Objective:Conservative approaches in muscle-invasive bladder cancer(MIBC)have been evolved to avoid aggressive surgery,but are limited to elderly,frail,and patients medically unfit for *** study aimed to assess the response rate of neoadjuvant chemotherapy(naCT)before radiotherapy(rT)in MIBC ***:Forty patients with urothelial carcinoma of stage T2-T4a,N0,M0 were enrolled between November 2013 and November 2015,and treated with three cycles of naCT with ***-naCT response was assessed using response Evaluation Criteria in Solid Tumors(rECIST)*** who achieved complete response(Cr)and partial response(Pr)>50%were treated with radical rT,and those who had Prrogressive disease(PD)underwent radical cystectomy(rC).Survival analysis was done with Kaplan-Meier method and point-to-time events were analyzed with Cox-proportional hazards regression ***:After naCT,35(87.5%)patients achieved either Pr>50%or Cr,and were treated with ***(12.5%)patients who had Prr PD underwent *** patients who received radiation showed Cr after 6 *** follow-up was 43 months(range:10-66 months)and median overall survival(OS)was not ***-year OS,local control,and disease-free survival were 70.1%,60.9%,50.6%,respectively,and 50%of patients preserved their functioning ***-year OS rate was 88.9%in patients who achieved Cr to naCT,73.1%in patients with Pr≥50%and 40%in patients with PrnaCT followed by rT provides a high probability of local response with bladder preservation in Cr *** use of this treatment regimen in carefully selected patients may omit the need for morbid surgery.

This paper presents a new feature descriptor, namely local extreme complete trio pattern （LECTP） for image retrieval application. The LECTP extracts complete extreme to minimal edge information in all possible directions using trio values. The LECTP integrates the local extreme sign trio patterns （LESTP） with magnitude local operator （MLOP） for image retrieval. The performance of the LECTP is tested by conducting three experiments on Corel-5 000, Corel-10 000 and MIT-VisTex color databases, respectively. The results after investigation show a significant improvement in terms of average retrieval precision （ArP） and average retrieval rate （Arr） as compared to the other state-of-the art techniques in content based image retrieval （CBIr）.

