目的分析20例中老年尿毒症患者肾移植前、后左室结构和功能的改变。方法二维、M型及多普勒超声心动图测量20例正常人、20例中老年尿毒症患者肾移植前、肾成功移植后1个月及三个月以后的左室结构及功能。结果病例组肾移植后一个月扩张的左房内径、左室舒末径均较术前减小,增厚的左室壁厚度开始变薄,三个月后增厚的左室壁厚度、收缩功能及舒张功能较移植术前明显改善(P<0.01),左房内径及左室壁厚度仍高于对照组(P<0.05),左室舒末径与正常无差异。结论中老年尿毒症患者在成功进行肾移植术后,左室的结构及功能逐渐得以改善,术后3个月变化明显。

The Influence of Climate Change and Variability on Aircraft Take-off and Landing Performance;a Case Study of the Abeid Amani Karume International Airport-Zanzibar

Climate change (CC) and variability have been world widely reported to pose number of risks in aviation industry including accidents, astray, and other operational difficulties. The impact of weather on landing and take-off performances has been several times experienced at Abeid Amani Karume International Airport (AAKIA);however, the influence of climate change and variability to the aircraft performance needs to be assessed. Thus, this study investigated the influence of climate change and variability on aircrafts take-off and landing performances. Specifically, the study investigated;i) the influence of climate change on Take-off Distance Required (TODR) and Maximum Take-off Mass (MTOM) for different types of aircraft;ii) the influence of climate variability to the aircraft landing performance on light, medium and heavy aircraft and lastly, iii) the study investigated the seasonal and annual variability on aircraft landing performance due to climate variability. The datasets used in this study include the eight years (2014-2021), aircraft operational records (diversion and missed approach events) and Aviation Routine Weather Reports (METAR) records which were utilized as the indicators for landing performance, the long-term (1990-2020) annual maximum temperatures (Tmax) which was used to determine the TODR and MTOM. Statistical tools including mean, percentage changes, correlations, regression, and the chi-square test were used for analysis and hypotheses testing. The results revealed that light and medium aircraft categories were significantly most affected on diversion events as compared to the heavy categories;however, for the missed approach events the impact was vice versa. Moreover, the seasonal and annual variability on diversion and missed approach events were significantly different (at p ≤ 0.001). Dairy cattle are frequently fed high-concentrate(HC)diets in modern intensive feeding systems,especially in the transition *** this period,cows face many alterations that include hormonal changes and shifting to a lactating *** to a HC diet that may disrupt the ruminal microbiota balance can lead to subacute ruminal acidosis(sara).Moreover,the main factor shaping the rumen microbiota is dietary composition,especially the ratio of starch to fibrous *** highly fermentable carbohydrate diets after adaptation to forage diets leads to a rumen fermentation rate that exceeds rumen absorption and buffering rates,resulting in a reduction in ruminal *** a result of Gram-negative bacterial cell lysis,an increase in harmful ruminal bacterial metabolites,including lipopolysaccharide,lactic acid,and histamine,is *** interactions between the host immune system and the ruminal microbiota play an essential role in many physiological processes and the development of the *** in DNA sequencing and bioinformatics platforms provides new opportunities to investigate the composition of ruminal microbes and yields unique advances in understanding ecology of the *** ruminal acidosis is linked with a change in the ruminal microbiota structure and richness and with other metabolic disorders;such as rumenitis,milk fat depression,laminitis,and liver ***,this review aims to explore a better understanding of the crosstalk between diet and microbiota in the prevalence of rumen acidosis and its consequences,which is crucial for control strategies such as feeding management,and supplementation with thiamine,prebiotics,and probiotics.

Perceived Barriers to Cervical Cancer Screening Using Pap Smear Test among Women Attending Saad Abu Al Ella Hospital in Khartoum State, 2022

Background: Cervical cancer is the second common cancer among women worldwide. It is a preventable cancer, and early detection of precancerous conditions through the Papanicolaou cytology screening (Pap smear) is a key aspect of prevention;it is accepted worldwide as an efficient tool for secondary prevention. While the PS test is simple, inexpensive, and relatively reliable as a method of diagnosing cervical cancer, most women do not take the test. Therefore, this study is sought to describe the barriers to pap smear uptake among Sudanese women. Materials and Method: This total coverage observational, analytical and cross sectional, hospital-based study was conducted in Saad Abu El Ella Hospital in April 2022. The study was conducted using an anonymous questionnaire to assess the perceived barriers of 93 participants. All data were computerized using Microsoft Excel’17 and the data were described and analyzed using statistical package for social science (SPSS23). Results: The findings revealed that the mean age of the participants was 39.5 years and only 3.2% had ever undergone a pap smear test. Identified barriers were lack of information, not knowing where to go, and fear of pain. The majority, 72% are willing to routinely perform a pap smear test if well informed about it. The study also demonstrates that there is a significant correlation between perceived barriers score and willingness to perform the pap smear test (p value = 0.008), and between the perceived barriers score and the sociodemographic factors: Age (p value = 0.006), educational level (p value = 0.028) and occupation (p value = 0.040), but no association with the economic status was found (p value = 0.378). Conclusion: The detection rate is too low compared to the national target of over 70%. Therefore, more work is needed to reduce perceived barriers to cervical cancer screening by providing education/raising for popular awareness;addressing misconceptions and false beliefs;informing women about

Early initiation of antiretroviral treatment: Challenges in the Middle East and North Africa

New World Health Organization guidelines recommend the initiation of antiretroviral treatment(ART) for asymptomatic patients with CD4+ T-cell counts of ≤ 500 cells/mm3. Substantial reduction of human immunodeficiency virus(HIV) transmission is addressed as a major public health outcome of this new approach. Middle East and North Africa(MENA), known as the area of controversies in terms of availability of comprehensive data, has shown concentrated epidemics among most of it's at risk population groups. Serious challenges impede the applicability of new guidelines in the MENA Region. Insufficient resources restrict ART coverage to less than 14%, while only one fourth of the countries had reportable data on patients' CD4 counts at the time of diagnosis. Clinical guidelines need to be significantly modified to reach practical utility, and surveillance systems have not yet been developed in many countries of MENA. Based on available evidence in several countries people who inject drugs and men who have sex with men are increasingly vulnerable to HIV and viral hepatitis, while their sexual partners- either female sex workers or women in monogamous relationships with high-risk men- are potential bridging populations that are not appropriately addressed by regional programs. Research to monitor the response to ART among the mentioned groups are seriously lacking, while drug resistant HIV strains and limited information on adherence patterns to treatment regimens require urgent recognition by health policymakers. Commitment to defined goals in the fight against HIV, development of innovative methods to improve registration and reporting systems, monitoring and evaluation of current programs followed by costeffective modifications are proposed as effective steps to be acknowledged by National AIDS Programs of the countries of MENA Region.
