AIM:To elucidate causes for false negative magnetic resonance imaging(MRI)exams by identifying imaging characteristics that predict viable hepatocellular carcinoma(HCC)in lesions previously treated with locoregional therapy when obvious findings of recurrence are ***:This retrospective institutional review board-approved and Health Insurance Portability and Accountability Act-compliant study included patients who underwent liver transplantation at our center between 1/1/2000 and 12/31/2012 after being treated for HCC with locoregional *** selected patients had a contrast-enhanced MRI after locoregional therapy within 90 d of transplant that was prospectively interpreted as without evidence of residual or ***,2 radiologists,blinded to clinica and pathological data,independently reviewed the pre transplant MRIs for 7 imaging *** explan histopathology provided the reference standard,with clinically significant tumor defined as viable tumor≥1.0cm in maximum ***’s exact test was firs performed to identify significant imaging ***:Inclusion criteria selected for 42 patients with 65 treated *** of 42 patients(33%and 16 of 65 treated lesions(25%)had clinically significant viable tumor on explant *** o the 7 imaging findings examined could reliably and reproducibly determine which treated lesion had viable tumor when the exam had been prospectively read as without evidence of viable ***:After locoregional therapy some treated lesions that do not demonstrate any MRI evidence o HCC will contain viable *** such even patients with a negative MRI following treatment should receive regular short-term imaging surveillance because some have occult viable *** possibility of occult tumo should be a consideration when contemplating any action which might delay liver transplant.

Comparison of the Hemodynamic Effects of the Induction Agents ketamine, Etomidate and Sevoflurane Using the Model of Electrical Velocimetry Based Cardiac Output Monitoring in Pediatric Cardiac Surgical Patients

Objective: To compare the haemodynamic effects of the induction agents ketamine, etomidate and sevoflurane using the model of electrical velocimetry based cardiac output monitoring in paediatric cardiac surgical patients. Design: Prospective randomized study. Setting: Tertiary care hospital. Participants: 60 children kg iv ketamine (group k), 0.2-0.3 mg/kg iv etomidate (group E) or upto 8% sevoflurane (group S) as the induction agent. Hemodynamic parameters were noted before and after induction of anaesthesia utilizing a noninvasive cardiac monitor based on the model of electrical velocimetry. Measurements and Main Results: The demographic characteristics of the patients were similar in the three groups. The HR decreased in all groups, least in group E (P ≤ 0.01) but the MAP decreased only in group S (P ≤ 0.001). In group S, the stroke volume improved from 9 ± 3.2 ml to 10 ± 3.2 ml (P ≤ 0.05) and the stroke volume variation decreased from 25% ± 6.4% to 13% ± 6.2% (P ≤ 0.001). The stroke index and systemic arterial saturation improved in all groups (P ≤ 0.01). The cardiac index and index of contractility were unchanged. The transthoracic fluid content reduced in groups E and S, but did not change in group k (P ≤ 0.05). Conclusions: Etomidate appeared to provide the most stable conditions for induction of anesthesia in children undergoing cardiac surgery, followed by ketamine and sevoflurane.

目的:研究柚皮苷对束缚性应激引起的小鼠生化指标改变、行为变化及线粒体功能紊乱的影响。方法:将小鼠按照体质量随机分组,分别给予不同药物治疗14 d。在第15天让所有小鼠接受束缚性应激刺激6 h,然后在接受各种不同的行为测试后处死。取小鼠的大脑匀浆进行各种生化指标测量和线粒体功能分析。结果:6 h的急性束缚性应激刺激能显著改变小鼠的行为(焦虑和记忆),引起生化指标变化和线粒体功能紊乱。用柚皮苷预处理(50或100 mg/kg口服)14 d则能显著减轻急性束缚性应激刺激引起的小鼠行为和生化指标变化以及线粒体功能紊乱(Pkg腹腔注射)可以拮抗柚皮苷的保护效应(Pkg腹腔注射)则导致柚皮苷的保护效应增强。结论:柚皮苷对束缚性应激刺激引起的小鼠行为和生化指标变化以及线粒体功能紊乱的保护作用与氮能神经通路有关。

Genome-wide association studies(GWASs) have been instrumental in understanding complex phenotypic traits. However, they have rarely been used to understand lineage-specific pathways and functions that contribute to the trait. In this study, by integrating lineage-specific enhancers from mesenchymal and myeloid compartments with bone mineral density loci, we were able to segregate osteoblast-and osteoclast(OC)-specific functions. Specifically, in OCs, a PU.1-dependent transcription factor(TF)network was revealed. Deletion of PU.1 in OCs in mice resulted in severe osteopetrosis. Functional genomic analysis indicated PU.1 and MITF orchestrated a TF network essential for OC differentiation. Several of these TFs were regulated by cooperative binding of PU.1 with BRD4 to form superenhancers. Further, PU.1 is essential for conformational changes in the superenhancer region of Nfatc1. In summary, our study demonstrates that combining GWASs with genome-wide binding studies and model organisms could decipher lineage-specific pathways contributing to complex disease states.

背景导尿管引起的尿管相关性膀胱不适（CRBD）令人痛苦。本研究评估了加巴喷丁用于预防CRBD的有效性。方法108例行择期经皮肾切开取石术的成年患者，ASAI～Ⅱ级，男女不限，随机分为2组，每组54例患者。对照组用安慰剂，G组用加巴喷丁600mg。2组患者均在手术前1小时口服药物。患者在麻醉诱导后将16F的Foley导尿管插入膀胱，气囊注入生理盐水10ml。手术后患者进入麻醉恢复室，分别在到达恢复室（0小时）、1、2和6小时的时间点评估CRBD的发生率与严重程度（轻、中、重度）。结果对照组CRBD的发生率达80％（43／54），而加巴喷丁组CRBD的发生率降低到50％（27／54）（P〈0．05）。加巴喷丁还可减轻CRBD的严重程度和手术后疼痛程度，该组手术后需要芬太尼镇痛的人数和芬太尼使用的总量都减少（P〈0．05）。结论手术前1小时口服加巴喷丁（600mg）可以减少CRBD的发生率和严重程度，减少手术后疼痛，使手术后需要芬太尼镇痛的患者例数和芬太尼需求量明显减少。
