Inflammatory bowel disease(IBD)is more common in adults than in children.Onset of IBD before 17 years of age is referred as pediatric onset IBD and is further categorized as very early onset IBD(VEO-IBD)for children who are diagnosed before 6 years of age,infantile IBD who had the disease before 2 years of age and neonatal onset IBD for children less than 28 days of life.Children presenting with early onset disease may have a monogenic basis.Knowledge and awareness of the clinical manifestations facilitates early evaluation and diagnosis.Next generation sequencing is helpful in making the genetic diagnosis.Treatment of childhood IBD is difficult;targeted therapies and hematopoietic stem cell transplantation form the mainstay.In this review we aim to summarize the genetic defects associated with IBD phenotype.We describe genetic location and functions of various genetic defect associated with VEO-IBD with their key clinical manifestations.We also provide clinical clues to suspect these conditions and approaches to the diagnosis of these disorders and suitable treatment options.

BACKGROUND Non-alcoholic fatty liver disease(NAFLD)increases the risk of cardiovascular diseases independently of other risk factors.However,data on its effect on cardiovascular outcomes in coronavirus disease 2019(COVID-19)hospitalizations with varied obesity levels is scarce.Clinical management and patient care depend on understanding COVID-19 admission results in NAFLD patients with varying obesity levels.AIM To study the in-hospital outcomes in COVID-19 patients with NAFLD by severity of obesity.METHODS COVID-19 hospitalizations with NAFLD were identified using International Classification of Disease-10 CM codes in the 2020 National Inpatient Sample database.Overweight and Obesity Classes Ⅰ,Ⅱ,and Ⅲ(body mass index 30-40)were compared.Major adverse cardiac and cerebrovascular events(MACCE)(all-cause mortality,acute myocardial infarction,cardiac arrest,and stroke)were compared between groups.Multivariable regression analyses adjusted for sociodemographic,hospitalization features,and comorbidities.RESULTS Our analysis comprised 13260 hospitalizations,7.3% of which were overweight,24.3% Class Ⅰ,24.1% Class Ⅱ,and 44.3% Class Ⅲ.Class Ⅲ obesity includes younger patients,blacks,females,diabetics,and hypertensive patients.On multivariable logistic analysis,Class Ⅲ obese patients had higher risks of MACCE,inpatient mortality,and respiratory failure than Class Ⅰ obese patients.Class Ⅱ obesity showed increased risks of MACCE,inpatient mortality,and respiratory failure than Class I,but not significantly.All obesity classes had non-significant risks of MACCE,inpatient mortality,and respiratory failure compared to the overweight group.CONCLUSION Class Ⅲ obese NAFLD COVID-19 patients had a greater risk of adverse outcomes than class Ⅰ.Using the overweight group as the reference,unfavorable outcomes were not significantly different.Morbid obesity had a greater risk of MACCE regardless of the referent group(overweight or Class Ⅰ obese)compared to overweight NAFLD patients admitted with COVID-19.

Dynamic Behaviour of Pond Ash Mixed with Coir and Crumb Rubber

An experimental program was conducted to determine the dynamic properties of pond ash(PA)and pond ash mixed with admixtures like crumb rubber(CR)and coir fiber(CF)which are wastes of different industries.Additives CR and CF were mixed separately with pond ash in a percentage ranging from 0%–20%at an interval of 5%and 0%–2%at an interval of 0.5%by weight of pond ash,respectively.Dynamic properties(Shear modulus,Number of cycles to initiate liquefaction,Degradation index and Damping ratio)for pond ash mixes were determined using strain-controlled cyclic tri-axial apparatus.The shear strain was kept constant at 1.2%and the frequency varied from 0.4 to 1 Hz.Liquefaction resistance improves when crumb rubber is added to pond ash,however adding Coir fiber to pond ash has essentially no effect on the number of cycles required to initiate liquefaction.The dynamic shear modulus decreases when crumb rubber was mixed in pond ash whereas when coir fiber is added to pond ash,the dynamic shear modulus increases significantly.A mix of PA,CR,and CF was also tried to balance the number of cycles required to initiate liquefaction and shear modulus.The mixed sample in the proportion of PA(84%)+CR(15)+CF(1%)can be termed a balanced mix as the number of cycles to initiate liquefaction was more than all PA-CF mixes and also the shear modulus was more than all PA-CR mixes.Damping ratio increases with increasing frequency of loading(0.4 to 1 Hz).

Polysiloxane coatings on yttria stabilized zirconia(YSZ)microspheres of 500μm were simulated in a spout fluidized bed coater using Multiphase Flow with Interface Exchange-Discrete Element Modelling(MFiX-DEM).Two different coater configurations were developed to study the influence of gas velocity and its distribution on particle dynamics.The presence of the Wurster tube not only enhances the dis-tribution but also increases the overall residence time of the particles.Investigations were also carried out with different Wurster tube positions(normal,10%and 20%lowered from its initial position).Among these,20%lowered Wurster tube position demonstrated the most effective coating process.The effects of gas inlet pressure on the average gas velocity and the distribution of particles were analyzed.More than 97%of the particles can be retained.The derived results,including average gas velocity,particle retention percentage,and distribution of particles with gas velocity,are being used to guide the experimental work in obtaining defect-free coatings for YSZ microspheres.

