Epidemiology of pelvic fractures in adults: Our experience at a tertiary hospital

Purpose: Pelvic fractures are severe injuries and are often associated with multiple system injuries, exacerbating the overall outcome. In India, the incidence of pelvic fractures is on a rise due to suboptimal roads and traffics but related literature regarding the overall epidemiology of these injuries is scarce and scanty. Our aim was to study the epidemiology of patients admitted with pelvic fractures at a level 1 trauma centre in India. Methods: A 16-month (between September 2015 and December 2016) prospective observational study was carried out on trauma patients with pelvic fractures at a level 1 trauma centre of a tertiary care hospital. Demography of patients, mechanism of injuries and complications were recorded prospectively. Results: We observed 75 patients who presented with pelvic fractures, where 56 were males and 19 were females. Mean age of the study populationwas 37.57 years. Road traffic accidents were the most common mode of injuries. Lateral compression injuries were the most common pattern. Associated injuries frequently encountered were lower extremities and acetabulum fractures, blunt abdominal trauma, urogenital injuries and head injuries. Out of the 75 patients, 52 were treated surgically and 23 were managed by conservative methods. Associated injuries of the extremities, head, abdomen and urogenital system indicated a longer hospital stay. Conclusion: Pelvic fractures, although belong to a relatively rare trauma subset, cause a high morbidity and mortality with considerable burden on the economy. Proper road safety training and driving etiquettes along with its strict implementation in true sense and spirit are the need of the hour.

Inhibition of proliferation of retinal vascular endothelial cells more effectively than choroidal vascular endothelial cell proliferation by bevacizumab

AIM： To evaluate the differential inhibitory effects of bevacizumab on cell proliferation of vascular endothelial growth factor （VEGF）-stimulated choroidal vascular endothelial cells （CVECs） and retinal vascular endothelial cells （RVECs） in ***： VEGF （400 ng/mL） enriched CVECs and RVECs were treated with escalating doses of bevacizumab （0.1, 0.5, 1, 1.5 and 2 mg/mL）. Cell proliferation changes were analyzed with WST-1 assay and trypan blue exclusion assay at 48, 72h and 1wk. Morphological changes were recorded with bright field ***： VEGF enriched RVECs showed significantly more decline of cell viability than CVECs after bevacizumab treatment. One week after treatment, RVEC cell proliferation decreased by 29.7%, 37.5%, 52.8%, 35.9% and 45.6% at 0.1, 0.5, 1.0, 1.5 and 2 mg/mL bevacizumab respectively compared to CVEC proliferation decrease of 4.1%, 7.7%, 2.4%, 4.1% and 17.7% （P〈0.05） by WST-1 assay. Trypan blue exclusion assay also revealed similar decrease in RVEC proliferation of 20%, 60%, 73.3%, 80% and 93.3% compared to CVEC proliferation decrease of 4%, 12%, 22.9%, 16.7% and 22.2% respectively （P〈0.05）. The maximum differential effect between the two cell types was observed at bevacizumab doses of 1.0 and 1.5 mg/mL at all time points. RVECs were 22 fold more sensitive （P〈0.01） compared to CVECs （52.8% vs 2.4%） at concentration of 1.0 mg/mL, and 8.7 fold more at 1.5 mg/mL （35.9% vs 4.1%） 1wk after treatment （P〈0.05 respectively）.CONCLUSION： VEGF-enriched RVECs are more susceptible to bevacizumab inhibition than CVECs at clinically used dosage of 1.25 mg and this differential sensitivity between two cell types should be taken into consideration in dosage selection.

Effect of plumbagin free alcohol extract of Plumbago zeylanica Linn,root on reproductive system of female Wistar rats

Objective:To assess the effect of plumbagin-free alcohol extract（PFAE） of Plumbago zeylanica Linn.（Plumbaginaceae）（***） root,on female reproductive system and ferlility of adult Female wislar ***:After the oral acute toxicity study,the PFAE was administered at two dose levels to perform the estrous cycle study,anti-implantation and ahortifacient activity and hormonal ***,the estrogenic/antiestrogenic activity was evaluated at only one most effective ***:LD50 cut-off was 5 000 mg/kg body *** extract exhibited significant anti-implantation and ahortifacient activity at the tested dose levels（300 and 500 mg/ kg.p.o.）（P<0.01）.The extract dose-dependency decreased the levels of serum progesterone, follicle stimulating hormone and luteinizing hormone,while a dose-dependent increase was observed in the concentration of serum *** extract did not show any significant changes in structure and function of Uterus when given alone,hut when given along with ethinyl estradiol,it exhibited significant antiestrogenic activity in immature overieclonzed female rats（P<0.001） Biochemical parameters in the serun/blood and haematological parameters did not show appreciable changes throughout and after the course of ***,all the altered parameters returned to normalcy within 30 days following withdrawal of treatment. Conclusions:All findings suggest that the anlifcrtility activity of extract could possibly he through the changes in the implantation site,altered hormonal levels,prolonged estrous cycle and anti-estrogenic ***,the extract possesses reversible antifcrtility activity without adverse toxicity in female rats.

