Nutritional Status and Physical Activities among Army Trainees in Public Institutions of Higher Education in Malaysia

This study was conducted to evaluate the nutritional status and physical activities of university students that enrolled in military training with the age range of 20 - 34 years. Nutritional status assessment was based on eating habit questionnaires, anthropometric measurements with body mass index (BMI) and waist-to-hip ratio (WHR), food intake records for three days and also food frequency questionnaire (FFQ). Assessment of physical activities was carried out using physical activities records. Food intake and physical activities data were analyzed using the Nutri-Antro Web (NAW) software. Generally, the subjects were found to have good nutrition knowledge and food intake habits, except for having an irregular meal time (85.0%). Based on BMI studies, 80.0% of male subjects and 78.6% of female subjects were within normal range with the average of 22.94 ± 2.97 kg/m2 and 21.40 ± 3.06 kg/m2 respectively. None of the subjects had WHR above the cut-off point of 1.0 for men and 0.85 for women. The average energy intake for male and female was 2589 ± 687 kcal and 1959 ± 460 kcal respectively. Overall, nutrient intake of both genders fulfilled Malaysia Recommended Nutrient Intake 2005 (RNI) except for the average energy intake of the female subjects. The daily physiccal activities patterns revealed that 73.6% was used for light activities, 19.7% for moderate while 7.4% for heavy activities. The average daily energy expenditure for male subjects (3161 ± 444 kcal) was higher than the female subjects (2492 ± 388 kcal). Both male and female subjects have negative energy balance of -572 kcal and -533 kcal respecttively. Majority of the subjects were categorized as physically active (77.9%). Pearson correlation test showed that physical activities were positively correlated (p < 0.05) with BMI, WHR and nutrient intakes except for the intake of vitamin C. In conclusion, nutritional status and physical activity level of subjects were satisfactory.

Kaempferia parviflora plants derived from in vitro culture were grown in the glasshouse. A comparison of the yield of total phenolics and total flavonoids under varying extraction conditions from rhizomes harvested from plants of different ages was undertaken. The results showed that phenolic and flavonoid contents in the rhizomes were highest 8 months after planting. Another study found that 2 g rhizomes extracted in 50 ml of water at 90°C for 120 minutes gave the best yield of phenolics and flavonoids. Under these conditions, an average of 210 mg GAE/g dry weight of total phenolics and 81 μg QCE/g dry weight of total flavonoids were obtained.

Electrical Performance Study of a Large Area Multicrystalline Silicon Solar Cell Using a Current Shunt and a Micropotentiometer

In this paper, a new technique using a Current Shunt and a Micropotentiometer has been used to study the electrical performance of a large area multicrystalline silicon solar cell at outdoor conditions. The electrical performance is mainly described by measuring both cell short circuit current and open circuit voltage. The measurements of this cell by using multimeters suffer from some problems because the cell has high current intensity with low output voltage. So, the solar cell short circuit current values are obtained by measuring the voltage developed across a known resistance Current Shunt. Samples of the obtained current values are accurately calibrated by using a Micropotentiometer (μpot) thermal element (TE) to validate this new measuring technique. Moreover, the solar cell open circuit voltage has been measured. Besides, the cell output power has been calculated and can be correlated with the measured incident radiation.

Homocystiene and C-Reactive Protein in Detection of Frailty

Background and Aim of the Work: Frailty is a state of reduced physiological reserve and is associated with increased susceptibility to disability. It is associated with a high morbidity and mortality. The aim of this work is to assess the association between the levels of homocystiene and C-reactive protein and frailty and to examine the ability of homocystiene as a new marker to detect frailty. Subjects and Methods: A total number of 104 elderly subjects (above 60 years old) were included in this study and they were subjected to history, examination, comprehensive geriatric assessment, and laboratory investigations including: C-reactive protein (CRP) and Homocystiene (Hcy). The patients were divided into frail and non-frail groups using Fried’s criteria as applied by Avila-Funes et al., 2008 and each group included 52 patients. Results: There was no significant difference between the two groups as regards age gender or smoking habits. But there were higher levels of CRP (>10 mg/dl) and homocystiene (>12 mmol/l) among frail cases and by comparing both homocystiene was more sensitive in detection of frailty. Conclusion: Frailty was associated with eleveted homocystiene and CRP and homocystiene is more specific than CRP in detection of frailty.

