势以及水平视线的作用。目标:本研究的主要目标是:1、评估和比较颈椎生理曲度在前路与后路手术后的矫正情况。2、解决在前路和后路手术同样适用时如何选择手术路径的争议。实验设计:这是一个回顾性临床对照研究,两组患者(前路手术及后路手术)手术部位颈椎Cobb角及整体颈椎前凸曲度分别在术前和术后利用中立位平片进行测量。实验方法:在本研究中,2010年01月01日到2016年10月01日之间共有75(n=75)例患者接受了颈椎手术(颈椎前路以及后路),我们进行了回顾性研究。将75名患者分为两组:A组44例(n=44,其中男30例,女14例)均采用颈前路手术治疗;B组31例(n=31,其中男23例,女8例),采用颈后路手术治疗。A组(前路组)平均年龄为53.7岁(从42到77岁不等),B组(后路组)平均年龄61.52岁(年龄从30到84岁不等)。利用x线测量两组术前及术后颈椎Cobb角,在手术部位同时测量从C 2下终板到C 6或C 7下终板的颈椎整体前凸曲度。所有测量记录在Excel表内。数据分析采用SPSS20版(IBM公司软件)。结果:使用独立样本T检验分别比较A、B两组患者手术部位的Cobb角及整体颈椎前凸,同时也比较了两组平均差。P值为0.001或P<0.05,有显著统计学差异,据此我们可以推出,颈椎前路手术术后Cobb角的纠正程度比后路手术要好。结论:相较于后路手术,颈椎前路手术能提供一个更好的颈椎前凸曲度,并且使颈椎整体前凸曲度和Cobb角得到更好的纠正。
