AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,***: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. Analysis of recent climate change over the Arctic using ERA-Interim reanalysis data

This study investigates recent climate change over the Arctic and its link to the mid-latitudes using the ERA-Interim global atmospheric reanalysis data from the European Center for Medium-Range Weather Forecast （ECMWF）. Since 1979, sub- stantial surface warming, associated with the increase in anthropogenic greenhouse gases, has occurred over the Arctic. The great- est warming in winter has taken place offshore in the Kara-Barents Sea, and is associated with the increase in turbulent heat fluxes from the marginal ice zone. In contrast to the marked warming over the Arctic Ocean in winter, substantial cooling appears over Siberia and eastern Asia, linked to the reduction of Arctic sea ice during the freezing season （September-March）. However, in summer, very little change is observed in surface air temperature over the Arctic because increased radiative heat melts the sea ice and the amount of turbulent heat gain from the ocean is relatively small. The heat stored in the upper ocean mixed layer in summer with the opening of the Arctic Ocean is released back to the atmosphere as turbulent heat fluxes during the autumn and through to the following spring. This warming of the Arctic and the reduced sea ice amplifies surface cooling over Siberia and eastern Asia in winter.

Prominent cycling reversibility and kinetics enabled by CaTiO3 protective layer on Zn metal for aqueous Zn-ion batteries

Aqueous Zn-ion batteries （AZIBs） have received considerable attention owing to their various advantages such as safety,low cost,simple battery assembly conditions,and high ionic ***,they still suffer from serious problems,including uncontrollable dendrite growth,corrosion,hydrogen evolution reaction （HER） from water decomposition,electrode passivation,and unexpected *** creation of a uniform artificial nanocrystal layer on the Zn anode surface is a promising strategy for resolving these ***,we propose the use of a perovskite CaTiO3（CTO） protective layer on Zn（CTO@Zn） as a promising approach for improving the performance of *** CTO artificial layer provides an efficient pathway for Zn ion diffusion towards the Zn metal because of the high dielectric constant （εr=180） and ferroelectric characteristics that enable the alignment of dipole moments and redistribute the Zn2+ions in the CTO *** avoiding the direct contact of the Zn anode with the electrolyte solution,the uneven dendrite growth,corrosion,parasitic side reactions,and HER are mitigated,while CTO retains its mechanical and chemical robustness during ***,CTO@Zn demonstrates an improved lifespan in a symmetric cell configuration compared with bare ***@Zn shows steady overpotential （～68 m V） for 1500 h at 1 mA cm-2/0.5 mA h cm-2,excelling bare ***,when paired with the V2O5-C cathode,the CTO@Zn//V2O5-C full battery delivers 148.4 mA h g-1（based on the mass of the cathode） after 300 *** study provides new insights into Zn metal anodes and the development of high-performance AZIBs.

We investigated the effects of Sn addition on the microstructural characteristics and mechanical properties of an extruded Mg-Bi binary alloy by comparing Mg-5 Bi(B5)and Mg-5 Bi-4 Sn(BT54).Both the extruded alloys exhibit a partially recrystallized grain structure with a strong extrusion fiber texture and numerous Mg3Bi2 ***,the addition of Sn significantly decreases the average grain size of the extruded alloy from 123.9 to 75.2μ*** Sn solute atoms inhibit the activity of dislocation slip,which reduces the internal strain energy accumulated in the dynamically recrystallized(DRXed)grains during ***,this reduced strain energy leads to the decrease in the DRXed grain size owing to weakened grain growth during natural *** extruded BT54 alloy exhibits higher tensile strength and ductility than the extruded B5 *** improvement in the strength by the Sn addition is attributed to the combined effects of grain refinement,Sn solute atoms,and increased dislocation *** formation of{10-11}and{10-11}-{10-12}twins during tension is suppressed by the grain refinement,thereby improving the tensile elongation considerably.

miR-21, miR-221 and miR-222 expression and prostate cancer recurrence among obese and non-obese cases

Recent evidence shows that certain microRNAs （miRNAs） play a role in both obesity and prostate cancer recurrence, but the association between the expression of these miRNAs and obesity in prostate cancer recurrence is unknown. In this study, we examined the effect of the interaction between obesity and miR-21, miR-221 or miR-222 expression on prostate cancer recurrence among 28 recurrent and 37 non-recurrent prostate cancer cases, miRNA expression was determined using quantitative real-time polymerase chain reaction. Cox proportional hazard models adjusting for age at diagnosis, clinical stage and Gleason score were used to estimate hazard ratios （HR） and 95% confidence intervals （95% CI） for recurrence free survival. A significantly （P=0.014） higher proportion of recurrent cases （78,6%） than non-recurrent cases （48.6%） had a low expression of miR-21 and the difference was more prominent in obese than non-obese patients. Multivariate analysis showed that the expression of miR-21 was an independent risk factor for recurrence in obese （HR=6.15, 95% CI= 1.04-36.48, P=-0.045）, but not in non-obese （HR= 1.28, 95% C1=0.30-5.49, P=0.74） cases. A significant association with recurrence was not observed for the expression of miR-221 and miR-222. In summary, our findings show that miR-21 is associated with prostate cancer recurrence after radical prostatectomy and suggest that the differential expression of miR-21 is more prominent in obese than in non-obese cases. Future larger studies are warranted to confirm these initial findings and to elucidate the mechanisms involved.

