Introduction: High blood pressure is a major public health problem worldwide due to its frequency and cardiovascular complications. Adherence to treatment for chronic diseases is a global problem. The aim was to study therapeutic adherence in hypertensive patients followed in ambulatory. Materials and Methods: This was a cross-sectional, descriptive study with prospective recruitment that took place from July 1 to December 31, 2022 (6 months) in the cardiology department of the university hospital of Kati. The variables studied were sociodemographic data, cardiovascular risk factors, comorbidities, the possession of insurance and compliance (the Girerd questionnaire was used to assess adherence). Results: A total of 1182 patients were consulted, including 887 for hypertension, a frequency of 75%. Fifty-six patients were included in the study. The average age was 58.18 ± 13.25 years with extremes of 30 and 80 years. There was a female predominance (75%) with a sex ratio of 0.3. The majority of patients lived in urban areas (89.3%). Out-of-school patients accounted for 44.6%, more than half of patients or 55.4% had no income, patients with medical coverage accounted for 67.9% of cases. The main risk factors were physical inactivity (25%) followed by smoking 14.3%. More than 71% of patients had a compliance problem and the main reasons were forgetting to take the drug with 73.2%, followed by delayed treatment of 50% and drug discontinuation of 28.6%. Conclusion: Compliance is a real challenge and a major public health issue. This study allowed us to find a real problem of compliance in our hypertensive patients. There was a statistically significant relationship between drug adherence and forgetting to take the drug and drug discontinuation.

Scleroderma (or systemic sclerosis) is a disease characterized by abnormalities in the functioning of small blood vessels and the immune system, ultimately leading to inflammation and excessive fibrosis of the skin and various organs, including the heart. Management must be multidisciplinary, to avoid complications that are often serious. We report the case of a 20-year-old patient with no known cardiovascular history who consults for dyspnea, and retrosternal pain associated with a dry cough. On physical examination, she had tachycardia, swelling of the lower limbs, jugular turgidity, and deafening heart sounds. Cardiac Doppler ultrasound shows dilation of the right cavities, paradoxical septum and significant pulmonary arterial hypertension, pericardial effusion of medium abundance. On oral examination, it presents an ulceration of the lips, dermatological examination finds scattered hypo chromic spots in the body, more accentuated in the face. Before the hypo chromic dermatosis, a dermatological consultation was carried out with an autoimmune assessment that came back positive for systemic scleroderma.

Rare Case of Abortion Complication: Bilateral Abscess of the Shoulders

Septic abortions usually result from intrauterine maneuvers using non-sterile instruments or materials;they are much more common when abortion is illegal. We report the clinical case of a 28-year-old patient following a complicated abortion of endometritis and bilateral abscess of the shoulders, requiring surgical drainage and antibiotic therapy. The bacterium identified was Escherichia coli, sensitive to ceftriaxone.

Cotton Leafhoppers, Amrasca biguttula (Ishida, 1913) (Hemiptera: Cicadellidae), Identified as a New Species on Okra and Guinea Sorrel in Niger

Leafhoppers (jassids) are well-known pests of vegetable crops in Niger. They are often part of a parasitic complex that causes varying degrees of damage. Over the last two crop years, the Niamey region has seen a heavy outbreak of leaf hoppers on okra and guinea sorrel. These insects alone have caused spectacular damage, resulting in losses ranging from 50% to 100% of unharvested plants. Following this observation, infested fields were surveyed, specimens sampled, and the responsible species identified. Two sites were surveyed in the city of Niamey. At each site, two plots of okra and two of guinea sorrel were visited. Fifty (50) plants were randomly sampled using the double “W” method per plot. The sampled plants were used for active and passive leaf hopper capture, damage description and loss assessment. Captured leaf hoppers were identified based on their morphology observed with a binocular magnifying glass and compared with the data in the identification keys. The symptoms observed in the plots were yellowing, leaf curling and drying, stunting, abortion of flowers and immature fruit and very low production. Identification results revealed the single species Amrasca biguttula (Ishida, 1913), which can be considered a new invasive species in Niger. Further confirmation of this identification by molecular tests, the distribution of the pest in Niger and the development of appropriate control methods would yield good prospects.

Double Discordance with Pulmonary Stenosis: About a Clinical Case

Introduction: Double discordance or corrected transposition of the great vessels is a rare congenital heart disease. It is an atrioventricular and ventriculo-arterial mismatch. It is a complex and unusual form of congenital heart disease. Often asymptomatic, in its isolated form, bradycardia, murmur and cyanosis can be a mode of revelation. Case presentation: We report the case of an 11-year-old child, asymptomatic at birth, after he started to have dyspnea on exertion, recurrent bronchitis, motivating a cardiological consultation. On cardiac physical examination, the heart sounds regular, not rapid at 81 BPM with a grade 3-4/6 systolic murmur at the 4th left EIC (Intercostal space). EKG (electrocardiogram) shows PR interval at 0.20 seconds. Cardiac ultrasound reveals atrioventricular discordance, vascular malposition, anterior aorta, the two vessels placed side by side, minimal pulmonary insufficiency, a small leak at the levels of the mitral and tricuspid valves, dystrophic pulmonary valves with an average gradient of 91 mmHg, max at 158 mmHg. Regular follow-up has been recommended through the performance of a clinical examination and cardiac ultrasound. Conclusion: The double discordance can be asymptomatic, and survival can be long in the isolated forms, but the evolution is not always benign, especially in the associated forms. It depends on the function of the systemic right ventricle and associated abnormalities.

