Current concepts on cytomegalovirus infection after liver transplantation

Cytomegalovirus (CMV) is the most common viral pa- thogen that negatively impacts on the outcome of liver transplantation. CMV cause febrile illness often ac com panied by bone marrow suppression, and in some cases, invades tissues including the transplanted allog raft. In addition, CMV has been signif icantly asso- ciated with an increased predisposition to allograft re- jection, accelerated hepatitis C recurrence, and other opportunistic infections, as well as reduced overall pa tient and allograft survival. To negate the adverse effects of CMV on outcome, its prevention, whether through antiviral prophylaxis or preemptive therapy, is regarded as an essential component to the medical management of liver transplant patients. Two recent guidelines have suggested that antiviral prophylaxis or preemptive therapy are similarly effective in preventing CMV disease in modest-risk CMV-seropositive liver trans plant recipients, while antiviral prophylaxis is the preferred strategy over preemptive therapy for the preven tion of CMV disease in high-risk recipients [CMV-ser o-negative recipients of liver allografts from CMV-seropositive donors (D+/R-)]. However, antiviral prophylax is has only delayed the onset of CMV disease in many CMV D+/R- liver transplant recipients, and at least in one study, such occurrence of late-onset primary CMV disease was significantly associated with increased mortality after liver transplantation. Therefore, optimized strategies for prevention are needed, and aggressive treatment of CMV infection and disease should be pursued. The standard treatment of CMV disease consists of intravenous ganciclovir or oral valganciclovir, and if fea sible, one should also reduce the degree of immuno-suppression. In one recent controlled clinical trial, val ganc iclovir was found to be as effective and safe as intravenous ganciclovir for the treatment of mild to mo d erate CMV disease in solid organ (including liver) tran splant recipients. BACKGROUND Malignant triton tumors(MTTs)comprise a subgroup of malignant peripheral nerve sheath tumors(MPNSTs)that exhibits rhabdomyosarcomatous differen-tiation and follow an aggressive course.MTTs are primarily located along peripheral nerves.Cases of MTTs in the abdominal wall have not been reported.MTT has a poorer prognosis than classic MPNSTs,and accurate diagnosis necessitates a keen understanding of the clinical history and knowledge of its differential diagnosis intricacies.Treatment for MTTs mirrors that for MPNSTs and is predominantly surgical.CASE SUMMARY A 49-year-old woman presented with a subcutaneous mass in her lower abdo-minal wall and a pre-existing surgical scar that had grown slowly over 3-4 months before the consultation.She had previously undergone radical hysterectomy and concurrent chemo-radiotherapy for cervical cancer approximately 5 years prior to the consultation.Abdominal computed tomography(CT)showed a 1.3 cm midline mass in the lower abdomen with infiltration into the rectus abdominis muscle.There was no sign of metastasis(T1N0M0).An incisional biopsy identified sporadic MTT of the lower abdomen.A comprehensive surgical excision with a 3 cm margin inclusive of the peritoneum was executed.Subse-quently,the general surgeon utilized an approach akin to the open peritoneal onlay mesh technique.The patient underwent additional treatment with an excision shaped as a mini-abdominoplasty for the skin defect.No complications arose,and annual follow-up CTs did not show signs of recurrence or metastasis.CONCLUSION An abdominal MTT was efficaciously treated with extensive excision and abdominal wall reconstruction,eliminating the need for postoperative radiotherapy.

Establishment of a chicken intestinal organoid culture system to assess deoxynivalenol‑induced damage of the intestinal barrier function

Background Deoxynivalenol(DON)is a mycotoxin that has received recognition worldwide because of its ability to cause growth delay,nutrient malabsorption,weight loss,emesis,and a reduction of feed intake in livestock.Since DON-contaminated feedstuff is absorbed in the gastrointestinal tract,we used chicken organoids to assess the DON-induced dysfunction of the small intestine.Results We established a culture system using chicken organoids and characterized the organoids at passages 1 and 10.We confirmed the mRNA expression levels of various cell markers in the organoids,such as KI67,leucine-rich repeat containing G protein-coupled receptor 5(Lgr5),mucin 2(MUC2),chromogranin A(CHGA),cytokeratin 19(CK19),lysozyme(LYZ),and microtubule-associated doublecortin-like kinase 1(DCLK1),and compared the results to those of the small intestine.Our results showed that the organoids displayed functional similarities in permeability compared to the small intestine.DON damaged the tight junctions of the organoids,which resulted in increased permeability.Conclusions Our organoid culture displayed topological,genetic,and functional similarities with the small intes-tine cells.Based on these similarities,we confirmed that DON causes small intestine dysfunction.Chicken organoids offer a practical model for the research of harmful substances.

