Covid-19 in Pregnancy: Cases of Women Managed in the City of Douala-Cameroon

Introduction: The SARS-COV-2 virus has been responsible for a health crisis in pregnancy, causing severe acute respiratory distress syndrome. Materno-foetal complications can be observed. Taking into account the materno-foetal risks associated with COVID-19 infection in pregnant women and the low sample size of the first publication in Cameroon, we considered it necessary to conduct an in-depth study on the maternal and foetal prognosis of this condition in pregnant women in three hospitals in Douala. Materials and Methods: We conducted a cross-sectional survey with retrospective data collection in the three state tertiary and reference hospitals in Douala (DGOPH, DGH and DLH) from November 1, 2021 to April 30 2022 after obtaining ethical and administrative clearances. All records of COVID-19 pregnant women confirmed by reverse transcription polymerase chain reaction (RT PCR) or COVID-19 rapid diagnostic test (COVID RDT) were included. The survey form contained socio-demographic data, clinical and para-clinical characteristics, management and materno-foetal outcome. SPSS.26 and Microsoft Excel 2016 software were used to analyze the data, and a logistic regression model was used to look for associations between the variables. Results: We found in total 96 files that met our inclusion criteria. The most represented sector of activity was the unemployed (44.46%). Patients with a secondary level of education constituted the majority with 47% (45). The main comorbidity found was diabetes (27.2%). The most frequent symptom was fever, found in 87 patients (90.1%). In our series, a chest CT scan was performed in 50 patients. Caesarean section was the most common mode of delivery (58.3%). We recorded 15.6% of maternal deaths, mainly in patients admitted to intensive care. Factors associated with maternal death were: Maternal diabetes, high LDH and D-dimer levels, and 75% lung involvement on chest CT scan. Oxygen saturation > 94% on admission was a protective factor. 随着全球经济的不断发展,无论大公司还是中小公司,都非常重视对人力资源实践的研究,而各相关人力资源实践活动可以直接促进企业的经济效益。对此,过往已有不少研究已经发现各种人力资源实践对企业绩效有重要的影响。但是在经济发展较为落后的非洲,学术界对这一问题研究甚少。对此,研究者选取这一研究主题,以非洲喀麦隆的中小企业为研究对象,调查中小企业人力资源实践行为的实施状况。本研究发现,喀麦隆中小企业人力资源实践行为存在实施不足、缺乏专门的人资管理人才、薪酬改革方面投入不足,特别是在人力资源培训计划和师徒计划方面投入有限等问题。通过SWOT分析法,对中小企业人力资源实施中存在的优势、不足,面临的机会和挑战,提出相应的对策。如:政府应为中小企业招聘优秀人才出台优惠政策;鼓励、引导有效的社会资源为中小企人力管理提供帮扶服务等。本研究可以有效促进喀麦隆中小企业人力资源管理活动,减少中小企业竞争压力、提高企业人力资源管理水平,提升喀麦隆的经济和社会效益。

Hybrid Composite Based on Natural Rubber Reinforced with Short Fibers of the Triumfetta cordifolia/Saccharum officinarum L.: Performance Evaluation

This article contributes to the development of the new class of fully biodegradable “green” composites by combining fibers (natural/bio) with biodegradable resin. The vegetable fibers (Triumfetta cordifolia and sugarcane bagasse) treated with NaOH and bleached were incorporated into a natural rubber matrix. The influence of the fiber ratio on the physical properties, tensile strength and surface hardness of the hybrid composites was analyzed. The results show that the addition of fibers in the natural rubber matrix increases the water absorption capacity but gradually reduces it with increasing fiber ratio. The hybrid composites of the NRT50-50B proportions show the best tensile strengths at 20 phr and a shore A hardness of 43.7 at 30 phr. The combination of two fibers has improved the physical and mechanical properties of the hybrid composites which can be used in engineering applications.