Neurological Sequelae in Cranioencephalic Trauma Patients at the National Hospital in Zinder (Niger)

Introduction: Cranioencephalic trauma is a frequent reason for admission to emergency departments and is a source of mild to severe neuropsychological defects that will persist over time. Their management remains difficult. Objectives: To evaluate the sequelae presented by patients suffering from cranioencephalic trauma. Methods: This was a retrospective, descriptive, cross-sectional study conducted at the National Hospital of Zinder. It will include all patients admitted and hospitalised in the emergency, intensive care and neurosurgery departments of Zinder National Hospital for head injury over a period of 28 months from 1 January 2016 to 30 April 2018. Brain scan, X-ray were the imaging tests used. Results: Out of a total of 974 admissions, 367 were retained, i.e. 37.6%, with a male predominance (82.6%). The average age was 26.5 years. MVAs were represented in 89.7% of cases. Moderate CTE accounted for 64% of cases. Altered consciousness was reported in 295 patients (80.38% of cases). Brain scans were used in 76.7% of cases and skull X-rays in 4.2% of cases. Medical management was performed in all patients. Treatment was operative in 78 patients (21.25%). Recovery without immediate sequelae was found in 187 patients (50.9%). Persistent headache represented 47.7% of the late sequelae observed in the patients, epileptic seizures represented 16.8% of the late sequelae, neurological deficit represented 14.7%. Conclusion: Cranioencephalic trauma represents a major public health issue. Although their in-hospital management remains a challenge, post-hospital management related to the appearance of sequelae remains another.

Introduction: The metabolic syndrome is a clinical entity defined by the association in the same subject of four of the following five factors: abdominal obesity (or android), type 2 diabetes, hypertension, hypo HDL cholesterolemia, hypertriglyceridemia. The aim of the work was to study the epidemiological, clinical-biological profiles and complications of patients in the cardiology and internal medicine department of the Mother-Child CHU “Luxembourg”. Materials and Methods: We carried out a cross-sectional study, from September 2019 to June 2020 in the cardiology and internal department of the mother-child CHU “Luxembourg”. All patients with metabolic syndrome admitted to the two departments who agreed to participate during the study period were included in the study. These patients meet the 2009 harmonization attempt criteria (IDF 2009). The variables studied were: socio-demographic characteristics (sex, age, profession), pathological history, clinical signs (functional signs, BMI, waist size), paraclinical signs (ECG, cardiac ultrasound, laboratory assessment) and complications. Results: A total of 104 patients were included. The female sex represented 58.65% of cases. The 60 to 69 age group was the majority with 40.78%, housewives occupied 41.34% of cases. Diabetes, arterial hypertension and dyslipidemia were the most represented cardiovascular risk factors with respectively: 80.77%, 79.81% and 53.85%. Visual blurring was the most common functional sign with 23.08% of cases. Moderate obesity was observed in 34% of patients. Waist circumference was high in 39.53% of male patients and 37.70% of female patients. Cardiac ultrasound found a severely collapsed ejection fraction in 25% of patients. Introduction: Cerebral palsy is the most common motor disorder of childhood according to the Center for Disease Control and Prevention (CDC). It is much more common in premature infants. The majority of fixed organic lesions occur during the development of the central nervous system. Objective: To study the epidemiological, clinical, paraclinical and prognostic profile of children aged 0 to 5 years with Cerebral Palsy (CP) seen in consultation in the medical neurology department of the National Hospital of Niamey. Methods: This was a transverse descriptive study with prospective collection of patient records (0 - 5 years) over a 6-month period. Results: During our study, 100 children aged 0 to 5 years had to be consulted. The prevalence was 61%. 62.3% of patients were male. The mean age was 2.01 years. The majority of the patients were from the urban area (63.93%). Almost half of the mothers had no schooling. 4.92% of patients were from consanguineous marriages. Pregnancies were monitored with at least 4 prenatal visit in 86.98% of cases and 83.61% of children were born at term. In 75.41% of cases, patients were born by eutocic delivery. In 36.07% of cases, the patients were resuscitated at birth. Fetal distress was present in 29.5%. Seizures were present in 49.18% of the patients of which 50% were generalized. Tonicity was not good in 35.15% and among them 52.63% had hypertonia. 18.03% of patients had a motor deficit. For 34.43% of the patients the first nerve was affected, followed by the third nerve with 13.11%. 65.57% of the patients had fixed retardation and for 78.69% of the patients the retardation was homogeneous. Neuromalaria, meningitis and hydrocephalus were the most incriminated causes of delay in 63.51%, 19.67% and 18.03% of cases respectively. Anticonvulsants and psychoanaleptics were the most used in 49.18% and 32.79% of cases respectively. 32.79% of patients had received motor rehabilitation. All mothers had received psychotherapy.