Electrical Stimulation of Auricular Acupressure for Dry Eye:A Randomized Controlled-Clinical Trial

Objective： To evaluate the clinical efficacy of electrical stimulation（ES） of auricular acupressure on reducing the ocular symptoms and signs before and after treatment for dry eye. Methods： The inclusion criteria were the tear film break-up time（TFBUT） below 5 s and a Schirmer test-Ⅰ below 5 mm in dry eyes with ocular symptoms for at least 6 months. Subjects were randomized into a treatment group（50 cases） with continuous low frequency ES under auricular acupressure at acupoints and a no ES under auricular acupressure（no-ES, control group, 50 cases） on the same acupoints. Auricular acupressure were stimulated with ES at 4 master points of both ears, which were performed twice a week for 4 weeks at each point for 30 s. The ocular symptoms, the TFBUT, and Schirmer test-Ⅰ were evaluated before and after this procedure. Results： There were significantly better scores in TFBUT（P=0.032）, the Schirmer test-Ⅰ（P=0.044） and ocular symptoms（P=0.029） at 3 months post-treatment in the treatment group than in the control group. The total effective rate in the treatment group was accomplished in 41（82%） of the 50 cases of dry eye. Conclusions： Auricular acupressure with ES at auricular acupoint improves ocular symptoms and signs of dry eye for a period of at least 3 months.

Castleman's disease(CD) is a rare lymphoproliferative disorder of unknown ***,it occurs as a localized(unicentric) disease or as a systemic(multicentric) *** Castleman's disease(UCD) presents as a solitary mass and primarily affects the mediastinal,retroperitoneal,and cervical lymph *** contrast to multicentric CD,which involves peripheral lymphadenopathy and numerous systemic symptoms,UCD is not typically associated withgeneralized *** main distinct histologic variants are recognized:hyaline-vascular type,plasma cell type,and mixed *** CD is ***,UCD exclusively in the spleen is extremely rare,with only 2 cases described in the literature to ***,we describe an asymptomatic 75-yearold man with a 5.7 cm × 4.5 cm sized heterogenous enhanced mass located in the *** underwent surgical resection for diagnosis and treatment.A pathologic examination indicated the hyaline-vascular type of *** this patient,the preoperative diagnosis was difficult to determine,and therefore,invasive procedures were required.

Enhanced thermoelectric composite performance from mesoporous carbon additives in a commercial Bi_(0.5)Sb_(1.5)Te_(3) matrix

Composites were prepared,through hot pressing,using carbon materials with different pore size distributions as additives for commercial Bi_(0.5)Sb_(1.5)Te_(3) thermoelectric material(BST,p-type).Thermoelectric properties of the composites were measured in a temperature range of 298-473 *** conductivity of the composites,especially lattice thermal conductivity,was effectively decreased due to the mesoporous properties of the incorporated carbon *** electrical conductivity of the composites slightly decreased due to the electron scattering at the interface between the carbon material and the commercial BST *** composite with 0.2 vol.%mesoporous carbon powder(36%mesoporosity)exhibited a figure of merit value approximately 10.7%higher than that of commercial BST without *** behavior resulted in 116%improved output power in the composite block-based single element compared with a bare BST thermoelectric *** enhanced figure of merit was attributed to the effective reduction of lattice thermal conductivity by acoustic phonons scattering at the interface between the BST matrix and the mesoporous carbon as well as at the pore surfaces within the mesoporous *** utilizing mesoporous carbon materials used in this study,the shortcomings and economic difficulties of the composite process with low dimensional carbon additives(carbon nanotubes,graphene,and nanodiamond)can be overcome for extensive practical *** carbon powder with a tailored porosity distribution revealed the validity of bulk-type carbon additives to enhance the figure of merit of commercial thermoelectric materials.

Non-microbial approach for Helicobacter pylori as faster track to prevent gastric cancer than simple eradication

Although the International Agency for Research on Cancer declared Helicobacter pylori(***)as a definite human carcinogen in 1994,the Japanese Society for Helicobacter Research only recently(February 2013)adopted the position that *** infection should be considered as an indication for either amelioration of chronic gastritis or for decreasing gastric cancer *** researchers have found that *** eradication halts progressive mucosal damage and that successful eradication in patients with non-atrophic gastritis most likely prevents subsequent development of gastric ***,those who have already developed atrophic gastritis/gastric atrophy retain potential risk factors for gastric *** chronic perpetuated progression of ***-associated gastric inflammation is associated with increased morbidity culminating in gastric carcinogenesis,a non-microbial approach to treatment that provides long-term control of gastric inflammation through nutrients and other interventions may be an effective way to decrease this *** non-microbial approach might represent a new form of prerequisite"rescue"therapy that provides a quicker path to the prevention of gastric cancer as compared to simple eradication.

Effects of mesenchymal stem cell on dopaminergic neurons,motor and memory functions in animal models of parkinson's disease:a systematic review and meta-analysis