Importance of dietary supplementation of soluble and insoluble fibers to sows subjected to high ambient temperatures during late gestation and effects on lactation performance

Heat stress adversely affects sows' performance, which can be improved by applying proper nutritional strategies. This study was conducted to investigate the interactive effects of dietary fiber levels and sources on sows' reproductive performance, metabolic response during gestation, and the carry-over influence on litter performance in the lactation period during heat stress(average room temperature of 27.1°C). Fifty-four multiparous sows(Landrace × Yorkshire; initial body weight of 236.3 ± 16 kg; 2, 3and 4 parities) at d 90 of gestation were assigned to a 2 × 3 factorial arrangement(9 sows/treatment),involving 2 dietary fiber levels(4.5% and 6% crude fiber) and 3 dietary fiber sources(wheat bran [WB],palm kernel meal [PK], and beet pulp [BP]). Sows fed the BP diet had highest(P < 0.01) feed intake and constipation index and lowest(P < 0.01) farrowing duration. Piglet weight(P = 0.041) and litter weight(P < 0.01) at weaning were higher in sows in the BP treatment compared to PK treatment. Sows in the BP treatment showed the greatest(P < 0.01) digestibility of crude protein and neutral detergent fiber. The fecal concentration of acetate was the lowest(P < 0.01) in the PK treatment. Total short-chain fatty acid production was increased in the WB and BP treatments compared with the PK. Sows in the BP treatment showed the lowest(P = 0.036) hair cortisol. The blood insulin concentration of sows was higher(P = 0.026) in the high fiber(6%) treatment compared with the low fiber(4.5%) treatment at 90 min and120 min after the meal. The concentration of phthalic acid, succinic acid, phenylethylamine, hydrocinnamic acid, iron, linoleic acid, glycerol, ketone, and formamide were increased(P < 0.05) in the BP treatment compared with the WB. The BP treatment with high soluble fiber content improved the constipation index, farrowing duration, and litter performance, while high insoluble fibers increased sows comfort and reduced stress factors including respiratory rate and rectal temperature. Weighted Forwarding in Graph Convolution Networks for Recommendation Information Systems

Recommendation Information Systems(RIS)are pivotal in helping users in swiftly locating desired content from the vast amount of information available on the Internet.Graph Convolution Network(GCN)algorithms have been employed to implement the RIS efficiently.However,the GCN algorithm faces limitations in terms of performance enhancement owing to the due to the embedding value-vanishing problem that occurs during the learning process.To address this issue,we propose a Weighted Forwarding method using the GCN(WF-GCN)algorithm.The proposed method involves multiplying the embedding results with different weights for each hop layer during graph learning.By applying the WF-GCN algorithm,which adjusts weights for each hop layer before forwarding to the next,nodes with many neighbors achieve higher embedding values.This approach facilitates the learning of more hop layers within the GCN framework.The efficacy of the WF-GCN was demonstrated through its application to various datasets.In the MovieLens dataset,the implementation of WF-GCN in LightGCN resulted in significant performance improvements,with recall and NDCG increasing by up to+163.64%and+132.04%,respectively.Similarly,in the Last.FM dataset,LightGCN using WF-GCN enhanced with WF-GCN showed substantial improvements,with the recall and NDCG metrics rising by up to+174.40%and+169.95%,respectively.Furthermore,the application of WF-GCN to Self-supervised Graph Learning(SGL)and Simple Graph Contrastive Learning(SimGCL)also demonstrated notable enhancements in both recall and NDCG across these datasets.

Background:The Korea National Health and Nutrition Examination Survey nonalcoholic fatty liver disease(K-NAFLD)score was recently developed with the intent to operationally define nonalcoholic fatty liver disease(NAFLD).However,there remained an external validation that confirmed its diagnostic performance,especially in patients with alcohol consumption or hepatitis virus infection.Methods:Diagnostic accuracy of the K-NAFLD score was evaluated in a hospital-based cohort consisting of 1388 participants who received Fibroscan®.Multivariate-adjusted logistic regression models and the contrast estimation of receiver operating characteristic curves were used for validation of the K-NAFLD score,fatty liver index(FLI),and hepatic steatosis index(HSI).Results:K-NAFLD-moderate[adjusted odds ratio(aOR)=2.53,95%confidence interval(CI):1.13-5.65]and K-NAFLD-high(aOR=4.14,95%CI:1.69-10.13)groups showed higher risks of fatty liver compared to the K-NAFLD-low group after adjustments for demographic and clinical characteristics,and FLI-moderate and FLI-high groups revealed aORs of 2.05(95%CI:1.22-3.43)and 1.51(95%CI:0.78-2.90),respectively.In addition,the HSI was less predictive for Fibroscan®-defined fatty liver.Both K-NAFLD and FLI also demonstrated high accuracy in the prediction of fatty liver in patients with alcohol consumption and chronic hepatitis virus infection,and the adjusted area under curve values were comparable between K-NAFLD and FLI.Conclusions:Externally validation of the K-NAFLD and FLI showed that these scores may be a useful,noninvasive,and non-imaging modality for the identification of fatty liver.In addition,these scores also predicted fatty liver in patients with alcohol consumption and chronic hepatitis virus infection.

