The aim of the present study was to measure the prevalence of multimorbidity in Bangladesh,India and China,and to assess the relationship between multimorbidity and patient＇s opinion regarding their involvement in healthcare decision-making and overall satisfaction of healthcare ***-sectional data on 18 696 men and women aged 18 and above were collected from the World Health Survey of World Health Organization（WHO）.Outcome variables were subjective rating of（1） healthcare system＇s ability to involve patients in decision-making,and（2） satisfaction with the way healthcare system runs in the ***-reported chronic conditions were used to measure the prevalence of *** of 9 chronic conditions,back pain,arthritis,and chronic cough appeared to be the most prevalent ones among majority of the *** one-third of the participants in China（30.7%） and two-thirds in Bangladesh（66.1%） and India（66.6%） reported having at least one chronic *** of multimorbidity was highest in India（34.3%） followed by Bangladesh（28.8%） and China（14.3%）.In Bangladesh,India and China,respectively 70.5%,41.7%,61.3% women and 54.5%,42.8% and 58.8 % men expressed dissatisfaction regarding the way healthcare system runs in their *** Bangladesh and India,men who were living with multimorbidity were more likely to rate the patient-centeredness as ＂bad＂ than those who had no disease *** study suggests that the prevalence of multimorbidity was remarkably high especially in Bangladesh and *** likelihood of dissatisfaction about healthcare system among multimorbid patients might be indicative of inadequacy in the provision of care in qualitative and quantitative *** words

Spondyloarthritis Associated with Inflammatory Bowel Disease: Study of 15 Observations in Guinea

Objective: Rheumatologic disorders of chronic inflammatory bowel disease (IBD) and reactive arthritis with a digestive origin are part of the spondyloarthritis family. In black Africa, the prevalence of SpA associated with IBD is not clearly established. Thus the objective of our study was to describe the clinical and radiological characteristics of spondyloarthritis associated with IBD. Patients and Method: We carried out a prospective study in the rheumatology department of CHU Ignace Deen between January and December 2019. The diagnosis of SpA was based on clinical and biological arguments in accordance with the criteria of Amor and ASAS. Results: Fifteen observations of spondyloarthritis associated with IBD were collected in patients mean age 52 years with extremes of 32 and 65 years. 9 (53.33%) were female. Ten patients had Crohn’s disease (CD) and 5 had ulcerative colitis (UC). The mean number of pushes was 2.5 ± 1.2. The average diagnostic time was 46 months. Sacroiliitis was present in 73.3% of cases and the mean mSASSS score at diagnosis was 32.11/72. In total, corticosteroids were used in 9 (60%) of patients, NSAIDs in 26.6% while DMARDs salazopyrine and methotrexate in 33.3% and 20% of patients, respectively. Conclusion: The MICI and SpA association is undoubtedly underestimated in our regions. Better collaboration between rheumatologists and gastroenterologists could facilitate diagnosis and improve care.

Epidemiologic, Diagnostic, and Therapeutic Features of Urinary Stone Disease in Northern Benin from 2018 to 2023

Objective: To determine the epidemiologic, diagnostic, and therapeutic features of urinary in northern Benin from 2018 to 2023. Patients and Method: The study was an observational and descriptive one. We exhaustively and retrospectively collected data from the medical records of the patients that were managed from February 1, 2018 to February 1, 2023 at the Department of Surgery of “Centre Hospitalier Universitaire Départmental Borgou-Alibori or CHUD-BA”. Results: Among the 4522 patients managed, 75% or 1.7% had urinary stone disease. Their mean age was 41 years. The sex ratio was 2.75, the males being predominantly affected. Of the 75 cases of urinary stone disease, 30.7% (n = 23) were renal, 46.7% (n = 35) were ureteral, and 22.7% (n = 17) were vesical. The patients came from variable geographical areas, ethnic groups, and professional sectors. Their main complaints were pain (57.3%), hematuria (14.7%), dysuria (12%), and fever (5.3%). 15 (20%) patients, i.e., 7 renal stone and 8 ureteral stone patients, had an obstructive renal failure. The imaging tools available were ultrasonography, tomography and computed tomographic scan. 11 of the bladder stone cases (64.7%) were associated with a bladder outlet obstruction. Overall, 38.7% of the patients had either diabetes mellitus (6.7%), hypertension or high blood pressure (6.7%), benign prostate hyperplasia (8%), urinary schistosomiasis (6.7%), ureteral pelvic junction obstruction (2.7%), and prostate cancer (1.3%). Of the 19 patients whose dietary behavior was investigated 15 (79%), 11 (57.9%) et 11 (57.9%) demonstrated a diet which was respectively rich in milk, fromage and meat. 100% of bladder stones were removed through cystolithotomy. Renal stones were removed by pyelolithtomy (52.2%) and nephrolithotomy (21.7%). Ureteral stones were removed through ureterolithotomy (40%), expelled by means of alpha-blockers (40%) or managed with ureteral pigtail stent (20%). Predicting the effects of selected reservoir petrophysical properties on bottomhole pressure via three computational intelligence techniques

