Does local vaginal estrogen after tension-free transobturator vaginal tape reduce overactive bladder symptoms in postmenopausal women? A prospective randomized, controlled study

Objective:We aimed to evaluate the efficacy of topical estrogen after transvaginal tension-free vaginal tape-obturator(TVT-O)in the treatment of de novo overactive bladder symptoms that appear after ***:This is a prospective randomized controlled study performed in the Urology and Gynecology Departments,Kasr Al Ainy Hospital,Cairo University,Cairo,*** hundred and ten postmenopausal females presenting during the period between January 2017 and November 2020 with stress urinary incontinence were included in the *** were divided into two groups,105 patients in Group A(treatment group)and 105 patients in Group B(control group).Patients in Group A underwent transvaginal TVT-O followed by local vaginal estrogen treatment for 6 months,while patients in Group B underwent transvaginal TVT-O *** study included any postmenopausal female with urodynamic stress urinary *** patients had to fulfill a 3-day bladder diary,overactive bladder symptoms score,urine analysis,urodynamic study,and post-voiding residual urine measurement by abdominal ultrasound preoperatively and at 3-month and 6-month ***:At 6-month follow-up,daytime frequency was reduced to 8%in Group A(increased to 21%in Group B)with a statistically significant difference between both groups(p=0.009).At 6-month follow-up,nocturia was 8%in Group A(11%in Group B)with no statistically significant difference between both groups(p=0.469).There was a statistically significant difference between both groups as regards to urinary urgency at 6-month follow-up(p=0.024).There was a statistically significant difference in postoperative wound healing events as regards to cure,hyperemia,gapping,and wound infection 1 week after intervention between both groups(p=0.008).No local or systemic side-effects were reported from local estrogen ***:Local vaginal estrogen treatment given to postmenopausal patients after midurethral sling procedures can reduce the symptoms o

Depression among Health Care Workers in Khartoum State, Sudan, 2022

Introduction: Depression is a serious issue affecting healthcare workers and is a leading cause of disability for both genders. Furthermore, it is one of the leading causes of mortality and morbidity, responsible for 4.4 percent of global disability. An estimated 350 million people are currently living with depression worldwide. Objectives: to estimate the prevalence of depression among healthcare workers in Khartoum State in 2022 and determine the associated factors. Methods: A cross-sectional survey was conducted among healthcare workers in Khartoum State, Sudan, in 2022 using a self-administered electronic questionnaire. Depression was screened using the self-reporting questionnaire (PHQ-9). Descriptive statistics in the form of frequencies and percentages were used to display the data. Odds ratios (ORs) with a 95% confidence interval were estimated using bivariate and multivariate logistic regression analysis to determine associations between depression and related factors. Results: A total of 341 valid responses were received, with a mean age of 33.91. The overall prevalence of depression (PHQ-9 > 8) was 258 (75.6%). The prevalence was significantly associated with marital status (single and divorced), occupation (psychologist), and working department (Emergency Department), showing a p-value of Conclusion: Depression is a serious mental health disorder that affects all people, including healthcare workers, and is a growing problem in Sudan. To address this, healthcare organizations must implement policies and strategies to reduce inequality and protect healthcare workers. A multidisciplinary approach that includes mental health professionals, the Ministry of Health, and universities is needed to prioritize mental health issues and ensure quality care and the overall well-being of both healthcare workers and patients.

Mental Distress among Medical Students in Khartoum, Sudan 2022

Background: Young adults in Sudan face numerous challenges that make it difficult for them to maintain their mental health. Due to the academic workload and performance expectations, medical students suffer from a high prevalence of mental distress. Objectives: to estimate the prevalence of mental distress among medical students and determine the associated factors. Methods: a cross-sectional survey was conducted among medical students in Khartoum, Sudan, using an electronic questionnaire. Mental distress was screened in the period from June 19 to July 30, 2022, using the self-reporting questionnaire (SRQ-20). Descriptive statistics in the form of frequencies and percentages were used to display data. Odds ratios (ORs) with a 95% confidence interval were estimated using univariate and multivariate logistic regression analysis to determine associations between mental distress and related factors. Results: A total of 432 valid responses were received with a mean age of 20.37. The Overall prevalence of mental distress (SRQ-20 > 8) in medical students was 241 (55.8%). The prevalence was significantly associated with gender (P-Value > 0.001), type of university (P-Value = 0.001), academic level of education (P-Value = 0.026), and family history of mental illness (P-value = 0.001). Conclusion: More than half of the medical students who were screened for mental distress had mental distress. Higher odds are associated with being a female student, attending a private university, studying at a higher academic level, and having a family history of mental illness. This study recommends a review of the academic processes and implementing institutional preventive strategies that target at-risk groups.

