Management of pancreaticobiliary disease using a new intra-ductal endoscope:The Texas experience

AIM:To evaluate a new single-operator mini-endoscope, Spyglass,for its performance,feasibility and safety in the management of pancreaticobiliary disease. METHODS:In a multicenter retrospective analysis of patients undergoing intraductal endoscopy,we evaluated 128 patients(71 men,mean age 57.6 years).Indications were therapeutic(TX)in 72(56%) and diagnostic(DX)in 56(44%). RESULTS:Peroral endoscopy was performed in 121 and percutaneous in *** indications included CBD stones in 41,PD stones in six,and biliary strictures in *** indications included abnormal LFT’s in 15, abnormal imaging in 38 and cholangiocarcinoma staging in *** of the stone(s)was considered good in 31,fair in six,and poor in four. Advancement of the electrohydraulic lithotripsy probe was not possible in three patients and proper targeting of the lesion was partial in four patients.A holmium laser was used successfully in three *** clearance was achieved in 37 patients after one procedure and in four patients after two *** of biliary strictures was modified in 20/29 and confirmed to be malignant in 10/*** the modified patients,no diagnosis was available in *** demonstrated malignancy in 8/17 and non-malignancy in *** pathology by imaging studies and abnormal LFT’s was modified in 43/63(66%).Staging of cholangiocarcinoma demonstrated multicentric cholangiocarcinoma in 2/*** was no morbidity associated with the use of Spyglass. CONCLUSION:Spyglass Spyscopeis a first generation, single operator miniature endoscope that can evaluate and treat various biliary and pancreatic tract diseases.

Performance studies on an oscillating water column employing a Savonius rotor

A new oscillating water column (OWC) design is proposed in this study to incorporate a simpler Savonius type turbine. Conventional OWC devices employ a bi-directional turbine such as a Wells or an Impulse turbine to extract energy from the air. The disadvantages of the Wells turbine include its inability to self start and stalling. The Savonius turbine is much cheaper and is an effective option at low Reynolds numbers. In the current rectangular OWC device, unlike the circular OWC, the width of entry of the capture chamber can be increased without being influenced by the diameter at the turbine section. To improve its primary capture efficiency, the front and rear walls of the OWC are inclined to minimize reflection. The Savonius rotor characteristics are studied with respect to the change in frequency of the incoming waves. The rotor rpm is sensitive to wave period and primary conversion efficiency while changes in depth only affect the rotor rpm at lower frequencies. The Savonius rotor shows promising results and can be incorporated into large scale OWC devices to reduce costs of the turbine component of the system.

Epidemiology of pelvic fractures in adults: Our experience at a tertiary hospital

Purpose: Pelvic fractures are severe injuries and are often associated with multiple system injuries, exacerbating the overall outcome. In India, the incidence of pelvic fractures is on a rise due to suboptimal roads and traffics but related literature regarding the overall epidemiology of these injuries is scarce and scanty. Our aim was to study the epidemiology of patients admitted with pelvic fractures at a level 1 trauma centre in India. Methods: A 16-month (between September 2015 and December 2016) prospective observational study was carried out on trauma patients with pelvic fractures at a level 1 trauma centre of a tertiary care hospital. Demography of patients, mechanism of injuries and complications were recorded prospectively. Results: We observed 75 patients who presented with pelvic fractures, where 56 were males and 19 were females. Mean age of the study populationwas 37.57 years. Road traffic accidents were the most common mode of injuries. Lateral compression injuries were the most common pattern. Associated injuries frequently encountered were lower extremities and acetabulum fractures, blunt abdominal trauma, urogenital injuries and head injuries. Out of the 75 patients, 52 were treated surgically and 23 were managed by conservative methods. Associated injuries of the extremities, head, abdomen and urogenital system indicated a longer hospital stay. Conclusion: Pelvic fractures, although belong to a relatively rare trauma subset, cause a high morbidity and mortality with considerable burden on the economy. Proper road safety training and driving etiquettes along with its strict implementation in true sense and spirit are the need of the hour.

Double segmental tibial fractures-- an unusual fracture pattern

A case of a 50-year-old pedestrian who was hit by a bike and suffered fractures of both bones of his right leg was presented. Complete clinical and radiographic assessment showed double segmental fractures of the tibia and multisegmental fractures of the fibula. Review of the literature revealed that this fracture pattern was unique and only a single case was reported so far. Moreover, we discussed the possible mechanisms which can lead to such an injury. We also discussed the management of segmental tibial fracture and the difficulties encountered with them. This case was managed by modern osteosynthesis technique with a pleasing outcome.

Automated Time Based Multi-Criteria Bug Triage Approach:Developer Working Efficiency and Social Network Based Developer Recommendation

In software development projects,bugs are common *** report bugs in open source *** is a need to develop high quality developer prediction model that considers developer work satisfaction,keep within limited development cost,and improve bug resolution *** address and resolve bug report as soon as possible is the main focus of triager when a new bug is ***,developer work efficiency is an important factor in *** address these issues,a proposed approach recommends a set of developers that could potentially share their knowledge with each other to fix new bug *** proposed approach is called developer working efficiency and social network based developer recommendation(DweSn).It is a composite model that builds developers'profile by using developer average bug fixing time,work efficiency to fix variety of bugs,as well as the developer's social interactions with other developers.A similarity measure is applied between new bug and bugs in corpus to extract the list of capable developers from the *** proposed approach only selects those developers who are active and less loaded with *** developer with the highest profile score is assigned the *** evaluated our approach on the subset of five large open-source projects including Mozilla,Netbeans,Eclipse,Firefox and OpenOffice,and compared it with the *** results demonstrate that combination of developers'efficiency with their average bug fixing time and interactions in their social network gives good accuracy and efficiently reduces bug tossing *** approach shows an improvement in prediction accuracy,precision,recall,F-score and reduced bug tossing length up to 93.89%,93.12%,93.46%,93.27%and 93.25%,*** proposed approach achieved a 93%hit ratio and 93.34%mean reciprocal rank,indicating that our proposed triager is able to efficiently assign bugs to correct developers.