This study investigates the effects of selected petrophysical properties on predicting flowing well bottomhole *** efficiently situate the essence of this investigation,genetic,imperialist competitive and whale optimization algorithms were used in predicting the bottomhole pressure of a reservoir using production data and some selected petrophysical properties as independent input variables.A total of 15,633 data sets were collected from Volvo field in Norway,and after screening the data,a total of 9161 data sets were used to develop apt computational intelligence *** data were randomly divided into three different groups:training,validation,and testing *** case scenarios were considered in this *** first scenario involved the prediction of flowing bottomhole pressure using only eleven independent variables,while the second scenario bothered on the prediction of the same flowing bottomhole pressure using the same independent variables and two selected petrophysical properties(porosity and permeability).Each of the two scenarios involved as implied in the first scenario,the use of three(3)heuristic search optimizers to determine optimal model *** optimizers were allowed to choose the optimal number of layers(between 1 and 10),the optimal number of nodal points(between 10 and 100)for each layer and the optimal learning rate required per task/*** results,showed that the models were able to learn the problems well with the learning rate fixed from 0.001 to 0.0001,although this became successively slower as the leaning rate *** the chosen model configuration,the results suggest that a moderate learning rate of 0.0001 results in good model performance on the trained and tested data *** the three heuristic search optimizers based on minimum MSE,RMSE,MAE and highest coefficient of determination(R^(2))for the actual and predicted values,shows that the imperialist competitive algorithm optimizer predicted

Application of artificial intelligence in predicting the dynamics of bottom hole pressure for under-balanced drilling:Extra tree compared with feed forward neural network model

This study used six fields data alongside correlation heat map to evaluate the field parameters that affect the accuracy of bottom hole pressure(BHP)*** six oil field data were acquired using measurement while drilling device to collect surface measurements of the downhole pressure data while *** the two case studies,measured field data of the wellbore filled with gasified mud system was utilized,and the wellbores were drilled using rotary jointed drill *** Randomized Tree and feed forward neural network algorithms were used to develop models that can predict with high accuracy,BHP from measured field *** modeling purpose,an extensive data from six fields was used,and the proposed model was further validated with two data from two new *** gathered data encompasses a variety of well data,general information/data,depths,hole size,and *** developed model was compared with data obtained from two new fields based on its capability,stability and *** result and model’s performance from the error analysis revealed that the two proposed Extra Tree and Feed Forward models replicate the bottom hole pressure data with R2 greater than *** high values of R^(2) for the two models suggest the relative reliability of the modelling *** magnitudes of mean squared error and mean absolute percentage error for the predicted BHPs from both models range from 0.33 to 0.34 and 2.02%-2.14%,for the Extra tree model and 0.40-0.41 and 3.90%e3.99%for Feed Forward model respectively;the least errors were recorded for the Extra Tree ***,the mean absolute error of the Extra Tree model for both fields(9.13-10.39 psi)are lower than that of the Feed Forward model(10.98-11 psi),thus showing the higher precision of the Extra Tree model relative to the Feed Forward *** has shown that underbalanced operation does not guarantee the improvement of horizontal well’s extension ability,because it mainly depends on t

