Durability of viral response after off-treatment in HBeAg positive chronic hepatitis B

AIM:To evaluate the durability in hepatitis B e antigen (HBeAg) positive chronic hepatitis B patients who discontinued antiviral treatment. METHODS:A total of 48 HBeAg positive chronic hepatitis B patients who were administered nucleoside analogues and maintained virological response for ≥ 6 mo [hepatitis B virus (HBV) DNA 104 copies/mL after HBeAg seroconversion with previously undetectable HBV-DNA level. RESULTS:During the median follow-up period of 18.2 mo (range:5.1-47.5 mo) after cessation of antiviral treatment, the cumulative serological recurrence rate was 15 % at 12 mo. The median duration between the cessation of antiviral treatment and serologic recurrence was 7.2 mo (range:1.2-10.9 mo). Of the 48 patients with HBeAg positive chronic hepatitis, 20 (41.6%) showed virological recurrence. The cumulative virologic recurrence rates at 12 mo after discontinuing the antiviral agent were 41%. The median duration between off-treatment and virologic recurrence was 7.6 mo (range:4.3-27.1 mo). The mean age of the virological recurrence group was older than that of the non-recurrence group (46.7 ± 12.1 years vs 38.8 ± 12.7 years, respectively; P = 0.022). 目的研究胎儿酒精效果对大鼠小脑皮质中脑源性神经营养因子(BDNF)和酪氨酸激酶B(TrkB)的影响及外源性甲状腺素(T4)的效应。方法选择怀孕第6天的SD孕鼠随机分为酒精组、正常组、酒精+T4组以及代理母鼠组。酒精组孕鼠摄取1 475 J/d的酒精;正常组孕鼠摄取与酒精组同热卡的奶粉;酒精+T4组孕鼠摄取与酒精组同量的酒精,同时皮下注射5μg/(kg.d)甲状腺素;代理母鼠组摄取正常鼠食。酒精组、正常组、酒精+T4组母鼠分娩6 h后处死采血,测试血中酒精浓度和甲状腺素含量。三组仔鼠由代理母鼠养育,并分别于生后第7、14、21、28、60、90天麻醉处死,采用免疫组织化学染色法,观察小脑皮质中BDNF和TrkB阳性浦肯野细胞的分布及形态,并测算小脑4/5小叶中单位面积所含的BDNF或TrkB阳性浦肯野细胞数。结果三组18只母鼠(每组6只)及其所生仔鼠中144只(每组均选择48只)纳入分析。酒精组、酒精+T4组母鼠的血液酒精浓度高于正常组,差异有统计学意义(P0.05);酒精组仔鼠小脑中BDNF和TrkB阳性浦肯野细胞数,从第28天起较正常组及酒精+T4组显著减少(P<0.01),并持续到第90天。结论外源性甲状腺素能促进小脑皮质中BDNF和其特异性受体TrkB的合成,促进BDNF-及TrkB-阳性浦肯野细胞的发育,进而改善胎儿酒精效果致低甲状腺素所引起的小脑发育障碍。

AIM:To evaluate and compare the clinical outcomes of prophylactic submucosal saline-epinephrine injection and saline injection alone for large colon polyps by conventional polypectomy. METHODS:A prospective study was conducted from July 2003 to July 2004 at 11 tertiary endoscopic centers. Large colon polyps (> 10 mm in diameter) wererandomized to undergo endoscopic polypectomy with submucosal saline-epinephrine injection (epinephrine group) or normal saline injection (saline group). Endoscopic polypectomy was performed by the conventional snare method,and early ( 2 cm) and neoplastic polyps were independently and significantly associated with the presence of PPB. CONCLUSION:The prophylactic submucosal injection of diluted epinephrine does not appear to provide an additional advantage over the saline injection alone for the prevention of PPB.

