AIM:To compare the outcomes between doubleguidewire technique(DGT) and transpancreatic precut sphincterotomy(TPS) in patients with difficult biliary ***:This was a prospective,randomized study conducted in single tertiary referral hospital in *** January 2005 and September 2010.A total of 71 patients,who bile duct cannulation was not possible and selective pancreatic duct cannulation was achieved,were randomized into DGT(n = 34) and TPS(n = 37) *** or TPS was done for selective biliary *** measured the technical success rates of biliary cannulation,median cannulation time,and procedure related ***:The distribution of patients after randomization was balanced,and both groups were comparable in baseline characteristics,except the higher percentage of endoscopic nasobiliary drainage in the DGT group(55.9% vs 13.5%,P < 0.001).Successful cannulation rate and mean cannulation times in DGT and TPS groups were 91.2% vs 91.9% and 14.1 ± 13.2 min vs 15.4 ± 17.9 min,P = 0.732,*** was no significant difference between the two *** overall incidence of post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis was 38.2% vs 10.8%,P < 0.011 in the DGT group and the TPS group;post-procedure pancreatitis was significantly higher in the DGT *** the overall incidence of post-ERCP hyperamylasemia was no significant difference between the two groups;DGT group vs TPS group:14.7% vs 16.2%,P < ***:When free bile duct cannulation was difficult and selective pancreatic duct cannulation was achieved,DGT and TPS facilitated biliary cannulation and showed similar success ***,post-procedure pancreatitis was significantly higher in the DGT group.

AIM: To evaluated the value of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) scan in diagnosis of hepatocellular carcinoma (HCC) and extrahepatic metastases. METHODS: A total of 138 patients with HCC who had both conventional imaging modalities and 18F-FDG PET/CT scan done between November 2006 and March 2011 were enrolled. Diagnostic value of each imaging modality for detection of extrahepatic metastases was evaluated. Clinical factors and tumor characteristics including PET imaging were analyzed as indicative factors for metastases by univariate and multivariate methods. RESULTS: The accuracy of chest CT was significantly superior compared with the accuracy of PET imaging for detecting lung metastases. The detection rate of metastatic pulmonary nodule ≥ 1 cm was 12/13 (92.3%), when characteristic curve, the optimal cutoff of average SUV to predict extrahepatic metastases was 3.4. CONCLUSION: 18F-FDG PET/CT scan is invaluable for detection of lung metastases larger than 1 cm and bone metastases. Primary HCC having larger than 5 cm and increased average SUV uptake more than 3.4 should be considered for extrahepatic metastases.

Pancreatic pseudocysts,which account for 70%-90% of pancreatic cystic lesions,characteristically are non-epithelially lined cystic cavities that are contiguous with the pancreas. Pancreatic pseudocysts can be caused by acute,chronic or traumatic pancreatitis and should be differentiated from other pancreatic diseases with cystic appearances,especially cystic neoplasms. We report a unique case of a pancreatic pseudocyst filled with semisolid lipids,which appeared by endoscopic ultrasound as a solid mass,and was therefore resected.

Administration of monoclonal antibodies(mAbs)against epidermal growth factor receptor(EGFR)such as cetuximab and panitumumab in combination with conventional chemotherapy substantially prolongs survival of patients with metastatic colorectal cancer(mCRC).However,the efficacy of these mAbs is limited due to genetic variation among patients,in particular K-ras *** discovery of K-ras mutation as a predictor of non-responsiveness to EGFR mAb therapy has caused a major change in the treatment of *** that inhibit transformation caused by oncogenic alterations of Ras and its downstream components such as BRAF,MEK and AKT seem to be promising cancer therapeutics as single agents or when given with EGFR *** multiple therapeutic strategies to overcome EGFR mAb-resistance are under investigation,our understanding of their mode of action is *** drug development based on stringent preclinical data,biomarker validation,and proper selection of patients is of paramount importance in the treatment of *** this review,we will discuss diverse approaches to overcome the problem of resistance to existing anti-EGFR therapies and potential future directions for cancer therapies related to the mutational status of genes associated with EGFRRas-ERK and PI3K signalings.

AIM:To investigate serotonergic Ca 2+ signaling and the expression of 5-hydroxytryptamine(5-HT) receptors,as well as Ca 2+ transporting proteins,in hepatic stellate cells(HSCs) . METHODS:The intracellular Ca 2+ concentration([Ca 2+ ]i) of isolated rat HSCs was measured with a fluorescence microscopic imaging *** PCR was per-formed to determine the transcriptional levels of 5-HT receptors and endoplasmic reticulum(ER) proteins involved in Ca 2+ storage and release in cultured rat HSCs. RESULTS:Distinct from quiescent cells,activated HSCs exhibited[Ca 2+ ]i transients following treatment with 5-HT,which was abolished by U-73122,a phospholipase C *** of 5-HT2A and 5-HT2B receptors,but not 5-HT3,was prominent during trans-differentiation of *** with ritanserin,a 5-HT2 antagonist,inhibited[Ca 2+ ]i changes upon application of *** of type 1 inositol-5'-triphosphate receptor and type 2 sarcoplasmic/endoplasmic reticulum Ca 2+ ATPase were also increased during activation of HSCs and serve as the major isotypes for ER Ca 2+ storage and release in activated *** 2+ binding chaperone proteins of the ER,including calreticulin,calnexin and calsequestrin,were up-regulated following activation of HSCs. CONCLUSION:The appearance of 5-HT-induced[Ca 2+ ]i response accompanied by upregulation of metabotropic 5-HT2 receptors and Ca 2+ transporting/chaperone ER proteins may participate in the activating process of HSCs.