Nepal is located in the central part of the greater Himalayan range with a unique series of mountain chains formed by recent mountain building geological events. As one of the youngest mountains in the world it contributes to diversity of plants and also provided barriers to and corridors through which plants migrated during the ice ages. The higher altitudinal variation with the high mountains, deep river valleys and lowland plains combine with the effects of the summer monsoon and dry winter result with an extraordinary diversity of ecosystems including flora and fauna in a relatively small land area. The existing checklists for Nepal record some 6000 species of flowering plants and about 530 ferns. However, the botanical experts estimate that numbers may go up to 7000 when the poorly known remote regions are fully explored. The information on plant endemism in Nepal Himalaya is not adequately known as Nepal is still struggling to complete long awaited Flora of Nepal project. Endemic species are confined to specific areas and are the first to be affected by land use and other global changes. We sought to explore the spatial distribution of endemic plant species in Nepal in relation to the consequences associated with climatic and geologic changes over time in the region with the help of published literature. It was found that the endemism showed marked spatial variation between open moist habitat and dry inner valleys, the former with higher endemism. The updated records showed 312 flowering plant species to be endemic to Nepal with higher endemism around the elevation of 3800e4200 m at sea level. The recent human population explosion, intensified deforestation, habitat fragmentation and modern day environmental changes are posing greater threats to endemic plant in Nepal. Cerebral Venous Sinus Thrombosis (CVST/CSVT) is occlusion of cerebral veins and venous sinuses of brain secondary to blood clot formation resulting in hindrance in the blood drainage system in brain, leading to disturbances the internal homeostasis of brain, raised intracranial pressure, cerebral edema, and 50% of cases will have venous infarction or venous hemorrhage (stroke). CVST although being a rare disorder but may be more common in children than adults with greater risk in neonatal period i.e. first 28 days of life. Here we are discussing a case of Pediatric CVST in a 7-month-old baby boy who presented to Emergency room (Er) with recurrent discrete episodes of vomiting, fever, seizures, drowsiness and respiratory distress. The fatal outcome in our child was attributed to delayed presentation in a tertiary care center, hence missed early diagnosis and treatment. In this child the CVST could be result of amalgamation of complex underlying ongoing multiple pathological processes: an acute systemic illness like sepsis, severe dehydration, undiagnosed and untreated complex congenital heart disease, tetralogy of fallot with osteum secondum atrial septal defect, worsening the coagulopathy. It takes this case even more unique. This discussion is to bring focus on the importance of knowledge about CVST amongst emergency physicians and primary care physicians, specially managing this rare disorder with flummox presentation mimicking other more common disorders, especially in pediatric and neonatal population where definitive history and chief complaints are often vague and difficult to obtain, making it more difficult to diagnose. We the authors hence reporting this case with intent to spread awareness of CVST, how to doubt it, detect it and then manage it, especially in places like Chhattisgarh, India, where CVST is not so uncommon. Several users use metasearch engines directly or indirectly to access and gather data from more than one data sources. The effectiveness of a metasearch engine is majorly determined by the quality of the results and it returns and in response to user queries. The rank aggregation methods which have been proposed until now exploits very limited set of parameters such as total number of used resources and the rankings they achieved from each individual resource. In this work, we use the neural network to merge the score computation module effectively. Initially, we give a query to different search engines and the top n list from each search engine is chosen for further processing our technique. We then merge the top n list based on unique links and we do some parameter calculations such as title based calculation, snippet based calculation, content based calculation, domain calculation, position calculation and co-occurrence calculation. We give the solutions of the calculations with user given ranking of links to the neural network to train the system. The system then rank and merge the links we obtain from different search engines for the query we give. Experimentation results reports a retrieval effectiveness of about 80%, precision of about 79% for user queries and about 72% for benchmark queries. The proposed technique also includes a response time of about 76 ms for 50 links and 144 ms for 100 links.

The composite, 0.5（BiGd0.15Fe0.85O3）-0.5（PbZrO3）, was synthesized using the solid-state reaction technique. The formation of the compound was confirmed by XrD with an orthorhombic structure at room temperature. The impedance parameters were studied using an impedance analyzer in a wide range of frequency （10z-106Hz） at different temperatures. The Nyquist plot suggests the contribution of bulk effect and a slight indication of grain boundary effect and the bulk resistance decreases with a rise in temperature. The presence of temperature-dependent relaxation process occurs in the material. Electrical modulus reveals the presence of the hopping mechanism in the materials. The value of exponent n, pre-factor A and adc were obtained by fitting ac conductivity data with Jonscher＇s universal power law. The activation energies calculated from the ac conductivity were found to be 0.50, 0.46, 0.44, 0.43, 0.42 and 0.38 eV at 1, 10, 50, 100, 500 kHz and I MHz respectively in the temperature region of 110℃-350℃. The dc conductivity was found to increase with the rise in temperature. The activation energy calculated from complex impedance plot and from the fitted Jonscher＇s power law are very close, which results similar type of charge carrier exist in conduction mechanism of the material.

Mine disasters occur predominantly due to methane or coal dust explosion or a combination of *** the top ten worst coal mine disasters in India, nine are due to coal dust explosion. The current paper describes a general overview of the parameters causing dispersion leading to coal dust explosion,and computational fluid dynamics(CFD) simulation study to observe the effects of particle size on dispersion in Indian coal mines. Turbulent kinetic energy(TKE) and velocity vector path of dust-air mixture and dust-free air were simulated to understand their effects on coal dust dispersion. The TKE contours and velocity vector paths for dust-free air were uniform and symmetrical due to resistance-free path available. The TKE contours and velocity vector paths for dust-air mixture shows the asymmetrical distribution of contours, due to entrainment of air with dust particles. Vortices were observed in velocity vector paths which gradually diminish on increment of time sequence. These vortices are dead centres where velocity and coal dust particles concentration are both zero.
