Incidence of coronary artery disease before valvular surgery in isolated severe aortic stenosis

Background Angina pectoris has been recognized as one of the principal symptoms of aortic valve stenosis (AS),even in patients without significant coronary artery disease (CAD).However,the incidence of angina pectoris and related CAD in such patients is controversial.There is continuing debate as to whether coronary angiography is necessary before aortic valve replacement (AVR) in patients with severe AS.The purpose of this study was to evaluate the incidence and predictors of CAD in patients with severe AS in a Korean population.Methods Data from all consecutive patients with severe AS undergoing AVR at a major tertiary cardiac and vascular center in Korea were entered in a prospective registry beginning in 1995.Clinical and echocardiographic follow-up data were recorded into the database annually.Significant CAD was defined as one or more major coronary arteries having an estimated narrowing of≥70% and left main coronary arteries having an estimated narrowing of≥50% on coronary angiography.We excluded patients with multiple valve disease,significant aortic regurgitation,or prior CAD or valve surgery.Results Totally 574 patients with severe AS (mean age,(65.9±9.6) years) were enrolled in this study.Significant CAD was found in 61 patients (10.6%).Factors associated with increased likelihood of CAD were age,hypertension,diabetes mellitus,chronic renal failure,carotid disease,and aorta calcification.In Logistic regression analysis,the independent predictor of the presence of CAD was age (P=0.011).The incidence of CAD increased significantly at 69.2 years of age.Having two risk factors for cardiovascular disease was the most useful cutoff to predict whether a patient was going to have significant CAD.Conclusions There was a low incidence of significant CAD in a population of Korean patients with severe AS.Therefore,coronary angiography before AVR will be considered in patients with multiple risk factors for cardiovascular disease or in patients more than 69 years of age without risk factors for cardiovascular disea

What is the real practice of exercise echocardiographic testing in asymptomatic patients with severe aortic stenosis？

Background Although exercise testing has been suggested to help predict clinical outcome, limited data are available to guide how exercise Doppler echocardiography (ECG) can be used clinically in asymptomatic patients with aortic stenosis (AS). The aim of this study was to assess the clinical value of exercise echocardiographic testing in asymptomatic patients with severe AS. Methods Symptom-limited treadmill exercise testing using the modified Bruce protocol was performed in 31 asymptomatic patients (mean age (62+11) years) with severe AS (aortic valve area 4m/sec, or a mean transaortic pressure gradient (AV mean PG) >40mmHg (1mmHg=0.133 kPa)) with normal left ventricular (LV) systolic function (LV ejection fraction (EF) >50%). Clinical symptoms, vital signs, ECG, and Doppler hemodynamics were obtained during and/or immediately after exercise. Results Aortic valve replacement (AVR) was performed in 18 patients during follow-up. The patients who had AVR exhibited higher baseline AV mean PG (51 (35-84) vs. 44 (25.2-57.0) mmHg; P=0.031). There were no significant differences between the AVR group and non-AVR group including exercise duration (7.47 (2.32-11.59) vs. 7.25 (4.06- 10.52) minutes, P=0.917), exercise capacity (10.1 (4.6-12.8) vs. 10.1 (7.0-12.8) metabolic equivalents, P=0.675), and an increment in AV mean PG by exercise (18.5 (3.2-48.0) vs. 12.6 (4.4-32.1) mmHg, P=0.366). Univariate regression analysis revealed that independent determinant of AVR was the baseline AV mean PG (P=-0.031). Conclusions Although additional value of exercise ECG was demonstrated, baseline transaortic mean pressure gradient is the major determinant of AVR. Further large-scale prospective studies are required to determine whether surgery should be recommended in the presence of an abnormal exercise ECG in asymptomatic severe AS.