The error-prone V(D)J recombination process generates considerable amounts of nonproductive immunoglobulin(Ig)*** recently demonstrated that aberrant Ig chains lacking variable(V)domains can be produced after nonsense-associated altered splicing(NAS)***,the expression of these truncated Ig polypeptides heightens endoplasmic reticulum stress and shortens plasma cell(PC)*** questions remain regarding the molecular mechanisms underlying this new truncated Ig exclusion(TIE-)checkpoint and its restriction to the ultimate stage of B-cell *** address these issues,we evaluated the extent of NAS of Ig pre-mRNAs using an Ig heavy chain(IgH)knock-in model that allows for uncoupling of V exon skipping from TIE-induced *** found high levels of V exon skipping in PCs compared with B cells,and this skipping was correlated with a biallelic boost in IgH transcription during PC *** analysis further revealed that the skipped V exon turned into a ***,we showed that hypertranscription of Ig genes facilitated V exon skipping upon passive administration of splice-switching antisense oligonucleotides(ASOs).Thus,V exon skipping is coupled to transcription and increases as PC differentiation proceeds,likely explaining the late occurrence of the TIE-checkpoint and opening new avenues for ASO-mediated strategies in PC disorders.

Traffic Engineering （TE） has become a challenging mechanism for network management and resources optimization due to the uncertainty and the difficulty to predict current traffic patterns. Recent works have proposed robust optimization techniques to cope with uncertain traffic, computing a stable routing configuration that is immune to demand variations within certain uncertainty set. However, using a single routing configuration for long-time periods can be highly inefficient. Even more, the presence of abnormal and malicious traffic has magnified the network operation problem, claiming for solutions which not only deal with traffic uncertainty but also allow to identify faulty traffic. In this paper, we propose two complementary methods to tackle both problems. Based on expected traffic patterns, we adapt the uncertainty set and build a multi-hour yet robust routing scheme that outperforms the stable approach. For the case of anomalous and unexpected traffic, we propose a fast anomaly detection/isolation algorithm which relies on a novel linear spline-based model of traffic demands to identify traffic problems and decide routing changes. This algorithm is optimal in the sense that it minimizes the decision delay for a given mean false alarm rate and false isolation probabilities. Both proposals are validated using real traffic data from two Internet backbone networks.

Cervical Conglutination: A Case Report

Conglutination is one of the rarest disorders encountered during labor. Reported by few authors over the centuries, its diagnosis and management are fraught with pitfalls in terms of the experience of the health-care team and the likely complications of vaginal delivery. We report on a case received at the university clinics in Kinshasa, Democratic Republic of the Congo, where we successfully delivered vaginally. Discussions are not clear-cut as to the preferred route of delivery in cases of external cervical conglutination, but in our context where society is pronatalist and women are unwilling to undergo caesarean section, every intervention [to be] performed should consider the impact of such intervention on the woman’s obstetrical future and the vaginal route should be preferred whenever reasonably possible.

Design and Multicriteria Optimization of a Two-Stage Reactive Extrusion Process for the Synthesis of Thermoplastic Polyurethanes

This paper presents the implementation of two multicriteria optimization methods based on different approaches, namely, Rough Set Method (RSM) and Net Flow Method (NFM), to the manufacture by reactive extrusion of linear thermoplastic polyurethanes (TPUs), appropriate for medical applications. A preliminary study allowed determining the process operating conditions for which the polymerization time and the average residence time of the reactants in the extruder are of the same order of magnitude. Prior to the optimization, a neural network model able to predict with acceptable accuracy the effect of the operating conditions on the output process variables, was constructed and validated. This model was then used to determine, using Pareto’s concept, a set of non-dominated solutions constituting Pareto’s domain. These solutions were then ranked according to the preferences of a decision maker using NFM and RSM. This allowed providing the 10% highest ranked solutions of Pareto’s domain and proposing a set of optimal operating conditions for the production, with the lowest energy consumption, of TPUs with targeted properties and high purity. Experimental validation runs carried out under similar operating conditions gave rise to criteria values confirming the su- perior performance of NFM, without rejecting, at the same time, the values obtained using RSM.
