BACKGROUND Slipped capital femoral epiphysis(SCFE)occurs in adolescents and has an incidence of around 10 per 100000 *** presenting with a unilateral SCFE are 2335 times more likely to develop a contralateral SCFE than the general *** factors that have been suggested to increase the risk of contralateral slip include a younger patient,an underlying endocrine disorder,growth hormone use and a higher radiographic posterior sloping ***,there is still much debate on the advantages and disadvantages of prophylactic fixation of the unaffected side in an otherwise healthy *** To investigate the risk rate of contralateral SCFE and assess the(dis)advantages of prophylactic fixation of the contralateral *** A systematic literature search was performed in the Embase,Medline,Web of Science Core Collection and Cochrane *** terms included‘slipped capital femoral epiphysis,’‘fixation,’‘contralateral,’and *** eligibility of the acquired articles was independently assessed by the authors and additional relevant articles were included through *** were considered eligible for inclusion if they presented data about otherwise healthy children with primarily unilateral SCFE and the outcomes of prophylactically pinning their unaffected side,or about the rates of contralateral slips and complications *** study quality of the included articles was assessed independently by the authors by means of the methodological index for non-randomized studies *** Of 293 identified unique publications,we included 26 studies with a total of 12897 patients.1762 patients(14%)developed a subsequent symptomatic contralateral *** addition,38%of patients developed a subsequent slip on the contralateral side without experiencing clinical *** most outspoken advantage of prophylactic fixation of the contralateral hip in the literature is prevention of an(asymptomatic)slip

The nitrogen(N)requirements of cotton are well understood and have been extensively studied since 1887,with practical guides,decision support systems,models and recent reviews providing information on the importance of providing cotton crops with a sufficient supply of nutrients and improving nitrogen use *** that the financial return to the grower depends on both the cotton quantity and quality and the growing importance of the latter,this review focusses specifically on information and knowledge about the effect of nitrogen application level on fibre quality.

X-ray absorption near-edge structure(XANES)spectra are the fingerprint of the local atomic and electronic structures around the absorbing ***,the quantitative analysis of these spectra is not *** with the most recent advances in this area,for a given spectrum,it is not clear a priori which structural parameters can be refined and how uncertainties should be ***,we present an alternative concept for the analysis of XANES spectra,which is based on machine learning algorithms and establishes the relationship between intuitive descriptors of spectra,such as edge position,intensities,positions,and curvatures of minima and maxima on the one hand,and those related to the local atomic and electronic structure which are the coordination numbers,bond distances and angles and oxidation state on the other *** approach overcoms the problem of the systematic difference between theoretical and experimental ***,the numerical relations can be expressed in analytical formulas providing a simple and fast tool to extract structural parameters based on the spectral *** methodology was successfully applied to experimental data for the multicomponent Fe:SiO_(2)system and reference iron compounds,demonstrating the high prediction quality for both the theoretical validation sets and experimental data.

Current study evaluated the effect of a fine and coarsely ground insoluble dietary fibre source on the gastrointestinal development of suckling *** hulls(OH)were selected as a model feedstuff,rich in cellulose,lignin,and insoluble dietary *** experimental supplemental diets were formulated:a finely ground,low fibre and nutrient dense diet served as control(CON).For the 2 high fibre diets,15%heat-treated starch in CON was exchanged with OH,either finely(OH-f)or coarsely ground(OH-c).Litters of 10 primi-and multiparous sows(mean litter size 14.6±0.84)were *** a litter,experimental diets were allotted to triplets of 4 *** approximately 12 d of age,piglets’individual feed intakes were recorded 2 times per day when separated from their dam for 70 *** could suckle with their dam for the remainder of the *** d 24 and 25,from the total pool of 120 piglets,seven healthy well-eating piglets per treatment were selected for post-mortem evaluation,resulting in 14 replicates per *** of OH-c and OH-f did not impede clinical health and production performance of *** full stomach weights tended to be greater for OH-c compared to OH-f whereas CON was intermediate(P=0.083).Supplementing OH significantly increased ileal villus height and caecal dry matter concentration(P<0.05).For the colon,OH increased its length,contents weight,short-chain fatty acid concentration and reduced total bacterial count as well asγ-proteobacteria count and proportion(P<0.05).The OH-c treatment specifically increased full gastrointestinal tract weight and caecum contents weight compared to piglets fed CON and ***,OH-c reduced colonic crypt depth when compared to OH-f(P=0.018).In conclusion,supplementing OH to a diet for suckling piglets exerted subtle developmental effects on gastrointestinal morphology and colonic microbial *** effects were largely independent from the particle size of the OH.