Effect of COVID-19 pandemic on blood transfusion service:an experience from a regional blood transfusion center

The unforeseen and uncertain life-threatening situation of the COVID-19 pandemic dramatically affected all areas of the human daily work schedule.This study was designed to assess the impact of the COVID-19 pandemic on blood transfusion services and discuss the adopted confrontation measures for uninterrupted blood supply during the pandemic situation.The data on blood donation,blood component preparation,and issue from January 2019 to December 2022 were collected from the inventory registers of the RBTC,Delhi,India.Compared to the non-pandemic year 2019,during the year 2020,all variables decreased gradually.The observed maximum decrease in variables such as blood collection(–79.16%)in the month of October,blood issue(–71.61%)in the month of August,random donor platelets(RDP)preparation(–98.09%)in the month of October,RDP issue(–86.08%)in the month of September,fresh frozen plasma(FFP)preparation(–100%)in the month of October,and FFP issue(–96.08%)in the month of July with an annual decrease of–45.52%,–42.87%,–33.00%,–59.79%,–40.98%,and–54.48%,respectively,as compared to year 2019.Compared to year 2020,in year 2021,the annual increase in blood collection,blood issue,FFP preparation,FFP issue,RDP preparation,and RDP issue was+50.20%,+21.68%,+65.31%,+78.52%,+116.23%,and+213.30%,respectively.Our study results show that the COVID-19 pandemic has significantly affected blood transfusion services at our blood bank.The adopted coping strategies to maintain the safe and uninterrupted blood transfusion chain at our blood bank gave us lessons for future preparedness if faced with a similar situation.

The purpose of the investigation was to study the effect of binary and ternary blends of cement on the mechanical properties of pervious concrete(PC)specimen through destructive(DT)and non-destructive testing(NDT).Various combinations of fly ash(FA),limestone powder(LP),metakaolin(MK),and silica fume(SF)as mineral admixtures have been investigated to partially replace the cement up to 30%by weight in PC.Standard cube specimens of size 150 mm×150 mm×150 mm of binary and ternary blends of mineral admixture of pervious concrete were prepared to conduct standard compressive strength test and split tensile test at 7 and 28 days of curing.The ultrasonic pulse velocity(UPV)test and Rebound Hammer test were used as a non-destructive testing tool to substantiate the robustness of PC and to determine the approximate mechanical properties where other destructive testing tools are not feasible in case of inplace pervious pavements.Overall the pervious concrete made with LP based ternary blends(PLM and PLS)were found to perform better than FA based ternary blends(PFM and PFS)and control mix(PC)in destructive and non-destructive testing.

The occurrence of diabetes mellitus(DM)in pancreatitis is being increasingly recognized lately.Diabetes can develop not only with chronic pancreatitis but even after the first episode of acute pancreatitis(AP).The incidence of diabetes after AP varies from 18%to 23%in 3 years and reaches up to 40%over 5 years.The exact pathogenesis of diabetes after AP is poorly understood and various mechanisms proposed include loss of islet cell mass,AP-induced autoimmunity,and alterations in the insulin incretin axis.Risk factors associated with increased risk of diabetes includes male sex,recurrent attacks of pancreatitis,presence of pancreatic exocrine insufficiency and level of pancreatitic necrosis.Diagnosis of post-pancreatitis DM(PPDM)is often excluded.Treatment includes a trial of oral antidiabetic drugs in mild diabetes.Often,insulin is required in uncontrolled diabetes.Given the lack of awareness of this metabolic disorder after AP,this review will evaluate current information on epidemiology,risk factors,diagnosis and management of PPDM and identify the knowledge gaps.

Diacerein protects against iodoacetate-induced osteoarthritis in the femorotibial joints of rats

The present study was undertaken to investigate the effect of diacerein on the histopathology of articular cartilage and subchondral bone of the femorotibial joint in rats. Osteoartbritis was induced in rats after single intra-articular injection of sodium iodoacetate. Rats were sacrificed 1, 2, 4, and 8 weeks post intra-articular injection to evaluate the progression of histopathogenesis of osteoarthritis. Diacerein was orally administered （15 mg/kg） once daily post 1 and 2 weeks of iodoacetate injection in two groups, respectively, for up to 12 weeks. Articular cartilage and sub- chondral bone of the rats of both groups were examined after 8 and 12 weeks, respectively. Quantitative histological analyses were performed by scoring these sections as per the OARSI system. Chondroitin sulfate was also estimated in articular cartilage by decrease in absorbance of methylene blue on complexation with chondroitin sulfate using a spectrophotometer. Intra-articular injection of iodoacetate induced loss of articular cartilage with progressive sub- chondral bone sclerosis and degeneration. Based on histopathological and biochemical findings, diacerein treatment showed chondroprotective effect. Furthermore, the chondroprotective effect of diacerein was found to be more pro- nounced after 12 weeks as compared to 8 weeks in both cases （i.e., post 1 and 2 weeks of iodoacetate injection）. Similar results were observed by investigation of chondroitin sulfate during biochemical study, showing the chon- droprotective effect. In conclusion, diacerein exhibits chondroprotective effect in rats with late onset of action.

Intuitionistic fuzzy sets(IFSs)are well established as a tool to handle the hesitation in the decision system.In this research paper,fuzzy sets induced by IFS are used to develop a fuzzy time series forecasting model to incorporate degree of hesitation(nondeterminacy).To improve the forecasting accuracy,induced fuzzy sets are used to establish fuzzy logical relations.To verify the performance of the proposed model,it is implemented on one of the benchmarking time series data.Further,developed forecasting method is also tested and validated by applying it on a financial time series data.In order to show the accuracy in forecasting,the method is compared with other forecasting methods using different error measures.