Thermoresponsive Al^(3+)-crosslinked poly(N-isopropylacrylamide)/alginate composite for green recovery of lithium from Li-spiked seawater

With the rapid increase in the demand for lithium as an energy-critical element,the recovery of Li+ions from seawater is a worldwide challenging issue.Herein,we propose a new facile and fast selective recovery approach of Li+using an Al^(3+)-crosslinked poly(N-isopropylacrylamide)(PNIPAAm)/alginate(Alg)(PNP/Alg(Al)) adsorbent.The in situ TEM images indicate that Alg-Al^(3+)coordination is reorganized via the rearrangement of PNIPAAm and Alg networks,as the temperature increases.The reorganization eventually leads to the formation of polycrystalline structure.The in situ FTIR results exhibit that PNP/Alg(Al) composite has peculiar phase transitions,which includes a retrogressive phase change from hydrophobic to hydrophilic.The synergetic effect of the strong repulsion force of Al^(3+) ions and the attractive force of negatively charged polymeric chains enables the efficient adsorption of Li+ions with a low affinity from Li-spiked seawater.7.3% of Li+ions are recovered from Li-spiked seawater although the concentration of Li-spiked seawater is very high.In addition,Li+ions can be extracted from PNP/Alg(Al) composite with the use of a small thermal energy.The proposed thermoresponsive IPN gel provides a strong potential in practical applications for Li+recovery as an innovative energy-material strategy.

Despite the enormous interest in inorganic/polymer composite solid-state electrolytes(CSEs)for solid-state batteries(SSBs),the underlying ion transport phenomena in CSEs have not yet been elucidated.Here,we address this issue by formulating a mechanistic understanding of bi-percolating ion channels formation and ion conduction across inorganic-polymer electrolyte interfaces in CSEs.A model CSE is composed of argyrodite-type Li_6PS_5Cl(LPSCl)and gel polymer electrolyte(GPE,including Li~+-glyme complex as an ion-conducting medium).The percolation threshold of the LPSCl phase in the CSE strongly depends on the elasticity of the GPE phase.Additionally,manipulating the solvation/desolvation behavior of the Li~+-glyme complex in the GPE facilitates ion conduction across the LPSCl-GPE interface.The resulting scalable CSE(area=8×6(cm×cm),thickness~40μm)can be assembled with a high-mass-loading LiNi_(0.7)Co_(0.15)Mn_(0.15)O_(2)cathode(areal-mass-loading=39 mg cm~(-2))and a graphite anode(negative(N)/positive(P)capacity ratio=1.1)in order to fabricate an SSB full cell with bi-cell configuration.Under this constrained cell condition,the SSB full cell exhibits high volumetric energy density(480 Wh L_(cell)~(-1))and stable cyclability at 25℃,far exceeding the values reported by previous CSE-based SSBs.

Deep Learning Model Ensemble for the Accuracy of Classification Degenerative Arthritis

Artificial intelligence technologies are being studied to provide scientific evidence in the medical field and developed for use as diagnostic tools.This study focused on deep learning models to classify degenerative arthritis into Kellgren–Lawrence grades.Specifically,degenerative arthritis was assessed by X-ray radiographic images and classified into five classes.Subsequently,the use of various deep learning models was investigated for automating the degenerative arthritis classification process.Although research on the classification of osteoarthritis using deep learning has been conducted in previous studies,only local models have been used,and an ensemble of deep learning models has never been applied to obtain more accurate results.To address this issue,this study compared the classification performance of deep learning models,includingVGGNet,DenseNet,ResNet,TinyNet,EfficientNet,MobileNet,Xception,and ViT,on a dataset commonly used for osteoarthritis classification tasks.Our experimental results verified that even without applying a separate methodology,the performance of the ensemble was comparable to that of existing studies that only used the latest deep learning model and changed the learning method.From the trained models,two ensembles were created and evaluated:weight and specialist.The weight ensemble showed an improvement in accuracy of 1%,and the proposed specialist ensemble improved accuracy,precision,recall,and F1 score by 5%,6%,6%,and 6%,respectively,compared with the results of prior studies.

