The hair follicle is a biological oscillator that alternates growth,regression,and rest phases driven by the sequential activation of the proliferation differentiation programs of resident stem cell *** activation of hair follicle stem cell niches and subsequent entry into the growing phase is mainly regulated by Wnt/β-catenin signalling,while regression and resting phases are mainly regulated by Tgf-β/Bmp/Smad activity.A major question still unresolved is the nature of the molecular switch that dictates the coordinated transition between both signalling *** we have focused on the role of Endoglin(Eng),a key co-receptor for members of the Tgf-β/Bmp family of growth *** an Eng haploinsufficient mouse model,we report that Eng is required to maintain a correct follicle cycling pattern and for an adequate stimulation of hair follicle stem cell *** further report thatβ-catenin binds to the Eng promoter depending on Bmp ***,we show thatβ-catenin interacts with Smad4 in a Bmp/Eng-dependent context and both proteins act synergistically to activate Eng promoter *** observations point to the existence of a growth/rest switching mechanism in the hair follicle that is based on an Eng-dependent feedback cross-talk between Wnt/β-catenin and Bmp/Smad signals.

Clinical decision support for drug related events: moving towards better prevention

Clinical decision support(CDS) systems with automated alerts integrated into electronic medical records demonstrate efficacy for detecting medication errors(mE) and adverse drug events(ADEs). Critically ill patients are at increased risk for mE, ADEs and serious negative outcomes related to these events. Capitalizing on CDS to detect mE and prevent adverse drug related events has the potential to improve patient outcomes. The key to an effective medication safety surveillance system incorporating CDS is advancing the signals for alerts by using trajectory analyses to predict clinical events, instead of waiting for these events to occur. Additionally, incorporating cutting-edge biomarkers into alert knowledge in an effort to identify the need to adjust medication therapy portending harm will advance the current state of CDS. CDS can be taken a step further to identify drug related physiological events, which are less commonly included in surveillance systems. Predictive models for adverse events that combine patient factors with laboratory values and biomarkers are being established and these models can be the foundation for individualized CDS alerts to prevent impending ADEs.

The studies performed in the frame of a project destined for the search of new (t,n) and (p,n) reactions of interest in nuclear reactors are described. Experimental evidences of the observations of the reactions: 46Ti(t,n)48V, 48Ti(p,n)48V, 52Cr(t,n)54mn, 56Fe(p,n)56Co, 72Ge(t,n)74As and 74Ge(p,n)74As, are presented. Additional data on some secondary reactions, already characterised for the production of 7Be, 56Co, 58Co, 65Zn and 88Y, were also obtained. The significance of these data is discussed.

Leishmaniasis is a set of diseases with a worldwide distribution that affects mainly economically underprivileged populations in developing countries. It has a major impact on public health, with a global cost of billions of dollars per year. The treatment and control of leishmaniasis vary according to the Leishmania species involved, which require reliable methods for species identification. Since most of the currently used methods have limitations, there is a need for assays that allow rapid, precise identification of the offending species. Real-time polymerase chain reactions in conjunction with dissociation curve analysis have been used to detect differences in the DNA composition of selected genes of Leishmania spp. Kinetoplast DNA is the main molecular target used because of its high copy number per parasite, but other targets have also been studied. As part of an effort to establish melting temperature standards for each target gene, we have reviewed the pertinent literature available in public databases, including Pub med, Web of Science, Sci ELO and LILACS, using the keywords "Leishmania", "leishmaniasis", "realtime PCR", "melting temperature", and "melting curve", alone or in combination. After applying eligibility criteria, 27 articles were selected for analysis. A considerable variation in the methodologies analyzed was found regarding molecular targets, standardization of the methods, reproducibility and specificity. Because of this, statistical analysis was not performed. In most cases, the methods were able to differentiate the parasite at the subgenus level or few species regardless of the target chosen.