Herein,we examine the performance of dye-sensitized solar cells containing five D-π-A organic dyes designed by systematic modification of π-bridge size and geometric *** dye has a simple push-pull structure with a triarylamino group as an electron donor,bithiophene-4,4-dimethyl-4 H-cyclopenta 1,2-b:5,4-b’]dithiophene(M11),4,4-dimethyl-4 H-cyclopenta[1,2-b:5,4-b’]dithiophenethiophene(M12),thiophene-4,4-dimethyl-4 H-cyclopenta[1,2-b:5,4-b’]dithiophene(M13),4,4-dimethyl-4 H-cyclopenta[1,2-b:5,4-b’]dithiophene-benzene(M14),and 4,4-dimethyl-4 H-cyclopenta[1,2-b:5,4-b’]dithiophene(M15)units asπ-bridges,and cyanoacrylic acid as an electron acceptor/*** extension of theπ-bridge linkage favors wide-range absorption but,because of the concomitant molecular volume increase,hinders the efficient adsorption of dyes on the TiO_(2) film ***,higher loadings are achieved for smaller dye molecules,resulting in(i)a shift of the TiO_(2) conduction band edge to more negative values,(ii)a greater photocurrent,and(iii)suppressed charge recombination between the photoanode and the redox couple in the ***,under one-sun equivalent illumination(AM 1.5 G,100 mW/cm^(2)),the highest photovoltage,photocurrent,and conversion efficiency(η=7.19%)are observed for M15,which has the smallest molecular volume among M series dyes.

Cold-junction compensation(CJC)and disconnection detection circuit design of various thermocouples(TC)and multi-channel TC interface circuits were *** CJC and disconnection detection circuit consists of a CJC semiconductor device,an instrumentation amplifier(IA),two resistors,and a diode for disconnection *** on the basic circuit,a multi-channel interface circuit was also *** CJC was implemented using compensation semiconductor and IA,and disconnection detection was detected by using two resistors and a diode so that IA input voltage became-0.42 *** a result of the experiment using R-type TC,the error of the designed circuit was reduced from 0.14 mV to 3μV after CJC in the temperature range of 0°C to 1400°*** addition,it was confirmed that the output voltage of IA was saturated from 88 mV to-14.2 V when TC was disconnected from *** output voltage of the designed circuit was 0 V to 10 V in the temperature range of 0°C to 1400°*** results of the 4-channel interface experiment using R-type TC were almost identical to the CJC and disconnection detection results for each *** implemented multi-channel interface has a feature that can be applied equally to E,J,K,T,R,and S-type TCs by changing the terminals of CJC semiconductor devices and adjusting the IA gain.

Indole-3-acetic acid （IAA）, a major plant auxin, is produced in both tryptophan-dependent and tryptophan-independent pathways. A major pathway in Arabidopsis thaliana generates IAA in two reactions from tryptophan. Step one converts tryptophan to indole-3-pyruvic acid （IPA） by tryptophan aminotransferases followed by a rate-limiting step converting IPA to IAA catalyzed by YUCCA proteins. We identified eight putative StYUC （Solanum tuberosum YUCCA） genes whose deduced amino acid sequences share 50%-70% identity with those of Arabidopsis YUCCA proteins. All include canonical, conserved YUCCA sequences： FATGY motif, FMO signature sequence, and FAD-binding and NADP-binding sequences. In addition, five genes were found with -50% amino acid sequence identity to Arabidopsis trypto-phan aminotransferases. Transgenic potato （Solanum tuberosum cv. Jowon） constitutively overexpressing Arabidopsis AtYUC6 displayed high-auxin phenotypes such as narrow downward-curled leaves, increased height, erect stature, and longevity. Transgenic potato plants overexpressing AtYUC6 showed enhanced drought tolerance based on reduced water loss. The phenotype was correlated with reduced levels of reactive oxygen species in leaves. The results suggest a func-tional YUCCA pathway of auxin biosynthesis in potato that may be exploited to alter plant responses to the environment.

BACKGROUND Retroperitoneal sarcoma(RPS) is a rare malignancy arising from mesenchymal cells that most commonly presents as an abdominal mass and is associated with poor *** several studies have assessed the survival benefits of wide excision,few have reported detailed methods for achieving wide excision in patients with *** To describe our experience with multidisciplinary surgical resection of RPS using intra-and extra-pelvic *** Multidisciplinary surgery is an anatomical approach that combines intra-and extra-peritoneal access within the same surgery to achieve complete RPS *** retrospective review of the records of patients who underwent multidisciplinary surgery for RPS analyzed surgical and survival *** Eight patients underwent 10 intra-and extra-pelvic surgical resections,and their median mass size was 12.75 cm(range,6-45.5 cm).Using an intrapelvic approach,laparoscopy-assisted surgery was performed in four cases and laparotomy surgery in *** an extrapelvic approach,ilioinguinal and posterior approaches were used in four cases each,and the prone position and midline skin incision were shared in *** patients’ RPS masses were removed completely,and four achieved R0 resection through intra-and extra-pelvic *** median estimated blood loss was 2000 m L(range,300-20000 m L) and the median hospitalization was 12.6 d(range,9-69 d).Reoperation was needed in two patients(one for wound necrosis and the other for bowel perforation and wound necrosis).The median overall survival rate and median progression-free survival were 64.6 and 13.7 mo,*** RPS is therapeutically challenging because of its location and high risk of ***,intra-and extra-pelvic surgical approaches can improve the macroscopic security of the surgical margin.
