Risk Factors for Geo-Helminthiasis in Children Aged 6 - 36 Months in a Rural Health District in Cameroon

Introduction and Objectives: Soil-Transmitted-Helminthiasis (STH) is a public health problem in Cameroon. The control strategies currently in place, particularly chemoprevention, has shortcomings linked to the target population, which are school-age children. The objective was to determine the prevalence and the risk factors associated with geo-helminthiasis in children aged 0 to 3 years in a rural health district. Method: From December 2020 to May 2021, a descriptive and analytical cross-sectional study of 376 children between 6 and 36 months was carried out in the Akonolinga health district. This was a cluster sampling in 4 health areas. Stool samples were collected and analysed using the mini-FLOTAC method. The results expressed as the number of eggs per gram of stool. A questionnaire on socio-demographic and lifestyle data was administered to the parents. The Chi-squared test was used to measure the association between geo-helminth infection and the data collected. A multivariate analysis using logistic regression was performed (p 0.05). Results: The prevalence of STH was 19.4% (Ascaris lumbricoides: 16% and Trichuris trichiura: 8%). Risk factors were: consumption of contaminated water (AOR = 1.93 [1.03 - 3.6];p = 0.040), early contact of the child with the ground (before age of 4 months) (AOR = 4.9 [2.1 - 11.37];p .001), habit of walking barefoot (AOR = 2.91 [1.1 - 7.97];p = 0.038), and living in a habitat with unpaved ground (AOR = 7.4 [1.55 - 35.7];p = 0.012). Conclusion: The prevalence of STHs in infants was high. Preventive chemotherapy should be extended to this age-group, and other measures intensified.

Parent-Reported Reasons for Loss-to-Follow Up in Patients Hospitalized at the Neonatology Unit at Laquintinie

Introduction: Despite the known benefits of newborn follow-up clinics, attendance has historically been difficult. Infants with reported follow-up difficulty have a greater incidence of severe sensorimotor and cognitive deficits and poorer access to early intervention programs. Our objective was to determine the parent-reported reasons for loss-to-follow up in patients hospitalised at the neonatology unit of Laquintinie Hospital. Methodology: We carried out a cross-sectional study through phone interviews with parents/caregivers of patients hospitalised at Laquintinie over a 2-year period from 1st January 2021 to 31st December 2022. A non-standardised structured interview guide was used for data collection. Loss-to-follow up referred to absence to at least one visit as recorded in the neonatal follow-up chart. All necessary administrative and ethical considerations were duly respected. Results: Most neonates were born through vaginal delivery (n = 313, 69.45%). The neonates were admitted at a median gestational age of 33 weeks (Q1-Q3;32 - 35) and the median duration of hospitalisation was 12 days (Q1 - Q3;8 - 18). A total of 23 neonates had died at the time of interview giving a mortality rate of 5.1%. The three most reported reasons for loss-to-follow-up was lack of money (n = 310, 68.13%), assumption that follow-up had ended (n = 37, 8.13%), and newborn that died (n = 23, 5.1%). Conclusion: This study highlights the significant impact of financial constraints and absence of a robust follow-up system on poor uptake of neonatal follow-up post-discharge in resource limited settings like Cameroon. Our results serve as advocacy for national health insurance especially in neonates.

Diagnosis of CAH in a Sub Saharan Country: Visible Part of Iceberg

Introduction :Congenital adrenal hyperplasia (CAH) is the most common cause of primary adrenal insufficiency. It is a rare monogenic recessive disorder. In African setting in absence of neonatal screening, the diagnosis is still late, based on a clinical approach. During this clinical enquiry, information from past history or pedigree of the patient is of a huge importance and may revealed surprises. Patients and Methods: In this observational study, we retrospectively included all patients with a diagnosis of CAH. The diagnosis of CAH was retained based on a high 17 hydroxyprogesterone level in addition to clinical and morphological findings. From patients’ files, we extracted data on family history of disease, pedigree, clinical findings and genetics when available of 39 patients from two endocrinopeadiatric centers. Results: In 13 (30%) families, we found 20 reported deaths of infant less than 12 months. In these 13 families, half of the patients followed had 21 hydroxylase deficiencies and had 11 hydroxylase deficiencies. Unsurprisingly, we suspected adrenal insufficiency in these patients at verbal autopsy even in families with a patient with 11 hydroxylase deficiency. Other non DSD malformations or genetic disorders with apparently no link with CAH were reported in 3 families. The father of a patient reported to have hypospadias. Conclusion: Each diagnosis of CAH made in our context is visible part of an iceberg. Behind a diagnosis of CAH made in our setting, is a long course of care, a dramatic past history revealing access to appropriate care disparity. Neonatal screening should thus be considered as an emergency.

SYNTHESIS AND CHARACTERIZATION OF GRAFT COPOLYMER(ALGINATE-g-POLY(N,N-DIMETHYLACRYLAMIDE))

The graft copolymerization of N,N-dimethylacrylamide onto alginate by free radical polymerization using potassium peroxymonosulphate-sarbose as a redox pair in an inert atmosphere was investigated. The reaction conditions for maximum grafting have been optimized by varying the reaction variables, including the concentration of N,N-dimethylacrylamide （7 × 10-2 mol/L to 23 × 10-2 mol/L）, potassium peroxymonosulphate （2 × 10-3 mol/L to 18 × 10-3 mol/L）, sarbose （0.4 × 10-3 mol/L to 3.4 × 10-3 mol/L）, sulphuric acid （1 × 10-3 mol/L to 8 × 10-3 mol/L） and alginic acid （0.4 g/L to 1.8 g/L） along with time duration （60 min to 180 min） and temperature （25℃ to 45℃）. Water swelling capacity, metal ion sorption and flocculation studies of the synthesized graft copolymer have been performed. The graft copolymer has been characterized by FTIR spectroscopy and thermogravimetric analysis.

GABOR ANALYSIS OF THE SPACES M(p,q,w)(R^d) AND S(p,q,r,w,ω)(R^d)

Let g be a non-zero rapidly decreasing function and w be a weight function. In this article in analog to modulation space, we define the space M（p, q, w）（Rd） to be the subspace of tempered distributions f ∈ S′（Rd） such that the Gabor transform Vg（f） of f is in the weighted Lorentz space L（p, q, wdμ）（R2d）. We endow this space with a suitable norm and show that it becomes a Banach space and invariant under time frequence shifts for 1 ≤ p, q ≤ ∞. We also investigate the embeddings between these spaces and the dual space of M（p, q, w）（Rd）. Later we define the space S（p, q, r, w, ω）（Rd） for 1 p ∞, 1 ≤ q ≤ ∞. We endow it with a sum norm and show that it becomes a Banach convolution algebra. We also discuss some properties of S（p, q, r, w, ω）（Rd）. At the end of this article, we characterize the multipliers of the spaces M（p, q, w）（Rd） and S（p, q, r, w, ω）（Rd）.