糖尿病相关的足病是造成糖尿病患者经济负担和增加社会成本的主要原因之一。本指南以主要目标为导向,以循证医学为依据,且在全球得到合理实施。践行国际糖尿病相关的足病指南对于减轻患者经济负担和社会成本有着重要影响。国际糖尿病足工作组(IWGDF)从1999年起开始发布并随后更新国际指南。2023年更新版指南相关推荐均基于推荐、评估、发展和评价分级(GRADE)系统,包括制定相关临床问题和重要结局,完成系统的文献系统综述和荟萃分析,完成评判表汇总,并且给予具体、明确、操作性强的推荐以及理论依据。本文阐述了《国际糖尿病足工作组:糖尿病相关的足病预防和管理指南(2023版)》(以下简称为指南)的制定过程。指南由7个部分组成,包括预防、糖尿病相关的足溃疡分类、减压、周围动脉病变、感染、创面愈合干预以及活动性夏科神经骨关节病,分别由7个独立的国际专家工作组编写。IWGDF编辑委员会对7个章节进行了整合和简化,形成IWGDF实践指南。指南中的每条推荐均经过IWGDF编辑委员会成员和相关领域的独立的国际专家审阅通过。本文是IWGDF指南的制定和方法学。我们相信,医务人员、公共卫生机构和政策制定者采用和实施2023年IWGDF指南,将会改善糖尿病相关的足病的预防和管理水平,从而减少全球该疾病患者和减轻该疾病引起的全球社会负担。

Introduction:This study aims to describe the occurrence of postoperative complications related to cholesteatoma surgery and to determine factors influencing the most common complication,*** surgical site infection(SSI)in cases with and without mastoid *** and methods:Retrospective analyses were performed on surgically treated cholesteatomas in our hospital between 2013 and *** characteristics,peri-and postoperative management and complications were *** cases were divided into two groups based on whether mastoid obliteration was performed or ***:A total of 336 cholesteatoma operations were performed,of which 248 cases received mastoid *** total 21 complications were observed,of which SSI was the most common(15/21).No difference in occurrence of any postoperative complication was seen between the obliteration and noobliteration group(p=0.798),especially not in the number of SSI(p=0.520).Perioperative and/or postoperative prophylactic antibiotics were not associated to the development of an SSI in both *** the no-obliteration group a younger age(p=0.015),as well as primary surgery(p=0.022)increased the risk for *** the obliteration group the use of bioactive glass(BAG)S53P4 was identified as independent predictor of SSI(p=0.008,OR 5.940).Discussion:SSI is the most common postoperative complication in cholesteatoma *** causes of SSI are multifactorial,therefore further prospective research is needed to answer which factors can prevent the development of an SSI in cholesteatoma surgery.

Myotonic dystrophy-related Cdc42-binding kinase alpha(MRCKα)is an integral component of signaling pathways controlling vital cellular processes,including cytoskeletal reorganization,cell proliferation and cell *** this study,we investigated the physiological role of MRCKα in milk protein and fat production in dairy cows,which requires a dynamic and strict organization of the cytoskeletal network in bovine mammary epithelial cells(BMEC).Within a selection of 9 Holstein cows,we found that both mRNA and protein expression of MRCKα in the mammary gland were upregulated during lactation and correlated positively(r>0.89)with the mRNA and protein levels of *** positive correlations(r>0.79)were found in a primary culture of BMEC stimulated with prolactin for 24 *** these cells,silencing of MRCKα decreased basal b-casein,sterol-regulatory element binding protein(SREBP)-1 and cyclin D1 protein level,phosphorylation of mTOR,triglyceride secretion,cell number and viabilitydwhile overexpression of MRCKα displayed the reversed ***,silencing of MRCKα completely prevented the stimulatory action of prolactin on the same *** data demonstrate that MRCKα is a critical mediator of prolactin-induced lactogenesis via stimulation of the mTOR/SREBP1/cyclin D1 signaling pathway.
