Management of pancreaticobiliary disease using a new intra-ductal endoscope:The Texas experience

AIM:To evaluate a new single-operator mini-endoscope, Spyglass,for its performance,feasibility and safety in the management of pancreaticobiliary disease. METHODS:In a multicenter retrospective analysis of patients undergoing intraductal endoscopy,we evaluated 128 patients(71 men,mean age 57.6 years).Indications were therapeutic(TX)in 72(56%) and diagnostic(DX)in 56(44%). RESULTS:Peroral endoscopy was performed in 121 and percutaneous in *** indications included CBD stones in 41,PD stones in six,and biliary strictures in *** indications included abnormal LFT’s in 15, abnormal imaging in 38 and cholangiocarcinoma staging in *** of the stone(s)was considered good in 31,fair in six,and poor in four. Advancement of the electrohydraulic lithotripsy probe was not possible in three patients and proper targeting of the lesion was partial in four patients.A holmium laser was used successfully in three *** clearance was achieved in 37 patients after one procedure and in four patients after two *** of biliary strictures was modified in 20/29 and confirmed to be malignant in 10/*** the modified patients,no diagnosis was available in *** demonstrated malignancy in 8/17 and non-malignancy in *** pathology by imaging studies and abnormal LFT’s was modified in 43/63(66%).Staging of cholangiocarcinoma demonstrated multicentric cholangiocarcinoma in 2/*** was no morbidity associated with the use of Spyglass. COnCLUSIOn:Spyglass Spyscopeis a first generation, single operator miniature endoscope that can evaluate and treat various biliary and pancreatic tract diseases.

Herein,we report the magnetic properties of α-MoO3nanofibers synthesized via a hydrothermal method.X-ray photoelectron and Raman spectroscopic studies have been employed to evidence the presence of oxygen vacancy defects in the α-*** elucidate the oxygen vacancy related ferromagnetism,post-thermal annealing in oxygen and vacuum was *** UV emission band of α-MoO3nanofibers reveals a red shift from oxygen annealed to vacuum annealed,indicating a bandedge *** saturation magnetization of oxygen annealed nanofibers decreases while that of vacuum annealed nanofibers *** results strongly confirm that the oxygen vacancies play a significant role in inducing *** origin of ferromagnetism may be due to the exchange interactions among localized electron spin moments resulting from oxygen vacancies of α-*** presence of such defects was further supported by the photoluminescence measurements.

Synergistic effect of hybrid Himalayan nettle/Bauhinia-vahlii fibers on physico-mechanical and sliding wear properties of epoxy composites

The cellulosic bast fibers are recognized as a justifiable and biodegradable substitute for producing moderate strength polymer composite materials because of their characteristics of renewability,ecofriendliness,and higher specific *** the aim of this research work is to fabricate Himalayan bast fibers(nettle fiber(nF)/bauhinia vahlii fiber(BF)) based mono/hybrid epoxy composites at varying weight percentage of 2-6 wt% and evaluate the physical(void fraction and water absorption),mechanical(tensile strength,flexural strength,hardness) and sliding wear properties of as-fabricated *** 6 wt% nBF reinforced composites exhibited higher mechanical properties as compared to nF and BF composites with tensile strength of 34.04 MPa,flexural strength of 42.45 MPa,and hardness of 37.01 Hv *** influence of various control factors(sliding velocity,nF/BF/nBF contents,normal load and sliding distance) on specific sliding wear rate of composites was evaluated by Taguchi(three factors at three levels) experimental design and the percentage contribution of these selected parameters on sliding wear performance was examined by Analysis of variance(AnOVA).The sliding wear property of as-developed composites was found to be greatly influenced by sliding velocity and the wear resistance was observed to be improved with the nF/BF/nBF *** wear mechanism of the as-fabricated composites has been elucidated by scanning electron microscopy *** research outcomes demonstrated that the hybridization of Bauhinia vahlii fiber with nettle fiber led to improve the mechanical and wear properties of epoxy composites.

Separation of metallic from semiconducting single-walled carbon nanotubes has been a major challenge for some time and some previous efforts have resulted in partial success. We have accomplished the separation effectively by employing fl uorous chemistry wherein the diazonium salt of 4-heptadecafl uorooc tylaniline selectively reacts with the metallic nanotubes present in the mixture of nanotubes. The resulting fl uoroderivative was extracted in perfl uorohexane leaving the semiconducting nanotubes in the aqueous layer. The products have been characterized by both Raman and electronic absorption spectroscopy. The method avoids the cumbersome centrifugation step required by some other procedures.