The supersonic dehydration of natural gas is gaining more attention due to its numerous advantages over the conventional natural gas dehydration ***,supersonic separators have seen minimal field applications despite the multiple benefits over other gas dehydration *** has been mostly attributed to the uncertainty in ascertaining the design and operating parameters that should be monitored to ensure optimum dehydration of the supersonic separation *** this study,the decision tree machine learning model is employed in investigating the effects of design and operating parameters(inlet and outlet pressures,nozzle length,throat diameter,and pressure loss ratio)on the supersonic separator performance during dehydration of natural *** model results show that the significant parameters influencing the shock wave location are the pressure loss ratio and nozzle *** former was found to have the most significant effect on the dew point *** dehydration efficiency is mainly dependent on the pressure loss ratio,nozzle throat diameter,and the nozzle *** the machine learning model-accuracy with a 1-D iterative model,the machine learning model outperformed the 1-D iterative model with a lower mean average percentage error(MAPE)of 5.98 relative to 15.44 as obtained for the 1-D model.

Hypertension, obesity, smoking, dyslipidemia, and type 2 diabetes (T2D) are the major risk factors for developing cardiovascular diseases (CVD). Recent studies revealed that taxi-motorbike drivers (TMDs) in Cotonou had higher rates of CVD risk factors, but their impacts on cardiovascular events have rarely been studied. The Framingham risk score (FRS) is an algorithm that considers CVD risk factors and estimates the risk of developing CVD in the next 10 years. Our objectives were to assess the 10-year CVD risk predicted by the FRS, and to examine the relationships of 10-year CVD risk with plasma iron and potassium levels among TMDs. We included 134 TMDs (22 - 59 years old) who had no prior diagnosis of CVD or T2D, and not taking medications affecting iron and potassium homeostasis. Conventional cardiovascular risk factors were used to calculate the 10-year CVD risk, which was categorized as low (20%). FRS > 2%, which corresponded to the 75th percentile of FRS distribution in our study population, was used as a cut-off value to classify participants into two groups. Plasma iron and potassium levels were segregated into tertiles and their associations with 10-year CVD risk were quantified by multivariate-adjusted logistic regression to calculate the odd ratios (ORs) to being above the 75th percentile of 10-year CVD risk with the corresponding 95% confidence intervals (CIs). We found that 62.0% of participants had at least one of cardiovascular risk factors. Approximately 97.8% of TMDs had 10-year CVD risk 4.8 mmol/L led to an 83% risk reduction of having 10-year CVD risk > 2% (OR = 0.17, 95% CI: 0.04 - 0.82, P = 0.027). In conclusion, our findings showed that high plasma potassium levels associate with reduced 10-year CVD risk among TMDs. Interventions focused on monitoring of plasma potassium, particularly in those with existing cardiovascular risk factors, may help prevent CVD.

High white blood cell count (WBC) and insulin resistance (IR) are interrelated events that contribute to non-communicable diseases (NCDs), including type-2 diabetes (T2D). However, associations between IR and hematological parameters have never been explored in populations of Benin. The aims of this study were to determine the prevalence of IR and associated hematological parameters in taxi-motorbike drivers (TMDs) working in Cotonou. A total of 133 participants were analyzed in this cross-sectional study. Complete blood count, including WBC and platelet, as well as fasting plasma glucose and insulin, were performed by standard procedures. IR was assessed using the Homeostatic Model Assessment for Insulin Resistance (HOMA-IR). Factors associated with IR, their odds ratios (ORs) and 95% confidence intervals (CIs) were determined by logistic regression analysis. The mean age of the study participants was 39.3 years. The HOMA-IR cut-off (75th percentile) for IR was 5.9. The overall prevalence of IR was 24.1%. IR increased with the increase of exposure duration and WBC levels. Logistic regression analysis revealed that the risk of IR increased significantly with higher total WBC, with adjusted ORs (95% CI) for the second and third tertiles of 3.56 (1.10 - 11.58) and 4.01 (1.21 - 13.31), respectively. Similar patterns of associations were observed in an analysis restricted to non-drinkers, although these estimates lacked statistical significance. BMI > 24.2 kg/m2 was independently associated with an increased risk of IR (OR = 3.82, 95% CI: 1.33 - 11.03, P = 0.013). In conclusion, the prevalence of IR in TMDs was 24.1%. IR was significantly associated with elevated WBC count and BMI. WBC may serve as a biomarker to identify individuals at the greatest IR risk.