Diagnostic and Prognostic Significance of Histidine-Rich Glycoprotein in Acute Lymphoblastic Leukemia

Background: Histidine-rich glycoprotein (HRG), a multifunctional plasma protein, has a regulatory role in homeostasis, angiogenesis, and immunity;which in turn could greatly affect tumor control and metastasis. Objectives: To assess the possible role of HRG in acute lymphoblastic leukemia (ALL) tumorgenesis and follow-up. Design and Methods: HRG was quantitatively measured in serum by ELISA and its expression was assessed by real-time PCR (qPCR) in 35 patients with ALL and compared to same 25 ALL patients after induction therapy and 30 age and sex matched healthy control subjects. Results: HRG-serum protein (at cutoff value 63.55 pg/ml) and HRG-RNA (at cut-off value 0.955) were positive in all ALL patients before therapy, but in only 76% after therapy for HRG-protein and 60% for HRG-RNA and they could not be detected in the control group;P < 0.001. Additionally, the serum HRG level showed a significant positive correlation with its expression level, bone marrow blast percentage, peripheral blood blast count, P < 0.01. Also its serum and expression levels were positively related to the poor risk Philadelphia chromosome;P < 0.01. Conclusions: HRG (protein and RNA) might be considered as a novel diagnostic and prognostic marker in ALL. HRG-serum protein level, detected by simple methodology of ELISA, has more significant advantages than its expression level, motivating its application in large clinical studies as a potential marker.

Perceived Barriers to Cervical Cancer Screening Using Pap Smear Test among Women Attending Saad Abu Al Ella Hospital in Khartoum State, 2022

Background: Cervical cancer is the second common cancer among women worldwide. It is a preventable cancer, and early detection of precancerous conditions through the Papanicolaou cytology screening (Pap smear) is a key aspect of prevention;it is accepted worldwide as an efficient tool for secondary prevention. While the PS test is simple, inexpensive, and relatively reliable as a method of diagnosing cervical cancer, most women do not take the test. Therefore, this study is sought to describe the barriers to pap smear uptake among Sudanese women. Materials and Method: This total coverage observational, analytical and cross sectional, hospital-based study was conducted in Saad Abu El Ella Hospital in April 2022. The study was conducted using an anonymous questionnaire to assess the perceived barriers of 93 participants. All data were computerized using Microsoft Excel’17 and the data were described and analyzed using statistical package for social science (SPSS23). Results: The findings revealed that the mean age of the participants was 39.5 years and only 3.2% had ever undergone a pap smear test. Identified barriers were lack of information, not knowing where to go, and fear of pain. The majority, 72% are willing to routinely perform a pap smear test if well informed about it. The study also demonstrates that there is a significant correlation between perceived barriers score and willingness to perform the pap smear test (p value = 0.008), and between the perceived barriers score and the sociodemographic factors: Age (p value = 0.006), educational level (p value = 0.028) and occupation (p value = 0.040), but no association with the economic status was found (p value = 0.378). Conclusion: The detection rate is too low compared to the national target of over 70%. Therefore, more work is needed to reduce perceived barriers to cervical cancer screening by providing education/raising for popular awareness;addressing misconceptions and false beliefs;informing women about

Shear strength characteristics of mixing slag-stone ballast reinforcement with tire geo-scrap using large-scale direct shear tests

Utilizing the ballast layer with more durable and stable characteristics can help avoid significant expenses due to decreased maintenance efforts. Strengthening the ballast layer with different types of reinforcements or substituting the stone aggregates with the appropriate granular materials could potentially help to achieve this goal by reducing the ballast deterioration. One of the exquisite and most effective solutions to eliminate these challenges is to use waste materials such as steel slag aggregates and useless tires. Utilizing these waste materials in the ballasted railway track will contribute to sustainable development, an eco-friendly system, and green infrastructure. So in a state-of-the-art insightful, the ballast aggregates, including a mixture of steel slag and stone aggregates, are reinforced with a novel kind of geo-grid made of waste tire strips known as geo-scraps. This laboratory research tried to explain the shear strength behavior of the introduced mixing slag-stone ballast reinforced with tire geo-scrap. To achieve this goal, a series of large-scale direct shear tests were performed on the ballast which is reinforced by tire geo-scrap and included various combinations of slag and stone aggregates. The concluded results indicate that the optimal mixing ratio is attained by a combination of 75% slag and 25% stone aggregates which is reinforced by tire geo-scrap at a placing level of 120 mm. In this case, the shear strength, internal friction angle, vertical displacement, and dilatancy angle of stone-slag ballast reinforced with geo-scraps exhibited average changes of + 28%, + 9%, - 28%, and - 15%, respectively.