A Lack of Effect on Patient Satisfaction Scores in One Large Urban Emergency Department

Background: To deal with emergency department crowding and long waits before patient care, many institutions have placed a doctor in the triage area to initiate treatment and testing. Objective: to determine the effect of a doctor in triage on patient satisfaction scores. methods: This is an observational study comparing the patient satisfaction scores from days when a physician was in triage to days when a physician was not present. The study was conducted in the ED of an urban academic medical center with excellent primary care resources and payer mix (7% self pay, 11% medicaid). Results: There was a mean of 4 (95% CI 3.1 - 4.5) surveys returned for each day when there was a doctor in triage and a mean of 5 (95% CI 4.3 - 5.7) surveys for each day without a doctor in triage. Overall satisfaction for the days with a doctor in triage was 79.9 ± 10.5 (95% CI 77.7, 82.1) compared to 78.8 ± 9.2 (95% CI 76.5, 81.1) (p = 0.9) on days without a doctor in triage. Conclusion: In this setting, there was no effect of a doctor in triage on patient satisfaction scores. While a doctor in triage may be valuable and cost effective in some settings, it may not provide appropriate benefit in all settings. We suggest that facilities trial a physician in triage program and measure predetermined outcomes such as patient satisfaction scores, length of stay and the percentage of patients left without being seen before investing in additional staffing and cost to sustain such a program.

Selective ligninolysis of wheat straw and wood chips by the white-rot fungus Lentinula edodes and its influence on in vitro rumen degradability

Background： The present work investigated the influence of lignin content and composition in the fungal treatment of lignocellulosic biomass in order to improve rumen degradability. Wheat straw and wood chips,differing in lignin composition, were treated with Lentinula edodes for 0, 2, 4, 8 and 12 wk and the changes occurring during fungal degradation were analyzed using pyrolysis-gas chromatography-mass spectrometry and detergent fiber ***： L. edodes preferentially degraded lignin, with only limited cellulose degradation, in wheat straw and wood chips, leaving a substrate enriched in cellulose. Syringyl（S）-lignin units were preferentially degraded than guaiacyl（G）-lignin units, resulting in a decreased S/G ratio. A decreasing S/G ratio（wheat straw： r =-0.72, wood chips： r =-0.75） and selective lignin degradation（wheat straw： r =-0.69, wood chips： r =-0.88） were correlated with in vitro gas production（IVGP）, a good indicator for rumen ***： L. edodes treatment increased the IVGP of wheat straw and wood chips. Effects on IVGP were similar for wheat straw and wood chips indicating that lignin content and 3D-structure of cell walls influence in vitro rumen degradability more than lignin composition.

Flexofytol^(■) (A Belgian Curcumin Extract)for the Treatment of Aged Patients with Osteoarthritis and Comorbidity

Introduction: Currently, the therapeutic arsenal of osteoarthritis includes several extracts of Curcuma including Flexofytol®, a bio-optimized extract of Curcuma longa. However, in older patients, indications for treatment might be limited by comorbidity and polymedication. Therefore, we aimed to assess the benefits and risks of Flexofytol®, in an older population with comorbidities. Patients and methods: This retrospective observational study included 31 patients over age 70 years (medianage: 77 years, range 71 - 81) that were treated with Flexofytol for painful osteoarthritis of the knee or lumbar spine. These patients were initially weakened by diabetes (48%) and/or renal insufficiency (71%), or they were taking anticoagulants (35%). The effects of Flexofytol®, were evaluated at 0, 6 and 12 weeks with visual analog scales for pain and disability and the SF-12 Quality of Life questionnaire. Adverse effects and drug interactions of Flexofytol®, were evaluated. In particular, we evaluated renal function, diabetes parameters and coagulation tests. The data were analyzed with Kruskal-Wallis and Wilcoxon-mann-Withney non-parametric tests. Results: Patients with Flexofytol®, showed significant improvement: pain improved by 50% (p = 0.0002) and functional disability improved by 33% (p = 0.0075). A series of quality of life parameters improved within the first 6 weeks of treatment and up to 3 months of treatment without impacting renal function, metabolic parameters or coagulation tests. We observed no significant adverse effects. Conclusion: In conclusion, our results suggested that Flexofytol®, may be useful in the management of painful osteoarthritis, particularly in older patients that are fragile due to comorbidity and polymedication. These initial results must be confirmed in future studies.