Geochronological and genetic characterization of basaltic basement from western offshore basins in India

^(40)Ar-^(39)Ar geochronology,geochemistry,and Sr-Nd isotopic compositions of 30 core samples from 24 offshore drill wells in the Western offshore basins have been used to characterize the genetics of the volcanic basement from the Kutch,Mumbai,and Kerala-Konkan offshore *** from the volcanic basement rocks demonstrate extremely varied isotopic and geochemical fingerprints,which are suggestive of significantly diverse parent magma compositions and emplacement *** tholeiitic basement from Kutch Offshore basin has Ar-Ar ages that range between 60 Ma and 62 *** basement is characterised by a within-plate basalt signature,with depleted isotopic signatures similar to least contaminated Deccan Traps basalts,and a component of subducted crustal *** basement from Mumbai Offshore Basin has eruption ages between 63 Ma and 65 Ma and show a strong within-plate OIB *** and isotopic signatures are consistent with Renuion lavas and an enriched-end member of Deccan Traps ***,Kerala-Konkan Offshore Basin shows significant E-MORB affinity and is characterized by primitive-mantle signatures and least contamination from the upper continental crust,with ages of eruption between 60 Ma and 61 Ma based on obtained^(40)Ar-^(39)Ar *** study suggests that the Mumbai Offshore Basalts’mode of the eruption was comparable with onshore Deccan volcanism,whereas the basaltic basement in Kutch Offshore was formed after the main phase of Deccan eruption and may have been an offshoot of a"tail"of the main Deccan *** parent magma for the volcanic E-MORB basement in the Kerala-Konkan Offshore Basin is thought to have come from mixing along the Carlsberg Mid-Oceanic Ridge and the material from the Réunion plume with the northward movement of the Indian Plate during the Early *** is thought to have occurred concurrently with the formation of the Chagos-Laccadive Ridge.

How do we manage post-OLT redundant bile duct?

AIM: To address endoscopic outcomes of post-Orthotopic liver transplantation (OLT) patients diagnosed with a "redundant bile duct" (RBD). METHODS: Medical records of patients who underwent OLT at the Liver Transplant Center, University Texas Health Science Center at San Antonio Texas were retrospectively analyzed. Patients with suspected biliary tract complications (BTC) underwent endoscopic retrograde cholangiopancreatography (ERCP). All ERCP were performed by experienced biliary endoscopist. RBD was defined as a looped, sigmoid-shaped bile duct on cholangiogram with associated cholestatic liver biomarkers. Patients with biliary T-tube placement, biliary anastomotic strictures, bile leaks, bile-duct stonessludge and suspected sphincter of oddi dysfunction were excluded. Therapy included single or multiple biliary stents with or without sphincterotomy. The incidence of RBD, the number of ERCP corrective sessions, and the type of endoscopic interventions were recorded. Successful response to endoscopic therapy was defined as resolution of RBD with normalization of associated cholestasis. Laboratory data and pertinent radiographic imaging noted included the pre-ERCP period and a follow up period of 6-12 mo after the last ERCP intervention. RESULTS: One thousand two hundred and eighty-two patient records who received OLT from 1992 through 2011 were reviewed. Two hundred and twenty-four patients underwent ERCP for suspected BTC. RBD was reported in each of the initial cholangiograms. Twentyone out of 1282 (1.6%) were identified as having RBD. There were 12 men and 9 women, average age of 59.6 years. Primary indication for ERCP was cholestatic pattern of liver associated biomarkers. Nineteen out of 21 patients underwent endoscopic therapy and 2/21 required immediate surgical intervention. In the endoscopically managed group: 65 ERCP procedures were performed with an average of 3.4 per patient and 1.1 stent per session. Fifteen out of 19 (78.9%) patients were successfully

Microstructure transformations and improving wear resistance of austenitic stainless steel additively fabricated by arc-based DED process

In this study, austenitic stainless steel(ASS) was additively fabricated by an arc-based direct energy deposition(DED) technique. Macrostructure, microstructure, mechanical characteristics at different spatial orientations(0°, 90°, and 45°), and wear characteristics were evaluated at the deposited structure top, middle, and bottom regions. Results show that austenite(γ) and delta-ferrite(δ) phases make up most of the microstructure of additively fabricated SS316LSi steel. Within γ matrix, δ phase is dispersed both(within and along) grain boundaries, exhibiting a fine vermicular morphology. The bottom, middle,and top regions of WAAM deposited ASS exhibit similar values to those of wrought SS316L in the tensile and impact test findings. Notably, a drop in hardness values is observed as build height increases. During SEM examinations of fractured surfaces from tensile specimen, closed dimples were observed, indicating good ductility of as-built structure. Wear test findings show signs of mild oxidation and usual adhesive wear. By depositing a mechanically mixed composite layer, an increase in the oxidation percentage was discovered to facilitate healing of worn surfaces. The findings of this study will help in design, production and renovation of products/components that are prone to wear. WAAM-deposited ASS has remarkable strength and ability to withstand impacts;it can be used in the production of armour plates for defence applications, mainly military vehicles and aircraft.