Performance studies on an oscillating water column employing a Savonius rotor

A new oscillating water column (OWC) design is proposed in this study to incorporate a simpler Savonius type turbine. Conventional OWC devices employ a bi-directional turbine such as a Wells or an Impulse turbine to extract energy from the air. The disadvantages of the Wells turbine include its inability to self start and stalling. The Savonius turbine is much cheaper and is an effective option at low Reynolds numbers. In the current rectangular OWC device, unlike the circular OWC, the width of entry of the capture chamber can be increased without being influenced by the diameter at the turbine section. To improve its primary capture efficiency, the front and rear walls of the OWC are inclined to minimize reflection. The Savonius rotor characteristics are studied with respect to the change in frequency of the incoming waves. The rotor rpm is sensitive to wave period and primary conversion efficiency while changes in depth only affect the rotor rpm at lower frequencies. The Savonius rotor shows promising results and can be incorporated into large scale OWC devices to reduce costs of the turbine component of the system.

Hepatocellular carcinoma in non-alcoholic steatohepatitis:Current knowledge and implications for management

With the prevalence of hepatitis C virus expected to decline, the proportion of hepatocellular carcinoma(HCC) related to non-alcoholic steatohepatitis(nASH) is anticipated to increase exponentially due to the growing epidemic of obesity and diabetes. The annual incidence rate of developing HCC in patients with nASH-related cirrhosis is not clearly understood with rates ranging from 2.6%-12.8%. While multiple new mechanisms have been implicated in the development of HCC in nASH; further prospective long-term studies are needed to validate these findings. Recent evidence has shown a significant proportion of patients with non-alcoholic fatty liver disease and nASH progress to HCC in the absence of cirrhosis. Liver resection and transplantation represent curative therapeutic options in select nASHrelated HCC patients but have placed a significant burden to our healthcare resources and utilization. Currently nASH-related HCC is the fastest growing indication for liver transplant in HCC candidates. Increased efforts to implement effective screening and preventative strategies, particularly in non-cirrhotic nASH patients, are needed to reduce the future impact imposed by nASH-related HCC.

A novel 5-（（4-（6,7-dihydrothieno[3,2-c]pyridin-5（4H）-ylsulfonyl）phenylamino）methyl）quinolin-8-ol（HTPSMQol） was synthesized by optimized reaction of 4-（6,7-dihydrothieno[3,2-c]pyridin-5（4H）-ylsulfonyl）aniline with 5-chloromethyl-8-hydroxy-quinoline hydrochloride（CMHQ）.Also complexes of HTPSMQol were prepared by using various M（Ⅱ） metal *** compounds were analyzed by spectroscopic techniques and screened against various strains of *** results showed higher antimicrobial activity of HTPSMQol compared to its metal complexes and comparable with Ciprofloxacin.

How do we manage post-OLT redundant bile duct?

AIM: To address endoscopic outcomes of post-Orthotopic liver transplantation (OLT) patients diagnosed with a "redundant bile duct" (RBD). METHODS: Medical records of patients who underwent OLT at the Liver Transplant Center, University Texas Health Science Center at San Antonio Texas were retrospectively analyzed. Patients with suspected biliary tract complications (BTC) underwent endoscopic retrograde cholangiopancreatography (ERCP). All ERCP were performed by experienced biliary endoscopist. RBD was defined as a looped, sigmoid-shaped bile duct on cholangiogram with associated cholestatic liver biomarkers. Patients with biliary T-tube placement, biliary anastomotic strictures, bile leaks, bile-duct stonessludge and suspected sphincter of oddi dysfunction were excluded. Therapy included single or multiple biliary stents with or without sphincterotomy. The incidence of RBD, the number of ERCP corrective sessions, and the type of endoscopic interventions were recorded. Successful response to endoscopic therapy was defined as resolution of RBD with normalization of associated cholestasis. Laboratory data and pertinent radiographic imaging noted included the pre-ERCP period and a follow up period of 6-12 mo after the last ERCP intervention. RESULTS: One thousand two hundred and eighty-two patient records who received OLT from 1992 through 2011 were reviewed. Two hundred and twenty-four patients underwent ERCP for suspected BTC. RBD was reported in each of the initial cholangiograms. Twentyone out of 1282 (1.6%) were identified as having RBD. There were 12 men and 9 women, average age of 59.6 years. Primary indication for ERCP was cholestatic pattern of liver associated biomarkers. nineteen out of 21 patients underwent endoscopic therapy and 2/21 required immediate surgical intervention. In the endoscopically managed group: 65 ERCP procedures were performed with an average of 3.4 per patient and 1.1 stent per session. Fifteen out of 19 (78.9%) patients were successfully