Ability of Comprehensive Geriatric Assessment to Detect Frailty

Background and Aim of the Work: Frailty is a state of reduced physiological reserve associated with increased susceptibility to disability. It is associated with a high morbidity and mortality. This work assessed the ability of comprehensive geriatric assessment to detect frailty in elderly patients. Subjects and Methods: A total number of 104 elderly patients (above 60 years old) were included in this study and they were subjected to a comprehensive geriatric assessment (CGA) including: (history and full clinical examination, Mini mental status examination (MMSE), Geriatric depression scale (GDS), Activities of daily living (ADL), Instrumental activities of daily living (IADL). The patients were divided into frail and non-frail groups using Fried’s criteria as applied by Avila-Funes et al., 2008, each group included 52 patients. Results: There were no significant differences between the two groups regarding age gender or smoking habits. But there was higher percentage of ADL, IADL dependence in addition to higher incidence of depression and cognitive impairment among frail cases. Conclusion: Comprehensive geriatric assessment can be used to detect frailty and its associated impairment and comorbidities.

Comparative Human Landing Catch and CDC Light Trap in Mosquito Sampling in Knowlesi Malaria Endemic Areas in Peninsula Malaysia

Adult mosquito sampling techniques are essential for monitoring transmission of malaria and other mosquito borne infections. Preference for any sampling technique depends on both its field efficiency and the characteristics of local vector populations. Surveys on adult mosquitoes using Human Landing Catch (HLC) and CO2-baited CDC light trap (CDC-LT) techniques were conducted in several knowlesi malaria endemic areas between the months of March to December 2012 in several states of Peninsula Malaysia. These two techniques were relatively compared to determine the preferences of anopheline mosquitoes towards CO2-baited CDC-LT technique using HLC technique as the reference method. Cx. gelidus, An. maculates and An. introlatus were the main three species collected by HLC technique, whereas the species collected by CO2-baited CDC-LT technique were mostly An. cracens, Ar. durhami and Coquillettidia species. Most of the Anopheles species were collected almost exclusively by the human collectors except for An. cracens and An. introlatus which were collected using both techniques. Anopheles cracens was the most dominant species captured using CO2-baited CDC-LT technique. This is the first report showing An. cracens was caught using CO2-baited CDC-LT technique in Malaysia.

Background: Poor postoperative pain control leads to longer postoperative care, longer hospital stay and decreased patient overall satisfaction. Aim: To compare the efficacy and safety of bilateral ultrasound-guided quadratus lumborum block versus lumbar epidural block on the management of postoperative pain following major lower abdominal cancer surgery. Methods: The study was a double-blinded, and randomized study, conducted in South Egypt Cancer Institute, Assiut University, Egypt. It included cancer patients scheduled for major lower abdominal cancer surgery in the period from 2019 to 2020. They were divided into two groups: Group Ι received pre-emptive ultrasound-guided Quadratus Lumborum Block (QLB) with 25 mL of 0.25% bupivacaine on each side of the abdominal wall before induction of General Anesthesia (GA), and Group II received pre-emptive lumbar epidural block with 15 mL of 0.25% bupivacaine before induction of GA. VAS score, and time of the first analgesic request and postoperative total analgesic consumption were evaluated. Results: Sixty patients were included in our study. Relationship between peripheral arterial disease, carotid intima-media thickness and C-reactive protein in elderly diabetic patients