AIM: To assess the efficacy and safety of weekly docetaxel plus a fixed-dose rate(FDR) of gemcitabine in metastatic esophageal squamous cell carcinoma(SCC).METHODS: A multi-center, open-label, prospective phase Ⅱ study was ***-three esophageal SCC patients with documented progression after fluoropyrimidine/platinum-based first-line chemotherapy were enrolled and treated with docetaxel 35 mg/m2 and gemcitabine 1000 mg/m2 iv at a FDR(10 mg/m2 per minute) on days 1 and *** was repeatedevery twenty-one days until disease progression, unacceptable toxicity, or consent *** primary endpoint was response rate(RR), and secondary endpoints were safety, progression-free survival(PFS) and overall survival(OS).RESULTS: Combination of weekly docetaxel and FDR gemcitabine was well tolerated: the most common treatment-related adverse events were anemia(97%), fatigue(64%) and neutropenia(55%).One patient with multiple lung and lymph node metastases died of respiratory failure after receiving four cycles of chemotherapy, and the possibility of drug-induced pneumonitis could not be completely *** control(objective response plus stable disease) in the ITT population was achieved in 88% of patients, and the overall RR was 30%(95%CI: 15%-46%).The median PFS and OS were 4.0(95%CI: 3.4-4.6) and 8.8 mo(95%CI: 7.8-9.8 mo), ***: A combination of weekly docetaxel and FDR gemcitabine showed promising antitumor activity and tolerability in previously treated, metastatic esophageal SCC.

Developing Stable Cultivar through Microspore Mutagenesis in ×Brassicoraphanus koranhort, Inter-Generic Allopolyploid between Brassica rapaand Raphanus sativus

A stable progeny was developed through induced mutation, using microspore culture, of the hybrid (F1F1) produced by crossing a newly synthesized, unstable allopolyploid (F1) and a stable cultivar, BB#1(F1) in xBrassicoraphanus. An F1F1 plant was subjected to the induced mutation system established during production of BB#1. Morphological characteristics of the progeny such as color, and leaf number and length, differed from those of BB#1. The bolting time of the progeny in spring cropping was very late compared to BB#1, allowing it to be grown to an adult plant in spring. Genomic in situ hybridization analysis of pollen mother cells at prophase I identified 19 bivalents, 10 from Brassica rapa and 9 from Raphanus sativus. The glucoraphenin content was almost identical to that of BB#1. Two cultivars are available in the baemoochae crop now. These results indicate that induced mutation using microspore culture is a viable method of stabilizing intergeneric allopolyploids between B. rapa and R. sativus.

Risk factors for local recurrence after en bloc endoscopic submucosal dissection for early gastric cancer

AIM: To investigate factors related to recurrence following en bloc resection using endoscopic submucosal dissection(ESD) in patients with early gastric cancer(EGC). METHODS: A total of 1121 patients(1215 lesions) who had undergone ESD for gastric neoplasia between April 2003 and May 2010 were retrospectively reviewed. Data from 401 patients(415 lesions) were analyzed, following the exclusion of those who underwent piecemeal resection, with deep resection margin invasion or lateral margin infiltration, and diagnosed with benign lesions. RESULTS: Local recurrence after en bloc ESD was found in 36 cases(8.7%). Unclear resection margins, long procedure times, and narrow safety margins were identified as risk factors for recurrence. Lesions located in the upper third of the stomach showed a higher rate of recurrence than those located in the lower third of the stomach(OR = 2.9, P = 0.03). The probability of no recurrence for up to 24 mo was 79.9% in those with a safety resection margin ≤ 1 mm and 89.5% in those with a margin > 1 mm(log-rank test, P = 0.03). CONCLUSION: Even in cases in which en bloc ESD is performed for EGC, local recurrence still occurs. To reduce local recurrences, more careful assessment will be needed prior to the implementation of ESD in casesin which the tumor is located in the upper third of the stomach. In addition, clear identification of tumor boundaries as well as the securing of sufficient safety resection margins will be important.

目的比较B超、增强CT及18F-FDGPET/CT在颈部复发性甲状腺癌的诊断效能。方法选取20例经手术病理证实患有分化型甲状腺癌(19例乳头状癌,1例髓样癌)并证实存在颈部复发病灶的病人。在进行进一步手术治疗之前,所有病人均在2个月内进行B超、CT和PET/CT检查。对于每例病人,通过B超、CT及PET/CT对局部复发病灶进行逐层图像回顾性分析。影像学结果均与组织学检查结构进行对照作为参考标准。结果经手术证实在110个颈部结节中52个为复发病灶。超声的敏感度、特异度及精确度分别为69.2%、89.7%及80%;CT分别为63.5%、94.8%及80.0;PET/CT为53.8%、79.3%及67.3%。ROC分析结果表明B超比PET/CT对于检测肿瘤复发病灶具有更高的诊断效能。结论虽然在3种检查方法中未发现显著性差异,但是在评价分化型甲状腺癌颈部复发灶方面,超声和CT的敏感性和特异性均高于PET/CT。