Multiyear Observations of Deposition-Mode Ice Nucleating Particles at Two High-Altitude Stations in India

Ice nucleating particle(INP) measurements were made at two high-altitude stations in India. Aerosols collected on filter paper at Girawali Observatory, Inter University Center for Astronomy & Astrophysics(IGO), and at the Radio Astronomy Center, Ooty(RAC), were activated in deposition mode using a thermal gradient diffusion chamber to determine the INP concentrations. The measurement campaigns at IGO were conducted during 2011, 2013 and 2014, and at RAC during 2013 and 2014. When the aerosol samples were exposed to an ice supersaturation of between 5% and 23% in the temperature range-17.6?C to-22?C, the maximum INP number concentration at IGO and RAC was 1.0 L-1and 1.6 L-1, respectively.A maximum correlation coefficient of 0.76 was observed between the INP number concentration and ice supersaturation. The airmass trajectories analyzed for the measurement campaigns showed that the Arabian Desert and arid regions were the main INP contributors. Elemental analysis of particles showed the presence of Na, Cl, Si, Al, Fe, Cu, Co, Cd, S, Mn and K, as well as some rare-Earth elements like Mo, Ru, La, Ce, V and Zr. When aerosols in the size range 0.5–20 μm were considered, the fraction that acted as INPs was 1 : 104to 1 : 106at IGO, and 1 : 103to 1 : 104at RAC. The higher ratio of INPs to aerosols at RAC than IGO may be attributable to the presence of rare-Earth elements observed in the aerosol samples at RAC, which were absent at IGO.

Comparison of two and three dimensional quantitative coronaryangiography to intravascular ultrasound in the assessment of leftmain coronary artery bifurcation lesions

Background Angiographic evaluation of left main coronary artery(LMCA) bifurcation lesions is often limited. two dimensional(2D) quantitative coronary angiography(QCA) with segmental analysis provides accuracy for quantification of the degree of stenosis in the main vessel and side branch ostium but can be affected by foreshortening and variable magnification. The accuracy of three dimensional(3D) QCA has recently developed to overcome 2D QCA limitations, however, accuracy and precision of 3D bifurcation QCA measurements in LMCA bifurcation lesions has not been *** We investigated whether such 3D and 2D bifurcation QCA measurements differ in their accuracy in assessing significant LMCA bifurcation lesions defined by intravascular ultrasound(IVUS) as a minimum luminal area(MLA) <6 mm2 of LMCA and MLA <4 mm2 of proximal left anterior descending(LAD) and/or proximal left circumflex(LCX)Results LMCA bifurcation lesions were assessed in 44 patients undergoing elective percutaneous coronary intervention. From 2D QCA measurements, MLA correlated moderately with threshold intravascular ultrasound MLA for LMCA(r=0.81, P <0.000 1), LAD(r=0.54, P=0.000 1) and LCX(r=0.58, P <0.000 1). Severity of lesion as MLA by derived 3D QCA, correlated moderately with threshold intravascular ultrasound MLA for LMCA(r=0.84, P <0.000 1), LAD(r=0.53, P=0.000 2); LCX(r=0.66, P <0.000 1). Overall, the C statistics tended to be slightly higher for 3D QCA and 2D QCA measurements in LMCA segment compared with proximal LAD and LCX segments, and there were no significant predictive power of percent diameter stenosis and percent area stenosis on 3D QCA for LCX IVUS MLA <4 mm2(percent diameter stenosis: area under curve 0.55, cutoff 23%, sensitivity 88%, specificity 37%, P=0.618 6; percent arer stenosis: area under curve 0.56, cutoff 41%, sensitivity 83%, specificity 38%, P=0.518 4, respectively).Conclusions The accuracy of 3D bifurcation QCA in detecting significant LMCA bifurcation les

The earthquake precursors and earthquake prediction are the burning issue among the community of earth scientists and engineers. Studies of earthquake precursory phenomena since the last several decades have shown that significant geophysical and geochemical changes may occur prior to intermediate and large earthquakes （Hartmann and Levy, 2005; Yang et al, 2005; Einarsson et al, 2008; Italiano et al, 2008; kumar et al, 2009）. Among the more intensely investigated geochemical phenomena there have been changes in the concentrations of dissolved ions and gases in groundwater as well as variations in the concentrations of crustal and mantle volatiles in ground gases.