Research on the quality of data in a structural calculation document(SCD)is lacking,although the SCD ofa bridge is used as an essential reference during the entire lifecycle of the facility.XML Schema matching enables qualitative improvement of the stored data.This study aimed to enhance the applicability of XML Schema matching,which improves the speed and quality of information stored in bridge SCDs.First,the authors proposed a method of reducing the computing time for the schema matching of bridge SCDs.The computing speed of schema matching was increased by 13 to 1800 times by reducing the checking process of the correlations.Second,the authors developed a heuristic solution for selecting the optimal weight factors used in the matching process to maintain a high accuracy by introducing a decision tree.The decision tree model was built using the content elements stored in the SCD,design companies,bridge types,and weight factors as input variables,and the matching accuracy as the target variable.The inverse-calculation method was applied to extract the weight factors from the decision tree model for high-accuracy schema matching results.

Porous Microspheres Comprising CoSe2 Nanorods Coated with N-Doped Graphitic C and Polydopamine-Derived C as Anodes for Long-Lived Na-Ion Batteries

Metal–organic framework-templated nitrogen-doped graphitic carbon(NGC)and polydopaminederived carbon(PDA-derived C)-double coated one-dimensional CoSe_(2) nanorods supported highly porous threedimensional microspheres are introduced as anodes for excellent Na-ion batteries,particularly with long-lived cycle under carbonate-based electrolyte system.The microspheres uniformly composed of ZIF-67 polyhedrons and polystyrene nanobeads(φ=40 nm)are synthesized using the facile spray pyrolysis technique,followed by the selenization process(P-CoSe_(2)@NGC NR).Further,the PDA-derived C-coated microspheres are obtained using a solution-based coating approach and the subsequent carbonization process(P-CoSe_(2)@PDA-C NR).The rational synthesis approach benefited from the synergistic effects of dual carbon coating,resulting in a highly conductive and porous nanostructure that could facilitate rapid diffusion of charge species along with efficient electrolyte infiltration and effectively channelize the volume stress.Consequently,the prepared nanostructure exhibits extraordinary electrochemical performance,particularly the ultra-long cycle life stability.For instance,the advanced anode has a discharge capacity of 291(1000th cycle,average capacity decay of 0.017%)and 142 mAh g^(-1)(5000th cycle,average capacity decay of 0.011%)at a current density of 0.5 and 2.0 A g^(-1),respectively.

Microglial activation in Alzheimer’s disease(AD):In 1907,Dr.Alois Alzheimer,a Bavarian-born German psychiatrist and neuropathologist,published an article describing the clinical and neuropathological features of an unclassified psychiatric disorder.The disorder was later named AD and is currently the most common brain disorder(Takata et al.,2021).AD involves the accumulation of amyloid-β(Aβ)and hyperphosphorylated tau proteins in the brain,which are associated with senile plaques and neurofibrillary tangles,respectively(Vergara et al.,2019;Takata et al.,2021).AD is characterized by cognitive impairment and memory loss with hippocampal neurodegeneration(Kim et al.,2021).

Pancreatic arteriovenous malformation treated with transcatheter arterial embolization:Two case reports and review of literature

BACKGROUND Various treatment methods are available for the treatment of pancreatic arteriovenous malformation(P-AVM);however,there are no established treatment options for asymptomatic P-AVM.CASE SUMMARY A 47-year-old and a 50-year-old male patients sought treatment for P-AVM in the pancreas,which was incidentally detected during routine abdominal computed tomography and magnetic resonance imaging conducted as part of a health check-up.They underwent transcatheter arterial embolization(TAE),and over the course of a 9-year follow-up period,the AVM did not worsen and was asymptomatic.CONCLUSION TAE can be considered as an alternative treatment option for P-AVM in selective cases where patients are asymptomatic or have a high surgical risk.