Purpose: To study the association between carotid intima media thickness (CIMT) and high sensitivity CRP (hs-CRP) level as markers for PAD in elderly patients with DM. Subjects: A case-control study on 90 participants aged 60 years and older divided into 60 cases (30 patients with DM alone and 30 patients with DM and comorbidities) and 30 healthy controls. All were assessed by measuring CIMT, ankle brachial index (AB), and markers for cardiovascular disease such as high-sensitivity CRP (hs-CRP), total cholesterol (TC), triglycerides (TG), high density lipoprotein (HDL), and low density lipoprotein (LDL). Results: hs-CRP levels showed statistically significant difference being highest among patients with DM and comorbidities and lowest among controls (P < 0.001). Also, symptoms of PAD were significantly higher among cases than controls. ABI was able to detect PAD in many asymptomatic patients. Color changes were present in only 43.30% (n = 26) of positive PAD cases while delayed wound healing, claudication pain, rest pain, cold extremities, and trophic changes were present in 23.30% (n = 14), 16.71% (n = 10), 16.71% (n = 10), 45.00% (n = 27), and 21.7% (n = 13). Using logistic regression analysis revealed that DM, CIMT, and hs-CRP were independent predictors for PAD (OR = 4.194, 7.236, 1.003;P value = 0.044, 0.25, 0.031) after adjustment of other coronary risk factors such as sex, smoking, hypertension, TC, and TG. Conclusion: Diabetic elderly have higher prevalence of asymptomatic PAD thannon-diabetics using solely ABI. DM, CIMT, and hs-CRP are independent predictors for the occurrence of PAD. Hs-CRP levels are highest among diabetics with comorbidities.

Effect of Local Wound Infiltration with Ketamine versus Dexmedetomidine Added to Bupivacaine on Inflammatory Cytokines, a Randomized Clinical Trial

Background: Ketamine or dexmedetomidine as an adjuvant to bupivacaine in local wound infiltration attenuated postoperative stress response, especially with ketamine in patients undergoing total abdominal hysterectomy. Objectives: Compare effect of local wound infiltration with ketamine or dexmedetomidine added to bupivacaine to bupivacaine alone on inflammatory cytokine response after total abdominal hysterectomy. Methods: Sixty female patients with endometrial carcinoma underwent total abdominal hysterectomy and scheduled to receive local wound infiltration before wound closure either with one of three;40 ml of 0.25% bupivacaine alone (C Group) or with the addition of 2 mg/kg ketamine (K Group) or 2 μg/kg dexmedetomidine (D Group). After extubation, they were followed up for postoperative interleukin 6 (IL6), IL1β, IL10, and TNF-α levels were assessed at baseline, pre-infiltration, 6, and 24 h by blood samples obtained from each patient, hemodynamic variables, analgesic profile and side effects. Results: Inflammatory cytokines response was attenuated in K and D groups, evidenced by decreased mean pro-inflammatory cytokines IL6, TNF-α, and increased anti-inflammatory IL10 at 6 and 24 h postoperatively compared to pre-infiltration levels (p ≤ 0.01) with preservation of IL1β at its preoperative level (p > 0.05). Attenuation was more in K and D groups than in the C group and was highest in the K group with decreased 1st request, total morphine consumption without serious side effect. Conclusion: Local wound infiltration with ketamine or dexmedetomidine added to bupivacaine has a good postoperative analgesic profile and attenuated cytokines inflammatory response more than bupivacaine alone after total abdominal hysterectomy, with highest attenuation in ketamine group.

降低制动器噪声已成为制动器工程师们具有挑战性的任务。目前关于处理盘式制动器噪声方面已有许多建议,但过去在鼓式制动器系统方面解决方法很少。本文企图提出一种采用有限元方法通过改进结构,降低鼓式制动器噪声。开发并证实了一个真实鼓式制动器的三维有限元(FE)模型。首先用模拟的基本有限元模型预测噪声,然后再提出几个改进结构,最后对选择的改进模型进行动态不稳定过程分析,以便确认由复杂的固有值分析得出的预测结果。