Health risks of pest control and disinfection workers after the COVID-19 outbreak in South Korea

The exposure patterns of pest control and disinfection workers have changed after the coronavirus disease 2019(COVID-19)outbreak,but the health risks of chemical exposure have not been assessed.We identified these workers’chemical exposure patterns and risks before and after the COVID-19 outbreak.We used data conducted between 2018(pre-pandemic)and 2021(post-pandemic)from three-year cross-sectional surveys on pest control and disinfection workers.Inhalation and dermal exposure concentrations were estimated using equations based on a biocidal product risk assessment model from the Korean National Institute of Environmental Research.The non-carcinogenic and carcinogenic risks of chemicals were calculated using the United States Environmental Protection Agency risk assessment model.We found that the annual work frequency(50^(th)percentile)of foggers using disinfectants increased the most among all the work types,from 140 uses/year to 176 uses/year after the COVID-19 outbreak.Moreover,all chemicals’non-carcinogenic and carcinogenic risks increased regardless of exposure routes.In the worst scenario(95^(th)percentile),the margin of exposure for citric acid,benzethonium chloride,benzyl-C12-16-alkyldimethyl chlorides,and sodium chlorite of inhalation exposure,and isopropyl alcohol and benzyl-C12-16-alkyldimethyl chlorides of dermal exposure were acceptable(>100)before the COVID-19outbreak but became unacceptable(10^(-6))before and after the COVID-19 outbreak but comparatively high after the COVID-19 outbreak.Additionally,significantly more workers experienced health symptoms after the COVID-19outbreak(p<0.05),with the most common being muscle lethargy(31%),skin/face stinging(28.7%),and breath shortness/neck pain(24.1%).

Extraction of intersecting palm-vein and palmprint features for cancellable identity verification

A novel method based on the cross-modality intersecting features of the palm-vein and the palmprint is proposed for identity verification.Capitalising on the unique geometrical relationship between the two biometric modalities,the cross-modality intersecting points provides a stable set of features for identity verification.To facilitate flexibility in template changes,a template transformation is proposed.While maintaining non-invertibility,the template transformation allows transformation sizes beyond that offered by the con-ventional means.Extensive experiments using three public palm databases are conducted to verify the effectiveness the proposed system for identity recognition.

Suboptimal Feature Selection Techniques for Effective Malicious Traffic Detection on Lightweight Devices

With the advancement of wireless network technology,vast amounts of traffic have been generated,and malicious traffic attacks that threaten the network environment are becoming increasingly sophisticated.While signature-based detection methods,static analysis,and dynamic analysis techniques have been previously explored for malicious traffic detection,they have limitations in identifying diversified malware traffic patterns.Recent research has been focused on the application of machine learning to detect these patterns.However,applying machine learning to lightweight devices like IoT devices is challenging because of the high computational demands and complexity involved in the learning process.In this study,we examined methods for effectively utilizing machine learning-based malicious traffic detection approaches for lightweight devices.We introduced the suboptimal feature selection model(SFSM),a feature selection technique designed to reduce complexity while maintaining the effectiveness of malicious traffic detection.Detection performance was evaluated on various malicious traffic,benign,exploits,and generic,using the UNSW-NB15 dataset and SFSM sub-optimized hyperparameters for feature selection and narrowed the search scope to encompass all features.SFSM improved learning performance while minimizing complexity by considering feature selection and exhaustive search as two steps,a problem not considered in conventional models.Our experimental results showed that the detection accuracy was improved by approximately 20%compared to the random model,and the reduction in accuracy compared to the greedy model,which performs an exhaustive search on all features,was kept within 6%.Additionally,latency and complexity were reduced by approximately 96%and 99.78%,respectively,compared to the greedy model.This study demonstrates that malicious traffic can be effectively detected even in lightweight device environments.SFSM verified the possibility of detecting various attack traffic on lightweight devices.

Over the years,zinc-ion batteries(ZIBs)have attracted attention as a promising next-generation energy storage technology because of their excellent safety,long cycling performance,eco-friendliness,and high-power density.However,issues,such as the corrosion and dissolution of the Zn anode,limited wet-tability,and lack of sufficient nucleation sites for Zn plating,have limited their practical application.The introduction of a protective layer comprising of tellurium(Te)nanobelts onto the surface of Zn anode has emerged as a promising approach to overcome these limitations and improve the electrochemical behav-ior by enhancing the safety and wettability of ZIBs,as well as providing numerous nucleation sites for Zn plating.In the presence of a Te-based protective layer,the energy power density of the surface-engineered Zn anode improved significantly(ranging from 310 to 144 W h kg^(-1),over a power density range of 270 to 1,800 W kg^(-1)),and the lifespan capability was extended.These results demonstrate that the proposed strategy of employing Te nanobelts as a protective layer holds great promise for enhancing the energy storage performance of zIBs,making them even more attractive as a viable energy storage solution forthefuture.
