Health Services Utilization and Health Status of Insured versus Uninsured Nigerian Children with Sickle Cell Disease

Introduction: Nigeria accounts for 150,000 infants born with the Sickle Cell Disease (SCD) every year. Children with SCD are affected by the deleterious effect of user fees which reduces affordability and utilization of health services. Evidences supported that institutionalized health insurance increases intensity of utilization. Methodology: The study was conducted at AKTH, Kano, and north-western, Nigeria. The study was retrospective comparative cross-sectional study. 100 patients were enrolled, 50 for each arm. Paediatric SCD clinic outpatients’ records and a specialty designed form containing the variables of interest were used. Data were analysed using Minitab 16. Proportions, percentages, tables, charts and chi squared test were used to compare the two groups. Result: The study found no association between the two groups in the likelihood of being insured based on age (χ2 = 1.478), gender (χ2 = 0.224) and dwellings (χ2 = 0.062). On health services utilization and insurance status, the study revealed that follow up clinic visits, unscheduled clinic visits and emergency room visits were more likely among the insured group compared to the uninsured group. While the health status and insurance status of the two groups over 12 months period showed a significant association with the insured more likely to have improved health status compared to uninsured (χ2 = 28.019, p = 0.0001). Similarly, health status and health services utilization were significantly associated with insurance status (χ2 = 12.191, p = 0.002). Conclusion: The insurance status of children with SCD is associated with their health services utilization and health status. However, age, gender and dwellings were not associated with insurance status of children with SCD. The optoelectronic properties of n-TiO2NW/p-Si heterojunction fabricated by depositing TiO2 nanowires on a p-Si substrate are studied. Under excitation at a wavelength of 370 nm, the TiO2 nanowires produce a light emission at 435 nm due to the emission of free excitons. The I-V characteristics are measured to investigate the heterojunction effects under the dark environment and ultraviolet （UV） illumination, n-TiOzNW/p-Si has a p-n junction formed in the n-TiOz/p-Si beterojunction. TiO2NW/Si photodiode produces a pbotocurrent larger than dark current under UV illumination. It is observed that UV photons are absorbed in TiO2 and the heterojunction shows a 0.034-A/W responsivity at 4-V reverse bias.

In tropical regions,heavy rainfall induces erosion and shallow landslides on road ***-based stabilization methods,common in these regions,contribute to climate change due to their high carbon *** study explored the potential application of coir fiber-reinforced laterite soil-bottom ash mixtures as embankment materials in the *** objective is to enhance engineered embankment slopes'erosion resistance and stability while offering reuse options for industrial *** study examined various mix designs for unconfined compressive strength(UCS)and permeability,utilizing 30%bottom ash(BA)and 1%coir fiber(CF)with varying sizes ranging from 10 to 40 mm,6%lime,and laterite soil(LS),followed by microstructural *** results demonstrate that the compressive strength increases as the CF length increases to 25 *** contrast,permeability increases continuously with increasing CF ***-treated mixtures exhibit superior short-and long-term strength and reduce permeability owing to the formation of cementitious materials,as confirmed by microstructural analyses.A lab-scale slope box was constructed to evaluate the surface erosion of the stabilized laterite soil *** on the rainfall simulation results,the LS-BA-CF mixtures show better resistance to erosion and deformation compared to untreated LS,especially when lime is added to the top *** study provides insights into a sustainable and cost-effective approach for slope stabilization using BA and CF,offering a promising solution for tropical regions susceptible to surface erosion and landslides.

Urinary Retention in Adults Male Patients: Causes and Complications among Patients Managed in a Teaching Hospital in North Western Nigeria

Background: Urinary retention is one of the common urologic emergencies constituting a significant workload of urologists and non-urologists alike. Aims and Objectives: This study was undertaken to identify the causes and complications associated with urinary retention in adult male patients at AKTH, Kano. Patients and Methods: It was a prospective hospital-based study of 110 consecutive adult male patients who presented to Aminu Kano Teaching Hospital, Kano with urinary retention over 12 months. On presentation, a brief history was taken and rapid physical examination done;the urinary retention was relieved by urethral catheterization or suprapubic cystostomy. A detailed history and thorough physical examination findings were later obtained. Results: One hundred and ten patients were seen during the study period. There ages ranged from 17 to100 years with a mean of 56 ± 19.3 SD years. Patients within the age range 55 - 74 years accounted for the largest group, (25.5% + 23.6% = 49.1%). The most common cause of urinary retention in this study was benign prostatic hyperplasia (51.8% of the patients). This was followed by urethral stricture (20%), cancer of the prostate (7.3%), urethral injury (7.3%) and bladder tumour (6.4%). More than half of the patients (53.4%) presented with acute urinary retention, 30.5% presented with chronic urinary retention, and 16.1% were diagnosed to have acute-on-chronic urinary retention. The complications of urinary retention found were: urinary tract infection (in 24.5% of patients), renal impairment (14.5%), and anaemia (11.8%). Conclusion: Urinary retention is commoner among the middle aged and the elderly. Benign prostatic hyperplasia remains the leading cause of urinary retention. Acute urinary retention was the commonest type of urinary retention;however, complications due to the retention were associated with chronic and acute-on-chronic urinary retention.

Model predictive controllers(MPC)with the two-loop scheme are successful approaches practically and can be classified into two main categories,tube-based MPC and MPCbased reference governors(RG).In this paper,an enhanced twoloop MPC design is proposed for a pre-stabilized system with the bounded uncertainty subject to the input and state *** proposed method offers less conservatism than the tube-based MPC methods by enlarging the restricted input *** to the MPC-based RGs,the investigated method improves tracking performance of the pre-stabilized system while satisfying the ***,the robust global asymptotic stability of the closed-loop system is guaranteed in a novel procedure with terminal constraint *** of the proposed method on a servo system shows its effectiveness in comparison to the others.

Using Syntactic-Based Kernels for Classifying Temporal Relations

Temporal relation classification is one of contemporary demanding tasks of natural language processing. This task can be used in various applications such as question answering, summarization, and language specific information retrieval. In this paper, we propose an improved algorithm for classifying temporal relations, between events or between events and time, using support vector machines （SVM）. Along with gold-standard corpus features, the proposed method aims at exploiting some useful automatically generated syntactic features to improve the accuracy of classification. Accordingly, a number of novel kernel functions are introduced and evaluated. Our evaluations clearly demonstrate that adding syntactic features results in a considerable improvement over the state-of-the-art method of classifying temporal relations.
